1.Research of association between the primary progressive type of pterygium and the tear film

To research the association between primary progressive pterygium and tear film.
●METHODS: Totally 60 cases of primary progressive pterygium from September 2012 to June 2013 in our hospital were enrolled. The pterygium eye was for observation group and the contralateral eye as the control group. The differences of eye symptoms, tear film break-up time ( BUT ), Schirmer Ⅰ test ( S Ⅰ t ), corneal fluorescein staining (FL), tear ferning test (TFT) and the conjunctival impression cytology ( ClC) were compared between two groups.
●RESULTS: The eyes in observation group had higher symptoms score, FL score, grades of conjunctival squamous metaplasia, percentage of abnormal tear ferning, but had lower BUT and density of goblet cell, the differences had statistically significance (P<0. 05); Similar S Ⅰ t results were presented in the two groups, the difference had no statistically significance (P>0. 05).
● CONCLUSlON: Primary progressive pterygium can cause a decrease in tear film stability, which in turn lead to some dry eye symptoms such as dry feeling and burning sensation, and its mechanism may be caused by multi-factors, such as density change of goblet cell and the tear fluid dynamics.
2.Study on establishing an indicator system for performance assessment of women and children health care institutions

Objective To establish an indicator system for performance assessment of women and children health care institutions,and provide basis for performance management of such institutions.Methods In this study,literature searching,expert discussion,Delphi method,coefficient of variation method,correlation analysis,principal components analysis,factor analysis and cluster analysis were used to establish the indicator system.In addition,expert scoring and CRITIC methods were called into play to calculate the weight coefficient of each index,and to test the validity and reliability of such an indicator system.Results Thanks to the framework of this indicator system,we set up six class 1 indicators,including heahhcare quality,healthcare efficiency,quality and efficiency of public health service,social efficiency,economic benefit,and development potential.Conclusions Scientific studies can help us build a scientific and practical general performance assessment indicator system applicable to women and children care hospitals at all levels.
3.Clinicopathological characteristics of non-muscle-invasive bladder cancer patients association with chronic kidney disease

Objective To explore the clinicopathological characteristics of non-muscle-invasive bladder cancer patients association with chronic kidney disease (CKD).Methods Between Jan 2009 and Dec 2010,536 patients (390 males and 146 females with mean age of 63 years) underwent surgical treatment at our institute for pathologically proven non-muscle-invasive bladder cancer.The clinical and pathological data of these patients were reviewed,and the relationships of these factors and CKD were analyzed.Presence of CKD was confirmed in patients with estimated glomerular filtration rate (eGFR)< 60ml/(min · 1.73 m2)calculated using the Modification of Diet in Renal Disease Study equation.Results Of the 536 consecutive cases,57 patients (10.6%) had CKD.Compared to the patients without CKD,there were more females and older patients in the patients with CKD (52.6% vs 24.2% and 69 years vs 62 years,both P < 0.05).The patients with CKD proned to have multiple bladder tumor (71.9% vs 50.9%,P < 0.05) and synchronous upper urinary tract urothelial carcinoma (7.0% vs 2.3%,P <0.05).The history of bladder cancer and upper urinary tract urothelial carcinoma were also predominated in these patients (43.9% vs 29.0% and 40.4% vs 6.5%,both P < 0.05).ConclusionConcurrent CKD in non-muscle-invasive bladder cancer patients is associated with greater risk of multiple tumors in urinary tract,particularly in female patients.
4.Anemia prevalence and its influencing factors for the infants aged 6-23 months in poor areas in Gansu Province

Objective To understand the prevalence of anemia and its influencing factors in infants aged 6 to 24 months in poor areas of Gansu Province, and to provide reference for improving the prevalence of anemia in local children. Methods A multi-stage sampling method was used to conduct a survey on infant and caregivers, in the 12 children nutrition improvement project counties in Gansu Province. T-test and analysis of variance were used to compare the hemoglobin content, the chi-square test was used to compare the prevalence of anemia, and the Logistic regression model was used to analyze the influencing factors. Results Among the 3 188 effective data, the detection rate of anemia was 25.69% (819). The detection rate of anemia among boys and girls was 24.54% and 26.90%, respectively. There was no significant difference in the detection rate of anemia among different sexes ( 2=2.326, P=0.127). The detection rate of anemia between different age groups were statistically significant ( 2=42.339, P<0.001); The results of multivariate analysis showed that children's age, children's ethnic groups, parents' awareness of feeding knowledge, the feeding method of 6 months after birth and the way of taking nutritional packs were associated with anemia (all P<0.05). Conclusions The prevalence of anemia in infants aged 6 to 24 months in poor areas in Gansu Province was relatively high. Strengthening the education of caretakers’ knowledge of guardian feeding and scientific child-rearing, and ensuring the nutrition packages intake can significantly reduce the prevalence of anemia prevalence in poor areas in Gansu Province.
5.A new steroidal glycoside from fermented leaves of Agave americana.

OBJECTIVETo investigate the constituents of fermented leaves of Agave americana, and discover new compounds.
METHODCompounds were purified with silica gel and C8 reverse--phase silica gel column chromatography. The structures were elucidated by chemical and spectroscopic evidence.
RESULTThree steroidal compounds were obtained and their structures were identified as (25R)-5 alpha-spirostan-3 beta, 6 alpha, 23 alpha-triol 6-O-beta-D-glucopyranoside(1), (25R)-5 alpha-spirostan-3 beta, 6 alpha, 23 alpha-triol-3, 6-di-O-beta-D-glucopyranoside (cantalasaponin-1) (2) and (25R)-5 alpha-spirostan-3 beta, 6 alpha, 23 alpha-triol(hongguanggenin) (3).
CONCLUSIONCompound 1 is new compound, named agamenoside C.

6.KUI Yu's Experience of "Abdominal Acupuncture,Collateral Bloodletting and Auricular Point Seed-Pressing "in the Treatment of Graves'Ophthalmopathy

Graves'disease,namely toxic diffuse goiter,is an autoimmune disease.Graves'ophthalmopathy is an ocular lesion with hypothalamic-pituitary-thyroid axis dysfunction.Its clinical features are characterized by exophthalmos,eyelid retraction,and delayed upper eyelid.It is often accompanied by eyelid edema,periorbital tissue infiltration,conjunctival erythema,and optic neuropathy.In clinic,Graves'ophthalmopathy has the characteristics of high incidence,rapid progression,high recurrence rate and single treatment.Therefore,Graves'ophthalmopathy has become one of the common refractory diseases in clinic.Professor KUI Yu believes that Graves'ophthalmopathy belongs to the category of'exophthalmos'in traditional Chinese medicine.He believes that the disease is located in the eye and is closely related to the liver,spleen and kidney.Qi stagnation,blood stasis and phlegm coagulation are important pathological factors in the process of disease occurrence and development.For the treatment of Graves'ophthalmopathy,Professor KUI Yu attaches great importance to the combination of whole and local thinking.Through the flexible use of a variety of external treatment methods of traditional Chinese medicine,the treatment path of Graves'ophthalmopathy is summarized as"abdominal acupuncture,collateral bloodletting and auricular point seed-pressing".The comprehensive treatment of abdominal acupuncture,collateral bloodletting and auricular point seed-pressing and other external treatment methods with traditional Chinese medicine characteristics has definite clinical efficacy and is worthy of summary and promotion.
7.A Clinical Analysis of 29 Patients with Acinic Cell Carcinoma of Parotid

Objective:This study was designed to summarize the clinical characteristics, biological behavior, and treatment results of acinic cell carcinoma of parotid. Method: Twenty nine cases of acinic cell carcinoma of parotid were collected to make clinical analysis. Results: The clinical feature of acinic cell carcinoma of parotid were observed as following: the incidence of male and female was equal and happen in any age group. The tumor usual located at superficial lobe of parotid, and growth slowly. The course was long with a mean of 5 years. The incidence of cervical lymph node metastasis was 17.2% and distant metastasis was much less, the local recurrence rate was 20.7% and the prognosis was relatively better. The survival rates of 3 year,5 year,10 year were 82.7% ,80% , and 76.9% ,respectively. Conclusion: The incidence of acinic cell carcinoma of parotid is low, and acinic cell carcinoma of parotid could be considered as lower malignant tumor. Radical surgical ressection is the most common modality for its treatment, while larger lesion or recurrent cases may receive postoperative radiotherapy so as to decrease local recurrence.

Objective To evaluate the value of ~(125)I permanent plantation in treating osseous metastases.Methods Twenty-two patients with osseous metastases were accepted radioactive seeds ~(125)I permanent plantation.The curative effect was appraised according to the degree of ostalgia relieving and the changing of the radiology imaging in patients.Results Accepted radioactive seeds ~(125)I permanent plantation,relief of pain was obtained and the effective rate is 91%(20/22).However none of the patients showed severe side-effect.Among 32 lesions in 22 cases followed-up by CT in 2 months,4 obtained CR, 18 obtained PR,10 NC and 0 PD.The responsive rate was 68.7%.Conclusion ~(125)I permanent plantation procedure can be a safe and effective method in treating osseous metastases and obtaining good clinical effects with minimal damage and few comnlications.
9.Molecular epidemiology of HFMD-associated pathogen coxsackievirus A6 in Fujian Province, 2011-2013.

In order to characterize the molecular epidemiology of HFMD-associated Coxsackievirus A6 (CVA6) in Fujian Province, a total of 1340 specimens from non-EV71 non-CVA16 HFMD patients were collected during 2011-2013. Isolated virus strains were identified and subtyped. Full-length coding regions for the VP1 gene of the predominant serotype CVA6 isolates were amplified and sequenced. Among the 375 non-EV71 non-CVA16 HFMD cases confirmed by virus isolation and molecular subtyping, 182 (48.5%) were found to be caused by CVA6, accounting for 7.9%, 16.2% and 39.6% HFMD-associated enteroviruses in FujianProvince during 2011, 2012, and 2013, respectively. Compared with general features observed in the HFMD epidemic, no difference in CVA6-specificity or severity rates was observed between geographical origins, gender, or age groups. Nucleotide sequence analyses of VP1 genes revealed high diversity levels of 16.2%-18.6% among CVA6 strains from Fujian Province, in contrast to the prototype CVA6 strain, and showed low levels of diversity in the amino acid sequences (4.3%-6.2%). Phylogenetic analysis also indicated that CVA6 isolates from Fujian Province were distinct from the prototype strain and other isolates from abroad; however, it was homologous to domestic strains, although the Fujian isolates clustered into multiple branches. These results suggested that significant changes in the pathogenic spectrum of HFMD in Fujian Province occurred during 2011-2013, as CVA6 was one of the predominant serotypes of HFMD. CVA6 isolates from Fujian Province were co-circulating and co-evolving with other domestic strains as multiple closely related CVA6 transmission chains were observed in Fujian Province overall and within each prefecture.

10.Evaluation of interference of thyroglobulin antibodies on the measurement of thyroglobulin by two chemiluminescence immunoassay

Objective To investigate the interference of thyroglobulin antibodies ( TgAb ) on the measurement of thyroglobulin ( Tg) by 2 chemiluminescence immunoassays ( CLIAs) .Methods Data of 199 315 individuals with determined TgAb and Tg , including physical checkup subjects , differentiated thyroid carcinoma ( DTC) patients and patients with other diseases , were retrospectively collected in Peking Union Medical College Hospital from November 2012 to April 2015.The correlation between serum Tg level and serum TgAb concentration was analyzed and the positive rate of TgAb in physical checkup subjects was calculated.Furthermore, 290 serum samples with different TgAb concentration were applied in the recovery test by adding in confirmed serum Tg .The correlation between the recovery of confirmed serum Tg and TgAb concentrations was evaluated using Pearson correlation analysis .Results The serum Tg was all decreased with the elevated TgAb concentration in each group of subjects .The positive rate of TgAb was 10.84%(8 416/77 634) in physical checkup subjects .It was higher in females than in males and was increased with age.Recovery test showed that the average recoveries of confirmed serum Tg in TgAb-negative serum were 107.28%(86.30%-117.60%) and 107.94% (85.60%-124.10%) respectively in Roche and Beckman systems.But in TgAb-positive serum samples , the average recoveries in Roche and Beckman systems were 88.59% (35.85% -141.53%) and 95.77% (36.48% -131.78%) respectively, and 12.63%(24/190) and 13.68%(26/190) samples displayed a recovery less than 80%.The recovery rate of confirmed serum Tg showed a significantly negative correlation with elevated TgAb concentration , with r=-0.239 (P=0.001) in Roche and r=-0.251 (P<0.001) in Beckman.Conclusions TgAb-positive serum, especially with high concentration of TgAb , significantly interfered the measurement of Tg .Thus, serum TgAb should be determined together with serum Tg to explore whether there was an interference .To avoid misdiagnosis and inappropriate therapy , clinician should be informed once serum TgAb displayed positive.
