1.Comparison of safety and generalizability between day surgery and traditional surgey for cataract

Objective To explore if the day surgery for cataract is safe and should be popularized. Methods From September to October in 2015, among the patients who received the ultrasonic phacoemulsification and intraocular lens implantation in the cataract department of Tianjin Medical University Eye Hospital, according to the admission time, one hundred and three patients were chosen respectively from the day ward(the experimental group)and the traditional ward(the control group)based on the purposive sampling. The average time of waiting for admission, the falls events before admission, the awareness about postoperative considerations, the postoperative complications and the infection rate between the two groups were compared. Results The average time of waiting for admission in the experimental group was much shorter than that in the control group, (6.36 ± 4.02) days vs. (21.35 ± 18.58) days. The difference was significant (Z=-8.386, P<0.01). The rate of falls before admission in the experimental group was lower than that in the control group and the difference was significant, 9.71%(10/103) vs. 20.39%(21/103),χ2=4.595, P<0.05. No infection happened in both groups. Conclusions The day surgery model doesn′t increase the risk and is as safe as the traditional surgery model. It should be extended as it also has the advantages of shorter hospital stay, less medical resource consumption and less hospitalization expenses, besides, it saves time and labor. What′s more, the mean waiting time for hospitalization for day surgery is shorter. It means the patients can receive the operation as soon as possible, which ensures the patients′safety, and reduce the unsafe factor in their life, bring the benefit to the society.
2.Effects of the grub extract on apoptosis of MCF-7 human breast cancer cell line

AIM:To investigate the apoptotic pathway of MCF-7 breast cancer induced by the grub extract in vitro.METHODS:MTT assay was used to determine the effect of the grub extract on proliferation of MCF-7 human breast cancer cell line and cell toxicity.Morphological changes of the apoptosis in cancer cells were observed by HE staining through invert microscope,light microscope,AO/EB double fluorescent staining under fluorescent microscope.FCM was used to assay the change of apoptotic rate.The expression of Bcl-2,Fas,caspase-9,caspase-3 in apoptotic pathway was detected with immunocytochemical method before and after exposure to the grub extract,and the effect of that on apoptotic pathway was explored.RESULTS:(1)The MTT test showed that the growth of MCF-7 human breast cancer cell line was significantly inhibited by the grub extract in dose and time dependent manners.The inhibitory rate in exposure group was significantly different from that in control group(P

5.Study on the connective tissue around extraocular muscle in cat

Background The extraocular muscles (EOMs) Pulley is considered as the functional origins of the recti EOMs,it is determinants of ocular motility.Objective The structure of the connective tissue around EOMs in cat was studied,and the role of EOMs Pulley to ocular movement was investigated.Methods Five adult cats were involved.The gross anatomy of an orbit in each cat was observed.The other orbit was processed with paraffin imbedding and coronal serial sections.A murine monocolonal antibody to α-smooth muscle actin (α-SMA) was used to show smooth muscle,while Masson trichrome stain was used to show muscle and collagen,and Weigert stain to show elastin.Results An encircling ring of collagen circled EOM was thin and connected to the orbital layer of muscle fiber loosely.Less elastin fibers and little smooth muscle cells were embedded in the collagen ring and connective band.Collagen ring around medial rectus and the connective band between medial rectus and inferior rectus was not more significantly developed than other bands.Conclusions The connective tissue around EOMs in cat may be related to its function of ocular movement,which is not developed.
6.Changes of the alpha competitive structure after administration of single dose methylphenidate in different subtypes of attention deficit hyperactivity disorder boys

Objective:To investigate methylphenidate response to different subtypes of attention deficit hyperactivity disorder(ADHD) boys by measuring the changes of the alpha competitive structure after administration of single dose of methylphenidate,explore the neuropathological mechanism and search for the sensitive index to predict methylphenidate response in this disorder.Methods:The study involved 88 ADHD boys(40 ADHD-C boys,48 ADHD-I boys) who met the DSM-Ⅳ diagnostic criteria of ADHD,and their continuous Electroencephalogram(EEG) data collected before and 2 hours after taking methylphenidate(10 mg) were analyzed with EEG-encephaloflutuographic technology(EEG-ET).Results:After taking methylphenidate,(1) The main frequency of alpha band obviously increased in ADHD-C boys and ADHD-I boys(from 9.05?0.96 Hz up to 9.72?0.99 Hz,P=0.000;from 8.90?0.93 Hz up to 9.25?0.86 Hz,P=0.002;separately),especially in ADHD-C boy group(Z=-2.111,P=0.035).(2) The dominant probability of 8 Hz was significantly decreased in ADHD-C boys and ADHD-I boys(from 24.34%?12.70% down to 20.74%?12.46%,P=0.002;from 28.82%?12.51% down to 25.64%?12.176%,P=0.003;separately).There was no significant difference in the changes of the dominant probability of 8 Hz between the two ADHD groups(Z=-0.494,P= 0.621).(3)The entropy value was significantly decreased in ADHD-I boys group(from 0.74?0.10 down to 0.70?0.13,t =3.579,P=0.001).There was no significant difference in the changes of the entropy value between the two ADHD groups(Z=-1.131 P= 0.258).Conclusion:After administra-tion of single dose of methylphenidate,slow ? wave decreased in ADHD boys,especially in ADHD-Cboy group;Single dose of methylphenidate can ameliorate the brain self-organization of ADHD-I boys.These findings suggest that the main frequency of alpha band may be the sensitive index to predict methylphenidate response in ADHD.

Objective:To explore the alpha competitive structure in children with attention deficit hyperactivity disorder(ADHD)with/without learning disabilities(LD).Methods:According to DSM-Ⅳ diagnostic criteria,the study involved ADHD children with LD,pure ADHD children and normal controls.Each group consisted of 68 subjects.All subjects were between the ages of 7 and 14 years,and the groups were matched by sex,age and ADHD subtypes.EEG data were recorded during an eye-closed resting period and then were analyzed with EEG-encephaloflutuographic technology(EEG-ET).Results:(1)The pure ADHD children showed significantly more 8 Hz activity(25.84%?14.81%)than that of the normal control group(16.50%?11.42%,P=0.000);The main frequency of alpha band was 10 Hz in the pure ADHD children,while the energy distribution among alpha components was diffuse.(2)ADHD children with LD showed significantly more 8 Hz and 13 Hz activity(25.11%?11.88%,1.14%?1.14%,separately)than that of the normal control(16.50%?11.42%,0.74%?0.97%,P=0.000,P=0.009,separately);The dominant probability of 10 Hz(27.80%?13.28%)in this group was significantly lower than that of the control group(36.06%?17.21%,P=0.011);The energy distribution among alpha components was diffuse in ADHD children with LD,whose main frequency of alpha band was 9 Hz;The entropy value of the ADHD children with LD was significantly higher than that of the control group in the right brain and the left parietal region,temporal region,occipital region(P
9.Predictive value of serum thyroid hormone in the short-term prognosis of patients with septic shock

Objective:To investigate the predictive value of serum thyroid hormone in the short-term prognosis of patients with septic shock..Methods:The 133 patients with septic shock admitted to Intensive Care Unit (ICU) of Nanjing Hospital of Nanjing Medical University were enrolled. Data of patient’s general clinical information, acute physiology and chronic health evaluationⅡ (APACHE Ⅱ), serum triiodothyronine ( T3 ), thyroxine (T4), free triiodothyronine (FT3) , free thyroxine (FT4) and thyroid stimulating hormone(TSH) in 24 h were collected, and the ratio of T3 to FT3 (T3/FT3) was calculated. The patients were divided into the survival group and death group according to the prognosis during ICU hospitalization. Univariate and multivariate analyses were used to analyze the prognostic factors of patients with septic shock. The receiver operating characteristic (ROC) curve and Kaplan-Meier analysis were plotted to evaluate the diagnostic efficacy of serum thyroid hormone for the short-term prognosis of patients with septic shock.Results:Compared to the survival group, FT3, T3 and T3/FT3 were significantly lower in the death group [1.73 (1.54, 2.52) vs. 1.32 (0.94, 1.54) pmol/L, 0.70 (0.56, 0.79) vs. 0.33 (0.25, 0.43) nmol/L, 318.18(299.44, 448.05) vs. 250.00 (192.31, 313.92), respectively; all P < 0.05 ]. The levels of serum T3 and FT3 were significantly correlated with the APACHEⅡ score in patients with septic shock (FT3: r = -0.25, P= 0.004; T3: r = - 0.24, P= 0.006). Binary Logistic regression analysis showed that FT3, T3 and T3/FT3 were independent risk factors of the short-term prognosis of patients with septic shock [FT3: OR = 6.533, 95% CI: 0.687 - 62.157, P = 0.012; T3: OR = 0.529, 95% CI: 0.372 - 0.975, P= 0.000; T3/FT3: OR = 1.719, 95% CI: 1.007 - 1.931, P= 0.002]. ROC curve analysis showed that FT3, T3 and T3/FT3 all had certain predictive value for the short-term prognosis of patients with septic shock, and the diagnostic value of T3 was the largest [AUC = 0.874, 95% CI: 0.794-0.954, P= 0.000]. The Kaplan-Meier curve showed a significantly higher survival rate in patients with T3 greater than 0.535 nmol/L than patients with T3 less than 0.535 nmol/L. Conclusions:The serum levels of thyroid hormone T3 and FT3 are closely related to the severity of septic shock. T3 is an effective ICU mortality during hospitalization prognostic indicator for patients with septic shock, which is better than FT3 and T3/FT3.
10.Predictive value of glycemic variability within 6 hours on the short-term prognosis of patients with septic shock

Objective:To investigate the predictive value of different glycemic variability indexes within 6 hours on the short-term prognosis of septic shock patients.Methods:A retrospective study was conducted. The 133 patients with septic shock admitted to intensive care unit (ICU) of Nanjing Hospital Affiliated to Nanjing Medical University from December 2014 to December 2019 were enrolled. Patients with septic shock admitted to ICU died during hospitalization were enrolled in the death group and others in the survival group. General data of the patients including gender, age, underlying disease, site of infection, duration of mechanical ventilation, length of ICU stay, whether to use continuous renal replacement therapy (CRRT) and acute physiology and chronic health evaluationⅡ (APACHEⅡ) scores within 24 hours were collected. The blood glucose (GLUadm), mean arterial pressure (MAP), serum creatinine (SCr) and procalcitonin (PCT) were recorded at ICU admission. The patients admitted to ICU received bundle therapy within 6 hours and blood glucose was observed every 2 hours. The blood glucose difference (GLUdif), average blood glucose (GLUave), blood glucose standard deviation (GLUsd) and blood glucose variation coefficient (GLUcv) within 6 hours were calculated. Multivariate Logistic regression analysis was used to analyze the prognostic factors of short-term prognosis of patients with septic shock, and receiver operating characteristic (ROC) curve was plotted to evaluate the diagnostic efficacy of glycemic parameters for short-term prognosis of septic shock patients.Results:A total of 133 patients with septic shock were admitted to ICU, among them 87 patients survived and 46 patients died during the ICU hospitalization. Compared with the survival group, the SCr at ICU admission and APACHEⅡ score within 24 hours were significantly higher in the death group [SCr (μmol/L): 208.5 (143.0, 286.5) vs. 172.0 (91.0, 234.0), APACHEⅡ score: 30.28±6.67 vs. 24.03±5.90, both P < 0.05], the length of ICU stay was shorter [days: 4.00 (2.00, 10.25) vs. 9.00 (4.00, 13.00), P < 0.01]. However, there were no significant differences in the baseline data of gender, age, underlying disease, infection site, CRRT ratio, MAP or PCT at ICU admission between the two groups. Compared with the survival group, the GLUsd and GLUcv within 6 hours in the death group were higher [GLUsd (mmol/L): 2.33 (1.95, 3.14) vs. 2.02 (1.66, 2.52), GLUcv: (31.00±7.06)% vs. (23.31±10.51)%, both P < 0.05]. There were no statistically significant differences in the levels of GLUadm, GLUdif or GLUave within 6 hours between the two groups. Multivariate Logistic regression analysis showed that APACHEⅡ score within 24 hours and GLUsd and GLUcv within 6 hours were independent risk factors of the short-term prognosis of septic shock patients [APACHEⅡ score: odds ratio ( OR) = 1.173, 95% confidence interval (95% CI) was 1.095-1.256, P = 0.000; GLUsd: OR = 1.465, 95% CI was 1.038-2.067, P = 0.030; GLUcv: OR = 1.089, 95% CI was 1.043-1.138, P = 0.000]. ROC curve analysis showed that GLUsd and GLUcv within 6 hours both had certain predictive value for the short-term prognosis of septic shock patients, the area under ROC curve (AUC) of GLUcv within 6 hours was higher than that of APACHEⅡ score (0.765 vs. 0.753), and AUC of GLUsd within 6 hours was close to APACHEⅡ score (0.629 vs. 0.753); and the diagnostic value of GLUsd combined with GLUcv within 6 hours was higher than the two respectively (AUC: 0.809 vs. 0.629, 0.765), the sensitivity was 97.8%, and the specificity was 66.7%. Conclusion:GLUsd combined with GLUcv within 6 hours can be used to estimate the short-term prognosis of septic shock patients.
