1.Analysis of health system responsiveness of Huzhou

Objective To analyse the health system responsiveness of Huzhou of Zhejiang Province. Methods Performance evaluation method proposed by WHO was employed for the study of responsiveness. The local residents of Huzhou of Zhejiang Province in 2007 were investigated with multilevel cluster stratified sampling. Data were obtained by household survey and involved items such as dignity,independence,privacy,communication,timely attention,equipment,social support and selection of medical staff,and were subjected to empirical analysis. Results Household survey was conducted in 1353 residents,among whom 28.16% lived in town and 71.84% lived in countryside. The index of health system responsiveness of Huzhou was 7.96,slightly higher than that of Zhejiang Province (7.71). 2.Comparison of Efficacy Between Pantoprazole and Gefarnate in Treating Upper Gastrointestinal Bleeding Caused by Post-PCI Dual Anti-platelet Therapy

Objective To investigate the effect of pantoprazole (proton pump inhibitor, PPI) and gefarnate (gastric mucosa protectant) on the prevention of upper gastrointestinal bleeding in patients undergoing post-percutaneous coronary intervention (PCI) dual anti-platelet therapy.Methods This research included 1263 patients taking enteric aspirin and clopidogrel after PCI.The cases were divided into 4 groups: routine treatment group (n=332), PPI group (n=318), gastric mucosa protectant group (n=299), and PPI+gastric mucosa protectant group (n=314).A follow-up for 6 months was observed including gastrointestinal symptoms, upper gastrointestinal bleeding, major adverse cardiac events (MACE), and adverse reactions.Results There were 52 cases of upper gastrointestinal bleeding within 6 months, including 21 cases from routine treatment group, 9 from PPI group, 15 from gastric mucosa protectant group, and 7 from PPI+gastric mucosa protectant group.The incidence of upper gastrointestinal bleeding among the 4 groups within 6 months was statistically different (X2=8.883, P=0.031).The routine treatment group had significant higher rate than the PPI group and the gastric mucosa protectant group (P<0.05), while among other groups there was no significant difference (P>0.05).The upper gastrointestinal bleeding occurred within 3 postoperative months in 34 out of 52 cases (65.4%).There was no statistical significance among the four groups in regard to bleeding occurrence time (X2=4.212,P=0.648).Conclusions Patients undergoing post-PCI dual anti-platelet treatment can reduce the incidence of gastrointestinal bleeding by taking pantoprazole or combined with gefarnate.Intervention against upper gastrointestinal bleeding should start on the first day after PCI and last for a minimum of 3-6 months.
3.Influence of chronic stress on vasoactive substances and inflammatory factor in rat model with acute myocardial infarction by factorial analysis

AIM:To investigate the effect of chronic unpredictable mild stress(CUMS) on plasma endothelin,nitric oxide and interleukin-6,high-sensitivity C-reactive protein(hs-CRP) in rats after acute myocardial infarction(AMI).METHODS:After the rats of myocardial infarction(MI) were established,the animals were treated under the condition of CUMS for 4 weeks,then the contents of plasma(or serum) endothelin,nitric oxide,interleukin-6 and hs-CRP were measured and data were analyzed by two factor factorial analysis. RESULTS:The results of factorial analysis showed:MI alone had no significantly effect on the level of nitric oxide(P>0.05),but CUMS had significantly effect on increasing the level of nitric oxide(P<0.01). The CUMS had significantly interaction with MI on increasing the level of nitric oxide(P<0.01). Both MI and CUMS increased the level of hs-CRP(all P<0.01). However,no interaction was discovered between MI and CUMS(P>0.05). Both MI and CUMS had no effect on the level of ET-1 and IL-6(P>0.05).CONCLUSION:CUMS increases nitric oxide content and has cooperative effect with MI on increasing NO,both MI and CUMS significantly increase the level of CRP,but have no effect on the level of ET-1 or IL-6,suggesting that the abnormal increase in nitric oxide and hs-CRP contents may be the important pathophysiological changes of post-MI depression.
4.Clinical Study on 38 Cases of Primary Sjogren's Syndrome Patients Treated with Xuanfei Bujin Particles

Objective To observe clinical efficiency of Xuanfei Bujin particles in the treatment of primary Sjogren's syndrome (PSS) and its impact on symptom score, ESR, immunoglobulin, as well as soluble intercellular adhesion molecule (sICAM-1). Methods Fifty eight cases of PSS patients were randomly divided into 38 cases of traditional Chinese medicine treatment group and 20 cases of western medicine treatment group. The symptom score, clinical efficiency, ophthalmic examination, sICAM-1, sIL-2R, ESR, immunoglobulin and other changes were observed. Results Total effective rate of Xuanfei Bujin particles treatment group was 84.21%, better than the control group. Xuanfei Bujin particles can significantly reduce dry mouth, dry eyes, and other symptoms of the main points, and improve the inspection of ophthalmology, sICAM-1. Conclusion Xuanfei Bujin particles is safe and efficient, and has certain degree in immune regulation.
5.Purification of total flavones from Bushen Recipe with macroporous resin

AIM: To study the absorption performance and purification process of total flavones from Bushen Recipe(Radix polygoni multiflori;Herba epimedii;Rhizoma drynariae;etc) with macroporous resin. METHODS: The contents of total flavones,icariin,tetrahydroxystilbene and naringin were worked as indexes,the adsorptive capacity of D101,DA201 and DM301 resins were investigated,and the static adsorption curve,dynamic leakage curve and elution curve were used for the selection of macroporous resin. RESULTS: The elution parameters of selected DM301 consisted of the 0.25 BV/h feed rate solution ratio of 4 BV water to elute resin and 4 BV 70% alcohol to be collected as elution solvent,and the elution rate of 2.4 BV/h was established. CONCLUSION: Macroporous resin method is a better candidate method for purifying flavones in Bushen Recipe.
6.Pharmacodynamic experiment of the antivirus effect of houttuynia cordata injection on influenza virus in mice.

It is to investigate the effect of two kinds of Houttuynia Cordata Injection on preventing and treating H1N1 influenza virus infection in mice. Pneumonia model was set up by intranasal infection of the normal and immunocompromised mice with influenza virus FM1 and PR8. The two injections were administered before and after the administration of virus, separately, and the lung index was observed. The results showed that the two preparations have obvious therapeutic effect on normal mice infected with influenza virus FM1 and PR8. And to FM1, the new injection's effect is better at small dosage. The results also showed that the two preparations have obvious prophylactic effect on immunodepressed mice infected with influenza virus FM1 and PR8. And to PR8, the old injection's effect is better at small dosage. Houttuynia Cordata Injection can improve the mice pneumonia caused by influenza virus H1N1 and decrease the lung index markedly. It has a remarkable preventive and therapeutic effect on H1N1 influenza virus in mice.
7.Pedicle screw fixation for thoracolumbar fracture with spinal cord injury:a comparison of the selective injury segment and the whole bone graft fusion

BACKGROUND:Spinal canal decompression is needed in posterior pedicle screw fixation surgery for thoracolumbar burst fractures combine with spinal cord injury. The structure of posterior spine is often damaged. The posterolateral bone fusion in al fixed segment is stil the main surgery. In order to further reduce fusion segment and maintain motor unit, it is necessary to perform selective segmental bone graft fusion during fixation and decompression.
OBJECTIVE: To discuss the advantages of selective posterolateral vertebral fusion for thoracolumbar fracture with spinal injury through comparing with posterolateral vertebral fusion.
METHODS: Data of 83 thoracolumbar burst fracture cases, who received posterior lumbar decompression and short segment fixation with pedicle screws and bone graft through injured vertebra from January 2006 to July 2013, were analyzed retrospectively. According to fusion segments, above patients were divided into selective posterolateral vertebral fusion group (n=42) and the whole posterolateral vertebral fusion group (n=41). Perioperative index, internal fixation, vertebral height loss rate, Cobb angle, spinal nerve recovery and Oswestry Disability Index were compared between the two groups.
RESULTS AND CONCLUSION:(1) Al cases were folowed up for 25-32 months. (2) There was no statistical significance in operation time, intraoperative blood loss and ambulation time between the two groups (P > 0.05). Postoperative drainage volume was less in the selective posterolateral vertebral fusion group than in the whole posterolateral vertebral fusion group (P < 0.05). Before removal of fixator, there was no loosened fixator or breakage of screw or stick. (3) There were improvements in the rate of vertebral front height loss and Cobb angle in both groups at various time points after operation (P < 0.05). There was no statistical significance in the rate of vertebral front height loss and Cobb angle in both groups (P > 0.05). (4) There was no significant difference in fusion rate at 6 months after treatment between the two groups (P > 0.05). Fusion was achieved in both groups before removal of the fixator. (5) Spinal nerve recovery was found after treatment in both groups. No significant difference in Oswestry Disability Index was detected in final folow-up (P > 0.05). (6) Results verified that compared with the whole posterolateral vertebral fusion, selective posterolateral vertebral fusion can obtain a good vertebral height and prevent Cobb angle loss again, reduce the internal fixation loosening and breakage. After removal of the fixator, selective posterolateral vertebral fusion can reduce spinal motion unit lost, and decrease the adjacent vertebral degeneration.
8.Isolation and biological characteristics of rat umbilical cord mesenchymal stem cells

BACKGROUND:There are many studies concerning rat bone marrow mesenchymal stem cells for immune tolerance following transplantation and tissue repair.However,there are no reports on umbilical cord mesenchymal stem cells(UCMSCs).OBJECTIVE:To establish a method of separating mesenchymal stem cells(MSCs)from rat umbilical cord,and to study its biological characteristics.METHODS:MSCs were separated from rat umbilical cord with enzyme method and tissue mass method,and then incubated in DMEM-LG medium.Cell morphology was observed under an inverted microscope.Growth curves of cells were drawn using cell counting.Cell cycle and surface antigen were detected with flow cytometry.Adipogenic differentiation and osteogenic differentiation were tested by immunohistochemistry.RESULTS AND CONCLUSION:Both of the two methods could obtain plenty of MSCs from rat umbilical cord.Primary culture showed that the efficiency of enzyme method was higher than tissue mass method.Passage time of the former was about 10 days and the latter was 14 days.The passage time of latter except primary culture was the same.Immunophenotype analysis showed that MSCs from rat umbilical cord expressed adhesion molecule and stromal cell markers,CD90 and CD106,but did not express hematopoietic cell markers,CD34 and CD45.In vitro induction test verified that rat UCMSCs have the potentials of adipogenic and osteogenic differentiation.
9.Preliminarily analysis on traditional Chinese medicine advices in Treatise on Febrile Diseases.

To make a systematic analysis on literatures concerning traditional Chinese medicine (TCM) advices in Treatise on Febrile Diseases, and summarize the main connotations of traditional Chinese medicine advices, relevant TCM advices in Treatise on Febrile Diseases were collected, screened, compared, summarized and analyzed according to TCM dosage form preparation methods, TCM administration methods, medication contraindications and nursing after TCM administration. The literatures concerning medications in Treatise on Febrile Diseases were consulted, summarized and compared to standardize medicine advices and facilitate rational clinical application of TCMs. The standard medicine advices were as follows. The boiling water for TCMs shall be tap water and well water. The decoctions that have effects in promoting blood and meridians can be boiled with wine. The decoctions containing toxic components can be boiled with honey. Some TCMs shall be boiled with special methods, e. g. Herba Ephedra that could be boiled before other medicine and skimmed. Japonica rice could be added in decoctions to measure the duration of decoctions. Different dosages were required for different forms (litre, pill, medicine spoon). Administration times, temperature and frequency shall be adjusted according to target positions, functions and stage of illness. As for dietary contraindications during medication, thick porridges are recommended, where foods impacting medicine efficacy are prohibited. Regarding nursing after medication is important to recover physical functions, particularly warm porridges can go with diaphoretic recipes, while thick porridges can go with purgative recipes. And drug efficacies shall be defined by observing urine and excrements, and blood form. In conclusion, Treatise on Febrile Diseases is the first book that discusses TCM advices and records them in details. In this study, new standard medicine advices were proposed to provide important basis for improving clinical advices of TCMs and supports for developing the TCM dispensing technology.

10.One-stage arthroscopic reconstruction of both cruciate ligaments using Achilles tendon-bone allografts

Objective To evaluate one-stage arthroscopic reconstruction of anterior crueiate ligament (ACL)and posterior cruciate ligament(PCL)using Achilles tendon-bone allografts. Methods From July 2000 to February 2005.we treated 15 patients(11 males and 4 females)whose ACL and PCL were ruptured at one knee but the eontralateral knee was intact.Their associated meniscus injuries were treated arthroscopically according to established procedures prior to ligament reconstruction.Thirty Achilles tendon-bone allografts were used to reconstruct torn ACL and PCL in 15 knees at one stage.Reconstruction of both ligaments was performed at subacute or chronic phase(>3 to 8 weeks)in 12 casses,and at acute phase in 3 cases(<3 weeks).All knees were graded pre-and postoperatively using the International Knee Documentation Committee(IKDC)and Lysholm scoring systems.At follow-up,functions were evaluated for all patients and compared with those of the contralateral healthy knee. Results All patients were followed up for a minimum of 3 years(mean,38 months).Preoperatively,the IKDC ratings showed all the injured knees were severely abnormal.At final postoperative f0Uow-up,9 knees received a normal rating,5 a nearly normal one and 1 an abnormal one.The differences in Lysholm score were statistically significant (t=15.660,P<0.05)between pre-and postoperative analyses.The most noticeable postoperative complication was a short localized fever coupled with arthroedema in 1 case. Conclusions Achilles tendon-bone allograft offers an alternative for simultaneous arthroseopic reconstruction of ACL and PCL.However,problems inherent in allograft tissues entail further investigation to ensure future application.
