1.Adverse Drug Reactions in Our Hospital:Analysis of 352 Cases

OBJECTIVE:To discuss the characteristics and regularities of adverse drug reactions(ADR)occurred in our hospital.METHODS:A total of 352 ADR cases submitted to State ADR Center by our hospital from 2006 to 2008 via network system were analyzed statistically in respect of patients'age and sex,the drugs involved,routes of administration,ADR-involved organs or systems,and turnover etc.RESULTS:Of the total 352 ADR cases,the majority(67.6%)were induced by antimicrobial drugs;44.9% manifested as lesions of skin and its appendants,and 43.2% were induced orally.CONCLUSION:It is urgent to intensify the publicizing of ADR-related knowledge so as to offer guidance for clinic rational,safe and effective drug use.
2.Stroma reaction and clinical significance in benign lesion and invasive carcinoma of breast

Objective To study the expression and clinical significance of CD34 and smooth muscle actin (SMA) in stroma of breast tissues and lesions. Methods Seventy cases of breast tissues and lesions, including 20 fibroadenomas, 10 sclerosing adnoses, 30 invasive ductal carcinomas and 10 invasive lobular carcinomas were investigated, and 10 normal breast tissues were served as controls. Immunohistochemical staining was applied to compare the distribution of CD34+ fibrocytes and SMA-reactive myofibroblasts. Results The stroma of normal breast tissue contained CD34+ fibrocytes, whereas SMA-reactive myofibroblasts were absent (100% for both). All benign breast lesions exhibited astromal CD34+ fibrobytes, and fibroadenomas showed SMA-reactive myofibroblasts as well. In invasive ductal and lobular carcinoma the stroma was devoid of CD34+ fibrocytes, but a varying number of stromal SMA-reactive myofibroblasts were detectable (100%). Conclusion In breast carcer, immunohistochemical staining used in detecting expressions of CD34 and SMA is helpful in distinguishing benign lesions from malignancies.
3.Effects of pioglitazone on angiotensin Ⅱ-induced proliferation and collagen expression of adventitial fibroblasts

Objective To investigate the effects of pioglitazone on angiotensin Ⅱ(AngⅡ) -induced proliferation and collagen typeⅠ expression of adventitial fibroblasts (AF). Methods The AFs were isolated from rat thoracic aorta. MTT colorimetry and flow cytometry were used to study the effects of pioglitazone on proliferation and cell cycles of AF. The expression of collagen typeⅠ regulated by pioglitazone was examined by the method of Western blot. 4. Effect of deactivation of enzymes on chemical composition of Forsythiae Fructus using 1H-NMR-based metabolomics approach

Objective: To investigate the effect of deactivation of enzymes on chemical composition of Forsythiae Fructus using NMR based metabolomics approach. Methods: 1H-NMR-based metabolomics approach combined with multivariate statistical analysis was used to investigate the differential metabolites between raw Forsythiae Fructus (RF) and Forsythiae Fructus with deactivation of enzymes (DF). And degradation pathway of significant metabolites was inferred by biosynthetic pathway. Results: Twenty-four metabolites were identified in the 1H-NMR spectrum of Forsythiae Fructus, and principal component analysis (PCA) showed clear separation between RF and DF. The S-plot of orthogonal partial least square discriminate analysis (OPLS-DA) revealed that ten compounds contributed to the separation of RF and DF, and the levels of forsythiaside A, forsythiaside C, phillyrin, rutin, and surcose were higher in DF than those in RF, while the levels of rengyol, rengyoside, rengyolone, α-glucose, and β-glucose were higher in RF than those in DF. Moreover, the degradation of forsythiaside A into rengyol was proposed. Conclusion: This study reveals the chemical effect of deactivation of enzymes on Forsythiae Fructus in a holistic manner, which confirms rationality and necessity of deactivation of enzymes during the processing of Forsythiae Fructus. This study also serves as a basis for the bioactive compounds and quality standards research of Forsythiae Fructus.
5.Protective effect and action mechanism of petroleum ether extracts from Saussurea involucrate on brain tissues of hypoxia rats.

OBJECTIVETo investigate the protective effect and action mechanism of petroleum ether extracts from Saussurea involucrate on brain tissues of hypoxia rats under constant pressure and closed conditions.
METHODThe PESI dosage-dependent experiment for hypoxia rats was conducted under constant pressure and closed conditions by intraperitoneally injecting 125, 250, 500 mg x kg(-1) to finalize that the optimum dosage is the high dose of PESI. Afterwards, 90 Wistar rats were randomly divided into the hypoxic model group, the acetazolamide 250 mg x kg(-1) group and the PESI high dose group. Each group was further divided into three subgroups according to different hypoxia times, with 10 rats in each subgroup. Under the same hypoxia and administration conditions, the rats were sacrificed after 0, 3, 6 h respectively. Their brain samples were collected for common pathological observation and immunohistochemical staining of HIF-1alpha. Real-time RT-PCR was used to detect HIF-1alpha, EPO, HO-1 and Caspase-3 gene expressions. And the Western blot assay was adopted to detect HIF-1alpha protein expression.
RESULTThe brain tissues of the hypoxia model group were severely damaged with the increase in the hypoxia time. The acetazolamide group and the PESI high does group were damaged in a much lower degree. According to the gene expression and the Western blot assay, high dose of PESI could inhibit HIF-1alpha expression. According to the pure gene expression test, high dose of PESI could increase EPO and HO-1 mRNA expressions, but inhibit Caspase-3 mRNA expression.
CONCLUSIONPESI's protective mechanism for brain tissues of hypoxia rats under constant pressure and closed conditions may be related to its effects in inhibiting HIF-1alpha expression, increasing EPO expression and resisting cell apoptosis.

6.Clinical observation on treatment of albuminuria in patients with pregnancy-induced hypertension syndrome in puerperium by Xiaobai Decoction.

OBJECTIVETo observe the effects of Xiaobai Decoction (XBD) in reducing albuminuria and shortening the duration of albuminuria in patients with pregnancy-induced hypertension syndrome (PIH) in puerperium.
METHODSEighty-five patients were given the conventional treatment with magnesium sulfate for relieving convulsion and lowering hypertension, at the same time, the treated group was given XBD additionally with the modification according to the symptoms. The treatment course for both groups was 14 days. Routine test of midstream urine was performed every three days, and 24 h-urinary protein was measured every week.
RESULTSThe therapeutic effect on the 43 patients of the treated group was markedly effective in 11 (25.6 % ), effective in 26 (60.4%) and ineffective in 6 cases (14.0%), the total effective rate being 86.0%; while in the 42 patients of the control group, the corresponding numbers were 5 (11.9%), 21 (50.0%), 16 (38.1%) and 61.9%, respectively, the efficacy of the former was significantly better (P < 0.05).
CONCLUSIONXBD is a simple, safe and effective drug for reducing albuminuria and shortening the duration of albuminuria in puerperium of PIH patients.

Correct clinical thinking always comes from solid basis and clinical theory and knowledge, careful history taking, meticulous physical examination, rigorous logical thinking and constant summing up experiences and lessons. Background Granulocyte colony-stimulating factor(G-CSF)has been reported to have beneficial effect on cardiac dysfunction in post infarction and doxorubicin-induced cardiomyopathy.Objective To investigate the effects of G-CSF on cardiac remodeling in cardiac hypertrophy induced by angiotensin Ⅱ(Ang Ⅱ).Methods Thirty-six male wild type mice(WT)were allocated randomly to receive subcutaneously G-CSF(10 ?g/kg per day, n=9),or Ang Ⅱ(2.88 mg/kg per day,n=9),or Ang Ⅱ plus G-CSF(Ang Ⅱ 2.88 mg/kg+G-CSF 10 ?g/kg,n =9)for 4 weeks with untreated WT(n=9)as controls.Blood pressure and cardiac function were measured. 10.In vitro study on microleakage of three adhesive materials

Objective To evaluate the influence of self-etch adhesive,total-etch adhesive and glass ionomer cement on the marginal microleakage of class II restorations. Methods Thirty human premolars were randomly divided into 3 groups(n=10),and cuboid class II cavities(4.0 mm?3.5 mm?2.5 mm) were prepared.Restoration was performed using self-etch adhesive+nano-resin(self-etch group),total-etch adhesive + nano-resin(total-etch group) or glass ionomer cement(glass ionomer group).Half of each group underwent 200 thermocyclings and the other half underwent 500 thermocyclings.The specimens were immersed in 0.5% basic fuchsin for staining.Each tooth was then evaluated the microleakage at the axial wall and the gingival wall section by section under a stereomicroscope.The data were statistically analyzed.
