1.Study of streptococcus pneumonia capsular polysaccharide antibodies in patients with chronic obstructive pulmonary disease

ObjectiveTo study streptococcus pneumonia capsular polysaccharide antibodies of patients with chronic obstructive pulmonary disease(COPD) in acute exacerbation and the feasibility of vaccination for patients with COPD.MethodsThe COPD in acute exacerbation without respiratory failure (RF)(COPD without RF group),COPD in acute exacerbation with RF (COPD with RF group),asthma (asthma group),healthy elderly examination(elderly examination group) and healthy youth examination (youth examination group) were selected,and each group had 15 cases.The levels of streptococcus pneumonia capsular polysaccharide antibodies IgG,IgM and IgA were measured in all groups.Results There was no significant difference in IgG among the five groups (P > 0.05 ).The levels of IgM in COPD without RF group and elderly examination group were significantly lower than those in COPD with RF group,asthma group and youth examination group (0.554 ± 0.309 and 0.538 ± 0.327 vs.0.810 ± 0.387,0.887 ± 0.278 and 0.852 ± 0.305,P < 0.05 ).Although the level of IgA in COPD without RF group was significantly lower than that in youth examination group(0.532 ±0.297 vs.0.930 ±0.502,P <0.05),there was no significant difference among the five groups.Conclusion Patients with COPD should inoculate pneumococcal vaccination.

3.Estrogen inhibition on the expression of amyloid precursor protein C-terminal in ovariectomized female rat hippocampus and cortex

AIM To study the effect of estrogen on the expression of amyloid precursor protein (APP) carboxyl terminal fragment in ovariectomized female rat hippocampus and cortex regions. METHODS Ovariectomized female rats were fed with Premarin or Progynova for 3 cycles before sacrificed. S P immunohistochemistry was performed to detect the APP C terminal fragment distribution, and semi quantitative H score was used to compare the expression. RESULTS APP C terminal fragment expression in both female rat hippocampus and cortex increased significantly after ovariectomy. Compared with ovariectomized control, C terminal fragment expression in the above regions decreased. It was even lower in progynova group than in premarin group. CONCLUSION Both premarin and progynova down regulate the APP C terminal expression in ovariectomized female rat hippocampus and cortex. The modulation of progynova is more effective.
4.Differential regulating effect on estrogen receptor ? and ? expression in female rat hippocampus and cortex regions between different types of estrogen regents

AIM: To study the regulatory effect two different estrogen reagents on expressions of estrogen receptor ? and ? in female rat hippocampus and cortex regions. METHODS: 12 cycles after ovariectomy, female rats were orally injected with premarin or progynova for 3 cycles before sacrificed. Semiquantitative RT-PCR was used to detect the ERs mRNA expression and SP immunohistochemistry was performed to measure the ERs protein distribution and expression. RESULTS: In premarin group, ER ? mRNA levels in both hippocampus and cortex tissues decreased significantly compared with control. ER ? protein level in hippocampus was lower than that in the control. However, ER ? protein level in cortex had no statistical difference. ER ? mRNA in the two regions and ER ? protein in cortex had no statistical differences compared with control, while ER ? protein level in hippocampus was higher than that in the control. In progynova group, both mRNA and protein levels of ER ? increased significantly in the two regions compared with the control, and ER ? mRNA level also increased in hippocampus, but ER ? mRNA level in cortex and ER ? protein levels in the above two regions showed no statistical differences. CONCLUSION: There were differential regulatory effects on ER ? and ER ? expression in female rat cognitive regions between the two different types of estrogen reagents, which may be one of the mechanisms of varied effects in different estrogen replacement therapy reagents.
5.Detection of Common Fungal Pathogens by Genenral Primer PCR

Objective To develop the detection method of common fungal pathogens by general primer PCR.Methods The primers were designed from the target genes of 5.8S rDNA and 28S rDNA of fungi,and the specificity and sensitivity were observed.Results The general primer PCR can be used to detected C.albicans,C.parapsilosis,C.krusei,C.glabrata,C.tropicalis,C.neoformans,A.fumigatus,A.flavus,A.nidulans,A.niger,C.carrionii,P.verrucosa,S.schenckii,F.pedrosoi,T.rubrum,T.mentagrophytes,M.gypseum,M.canis,E.floccosum,M.racemosus,the sensitivity was 15 pg/ml of DNA.Conclusion The general primer PCR can be used to detect common fungal pathogens.
6.Repeated tissue expansion in head and neck

Objective To summarize the experiences in treating 30 cases of skin soft tissue defects in head and neck by applying repeated expansion, and to explore an effective and satisfactory method of massive defect repair in head and neck. Methods Skin soft tissues were repeatedly expanded for 1 to 3 times after the first expansion. Massive skin defects in head and neck were repaired several times by useing repeated expansion of flap. The size of the skin defects ranged from 29cm?24cm to 10cm?8cm. The indications, complications and experiences in the treatment were discussed. Results A total of 30 cases were treated. Complication incidences of every expansive procedure were 27%, 15%, 14% and 0% respectively . After 6~12 months postoperative follow-up, the appearance and functional restoration was satisfactory. Conclusion Repeated expansion is an excellent method of repairing massive defects in head and neck.
7.CT Features of Peritoneal Metastasis

Objective To study the CT features of peritoneal Metastasis in post-operative patients of ovarian carcinomas.Methods CT appearance of peritoneal metastasis of ovarian carcinomas proved by surgery and pathology in 33 post-operative patients were reviewed.The CT features of the foci were recorded and analyzed,especially on the location,quantity,density and size.Results In the peritoneal cavity,186 implant foci and 10 recurrent foci were found.metastasis often occurred in the right upper abdomen,especially the right subphrenic spaces.The most frequent locations were the right suprahepatic and subhepatic spaces,the small bowel mesentery,the gastrocolic ligament and the omentum.The density of the foci was most of solid.The size was ranged from 0.5~13 cm.Conclusion Peritoneal metastasis is the most frequent route of metastases for ovarian carcinomas.It is frequently found in upper abdomen,especially in the subphrenic spaces.Localized ascites in the peritoneal cavity is another important sign suggesting peritoneal implants.CT scan from the diaphragm to the pelvic floor will be helpful to diagnose peritoneal implants in cases of post-operative ovarian carcinomas.
8.Current situation survey and analysis of a district general hospital in Shanghai

Objective To analyse the changes of a district general hospital before and after management by Shanghai Renji Medical Group through current situation survey,and explore the conditions for the development of district general hospitals.Methods The Zhujiajiao People's Hospital of Qingpu District in Shanghai was selected,and patients(n=50)and hospital staff(n=100) were conducted current situation survey with questionnaires which included the knowledge and evaluation of the hospital's current situation as well as the changes before and after management by Shanghai Renji Medical Group.Results Half year after management by Shanghai Renji Medical Group,great progress in medical work was achieved,with a favourable development trend.The rate of satisfaction on medical service evaluated by inpatients reached 89.60%,and 81.91% hospital staff had confidence in the sustainable development of the hospital. Conclusion Management by Shanghai Renji Medical Group has brought prospects for the Zhujiajiao People's Hospital.Under current environment,only certain conditions are met can a district general hospital achieve sustainable development.
9.The diagnostic value of confocal laser endomicroscopy in colorectal polyps

Objective To analyze endoscopic histological characters and establish diagnostic standards of colorectal polyps with confocal laser endomicroscopy (CLE) examination and to explore the diagnostic value of the CLE in adenomatous polyps and non-adenomatous polyps. Methods From June to December in 2009, 90 patients were recruited in this study, which included 40 pathologically confirmed colorectal polyps patients (total 48 colon and rectal polyps) and 50 patients for prospective study (total 106 colon rectal polyps). At same time 10 spots of normal mucosa was taken for comparison. Firstly the CLE images of 48 pathologically confirmed colorectal polyps (22 adenomatous polyps and 26 non-adenomatous polyps) were analyzed, and the diagnosis criteria for distinguishing adenomatous polyps and non-adenomatous polyps was established with CLE. Then according to the criteria, 106 colorectal polyps underwent prospective diagnosis with CLE. Finally, the CLE diagnosis result was compared with pathologically result to evaluate the diagnosis efficiency of CLE. Results In the 48 colorectal polyps of 40 pathologically confirmed colorectal polyps patients, there were 22 adenomatous polyps and 26 non-adenomatous polyps. The sensitivity, specificity and accuracy of CLE in adenomatous polyps diagnosis were 94. 0%, 92. 9%, and 93. 4%, respectively. The positive predictive value was 92.2% and the negative predictive value was 94.5%. The sensitivity, specificity and accuracy of CLE in non-adenomatous polyps diagnosis were 92. 9%, 94.0%, and 93. 4%respectively, the positive predictive value was 94.5% and the negative predictive value was 92.2%.The coherence of CLE and histopathology in adenomatous polyps diagnosis was pretty good (Kappa=0. 893). Conclusion The accuracy of CLE in adenomatous polyps and non-adenomatous polyps differential diagnosis was high, and the coherence with histopathology diagnosis was good, which provided experience for further detection of early rectal precancerous.
