OBJECTIVE To evaluate distribution and resistance of pathogens in patients with pulmonary infection after abdominal operation by stages,and provide reference to select antibiotics in clinics.METHODS Forty five patients with pulmonary infection after abdominal operation were included from Jan 2004 to Sep 2007.Their course was divided into 3 stages: onset stage,middle stage and last stage,at every stage to identify pathogens and to test drug sensitivity and the sputa and broncho-alveolar lavage fluid(BALF) were sampled.RESULTS A total of 189 strains of pathogens were isolated,from them 110(58.2%)were Gram-negative bacilli including Pseudomonas aeruginosa,Escherichia coli and Klebsiella pneumoniae;and 59(31.2%)were Gram-positive cocci including Staphylococcus aureus and Enterococcus;and 20(10.6%)were fungi,Candida albicans was the main fungus.At onset stage the most pathogens were Gram-negative bacilli;at middle stage the Gram-positive cocci increased distinctly;and the fungi were detected at middle and last stages.The mixed infection rate was high.The result of drug sensitive test showed that there were high rates of multidrug resistance in P.aeruginosa.The resistance rates of all isolates of Enterobacteriaceae to imipenem or meropenem were 0.The resistance rates of all Gram-positive cocci to linezolid or vancomycin were 0.CONCLUSIONS Distribution of pathogens in patients with pulmonary infection after abdominal operation is different at each stage.The most are multidrug resistant,and rationl use of antibacterial drug in clinics must be based on the distribution and drug-resistance of pathogens.

Objective To investigate the expression and significance of Histidine decarboxylase (HDC) in liver and intestinal tissue for early diagnosis of strangulated intestinal obstruction.Methods Ten male Wistar rats for the control group and 20 ones for the test group which was divided into A,B groups.Group A (n =10) was made by using ligating ileum for 1 h and group B (n =10) was for 3 h.When intestinal ob struction models were built,the changes of pathology in liver and intestine tissue were observed by light microscope.Real time quantitative PCR was used to detect the expression of HDC in liver and intestinal tissue.Results Under light microscopic level,the injury score of intestine in test group was increased in comparison with that in control group.At 3 h after strangulated intestinal obstruction,RT-PCR showed that brightness of HDC band in intestine tissue was significantly higher than the control.Real time quantitative PCR also showed that the expression of HDC in liver tissue in group A and group B increased 1.34 fold(P ＞0.05) and 2.21 fold (P ＜ 0.05) respectively,when compared with the control group.The expression of HDC in intestine tissue in group A and group B increased 1.81 fold(P ＞ 0.05) and 8.02 fold (P ＜ 0.01) respectively,when compared with the control group.Conclusion Expression of HDC can be used for early diagnosis of strangulated intestinal obstruction.

Objective To explore the protective effects of lycium barbarum polysaccharides(LBP) on fibroblast in vitro irradiated by ultraviolet A(UVA). Methods Taking the primary cultured fibroblast as objects, the fibroblast was irradiated by UVA ( irradiation intensity: 2.4 J/cm2). The fibroblast was randomly divided into six groups, control group, UVA radiated group and four protective groups(0.1 mg/ml LBP, 0.2 mg/ml LBP, 0.4 mg/ml LBP and 0.8 mg/ml LBP). The activities of cell proliferation were measured by MTT methods. The contents of MDA, the activities of SOD in the fibroblasts, and the activities of LDH in the supernatants of fibroblasts were determined by biochemical methods. Results The fibroblasts were irradiated by UVA (irradiation intensity: 2.4 J/cm2),the activities of cell proliferation was decreased,the activities of SOD was decreased too, the content of MDA and LDH increased. Compared with UVA irradiated group,in the given concentration,LBP could improve the activities of cell’s proliferation,improve the activities of SOD and decrease the contents of MDA in the cell, and decrease the content of LDH in the supernatants of cells significantly (P

Objective To study the modulation alterations of calcium activated potassium channel (Kca) and calcium activated chloride channels (Clca) on detrusor instability(DI) and their roles in DI. Methods The detrusor instability (DI) animal model was established using female Wistar rats. Detrusor strips were obtained from normal and DI rats. The contraction frequency and amplitude of the strips were measured by an in vitro tension test. The actions of channel openers and blockers were studied. Results Spontaneous contraction frequency and amplitude of DI increased significantly. The block of big conductance calcium activated potassium channel(Bkca) by Iberiotoxin resulted in decrease of frequency but increase of amplitude in the control, but only increase of frequency in DI. The opener, NS1619, resulted in decreases of frequency and amplitude in the control, but only decreases of frequency in DI. The block of small conductance Kca (Skca) by apamin resulted in increases of frequency and amplitude in both control and DI groups, but the opener, Chlorzoxazone, resulted in decreases of frequency and amplitude in the control, but only decrease of frequency in DI. The contraction alterations of DI were significantly lower than those of the control no matter Kca was blocked or opened. NFA, a selective Clca blocker, resulted in no significant frequency and amplitude alteration in the control, but significant decrease in DI. Conclusion These results suggested that Kca and Clca play a critical role in the modulation of detrusor contraction, and the down regulation of Kca and up regulation of Clca suggests that disorder of calcium related regulation may play an important role in DI.

Objective To study the different mRNA expressions of big and small conductance calcium activated potassium channels (Bkca and Skca), and calcium activated chloride channels (Clca) in normal and instable detrusors. Methods Model of bladder outlet obstruction (BOO) of female Wistar rats was prepared by ligating the proximal urethra in the perineum. Detrusor instability (DI) was confirmed by conscious cystometry. The mRNA was extracted from the detrusors of normal and DI rats for the detection of the expressions of Bkca, Skca2, Skca3, and Clca by RT PCR. The different channel expression between normal and instable detrusors was identified by gel imaging. Results The incidence of DI in BOO rats was 76.17%. Objective To explore the possibility of non-discarding blood after cisplatin hyperther-mic antiblastic perfusion(HAP)in treating the patients with malignant bone tumor of lower extremity.Methods Forty patients consisted of 31osteosarcomas and9malignant fibrous histiocytomas of bone.The clinical stages were wholly classified intoⅡB stage according to Ennekings surgical staging system for muscu-loskeletal tumors.The patients were divided into three groups:group1:discarding blood group(10cases),in whom200ml of the400ml blood remaining in heart-lung machine was discarded after HAP;group2:Non-discarding blood group(25cases),the blood remaining in heart-lung machine was not discarded after HAP and re-infused into systemic circulation;group3:systemic chemotherapy group(5cases),cisplatin was infused intravenously.The regional and the systemic blood cisplatin concentrations were determined at3,30and60minutes during HAP,and the systemic blood cisplatin concentrations were determined at0,0.5,1,2,4,8,12,24,48and72hours after HAP in the group1and group2.While1h after cisplatin infusion in-tra venously in the group3,the blood cisplatin concentration were also measured.Results The regional blood cis platin concen tra tions were high er than that in systemic blood at the time during HAP.The systemic blood cisplatin concen trations after HAP,ex cept at0minute in the group2were higher than that in the group1,there were sig nificant statistic differences between the two groups.The systemic blood cisplatin con-cen trations after HAP in the group2were simi lar to that after systemic chemotherapy.No severe adverse ef-fects in the group1were found.The Ennek ings surgical stagings for the tumors were turned fromⅡB intoⅡA in all the patients.Conclusion The method of non-discarding blood after HAP is available,it is not only local treatment ,but also systemic chemotherapy in the patients with malignant bone tumor of lower ex-tremi ty for limb-salvage surgery.
