Objective To investigate the clinical efficacy of modified YV-plasty for refractory bladder neck contracture (BNC) caused by transurethral resection of prostate (TURP).Methods From June 2013 to March 2016,11 patients with BNCs secondary to TURP were included in this study.Their mean age was 63.7 years (range,56-73 years).All patients presented voiding difficulty and failed after 2 or more prior endoscopic treatments.Modified YV-reconstruction of bladder neck was performed,by incising the anterior wall of bladder neck in a T-shaped manner,and creating two well-vascularized and tension-free flaps,which offer the possibility to reconstruct a wide bladder neck.Results After a mean follow-up of 14.6 months (ranging 3-24 months),successful outcome was achieved in 9 patients without incontinence secondary by surgery.Recurrent voiding difficulty developed in 2 patients,which was cured after a following endoscopic treatment.Conclusion A wider bladder neck can be obtained through modified YV-reconstruction of bladder neck,while avoiding external urethral sphincter injury.It is an available option for refractory bladder neck contracture.

AIM To study the effects of dexamethasone on expressing monocyte chemoattractant protein 1(MCP 1 ) mRNA in the rats with pulmonary fibrosis, elaborate the molecular mechanism of dexamethasone (Dxs) in pulmonary fibrosis therapy. METHODS The model of pulmonary fibrosis was established by instilling bleomycin intratracheally. After treating with Dxs ip , the levels of MCP 1 mRNA were determined by RT PCR. The histological changes were observed and the numbers of inflammatory cells were counted in optical microscopy field. RESULTS The accumulation of inflammatory cells decreased markedly, and the symptom of pulmonary fibrosis was alleviated. Furthermore, Dxs evidently inhibited the expression of MCP 1 mRNA in lung tissues with pulmonary fibrosis. CONCLUSION The molecular mechanism of Dxs in pulmonary fibrosis therapy was associated with inhibiting the expression of MCP 1 mRNA.

Objective: To design the ribozymes to cleave human TIMP 1 mRNA, and embed them into U 6snRNA to make them stable. Methods: Ribozymes were designed according to the “hammerhead structure” described by Symons.Computer was used to analyze the possible cleavage sites. Results: Three ribozymes targeting the nt123, nt299 and nt353 on TIMP 1 mRNA were designed. Embedding ribozyme in U 6snRNA had little effect on its binding with the substrate. Conclusion: Computer assisted design is indispensable in studying ribozyme. Embedding ribozymes in U 6snRNA may be a good way to solve the problems existing in ribozyme study. [

Objective To systematically evaluate the effectiveness of diabetes self-management education through telemedicine on glycosylated hemoglobin (HbA1c). Methods Randomized clinical trials (RCTs) that assessed the effectiveness of remote diabetes self-management education in patients with diabetes were systematically reviewed using multiple electronic databases. The searching time was from database establishment to May 2015. These articles were analyzed after evaluation of their quality. Results Eight RCTs were included involving 2 047 patients.The results of Meta-analysis showed the effectiveness of remote DMSE on reduction of HbA1c of patients with type 2 diabetes was more significant than that of the control group [WMD=-0.50,95%CI (-0.88,-0.12),P < 0.05],while failing to support the effectiveness of remote DMSE on patients with type 1 diabetes [WMD=0.62, 95%CI(-0.19,1.43), P>0.05]. Conclusions Remote DMSE is effective to improve the glycemic control in patients with type 2 diabetes,while the effect on patients with type 1 diabetes is still not certain. Large-scale and multi-centered RCTs are needed to confirm its effectiveness.

BackgroundThe study of the molecular mechanism of visual plasticity is helpful for the explaining and prevention of strabismus and amblyopia.The effect and significance of NogoA in the strabismus and amblyopia formation are attracting more attention.ObjectiveThe present study was to investigate the expression of NogoA in 21a area of visual cortex in strabismic-induced amblyopia cats and explore the possible molecular mechanism of strabismic-induced amblyopia.MethodsSixteen 4-week old clean cats were randomizedly divided into normal group and strabismic-induced amblyopia group and eight for each group.The strabismic-induced amblyopia models were created by cutting off the external rectus in 8 cats.Pattern visual evoked potentials ( P-VEP ) were recorded 1 week after operation and compared with normal cats,and depression of amplitude and prolongation of implied time of P100 wave were as the successful criterion of model.The 200 ml paraformaldehyde was infused via heart to fax the brain under the deep anesthesia and then the cats were sacrificed and the brain cortex sections were prepared.The morphology of 21a zone of cat visual cortex was examined by haematoxylin and eosin staining,and the expressions of NogoA in 21a area of visual cortex were detected by immunohistochemistry with a monoclonal antiNogoA antibody.The use of the animals complied with the Regulations for the Administration of Affair Concerning Experimental Animals by State Science and Technology Commission.ResultsThe implied time and amplitude ratio of VEP P100 wave were (98.10±7.07)ms and (0.83±0.14) in normal group,and those in strabismic-induced amblyopia group were (108.50±6.95 )ms and (0.35 ±0.09 ),showing significant differences between two groups (t=4.450,P=0.005 ; t =5.970,P =0.005 ).The numbers of neurons were similar in 21a area of visual cortex between the two groups,but the volume of the neurons was lessen in strabismic-induced amblyopia group.The positive cell densities for NogoA were ( 387.37±2.01 ) cells/mm2,( 354.58± 1.85 ) cells/mm2 and ( 289.68± 1.81 ) cells/mm2 in layer Ⅱ/Ⅲ,Ⅳ and V/V[ respectively in strabismic-induced amblyopia group,and those in normal group were ( 161.39± 1.98 ) cells/mm2,( 128.93 ± 1.26 ) cells/mm2 and ( 96.25 ± 1.49 ) cells/mm2,indicating a significant increase in model cats ( t =- 160.400,- 201.890,- 164.740,P =0.000 ).Conclusions NogoA may play an important role in the regulation of the sensitive developmental period of visual sense and its plasticity.

The biochemical mechanism of degrading keratins by S.fradiae var S-221 was primarily studied.The compounds (Na_ 2 SO_ 4 , Na_ 2 SO_ 3 and sulfdryl acohol), which respecitively enhance specific activity of keratinase, activate keratinase intensively and mainly act on the disulfide bonds reductase in the keratinase, Na_ 2 SO_ 3 activates intensively both disulfide bonds reductase and polypeptide hydrolytase at 0.01 mol/L, whereas Na_ 2 S_ 2 O_ 3 , which acts on the disulfide bonds reductase, inhibits keratinase.On the condition that substrate, keratins exists, S.fradiae var S-221 is induced to produce exo-keratinase, which is a multiproteinase, containing disulfide bonds reductase, which is a key enzyme degrading keratins, then, with polypeptidic, hydrolytase, graduately hydrolyzates denatured keratins into polypeptides, oligopeptides and free amino acids, so that keratins have been decomposed completely.Sulfur in the keratins was transferred into sulfhydryl compounds, H_ 2 S and sulfates in the course of keratinolysine.

Objective To investigate the prevalence and charateristics of carotid atherosclerosis among healthy middle-aged people of community. Methods Totally 1930 subjects aged 50 years and above were selected randomly from 10027 participants recruited during Phase 3 of the Guangzhou Biobank Cohort Study (GBCS).Common carotid artery intima-media thickness (CCA-IMT) and carotid plaques (CP) were measured by B-mode ultrasonography using ALT HDI 3000 mainframe with a high-resolution. Results The prevalences of CCA-IMT and CP in men were higher than women [41.5％(400/963)vs.16.2 (157/967),34.1％ (328/967) vs.13.8％ (133/967),both P＜0.001].The prevalence of CCA-1MT was higher with age increasing in either men or women in groups of 50-yrs,60-yrs and 70-79 yrs (all P＜0.001).The most of CP was cankerous spot (37.8％),secondly hard spot (24.9％),soft spot (22.1％) and the prevalence of flat spot was lowest (15.2％).The number of CP was not significantly different between the left and right side (51.0％ and 49.0％).The most common location of CP was bifurcations of carotid arteries (61.7 ％),next the common carotid artery (21.0％) and internal carotid artery (ICA) was lowest (17.3 ％).The prevalence of CP was 44.4％ (83/87) in women aged 50- yrs,it was higher than that in men 20.9％ (108/517).However,the prevalence of CP was 79.1％ (409/517) in men aged more than 60 yrs,higher than that in women (55.6％). Conclusions The carotid atherosclerosis is commonly encountered disease in the male elderly,and the most of CP was cankerous spot in bifurcations of carotid arteries
