2.Effects of qingchang huashi recipe on the dendritic cells of the colonic mucosa and the mesenteric lymph nodes in experimental colitis rats.

OBJECTIVETo observe the effects of qingchang huashi recipe (QHR) on the dendritic cells (DCs) of experimental colitis rats, thus exploring its possible mechanisms for treating ulcerative colitis (UC).
METHODSThe UC rat model was induced by TNBS/anhydrous alcohol. Forty male Wistar rats were randomly divided into 4 groups, i.e., the normal group, the model group, the QHR group, and the Mesalazine group, 10 in each group. Since the 2nd day of modeling, corresponding medication was respectively administered to each treatment group by gastrogavage for 10 successive days. The number of DCs in the colonic mucosa was observed using iMmunohistochemical assay. The DCs ratio in the mesenteric lymph nodes, and the expressions of surface molecules MHC-II and CD86 were detected using flow cytometry.
RESULTSCompared with the model group, the number of DCs in the colonic mucosa significantly decreased, the expression of MHC-II in the mesenteric lymph nodes significantly decreased in the QHR group and the Mesalazine group, showing statistical difference (P < 0.01). There was no statistical difference between the two groups (P > 0.05). There was no statistical difference in the DCs ratios and the CD86 expression among the 4 groups (P > 0.05).
CONCLUSIONQHR could decrease the infiltration of DCs in the colonic mucosa, and suppress the activation of DCs in the mesenteric lymph nodes, which might be one of its mechanisms for treating UC.

3.Bone mineral density changes in coal workers' pneumoconiosis.

OBJECTIVETo observe the bone mineral density changes of coal workers' pneumoconiosis.
METHODSWe chose 150 cases of One-Triple coal workers pneumoconiosis in Jincheng Coal Mining Group, all of workers were male, of 55-years old-80 years old, an average of 67 years old. 10 years of age to grouping, Whole body bone mineral density and T value were measured by body dual-energy X-ray absorptiometry. We analyzed the BMD changes bone loss, osteoporosis occurrence.
RESULTSThe BMD of six parts were not declined obviously in One stage of coal workers' pneumoconiosis; the BMD of Chest bone, pelvis and spine were declined obviously in two stage of coal workers' pneumoconiosis; the BMD of six parts were declined obviously in Triple stage of coal workers' pneumoconiosis; The occurrence rate of bone loss was significantly higher in Two and Triple coal workers pneumoconiosis. The occurrence rate of osteoporosis was significantly higher in Triple coal workers pneumoconiosis.
CONCLUSIONWith the increase in the severity of coal workers' pneumoconiosis, the BMD of six parts were declined, The occurrence rate of bone loss osteoporosis was significantly higher.

4.Endoscopic transoropharyngeal approach to the ventral region of craniocervical junction.

OBJECTIVETo discuss the surgical methods and clinical effect of endoscopic transoropharyngeal approach to deal with the lesions of the ventral region of the craniocervical junction.
METHODSFrom August 2002 to November 2006, endoscopic transoropharyngeal approach was taken to deal with the lesions of the ventral region of the craniocervical junction, including 6 patients with chordoma, 5 patients with craniovertebral malformation, 3 patients with skull-base invagination combined with Chiari malformation, 2 patients with odontoid fracture . 0 degrees endoscope was used to deal with the lesions of middle and lower clivus, 30 degrees endoscope was used to deal with the lesions of craniovertebral malformation.
RESULTSFollow-up ranged from 2 to 48 months. Three patients with chordoma recurred. All patients with craniovertebral malformation recovered.
CONCLUSIONSEndoscopic transoropharyngeal approach is a new method to deal with the lesions of the ventral region of the craniocervical junction, and it has many advantages, such as direct approach, good exploration, micro-invasive, wide operation space, and good results.

We found that the rate of Pelvic BMD decline of coal workers' pneumoconiosis patients in Two stage was significantly faster after 65 years of age. In Different age groups of coal workers' pneumoconiosis patients in Two and Triple stages, incidence of bone loss and osteoporosis were significantly increased.
CONCLUSIONWith the age increasing, Coal workers' pneumoconiosis in Two and Triple stages significantly accelerated the speed of the rate of BMD decline. This phenomenon was most obvious in the chest bone, pelvis, and spine.

6.Effect of qingchang huash recipe on NF-kappaB/Tolls pathway in ulcerative colitis patients and mechanism study.

OBJECTIVETo observe the effect of Qingchang Huashi Recipe (QHR) on the activation and expressions of nuclear factor kappaB (NF-kappaB), Toll-like receptors (TLRs), and contents of interleukin-8 (IL-8), thus exploring its possible mechanisms for treating ulcerative colitis (UC).
METHODSThe HT-29 cells were induced to inflammation model by tumor necrosis factor-alpha (TNF-alpha) and lipopolysaccharides (LPS). HT-29 cells were divided into 6 groups, i.e., the vehicle control group, the model control group, the sulfasalazine (SASP) group, the high dose QHR group, the middle dose QHR group, the low dose QHR group. Effects on the cell growth were detected by MTT. The chemoattractant of macrophages was observed using Transwell. The expressions of NF-kappaB and TLR4 protein were detected using immune cell fluorescence method. The content of IL-8 was detected by ELISA.
RESULTSThe growth of cells were not inhibited in each group. Statistical difference existed in each dose QHR group in inhibiting the chemoattractant of macrophages, reducing activation of NF-kappaB, lowing expressions of TLR4 protein, and decreasing the secretion of IL-8, when compared with the model control group (P < 0.05). No statistical difference existed in inhibiting the chemoattractant of macrophages between the high dose QHR group and the vehicle control group (P > 0.05). But its inhibition on NF-kappaB activation was higher in the high dose QHR group than in the SASP group (P < 0.05).
CONCLUSIONQHR could obviously attenuate the inflammatory reaction of HT-29 cells, inhibit the chemoattractant of macrophages, reduce the activation of NF-kappaB, lower expressions of TLR-4, and attenuate the secretion of IL-8, which might be one of its mechanisms for treating UC.

7.Management of elderly patients with symptomatic vertebrobasilar insufficiency.

OBJECTIVETo evaluate the feasibility, safety and short-term efficacy of stent-assisted angioplasty and/or drug therapy for elderly patients with symptomatic vertebrobasilar insufficiency.
METHODSElderly patients (> or = 60 years old) with symptomatic vertebrobasilar stenosis (> or = 50%) demonstrated by cerebral angiography were treated with drug therapy and some with endovascular stenting further from April 2001 to June 2006. The clinical, imaging, intervention and follow-up data were collected and analyzed.
RESULTSEighty-one elderly patients were chosen for study, including 68 males and 13 females. The mean age is 70 years (60 - 87 years); stroke rate of 4.9% (4/81) and stroke-related mortality rate of 2.5% (2/81) were found in this group during hospitalization and follow-up (mean 28.1 months), and symptoms resolved or improved clinically in 66 (81.5%). Fifty-two balloon expandable stents were placed in 48 (59.3%) patients of this group with a technical success rate of 98.1% and the mean degree of stenosis was reduced from (82.4 +/- 13.1)% to (6.4 +/- 3.2)% (t = 22.4, P = 0.00).
CONCLUSIONSAppropriate management including endovascular stenting and/or drug therapy may improve short-term outcomes of elderly patients with symptomatic vertebrobasilar insufficiency; meanwhile, stent-assisted angioplasty is technically feasible and relatively safe.

8.Fancd2os Reduces Testosterone Production by Inhibiting Steroidogenic Enzymes and Promoting Cellular Apoptosis in Murine Testicular Leydig Cells

Background:
It is well-established that serum testosterone in men decreases with age, yet the underlying mechanism of this change remains elusive.
Methods:
The expression patterns of Fancd2 opposite-strand (Fancd2os) in BALB/c male mice and testicular tissue derived cell lines (GC-1, GC-2, TM3, and TM4) were assessed using real-time polymerase chain reaction (RT-PCR), Western blot and immunofluorescence. The Fancd2os-overexpressing or knockdown TM3 cells were constructed by infecting them with lentivirus particles and were used to evaluated the function of Fancd2os. The testosterone production was measured using enzyme linked immunosorbent assay (ELISA) and the steroidogenic enzymes such as steroidogenic acute regulatory protein (StAR), P450 cholesterol side-chain cleavage (P450scc), and 3β-hydroxysteroid dehydrogenase (3β-HSD) were analysed using RT-PCR. The apoptosis of TM3 cells induced by ultraviolet light or testicular tissues was detected using flow cytometry, Western blot or dUTP-biotin nick end labeling (TUNEL) assays. Pearson correlation analysis was used to assess the correlation between the Fancd2os expression and TUNEL-positive staining in mouse testicular Leydig cells.
Results:
The Fancd2os protein was predominantly expressed in mouse testicular Leydig cells and its expression increased with age. Fancd2os overexpression inhibited testosterone levels in TM3 Leydig cells, whereas knockdown of Fancd2os elevated testosterone production. Fancd2os overexpression downregulated the levels of StAR, P450scc and 3β-HSD, while Fancd2os knockdown reversed this effect. Fancd2os overexpression promoted ultraviolet light-induced apoptosis of TM3 cells. In contrast, Fancd2os knockdown restrained apoptosis in TM3 cells. In vivo assays revealed that higher Fancd2os levels and mouse age were associated with increased apoptosis in Leydig cells and decreased serum testosterone levels. Pearson correlation analysis exhibited a strong positive correlation between the expression of Fancd2os and TUNEL-positive staining in mouse testicular Leydig cells.
Conclusion
Our findings suggest that Fancd2os regulates testosterone synthesis via both steroidogenic enzymes and the apoptotic pathway.
9.Different dissecting orders of the pulmonary bronchus and vessels during right upper lobectomy are associated with surgical feasibility and postoperative recovery for lung cancer patients

Background: Right upper lobectomy (RUL) for lung cancer with different dissecting orders involves the most vari-able anatomical structures, but no studies have analyzed its effects on postoperative recovery. This study compared the conventional surgical approach, VAB (dissecting pulmonary vessels first, followed by the bronchus), and the alter-native surgical approach, aBVA (dissecting the posterior ascending arterial branch first, followed by the bronchus and vessels) on improving surgical feasibility and postoperative recovery for lung cancer patients. Methods: According to the surgical approach, consecutive lung cancer patients undergoing RUL were grouped into aBVA and VAB cohorts. Their clinical, pathologic, and perioperative characteristics were collected to compare periop-erative outcomes. Results: Three hundred one patients were selected (109 in the aBVA cohort and 192 in the VAB cohort). The mean operation time was shorter in the aBVA cohort than in the VAB cohort (164 vs. 221 min, P < 0.001), and less blood loss occurred in the aBVA cohort (92 vs. 141 mL, P < 0.001). The rate of conversion to thoracotomy was lower in the aBVA cohort than in the VAB cohort (0% vs. 11.5%, P < 0.001). The mean duration of postoperative chest drainage was shorter in the aBVA cohort than in the VAB cohort (3.6 vs. 4.5 days, P = 0.001). The rates of postoperative complica-tions were comparable (P = 0.629). The median overall survival was not arrived in both cohorts (P > 0.05). The median disease-free survival was comparable for all patients in the two cohorts (not arrived vs. 41.97 months) and for patients with disease recurrences (13.25 vs. 9.44 months) (both P > 0.05). The recurrence models in two cohorts were also comparable for patients with local recurrences (6.4% vs. 7.8%), distant metastases (10.1% vs. 8.3%), and both (1.8% vs. 1.6%) (all P > 0.05). Conclusions: Dissecting the right upper bronchus before turning over the lobe repeatedly and dissecting veins via the aBVA approach during RUL would promote surgical feasibility and achieve comparable postoperative recovery for lung cancer patients.
10.Investigation of willingness in individualized medication of voriconazole in fungal infectious patients

Objective To investigate the perception and willingness of patients on individualized medication of voriconazole,to discuss the factors influencing patients' decision-making,and to support the development of the practice guideline for individualized medication of voriconazole.Methods A cross-sectional study involving various types of patients in 9 hospitals was conducted by questionnaire to evaluate the perception and willingness on therapeutic drug monitoring (TDM) of voriconazole and CYP2C19 genotype testing.Results A total of 119 patients were involved,including 24 (20.17%) children.Ninety five (79.83 %) patients thought voriconazole TDM was necessary during the treatment of fungal infections.There was a significant difference on preference towards voriconazole TDM between patients who had received voriconazole TDM,before and patients who had never taken voriconazole TDM (100% vs 75.76%,P < 0.05).Fifty four patients (45.38%) agreed on CYP2C19 gene test.For this question,there was a significant difference among patients with different types of medicare (P < 0.05).The ranking of factors that contributed to the 2 decision-making was consistent (P <0.05).The clinical efficacy was the most important factor (mean rank 3.46 and 3.87,respectively),while cost (mean rank 1.89) and potential benefit (mean rank 2) was the least important factor for the 2 decision-making,respectively.Conclusion There was a significant variance among different types of patients on preferences towards voriconazole TDM and CYP2C19 genotype testing,which should be considered for clinicians to make clinical decisions.The perception and willingness of patients should also be considered during the development of clinical practice guidelines and clinical practice.
