1.Status and future of metabolic surgery in China

[Summary] With the rapid development of economy ,obesity and type 2 diabetes (T2DM ) have become a serious social problem in China. Metabolic surgery was started to carry out in late 20th century in our country . Currently ,there were many deficiencies in the selection of operation indication ,surgical pattern and procedure ,prevention and treatment of complications ,perioperative management and postoperative follow‐up. Therefore ,to standardize nationwide surgical treatment of obesity and diabetes ,to reduce the incidence of postoperative complications ,Chinese Society for Metabolic & Bariatric Surgery (CSMBS) ,surgeon branch of Chinese Medical Doctor Association called domestic famous experts together to jointly draft a practical manual :Guide to Surgical Treatment of Obesity and Type 2 Diabetes in China (2014). With the spread of this guide ,the number of metabolic surgery increased significantly ,the procedure and management of surgery were further standardized.
2.Protective effect of quercetin in lens

?With the deepening study of quercetin, some studies show that quercetin can protect lens, with pharmacological activities such as antioxidation and anti-apoptosis and so on. And they can delay the occurring and developing of cataract. The development and utilization of the quercetin will be expected to be a new means of prevention and treatment of cataract. The article takes in the research progress of that quercetin on the lens with antioxidation, inhibiting aldose reductase and anti- apoptosis and its mechanisms.

5.The effect of different staining reagent and time on acid fast bacilli staining and its quality control significance

Objective To observe the effect of different staining reagent and time on acid fast bacilli staining and study its quality control measures.Methods We collected 38 cases for positive acid fast bacilli stain.Every wax blocked into 4 pieces,with different staining reagent and time of acid fast staining.Results Using xylene and ethanol staining for 20 minutes,acid fast bacilli was discontinuous and red punctate.Zero cases were positive and the positive rate was 0%.Turpentine dyeing for 1 5 minutes group,acid fast bacilli was clear,bright red,slightly bent branched,and easy to identify.A total of 34 cases were positive and posi-tive rate was 89%.Turpentine dyeing process for 20 minutes,the whole background was red,and was not easy identification.A total of 28 cases were positive and the positive rate was 74%.Turpentine dyeing for 30 minutes,the background was entirely deep red, and was hard to discern the acid fast bacilli.A total of 27 cases were positive and the positive rate was 71%.Conclusion Different staining reagent and time had different positive staining results.Suitable turpentine process was stained for 1 5 minutes for acid fast bacilli.
6.Reduction mammoplasty by central pedicle flap transposition

Objective To explore a surgical procedure of reduction mammary plasty to assure the viability and sensory of the nipple-areolar complex (NAC) after transposition and to keep the lactiferous function partly. Methods Reduction was accomplished by using perforating vascular branches from the pectoralis major muscle and its fascia supplying the nipple and breast parenchyma instead of the subdermal plexus. The central vascular pedicle supplying the nipple-areola complex was preserved. And the sensory nerve of the NAC was not injured. The erection of the nipple could be observed in operation. Lactation was possible because of preservation of the lactiferous ducts. The technique had been used in 17 patients since 1996. Results The nipple-areola complex was safely transposed with great freedom, and the amount of resection was accurately adjusted for symmetry. No cases of nipple-areola complex sensory change occurred postoperatively in all 17 patients. Breast-feeding was possible at least in one patient after operation. Conclusion This method is ideal for the patients, which keeps the viability and sensory of the nipple-areolar complex and the function of lactation. In another case, the amount of resection is easily adjusted for symmetry.
7.Correlation between revascularization effect and calcium content of a kind of man-made compound bone on the femoral head defects and necrosis of canines

[Objective]To observe the effect of composite(AACB/BMP/bFGF)of allogeneic antigen-extracted cancellous bone(AACB),bone morphogenetic protein(BMP)and basic fibroblast growth factor(bFGF)on the repair of defective and necrotic areas of the femoral head,and evaluate the revaseularization the femoral head defect and necrosis model using liquid nitrogen,and randomly divided into 3 groups with 12 femoral heads in each group,which were implanted with nothing(group A),AACB/BMP(group B)and AACB/BMP/bFGF(group C)respectively.Every femoral head was implanted with about AACB 0.33 g,BMP 12.5 mg and bFGF 2000 U.Two canines were sacrificed respectively at 3th,6th,and 12th weeks after operation in every group.Histologically,immunohitochemical vessel staining,vessel count and vessel surface area image analysis,calcium content and the correlation between revascularization effect and calcium content were investigated in the femoral heads after the operation.[Result]Histology,the detects were still infilled by fibrillar tissue in group,A;there were a great deal of new bone formed in group B;and the detects were repaired completely in group C 12 weeks after operation.Measurement of vessel count and vessel surface area showed a large quantity of new vessel proliferated around the infilling material in group C on the 3rd week.The vessel count and vessel surface area continuously increased on the 6th week and 12th week.They were more than those of group B and group A at every definite time point(P
8.Effects of noxious coldness and non-noxious warmth on the magnitude of cerebral cortex activation during intraoral stimulation with water.

OBJECTIVEWe used functional magnetic resonance imaging (fMRI) to explore the effects of noxious coldness and non-noxious warmth on the magnitude of cerebral cortex activation during intraoral stimulation with water.
METHODSSix male and female subjects were subjected to whole-brain fMRI during the phasic delivery of non-noxious hot (23 °C) and no- xious cold (4 °C) water intraoral stimulation. A block-design blood oxygenation level-dependent fMRI scan covering the entire brain was also carried out.
RESULTSThe activated cortical areas were as follows: left pre-/post-central gyrus, insula/operculum, anterior cingulate cortex (ACC), orbital frontal cortex (OFC), midbrain red nucleus, and thalamus. The activated cortical areas under cold condition were as follows: left occipital lobe, premotor cortex/Brodmann area (BA) 6, right motor language area BA44, lingual gyrus, parietal lobule (BA7, 40), and primary somatosensory cortex S I. Comparisons of the regional cerebral blood flow response magnitude were made among stereotactically concordant brain regions that showed significant responses under the two conditions of this study. Compared with non-noxious warmth, more regions were activated in noxious coldness, and the magnitude of activation in areas produced after non-noxious warm stimulation significantly increased. However, ACC only significantly increased the magnitude of activation under noxious coldness stimulation.
CONCLUSIONResults suggested that a similar network of regions was activated common to the perception of pain and no-pain produced by either non-noxious warmth or noxious coldness stimulation. Non-noxious warmth also activated more brain regions and significantly increased the response magnitude of cerebral-cortex activation compared with noxious coldness. Noxious coldness stimulation further significantly increased the magnitude of activation in ACC areas compared with noxious warmth.

10.Network pharmacology study on major active compounds of siwu decoction analogous formulae for treating primary dysmenorrhea of gynecology blood stasis syndrome.

OBJECTIVETo explore the molecular mechanisms of main active ingredients of Siwu decoction analogous formulae for treating primary dysmenorrhea of gynecology blood stasis syndrome by network pharmacology study, and to investigate the correlations between multi-compounds, multi targets and multi pathways.
METHODMajor active compounds from Siwu decoction analogous formulae, including ligustilide, butylidene phthalide, senkyunolide, ferulic acid, gallic acid, peoniflorin, jioglutin A, catalpol, transanethole, zingiberone, commiphoric acid, eugenol, isorhamnetin-3-O-neohesperidoside, wulingzhic acid, alpha-cyperone, cyperene, costunolide, costic acid, tetrahydropalmatine, protopine, amygdalin, 24-methylene cycloartanol, oleic acid, linoleic acid, 3-p-coumaroylquinic acid, hydroxysafflor yellow A, coptisine, berberine, jatrorrhizine, baicalein, baicalin, wogonin were collected to build component-protein networks based on PharmMapper database. The targets information access was used to construct and visualize components-targets-pathways network model using the kyoto encyclopedia of genes and genomes (KEGG) pathway database and Cytoscape software.
RESULT AND CONCLUSIONSerine threonine protein kinases play an important role in the process of cells. They were potential targets in the effect of Siwu decoction analogous formulae. The effect of main active ingredients involved 51 the pathway. Besides the same ones, Shaofu Zhuyu decoction had more effect on lipid metabolism, Xiangfu Siwu decoction on amino acid metabolism pathways, Taohong Siwu decoction on carbohydrate metabolism, while, Qinlian Siwu decoction on ErbB, VEGF signal transduction pathway. Siwu decoction and its derived formulae not only had common targets and pathways, but also had their own emphasis. This reflected the formulae effect mode of multi-ingredients, multi-targets and multi-pathways. It may provide clues to deeper study of molecular mechanism of Siwu decoction analogous formulae action.
