1.Urachal Adenocarcinoma -Report of Two Cases-.

Urachal carcinoma is an uncommon neoplasm associated with a poor prognosis. We have encountered two another cases of urachal adenocarcinoma recently 5 years after our first report. The common sign of urachal cancer was painless hematuria. We diagnosed the specific site of origin preoperatively by means of computerized tomography. One patient was performed partial cystectomy and another was total cystectomy with double barrel ureterocutaneostomy. A brief review of the literature of urachal adenocarcinoma was also made.

2.Milk-of-Calcium in a Pyelocaliceal Diverticulum; A Case Report.

The term "Milk of calcium" has been borrowed from a somewhat similar condition encountered in the gallbladder. The coexistence of stone in a caliceal diverticulum is well known but milk-of-calcium is unusual in this structure. Recently we experienced a case of milk-of-calcium in a pyelocaliceal diverticulum in a 44 year old man, so we report this case with a brief review of the literature.

4.Open and Closed Mouth Impression Techniques for Mandibular Implant Overdenture: Two Cases Report

Mandibular implant overdenture is a good treatment option for complete edentulous patients with restoring removable prosthesis. Mandibular implant overdenture with two implants and locator attachments is widely used. It is tissue-supported overdenture that is made with the concept of conventional complete denture fabrication. There are two patients who provided impressions by open mouth technique and closed mouth technique in each case. In both cases, mandibular implant overdentures were restored with functionally and aesthetically satisfying results.

6.Bilateral Visual Field Defect Following Laser in Situ Keratomileusis(LASIK).

We report a 36-year-old male with the bilateral visual field defect following LASIK was observed. Preoperatively, patient had high myopia(right. -7.25 0.75x18, left. -7.5-0.5x12), normal intraocular pressure(IOP)(right/left: 16/14 mmHg), normal visual field(Humphrey 30-2 visual field)and revealed the tilted discs with symetric optic nerve head cupping(cup-to-disc ratio, 0.70), parapapillary atrophy. Bilateral LASIK keratorefractive surgery was on the same day and intraoperative complication was not noted. After the first operative visit day, examination revealed uncorreted visual acuity right 0.8 and left 0.6 but scotoma was reported. Three months later, Humphrey 30-2 visual field revealed a near-superior altitudinal defect in the right eye and nasal side visual field defect in the left eye. Best corrected visual acuity was 1.0 in the right eye, 0.8 in the left eye. Repeated visual fields over a 11-month period postoperatively showed stability of the defect, with IOP of 12 mmHg to 15 mmHg in each eye. We considered LASIK uses a microkeratome vaccum ring to make a corneal flap, transient elevation of IOP alters the microcirculation of optic nerve head and may have precipitated visual field defect.

7.Clinical Study on Intrauterine Fetal Growth Restriction.

This study was undertaken at the department of Obstetrics and Gynecology, Chonnam University Medical School, to investigate the association between some of the risk factor and the incidence of intrauterine fetal growth restriction(IUGR). The studied population was selected from patients who admitted at Chonnam University Hospital during January, 1992 through May, 1997, with following criteria, Korean, singletone pregnancy with live birth and known gestational weeks with 28 or more. And then, the risk factors were analyzed in terms of maternal factor, placental factor, and fetal factor. The following results were obtained. 1) The incidence of IUGR was 6.1%. 2) The incidence of IUGR was higher at young aged mother and nullipara. 3) Only 39.1% of etiologic factors for IUGR was found to have known causes. According to the risk factors for IUGR, hypertensive disorder during pregnancy, anemia, cardiac disease, leukemia, and pulmonary tuberculosis were associated with increased incidence of IUGR. 4) The relative risk of IUGR was much higher in neonates born with congenital anomalies. 5) According to the placental causes of IUGR, placenta previa and placenta abruption showed some association with IUGR.

8.Usefulness of Treadmill Exercise Test on Diabetes Mellitus.

We studied 193 Diabetics without any symptoms of coronary artery disease and 39 Normal healthy subjects as Control group, using treadmill exercise test and obtained following results. 1) The positive rate was significantly higher in Diabetics(19.4% in Normal ECG groups and 25.4% in total Diabetics)than Control groups(5.1%)(0.01
0.05). 5) There was no difference in HbAIC, Total cholesterol and Triglyceride between positive and negative groups(P>0.05). In summary, with exercise test, the positive rate was higher in Diabetics than in Normal control groups and it seemed to be treadmill exercise test is a useful screening method for evaluation of coronary artery disease as well as for assesment of treatment and follow up care.

9.Psoas Hitch Ureteroneocystostomy.

The Psoas hitch procedure is a universal approach for ureteral reimplantation in all diseases of the distal ureter. Digital ureteral defect due to iatrogenic lesion and chronic inflammation such as tuberculosis, congenital ureteral anomalies including complicated reflux in child are the main indications for the Psoas hitch plasty. We performed ureteroneocystostomy with Psoas hitch procedure in l4 patients in recent 3 years. We evaluate the preoperative state. operative technique, postoperative course and follow-up of these cases and report with the review of the articles. The results were as follows: l. Fourteen cases included 7 ureteral injuries by 3 cases of transabdominal hysterectomy and 4 cases of ureterolithotomy, 3 chronic inflammatory stricture ie. tuberculosis, 2 traffic accidents, l bladder CA. in bladder diverticulum, 1 con genital megaureter. 2. Psoas hitch ureteroneocystostomy was done by submucosal tunnel with Psoas hitch in 9, end to side with Psoas hitch in 3, submucosal tunnel with Psoas hitch and Boari flap in 2 cases. 3. There were improvements in all l4 cases without ureterovesical stenosis or VUR. And functional capacity of the bladder has been well preserved. Postoperative complications were UTI in 3 cases.

10.Congenital Hepatic Fibrosis: A case report.

Congenital hepatic fibrosis is an uncommon disease of children and young adults with two major risks: gastrointestinal hemorrhage caused by portal hypertension, and cholangitis related to bacterial infection of dilated intrahepatic bile ducts. It is characterizeed by stony hard hepatomegaly and portal hypertension with rather well preserved hepatic function and architecture, and frequent association of the renal lesions. We have recently experienced a case of congenital hepatic fibrosis in a 24 year-old Korean male. The chief complaint was hematemesis from esophageal varices. There were marked hepatosplenomegaly, mild pancytopenia and the liver function test was within normal engorgement and dilatation of portal and splenic veins and multiple cysts of both kidneys.
