1.Clinical Effect of Neoadjuvant Chemotherapy Combined with PA-MSHA Injection on Breast Cancer

Objective: To evaluate the effect and safety of neoadjuvant chemotherapy combined with PA-MSHA injection for breast cancer patients. Methods: An open randomized controlled clinical trial was con-ducted. Fourty patients with breast cancer were randomly assigned to neoadjuvant chemotherapy group (the control group, n=20) and neoadjuvant chemotherapy combined with PA-MSHA injection group (the experi-ment group, n=20). The evaluation of therapeutic effect was carried out when the treatment was completed. Kamofsky score was recorded before and after therapy. Venous blood was drawn before and after therapy and immune function (IFN-γ, IL-2, IL-4, and IL-10) and other indicators (Caspase-3, VEGF, MMP-2 and MMP-9) were measured by double antibody ELISA test. Adverse effects of PA-MSHA during therapy were ob-served and recorded. Results: The overall response rate (RR) in the experiment group was significantly higher than that in the control group (P<0.05). No significant difference was found in the pathologic complete remis-sion (pCR) between the experiment group and the control group (P>0.05). In the experimental group, pCR was significantly different before and after therapy (P<0.01). The score in the experimental group was signifi-cantly higher than that in the control group after therapy (P<0.01). With the treatment of chemotherapy and PA-MSHA injection, IFN-γ and IL-2 levels were significantly higher while IL-4 and IL-10 levels were significant-ly lower in the experiment group (P<0.05). A significant increase in serum Caspase-3 and a significant de-crease in serum VEGF, MMP-2 and MMP-9 (P<0.05) after therapy were also observed in the experimental group. The level of serum MMP-9 was decreased significantly (P=<0.05) after therapy in the control group. Con-clusion: Neoadjuvant chemotherapy combined with PA-MSHA injection can significantly improve the RR of breast cancer patients, enhance their cellular immune function, induce the apoptosis and restrain the metasta-sis of breast cancer cells. The PA-MSHA has been proved to be an ideal supplementary therapy for breast cancer.
3.Transumbilical single-site single-port versus single-site double-port laparoscopic varicocelectomy for varicocele in adolescents.

OBJECTIVETo compare the effect of transumbilical single-site single-port with that of transumbilical single-site double-port laparoscopic varicocelectomy in the treatment of varicocele in adolescents.
METHODSWe randomly assigned 80 varicocele patients aged 10 - 16 years to two groups of equal number to receive transumbilical single-site single-port and single-site double-port laparoscopic varicocelectomy, respectively. We compared the operation time, postoperative hospital stay, incisional pain, complications and satisfaction with the abdominal cosmetic outcomes between the two groups.
RESULTSAll the operations were successfully performed. The double-port group showed a significantly higher score on the Visual Analogue Scale than the single-port group (4.8 +/- 1.4 vs 3.6 +/- 1.1, t = -4.986, P < 0.01), but there were no significant differences between the two groups in the operation time ([29.8 +/- 4.2] vs [31.2 +/- 4.6] min, t = 1.383, P = 0.171), postoperative hospital stay ([1.95 +/- 0.7] vs [1.82 +/- 0.8] d, t = -0.784, P = 0.436), complications (0 vs 0) and scores on the satisfaction with abdominal cosmetic outcomes (4.6 +/- 0.6 vs 4.8 +/- 0.5, t = 1.253, P = 0.214). No recurrence, umbilical hernia, hydrocele and orchiatrophy were found in the two groups of patients at 6 months after operation, and no visible scar was observed on the abdominal surface.
CONCLUSIONWith strict surgical indications, single-site single-port and single-site double-port laparoscopic varicocelectomies have similar clinical effects in the treatment of varicocele, which leave no scar on the abdominal surface. Single-site double-port laparoscopy needs no special instruments and therefore is worthier of wide clinical application.

4.Diagnostic value of diffusion kurtosis imaging in assessing liver fibrosis

Objective To investigate the diagnostic value of diffusion kurtosis imaging(DKI)in the classification of hepatic fibrosis. Methods Thirty-five male SD rats were randomly divided into two groups:the hepatic fibrosis group(n=28)and the control group(n=7). The rats in hepatic fibrosis group were randomly divided into 4 subgroups and seven rats per group, the rats were administrated 50% CCl4 intraperitoneally twice a week to establish hepatic fibrosis , and the four subgroups were injected 2, 4, 6, and 8 weeks, respectively. The rats in the control group were administrated same dose of olive oil for 8 weeks. One rat in hepatic fibrosis group was died of liver failure in the 7th week, and a total of 27 fibrosis experimental rats and 7 control rats were finally included in this study. DKI was performed at the end of the injection period for all rats, the apparent diffusion(D)and kurtosis(K)values were evaluated. Rats were sacrificed immediately after MRI scan and liver specimens were collected. The liver tissues were examined by pathology, liver fibrosis degree, which was graded from S0 to S4, and inflammatory activity, which was graded from G0 to G3 were graded. The difference of D value and K value between different liver fibrosis and inflammatory activity scores was compared by one-way ANOVA(normal distribution)or Kruskal-Wallis test(skewed distribution). Spearman correlation analysis and multiple regression analysis were used to reveal the correlation between DKI parameters and fibrosis staging/necroinflammatory activity grade. To confirm the efficiency of using the ROC curve of DKI parameters to qualify the liver fibrosis grade, which grade was≥3. Results Seven, 6, 6, 7, 8 rats were diagnosed as S0 to S4, respectively. The difference of D value and K value among different fibrosis grades was statistically significant(P<0.05). D value and the degree of fibrosis was negatively correlated(r=-0.650, P<0.01);K value and liver fibrosis grade no correlation(r=0.336, P=0.080). Thirteen, 6, 8, 7 rats were diagnosed as G0 to G3, respectively. D value was negatively correlated with inflammatory activity(r=-0.590, P=0.001);K value was no correlation with inflammatory activity(r=0.169, P=0.389). Compared with inflammatory activity, fibrosis classification was an independent factor in determining D values(P=0.001). ROC analyses demonstrated an area under the curve(AUC)of D value, K value, D value combined with K value in the diagnosis of liver fibrosis grading ≥ 3 level were 0.781, 0.672 and 0.833, respectlively. The sensitivity and specificity of D value combined with K value were 83.3% and 75.0%, respectively. Conclusion DKI imaging is of great value in the classification of hepatic fibrosis and can be used as an effective method for the diagnosis of fibrosis.
5.Two-dimensional electrophoresis for the screening of biomarker in carcinomas of the urinary bladder

Objective This study was designed to establish and optimize the two-dimensional gel electrophoresis (2-DE) for the proteome analysis of human Transitional Cell Carcinomas of the Urinary Bladder ,and to established the two-dimensional gel electrophoresis (2-DE) profile of human Transitional Cell Carcinomas of the Urinary Bladder(TCC) of different stages in carcinogenic process. The basic aim is to perform differential analysis and provide a basis for identifying carcinogenesis-associated protein of TCC.Methods After obtaining samples of the normal,invasive tissue,noninvasive tissue,a series of methods Such as sample preparation,electrophoresis parameters,gel concentration,PDQuest 2DE analysis software,peptide mass fingerprint (PMF,MALDI-TOF-MS)were used to analyses these tissues.Results The good 2DE patterns of different tissues including resolution and reproducibility were obtained. After coomassie Brilliant Blue R-250 staining, the average matching ratio was 76%.There was a good reproducibility of spot position in 2-DE map, with average deviation in IEF direction of (0.98?0.26) mm, while in SDS-PAGE direction it was(1.23?0.22) mm. The 2-DE patterns with good quality had been obtained. Ten protein spots chosen randomly from different stages were identified by PMF, some of which were involved in the cell proliferation, differentiation, cycle regulation, and tumor occurrence, such as Heat shock protein-27,SFN etc.Conclusions The results provide a fundamental basis for further study of mechanism of human Transitional Cell Carcinomas of the Urinary Bladder and screening its specific marker.

7.Clinical efficacy of artificial skin combined with vacuum sealing drainage in treating large-area skin defects.

OBJECTIVETo observe the clinical efficacy of artificial skin combined with vacuum sealing drainage (VSD) in treating large-area skin defects.
METHODSTotally 18 patients with skin defects, treated with artificial skin combined with VSD from September 2008 to May 2009 in our hospital, were retrospectively analyzed in this study. There were 15 males and 3 females, aged 7-66 years, 34.3 years on average. Among them, 10 cases had skin laceration caused by traffic accidents (7 with open fractures), 1 mangled injury, 1 blast injury, 1 stump infection combined with skin defects after amputation and 5 heel ulcers.
RESULTSAll skin grafts in 16 cases survived after being controlled by VSD for one time. For the rest 2 patients, one with skin avulsion on the left foot was given median thickness skin grafts after three times of VSD, the other with open fractures in the left tibia and fibula caused by a traffic accident was given free flap transplantation. Skin grafts of both patients survived, with normal color and rich blood supply.
CONCLUSIONSkin grafting in conjunction with artificial skin and VSD is much more effective than traditional dressing treatment and worth wide application in clinic.

8.Correction of claw hand deformity after burns by elastic traction.

OBJECTIVETo investigate a new method for correction of claw hand deformity after burns.
METHODSFrom May 2006 to Jul. 2010, 12 patients with claw hands deformities after burns were treated with skin grafts (11 hands) and skin flap (1 hand) with unsatisfactory results. Then elastic traction (skin traction or skeletal traction) were performed with individual functional brace.
RESULTSAll patients were followed up for 0.5 to 2 years. Elastic traction was effective in the correction of metacarpophalangeal joint deformity, buttonhole deformity, thumb-in-palm deformity, scar contracture, and palmar arch deformity.
CONCLUSIONSElastic traction is a simple and effective way for the correction of claw hand deformity after burns with less morbidity and stable results.

9.Progress on the Study of Sex-specific Membrane Proteins in Mammalian Spermatozoa

The expression of specific genes in sex chromosomes is the basis of sex-specific membrane protein in mammalian spermatozoa. The gene expression products are shared among spermatozoa through intercellular bridges, however, the phenomena of male transmission-ratio distortion and sex ratio distortion proved that differential proteins exist between X and Y spermatozoa. In addition, the existence of sex-specific proteins was confirmed by the separation experiment of X/Y chromosome bearing spermatozoa and the detection result of sex specific proteins. At the same time, it was also confirmed that the difference of the sex-specific protein is weak . The advance of separation techniques as well as the integration and optimization among these techniques has made it possible to separate sex-specific membrane proteins in mammalian spermatozoa.
10.Study on incompatibility of traditional Chinese medicines.

The incompatibility of traditional Chinese medicines is related to the clinical medication safety, so has attracted wide attentions from the public. With the deepening of studies on the incompatibility of traditional Chinese medicines represented by 18 incompatible herbs, the incompatibility of theory traditional Chinese medicines has raised to new heights. From the origin of incompatibility theory of traditional Chinese medicines, relationship of herbs, harms of incompatible herbs and principle of prevention to toxic effects of specific incompatible medicines, the innovation and development of the traditional Chinese medicine incompatibility theory was explored. Structurally, the incompatibility of traditional Chinese medicines refers to the opposition of two herbs based on seven emotions and clinical experience. The combination of incompatible herbs may lead to human harms, especially latent harm and inefficacy of intervention medicines. The avoidance of the combination of incompatible herbs and the consideration of both symptoms and drug efficacy are the basic method to prevent adverse reactions. The recent studies have revealed five characteristics of incompatible herbs. Toxicity potentiation, toxication, efficacy reduction and inefficacy are the four manifestations of the incompatible relations. The material changes can reflect the effects of toxicity potentiation and toxication of opposite herbs. The accumulation of toxicity and metabolic changes are the basis for latent harms. The antagonistic effect of main efficacies and the coexistence of positive and negative effects are the distinctive part of the incompatibility. The connotation of incompatible herbs plays an important role in the innovation of the traditional Chinese medicine incompatibility theory.
