3.Clinical effects of biofeedback therapy on functional constipation caused by pelvic floor dys-synergia

Objective To explore the clinical effects of biofeedback (BF) therapy on functional constipation (FC) caused by pelvic floor dys-synergia (PFD).Methods Forty patients with FC caused by PFD were enrolled and given BF therapy twice a week for 5 weeks.Constipation symptom scores and anal motility were recorded before and after the therapy.Results All the patients enrolled finished the BF therapy,with a total effectiveness rate of 80%.Both the constipation symptom scores and anal residual pressure during defecation had decreased significantly after therapy.Rectal gradient pressure,however,had increased significantly.Conclusion Biofeedback can relieve the clinical symptoms of FC and improve pelvic floor coordination during defecation.It is worth applying in clinical practice.

Objective To explore the changes of atrial natriuretic peptide(ANP)and parathyroid hormone(PTH)in umbilical cord blood of newborn infants with intrauterine growth retardation(IUGR)and their relationships with electrolytic.Methods A total of 71 IUGR infants borned between Jan.2006 and Aug.2007 were enrolled in this study.Another 40 normal appropriate for gestational age neonates were selected as control group.The study group were divided into 2 groups:mature IUGR group(n=29)and premature IUGR group(n=42).The samples of umbilical cord blood of every group were collected at the time of delivery,and ANP,PTH levels in umbilical cord blood were mea-sured by radioimmunoassay.The sodium,calcium levels in their peripheral vein were measured simultaneously.Results 1.Compared with control group[(0.78?0.42)?g/L],the ANP levels of premature IUGR group[(1.26?0.47)?g/L] and the mature IUGR group[(1.09?0.51)?g/L] were significantly increased(t=5.98,2.76 Pa0.05).The calcium levels of the premature IUGR group[(1.85?0.37)mmol/L]significantly decreased(t=1.93 P0.05)compared with control group [(2.02?0.44)mmol/L].3.The serum sodium level was negatively correlated with the umbilical ANP level(r=-0.93 P
5.Interferin-γ and interleukin-10 levels in expressed prostatic secretion of chronic prostatitis/chronic pelvic pain syndrome

Objective To investigate the levels of pro-inflammatory cytokine interferon (IFN)-γand anti-inflammatory cytokine interlenkin (IL)-10 in expressed prostatic secretions (EPS) of chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) and their clinical significance. Methods The levels of IFN-γ and IL-10 in EPS of 20 patients with CP (type Ⅲ A, group A ), 20 patients with CPPS (type Ⅲ B,group B) and 10 healthy men (control group) were measured using enzyme-linked immunosorbent assay (ELISA). The results were analyzed comparatively with NIH-chronic prostatitis symptom index (NIH-CPSI).Results The levels of IFN-γ in EPS of group A and group B were ( 14.92 ± 7.85 ), ( 13.74 ± 5.96) ng/L respectively, which were higher than that in control group [(7.47 ± 1.40) ng/L] (P < 0.05 ). And the levels of IL-10 in EPS of group A and group B were (216.40 ± 33.65 ), (223.70 ± 45.21 ) ng/L respectively, which were higher than that in control group [( 162.10 ± 43.81 ) ng/L] ( P < 0.05 ). There was no statistically significant difference between group A and group B. No correlation between NIH-CPSI and IFN- γ,IL-10 levels in EPS(P > 0.05 ). Conclusion IFN-γ and IL-10 have a very important role in etiology of CP/CPPS and they can be the objective parameters in the diagnosis of CP/CPPS.
6.Analysis of the Utilization of TCM Decoction Pieces for Clearing Damp and Promoting Diuresis in Affiliated Hospital of Nanjing University of TCM During 2009-2013

Objective To know the utilization and development trend of TCM decoction pieces for clearing damp and promoting diuresis in our hospital;To provide references for rational medicine use and purchase in clinic. Methods Excel software was used to analyze the utilization of TCM decoction pieces for clearing damp and promoting diuresis in our hospital during 2009-2013 from the aspects of consumption quantity, consumption sum, and annual average growth. Results The consumption quantity of TCM decoction pieces for clearing damp and promoting diuresis in our hospital increased year by year with average growth rate of 10.58%;the consumption sum increased year by year with average growth rate of 20.21%. The consumption quantity of TCM decoction pieces for inducing diuresis to alleviate edema took the lead among all kinds of medicine, followed by medicine for inducing dampness for treating jaundice, and medicine for inducing diuresis for treating stranguria. Conclusion Both the consumption quantity and the consumption sum of TCM decoction pieces for clearing damp and promoting diuresis in our hospital have been increasing year by year. The proportion of which in total quantity and the consumption sum of TCM decoction pieces and the medicine utilization structure have kept stable, and its usages is rational on the whole.
7.Effect of stellate ganglion block on heart rate variability during thyroidectomy performed under cervical plexus block

Objective To investigate the effect of stellate ganglion block(SGB)on heart mte variability (HRV)during thyroidectomy performed under cervical plexus block.Methods Forty-five ASA Ⅰ or Ⅱ pafients of both sexy8(8 males,37 females) aged 20-60 yr,weighing 40-70 kg undergoing elective thyroidectomy under cervical plexus block were randomly divided into 3 groups(n=15 each):group Ⅰ control,group Ⅱ left SGB and group Ⅲ right SGB.The parameters of HRV included low frequency(LF),high frequency(HF),LFnu,HFnu and LF/HF ratio.SBP,DBP,HB and HRV parameters were recorded before anesthesia(To,baseline),immediately(T1) and at 5 and 10 rain after cervical plexus block(T2,T3),at the beginning and end of operation (T4,T5).Results SBP and DBP were significantly increased at different time points after cervical plexus block in all 3 groups.Compared with the baseline at T0,LFnu and LF/HF ratio were significantly increased while HR,HF and HFnu decreased during operation in group Ⅰ,LF,HF and HFnu were significantly decreased while HR increased during operation in group Ⅲ(P<0.05),but no significant change in HRV parameters was found during operation in group Ⅱ(P>0.05).Compared with group Ⅰ,HR and LFnu were significantly decreased while HF and HFnu increased in group Ⅱ(P<0.05),but no significant change was found in the indexes mentioned abovein group Ⅲ (P>0.05).Conclusion SGB can adjust cervical plexus block-induced sympathetic excitability and maintain the balance between sympathetic and parasympathetic tension.Left SGB outweighs right SGB.
8.Screening of specific microRNAs and bioinformatics analysis on cardiomyocytes in rat with heart failure

Objective To investigate microRNAs ( miRNAs) expression profiling of cardiomyocytes in rats with heart failure, and predict miRNAs-regulated target genes and their functions.Methods Total of 18 male SD rats weighing 200-220 g were randomly divided into 2 groups:the control group ( CON) and the heart failure group (HF).The rats in HF group were injected by adriamycin via tail vein to induce heart failure, meanwhile in CON group, rats were received an equal volume of 0.9% sodium chloride intravenously.The cardiomyocytes isolated from the rat hearts in two groups and cultured overnight.After that, total RNA was extracted and then subjected to miRNA microarray to screen differentially expressed miRNAs.The reults of microarray were further verified by quantitative real-time PCR ( qRT-PCR ) .The target genes regulated by differentially expressed miRNAs were predicted by the software of Targetscan and miRanda.Bioinformatics analysis was performed to predict the miRNAs-regulated target genes and analyze the enriched gene ontology ( GO) and signaling pathway ( KEGG Pathway) .Results The results of miRNA microarray showed that a total of 37 miRNAs were differentially expressed in HF group as compared to CON group, among which 22 miRNAs were up-regulated and 15 miRNAs were down-regulated (P<0.01, FDR<0.05).The expression of miR-133b-5p (t=14.56, P<0.01), miR-6216 (t=9.32, P<0.01) and let-7e-5p (t=13.92, P<0.01) which were detected by qRT-PCR exhibited the similar tendency of up or down regulation to those shown in microarray results.Bioinformatics analysis indicated that miRNAs-regulated target genes were significantly enriched in 31 GOs (P<0.01, FDR<0.05) and 12 signal pathways (P<0.05, FDR<0.05), among which ubiquitin-proteasome system, MAPK signaling pathway and Toll like siganling pathway exhibited a higher enrichment. Conclusion MiRNA expression profile on cardiomyocytes in rat with adriamycin-induced heart failure was significantly changed.These differentially expressed miRNAs might participate in the process of heart failing by regulating their target genes in rat cardiomyocytes.
9.Anterior Cruciate Ligament: Evaluation with Oblique Coronal MR Images

Objective To investigate the value of obli que coronal MR images in visualization of the anterior cruciafe ligment(ACL).Methods Conventional axial,oblique sagittal and coronal MR images were performed in 30 normal knees.Then oblique coronal T 2-weighted i mages,which was parallel to ACL,based on oblique sagittal T 2 -weighted images ,were acquired.The visualization of ACL on oblique coronal,oblique sagital and coronal T 2-weighted images was analysed.Results The visualization of the whole ACL was 100%(30/30 ) on one image of the oblique coronal T 2-weighted images, 83.3%(25/30) on on e image of the oblique sagaital T 2-weighted image. On the coronal T 2-weighte d images,the ACL appeared as segmental bundle.The whole ACL could not be visua lized on one image of coronal T 2-weighted images.Conclusion The whole ACL can be showed on one image of ob lique coronal T 2-weighted image,which is helpful in evaluation of the injure o f ACL.
10.Efficacy of double-lumen central venous catheter for airway management in children undergoing resection of laryngeal papilloma under inhalation anesthesia with sevoflurane-spontaneous breathing

Objective To evaluate the efficacy of double-lumen central venous catheter (DLCVC) for airway management in children undergoing resection of laryngeal papilloma under inhalation anesthesia with sevoflurane-spontaneous breathing.Methods Twenty-nine ASA physical status Ⅰ or Ⅱ podiatric patients,aged 1-9 yr,weighing 10-35 kg,scheduled for elective resection of laryngeal papilloma under self-retaining laryngoscope,were included in this study.Anesthesia was induced with inhalation of 8 % sevoflurane and the children kept spontaneous breathing.The larynx was sprayed with 2% lidocaine for topical anesthesia.A self-retaining laryngoscope was inserted and a 7F DLCVC was placed below the glottis via the self-retaining laryngoscope.4%-6% sevoflurance was insufflated via the main lumen of DLCVC (14G) to maintain anesthesia.PETCO2 was monitored through the branch lumen of DLCVC (18G).Before surgery (T1),at 20 min after the beginning of surgery (T2),and at the end of surgery (T3),arterial blood samples was obtained for blood gas analysis,and body movement,hypoxemia (SpO2 < 95 % during oxygen inhalation),and airway management failure (SpO2 < 90% during oxygen inhalation) were also recorded.Results Compared with the baseline value at T1,although there were significant changes in PaCO2,base excess,pH value and PaO2 at T2 and T3 (P < 0.05),they were all within the clinical reference ranges.There were no significant differences between PaCO2 and PETCO2 at T1,T2 and T3 (P > 0.05).The incidence of body movement and hypoxemia was 10%,and the rate of airway management failure was 3% (due to degree Ⅲ laryngeal obstruction).Conclusion DLCVC can be safely and effectively used for airway management in children with degree Ⅰ or Ⅱ laryngeal obstruction undergoing resection of laryngeal papilloma under inhalation anesthesia with sevoflurane-spontaneous breathing.
