Eight mongrel dogs were involved in the investigation of sevoflurane effects on renal blood flow (RBF). After intravenous anesthesia and surgical preparation, RBF, MAP, HR, CVP, CO and SV were recorded as baseline. Sevoflurane was then administered to achieve end-tidal concentrations of 1, 2, 3 and 4% in succession. Following 20 mins equilibration at each concentration, and 30 mins after the termination of inhalation, the above parameters were measured repeatly for comparing. Results showed significant dose-related depressions of RBF, MAP, HR and CO with sevoflurane. RBF didn't reduce significantly until MAP decreased to 9.3kPa, with end-tidal concentration of sevoflurane no less than 3%, it didn't recover in step with improved hemodynamic parameters after sevoflurane withdrawal. This study indicates that sevoflurane may influence little on autoregulation of RBF, its dose-related cardiac depression may be responsible for the reduction of RBF.

Objective The effect of angiotensinⅡ(AngⅡ) on cardiac hypertrophy were studied in SD rat with different ages. Methods Cardiocyte and cardiac fibroblasts in neonatal SD rat group and 12 months SD rat group were cultured in vitro. In each ages group, control MEM, angⅡ, angⅡantagonist(Losartan) and angⅡ+losartan were administrated respectively. 3 H leu incorporation into cardiocyte and 3 H TdR incorporation into cardiac fibroblasts were measured in neonatal rat group and 12 month rat group. To investigate the effect of cardiac fibroblasts on cardiocyte under the influence of angiotensinⅡ, the supernatant of cultured cardiac fibroblasts with angiotensinⅡ, the supernatant of cultured CBS with losartan and the supernatant of cultured cardiac fibroblasts with angiotensinⅡ and Losartan were added into cultured cardiocyte respectively. 3 H leu incorporation into cardiocyte were tested. Objective To investigate the clinical outcomes of radical surgery for pancreatic body and tail tumors accompanied with sinistral portal hypertension (SPH).Method The clinicopathological data of 35 patients with pancreatic body and tail tumors accompanied with SPH operated from January 2004 to December 2014 were retrospectively analyzed.Results Of 35 patients,22 patients had body and tail pancreatic carcinomas,10 patients had malignant solid pesudopaillary tumors and 1 patients had a neuroendocrine tumor.All these patients developed splenomegaly and varices in the gastric fundus with normal hepatic function.The splenic vein pressure was (27.3 ±3.8)cmH2O (1 cmH2O =0.098 kPa),its average diameter was (1.3 ± 0.3) cm,and the speed of splenic vein blood flow was (8.9 ± 0.8) cm/s.Of the 35 patients with pancreatic body and tail tumors who underwent radical resectional operations,22 patients in addition underwent devascularization.There were 13 of these 22 patients who underwent pericardical devascularization and the remaining 9 underwent total or proximal gastrectomy.The main complications were pancreatic fistula (n =5,14.2％),intra-abdominal infection (n =4,11.5％),delayed gastric emptying (n =1,2.8％),lymphatic fistula (n =1,2.8％) and gastric perforation (n =1,2.8％).The post-complication morbidity rate was 34％.All these patients were followed-up for 6 to 60 months after operations.There was no upper gastrointestinal bleeding which occurred within 6 months of operation.Conclusion Multi-disciplinary treatment,complete preoperative evaluation,correct perioperative and individualized management enhanced efficacy in the surgical treatment of patients with pancreatic body and tail tumors with SPH.

Objective To investigate the clinical efficacy of radical operation for gastric cancer with liver cirrhosis.Methods The clinicopathological data of 56 gastric caner patients with liver cirrhosis performed on between Jan 2000 and Dec 2013 were analyzed retrospectively.Results The morbidity rate was 71％.The main complications of the group with liver cirrhosis were postoperative ascites (7 cases),delayed gastric emptying(6 cases),pulmonary infection(8 cases),intra-abdominal infection (5 cases),anastomotic leakage (3 cases).wound infection (2 cases)and,astrointestinal bleeding(1 cases).Postoperative complications of gastric cancer with liver cirrhosis were associated with age (＞ 60 yrs),Child-pugh grade(B),lymph node dissection(D2).There were no mortality.The 1-,3-and 5-year survival rates of the 56 cases were 73.2％,41％ and 26.7％.Conclusion Multi-disciplinary treatment,complete preoperative evaluation,correct perioperative management,individualized principle were the best mode on effective treatment of the patients with liver cirrhosis.

AIM:To explore the effects of small interfering RNA(siRNA) targeting PCNA gene on nasopharyngeal carcinoma CNE2 cells growth and cycle.METHODS:Three synthesized siRNA targeting PCNA gene was transfected into CNE2 cells by using LipofectamineTM reagent.The PCNA mRNA and PCNA protein were detected by real-time polymerase chain reaction(RT-PCR) and immunohistochemical method.Inverted phase contrast microscope was used to determine the CEN2 cells growth before and after PCNA-siRNA transfected.Flow cytometry was used to observe the cell cycle.RESULTS:In CNE2 cells after PCNA-siRNA transfection,the expressions of PCNA mRNA and protein were down-regulated at different degree.Inhibition ratio of PCNA mRNA was 98.5%.Meanwhile,the cell cycle was suffocated at G0/G1 stage.CONCLUSION:The synthesized PCNA-siRNA effectively interferes nasopharyngeal carcinoma cells by down-regulating the expressions of the PCNA mRNA and its protein,therefore inhibits the growth of CNE2 cells.Future application of PCNA-siRNA in the gene therapy of nasopharyngeal carcinoma might be expected.

Objective To observe the correlation between neutrally adjusted ventilatory assist (NAVA) mode and improvement of inflammation and oxidative stress in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD),and as well as to investigate the effects of NAVA mode versus pressure support ventilation (PSV) mode on improving the success rate of weaning advantages.Methods A total of 40 patients with AECOPD-Ⅲ supported by mechanical ventilation treatment admitted from November 2012 to January 2014 into intensive care unit (ICU) were enrolled for prospective study.The patients were randomly divided into PSV mode (n =20) and NAVA mode (n =20) according to gender,age,APACHE Ⅱ score,medical history,PaCO2 of five factors and adopting the principle of minimum distribution of the imbalance index.The comparisons of the successful rate of weaning,48 h re-intubation rate and length of ICU stay were made between two groups.The level of C-reactive protein (CRP),serum amyloid A (SAA),human cartilage glycoprotein 39 (YKL-40) in serum were measured by enzyme-linked immunosorbent assay (ELSIA) on the 1st day,3rd day,5th day and 7th day after initiation of mechanical ventilation,and results of these laboratory tests in patients were compared with those in healthy subjects of control group.And simultaneously,the broncho-alveolar lavage fluid (BALF) was collected with Gibot method by employment of optic fiber bronchoscope on the given days for detection of YKL-40,and levels of YKL-40 were compared between NAVA mode and PSV mode.Enumeration data were analyzed with x2 test,measurement data were analyzed with t test or repeated measures analysis of variance,and P ＜ 0.05 was considered to be significant.Results (1) There were no significant differences in leukocyte count and neutrophils percentage between NAVA mode and PSV mode (P ＞ 0.05).The magnitudes of decrease in concentrations of blood CRP and SAA in NAVA mode were significantly greater than those in PSV mode (P ＜ 0.01),but there was no significant difference in blood YKL-40 between NAVA mode and PSV mode (P ＞ 0.05).The magnitude of reduction in concentration of BALF YKL-40 in NAVA mode was significantly greater than that in PSV mode (P ＜0.01).(2) There was on significant difference in rate of final weaning between NAVA mode and PSV mode,but the rate of direct weaning was higher in NAVA mode than that in PSV mode (P =0.046),and the 48 h re-intubation rate was lower in NAVA mode than that in PSV mode (P =0.032).The length of ICU stay was shorter in NAVA mode than that in PSV mode (P =0.031).The peak of EAdi (electric activated diaphragma trigger) in 8 patients failing in direct weaning before first attempt was significantly higher than that in other patients with successful weaning patients (P =0.002).Conclusions NAVA mode can attenuate inflammation and oxidative stress in patients with AECOPD,and ultimately improve the rate of direct weaning and shorten the length of ICU stay.Further research is necessary to confirm the capability of NAVA mode for improving the ultimate rate of weaning in AECOPD patients.

Objective: To investigate the efficiency of the three attentional networks in attention deficit hyperactivity disorder (ADHD). Methods: Subjects were 25 children, aged 7 to 12 years, with DSM Ⅳ ADHD and 25 non ADHD controls matched by age, sex and IQ. Attentional networks tests were performed in all the subjects on an IBM compatible computer. Results: No differences were found between the ADHD children and the normal controls in alerting and orienting effects. However, ADHD children had a significantly larger executive control effect than their normal counterparts. The general accuracy of the ADHD children was significantly lower than that of the normal controls. Conclusion: ADHD children may have impairments in their executive control network of attention.

Compound Danshen aerosol remarkably increased hypoxia tolerance after isoproterenol injection and prolonged survival time in mice. The electrocardiogram of rats and rabbits with pituitrin-induced ischemia was significantly improved. The aerosol also decreased the elevation of lactic dehydrogenase in rats with experimental myocardial infarction.

Objective To explore the effect of hesperidin on cardiac function and ventricular remodeling following myocardial infarction (MI) in mice.Methods Ligation of left anterior descending (LAD) was operated to establish MI model.Forty-two C57BL/6 mice were randomly (random number) divided into control and MI group;and 24 h after LAD ligation,mice in MI group were further divided into MI control and hesperetin group.Eight weeks later,cardiac function and structure changes were determined by the methods of hemodynamic measurement and echocardiography.HE staining was used to measure crosssectional area (CSA) of atrial myocytes,and PSR staining was applied for observe collagen deposition and calculation of collagen volume fraction (CVF).Real-time PCR was used to detect the mRNA expressions of cardiac hypertrophy markers (ANP,BNP and β-MHC) and cardiac fibrosis markers (Collagen Ⅰ,Collagen Ⅲ and CTGF).The contents of superoxide anion and hydroxy radical were detected by colorimetric method.Results Compared with control group,left ventricular posterior wall thickness (LVPWT) and interventricular septum thickness (IVST) were increased to be thicker,left ventricular fractional shortening (LVFS) and left ventricular ejection fraction (LVEF) were significantly lower,and ± dp/dtmax was remarkably reduced in MI control group (P ＜ 0.05).Compared with MI control group,hesperetin could increaseLVFS [(29.48±3.87)％ vs.(20.69±3.99) ％],LVEF [(46.40±1.68)％ vs.(30.51± 1.17) ％] and ±dp/dtmax [(3 344.33 ±269.57) mmHg/S vs.(2 205.19 ±224.17) mmHg/S;(2250.40±218.35) mmHg/S vs.(-1 566.91 ±217.37) mmHg/S];but could reduce LVPWT [(2.29±0.05) mm vs.(2.85±0.10)mm]andIVST[(1.44±0.09) mm vs.(1.89±0.06) mm].Compared with control group,CSA and CVF were significantly increased in MI control mice.However,hesperetin could reduce CSA and CVF.Compared with control group,the mRNA expressions of cardiac hypertrophy and cardiac fibrosis markers were significantly increased in MI control mice;but hesperetin could significantly inhibit the mRNA expressions of cardiac hypertrophy and cardiac fibrosis markers.Additionally,hesperetin could significantly reduce the contents of superoxide anion and hydroxy radical.Conclusion Hesperetin intervention can inhibit ventricular structure change,and improve hemodynamics and cardiac function after acute myocardial infarction via inhibiting the production of reactive oxygen species (ROS).
