1.Clinical significance of urine ferritin in patients with cerebral infarction

Objective To find a indicator in urine to assist diagnosis of cerebral infarction,we investigated the changes of urine ferritin in patients with cerebral infarction.Methods Collected serum from 30 healthy volunteers and 53 patients with cerebral infarction(CI),with ratio of males to females 19∶11 vs.30∶ 23,and average age was (56 ± 11) vs.(63 ± 11) respectively.Collected urine from 39 heathy volunteers and 33 patients with CI,with ratio of males to females 25∶14 vs.20∶ 13,and average age was (57 ± 10) vs.(64 ± 11) respectively.Ferritin concentration in all samples was detected with the chemical luminescence method,and the urine ferritin/urine cretinin (uFer/uCr) ratio was calculated.Differences of serum ferritin and uFer/uCr between healthy volunteers and CI patients were analyzed with the statistical software under Wilcoxon test.Urinary proteins were extracted with absolute ethanol.The extracted ferritin was displayed with Western blot for light chains and heavy chains to investigate the difference between healthy volunteers and CI patients.Results uFer/uCr was higher than that from healthy volunteers (U =292.00,P <0.05),and the median was (14.86 vs.6.22) μg/g.Moreover,after extraction of urinary proteins with alcohol,both light chains and heavy chains of ferritin in urine from CI patients were much more obvious than that from healthy volunteers.Conclusion The uFer/uCr ratio could provide a new proof to assist the diagnosis of cerebral infarction.
2.Preparation and evaluation of recombinant human ferritin light chain for quality control materials

Objective To construct a prokaryotic expression system of human ferritin L which could be overexpressed in E.coli, and then prepare quality control materials of rFL and evaluate the homogeneity, stability, precision and application value of the rFL.Methods RT-PCR was used to clone ferritin L gene with total RNA from peripheral blood.Ferritin L gene was inserted into plasmid pGM-T to construct subclone plasmid pGM-T/ferritin L, which was identified by sequencing.The recombinant plasmid pET-30a/ferritin L was transformed into E.coli BL21 (DE3) and efficiently expressed under IPTG induction.rFL was identified by SDS-PAGE, and its antigenicity was examined by WB.The concentration of rFL was measured by ACCESS immunoassay system.Fifteen control samples were randomly selected using random number table.The homogeneity and stability of rFL were evaluated by one-way analysis of variance (ANOVA) and a linear regression model, respectively.The uncertainties of homogeneity, stability and the precision were evaluated.Results A prokaryotic expression system of ferritin L was successfully constructed.Sequence analysis showed that the ferritin L gene inserted was identical with the one from GenBank(NM_000146.3).The PCR products were 527 bp long, in complete agreement with length of ferritin L gene.The molecular weight of rFL highly expressed in E.coli induced by IPTG was 19 000 Da.The WB analysis indicated that this rFL protein had good antigenicity.The concentration of rFL(1 000 times dilution) measured by ACCESS immunoassay system was (1115.84±38.38) ng/ml.Homogeneity evaluation of low-concentration QC sample and high-concentration QC sample of rFL showed that there were no statistical significances in the within-groups and between-groups (for high-concentration F=2.336, P>0.05 and for low-concentration F=0.730, P>0.05).A linear regression based on the stability test indicated that there was no statistically significant trend of instability in five and a half months (F=1.755, P>0.05). Precision analysis showed the within-run CV, the between-run CV, the between-day CV and the total CV of high-concentration QC sample were 2.06%, 2.12%, 0% and 2.96% respectively; the within-run CV, the between-run CV, the between-day CV and the total CV of low-concentration QC sample 2.03%, 2.08%, 0% and 2.90%, respectively.Conclusion This rFL for quality control materials with independent intellectual property rights could meet the clinical requirement of homogeneity, stability and precision, and could provide raw materials for preparation of mixed ferritin for quality control.
3.Expression of Interleukin-1β in Synovia and Synovium of Rats after High Intensity Exercise

Objective To study the expression of interleukin-1β (IL-1β) in synovia and synovium of rats after high intensity exercise.Methods 24 healthy male Sprague-Dawley rats were randomly divided into control group and high intensity group, and the latter were forced to run 60 min at the speed of 28 m/min for 6 weeks. The histological changes of cartilage from the lesion areas of medial femoral condyle and synovium of knee joint were examined with HE stain and the expression of IL-1β in synovium with immunohistochemistry, in synovial fluid with the enzyme-linked immunosorbent assay (ELISA). Results The articular cartilage of knee were injured and synovitis were found in high intensity group. The expression of IL-1β both in synovial fluid and in synovium of the knee were more in high intensity group than in control group (P<0.05). Conclusion High intensity exercise could cause aticular cartilage injury, synovitis and over expression of IL-1β in synovia and synovium, which may associated with post-traumatic osteoarthritis.
4.Influence of different sample processing methods on yield of endoscopic ultrasound-guided fine needle aspiration

Objective To compare the yield of endoscopic ultrasonography guided fine needle aspira-tion (EUS-FNA) with 3 different sample processing methods. Methods The clinical data of 118 patients, who underwent EUS-FNA performed by one physician from February 2005 to September 2008, were retrospectively analyzed. The FNA sample processing methods included liquid-based cytology, on-site cytology and smear method. The pathological diagnosis was classified as definite, suspicious malignancy, dissatisfying sampling and indefinite. Results The success rate of obtaining samples through on-site cytological procedure was 95.2% (40/42), which was significantly higher than that of conventional smear (32/47, 68%, P <0. 05), and was higher than that of liquid-based cytological method (26/29, 89. 7% ), but without significant differ-ence (P>0.05). The yield of definite diagnosis with liquid-based cytology and on-site cytology were 82.8% (24/29) and 78. 6% (33/42), respectively, which were both significantly higher than that of smear method (57. 4%, 27/47, P <0. 05). The sensitivity and accuracy of on-site cytology were higher than those of smear method and liquid-based cytology, but without significant differences (P >0. 05). Conclusion Compared with conventional smear method, an-site cytology and liquid-based cytology yield more results from EUS-FNA.
5.The diagnosis and treatment value of fibroptic ductoscopy in non-malignant mammary lesion

Objective To evaluate the clinical value of fiberoptic ductoscopy in diagnosis and treatment of patients with galactophoritis or mammary duct ectasia. Methods From November 2005 to March 2008, fiberoptic ductoscopy were performed in 120 women with nipple discharge. The duct of 95 cases as non-malignant lesion were insufflated and perfusioned with entamycin and dexamethasone. Results Ninty-five of 120 cases were non-malignant disease,which contained one side 81 and two sides 14; the discharge was bloody,ivory, stramineous in 21, 17, 57 patinents, respectively; and the dignosis were 17 mammary duct ectasia, 53 galactophoritis, and 25 mammary duct ectasia with galactophoritis. Of the 95 cases, hich were intradutal insufflated and perfusioned with gentamycin and dexamethasone, the nipple discharge were decreased or disappeared in 81 cases, the effective rate was 85.3%. Conclusion Fiberoptic ductoscopy is a convenient,safe, accurate method in diagnosis and treatment of patients with galactophoritis or mammary duct ectasia.
6.Genetic Variation Analyses of nsp2 Gene of PRRSV in Ningxia Hui Autonomous Region of China

To gain a better understanding of the genetic diversity and evolution of PRRSV in the Ningxia Hui Nationality Autonomous Region (Ningxia) of China, the nsp2 genes from a series of PRRSV strains collected from the region in 2007 were partially sequenced. These sequences were then analyzed along with the classical strain (ch-la) and two other epidemic strains SD (3) and SD2006. Comparison of the nucleotide sequence with ch-la indicated that nsp2 genes of seventeen Ningxia isolates (NX strain) have deletions of 87 nucleotides. Sequence analysis indicated that homology between the Ningxia strain and ch-la was 60.3%-79.9% in the nucleotide sequence, and homology between the NX strains and SD strains was 80.3%-98.8% in the nucleotide sequence. The nsp2 genes of the seventeen isolates had 74.9%-100% nucleotide sequence identities with each other. This study was undertaken to assess the regional variation of prevalent PRRSV and to establish a sequence database for PRRSV molecular epidemiological studies.
7.Formulation Screening and Determination of Troxerutin Microemulsion

Objective To optimize a W/O microemulsion formulation of troxerutin and evaluate its physical properties such as morphology, droplet size, stability and content of troxerutin. Methods The W/O microemulsion was optimized using a pseudoternary phase diagram and the area of the microemulsion region was used to screen and determine microemulsion components.HPLC assay was used for determination of the loading content. Results The optimal formulation contained lecithin, ethanol, isopropyl myristate and water (23.30:11.67:52.45:12.59).The microemulsion was physicochemically stable with round shape and uniform size, and the mean droplet size was about 50. 20 nm. Conclusion Microemulsion was developed successfully.It will expect to be the new preparation for troxerutin.
8.Role of extracellular signal-regulated kinase 1/2 in pathogenesis of stress-induced gastric ulcer

Objective:To investigate the role of extracellular signal-regulated kinase 1/2(ERK1/2)signal pathway in the pathogenesis of stress-induced gastric ulcer.Methods:Animal model of stress-induced gastric ulcer was established in rats with water-immersion restraint(WIR)stress.The mucosal activation of ERK1/2 was observed before and 5,15 and 30 min,and 1, 2 and 3.5 h after WIR stress.Some animals were also treated with an intravenous injection of PD98059(1 mg/kg),a specific ERK1/2 inhibitor,1 h prior to WIR stress.Expression of total ERK1/2 and caspase-3 were detected by Western blot analysis; ERK1/2 activity was measured by kinase activity assay using myelin basic protein as a specific substrate.DNA-binding activities of the transcription factors activator protein-1(AP-1)and nuclear factor-?B(NF-?B)were determined by electrophoretic mobility shift assays(EMSA).Mucosal TNF-?and IL-1?mRNA expression was analyzed by Northern blot analysis.The degrees of the gastric mucosal lesions were expressed as ulcer index(UI)and pathological evaluation.Apoptosis in the gastric mucosa was examined by an in situ TdT-mediated dUTP nick-end labeling(TUNEL)method.Results:Activated ERK1/2 was very weakly expressed in the gastric mucosa of normal rats.ERK1/2 was rapidly activated in the gastric mucosa of rats 15 min after WIR stress and the activity reached the maximal after 3.5 h.Pretreatment with PD98059 significantly inhibited ERK1/2 activation,decreased AP-1 and NF-?B activities and TNF-?and IL-1?mRNA expression,and obviously relieved gastric mucosal lesions,accompanied by caspase-3 activation and increased apoptosis.Conclusion:The present results indicate that ERK1/2 activation plays an important role in the development of stress-induced gastric ulcer.
9.Comparison of surgical thrombectomy and interventional thrombectomy for acute deep vein thrombosis of the lower extremity.

OBJECTIVETo study the short- and long-term results for acute deep vein thrombosis (DVT) of the lower extremity treated by surgical thrombectomy and interventional thrombectomy.
METHODSOne hundred and thirty-six acute DVT cases treated by surgical thrombectomy or interventional thrombectomy from March 2000 to August 2008 were reviewed. There were 80 patients treated by surgical thrombectomy. Among them, 30 cases were male, 50 cases were female, aged from 26 to 81 years with a mean of (58 ± 14) years. The other 56 cases were treated by interventional thrombectomy. Among them, 25 cases were male, 31 cases were female, aged from 22 to 92 years with a mean of (57 ± 17) years. All the 136 patients received district anticoagulation with heparin and thrombolysis with urokinase after operation.
RESULTSAfter operation, the circumference difference between bilateral thigh in intervention group were less than that in surgical group [(0.8 ± 1.3) cm vs. (1.5 ± 1.7) cm, P = 0.002]. The circumference difference of bilateral calf had no significant difference [(0.7 ± 1.1) cm vs. (1.0 ± 1.1) cm, P = 0.152]. The average hospital stay in intervention group was shorter than that in surgical group [(7 ± 4) d vs. (15 ± 7) d, P = 0.000]. The morbidity of complications in intervention group was less than that in surgical group (8.9% vs. 32.5%, P = 0.000). One hundred and eight patients were followed up, who was 79.4% of the total patients, and were followed up for an average of (46 ± 29) months. The circumference difference of thigh and calf, the symptom grade, the pigmentation, varicose veins, intermittent claudication and ulceration between the two groups had no significant difference (P > 0.05).
CONCLUSIONCompared with surgical group, intervention group has a better short-term effect, shorter hospital stays, less complications and similar long-term result.

10.HIV prevalence and its correlation among the community-based drug users in Dongguan in 2008.

OBJECTIVETo understand the HIV epidemic and its correlation among the community-based drug users in Dongguan, Guangdong province.
METHODSIn 2008, 13 drug users were identified as seeds in Dongguan. Respondent driven sampling was applied to recruit community-based drug users. A structured questionnaire was used for a face to face interview. Intravenous blood samples were collected for the measurement of HIV, HCV and syphilis antibodies. RDS analysis tool (RDSAT) was applied for adjusting the estimates and bivariate analysis.
RESULTSA total of 303 drug users were recruited within 15 weeks and 300 blood samples were collected. After RDSAT adjustment, the prevalence rates of HIV, HCV and syphilis were 14.7% (95%CI: 6.1% - 25.4%), 63.2% (95%CI: 55.7% - 71.7%), and 4.7% (95%CI: 1.8% - 8.2%) respectively. The HIV prevalence rates among those who were HCV seropositive, migration, younger than 26 years at their first drug use, and ever shared needles or syringes were 22.9% (95%CI: 10.8% - 37.0%), 26.9% (95% CI: 11.4% - 44.9%), 19.9% (95%CI: 8.3% - 35.7%), and 29.1% (95%CI: 12.3% - 45.1%) respectively. The proportions of the drug users who were enrolled at methadone maintenance treatment (MMT) of local residency, migration were 20.9% (95%CI: 4.1% - 44.6%), 2.3% (95%CI: 0.0% - 7.0%) respectively.
CONCLUSIONThe HIV epidemic among the community-based drug users in Dongguan is serious. Drug users who are HCV seropositive, migration, younger than 26 years at their first drug use, and ever shared needles or syringes are more likely to be HIV seropositive.
