1.Mechanism research of miR - 181 regulating human lens epithelial cell apoptosis

?AlM: To investigate the expression of miR-181 in the lens tissue of cataract and the regulating mechanism of miR-181 on apoptosis of human lens epithelial cell.
?METHODS:Real time q-PCR was used to measure the expression of miR-181 in the anterior lens capsules of age - related cataract and human lens epithelial cell apoptosis model. miR- 181 mimic and inhibitor were transfected using Lipofectamine 2 000 to regulate the expression of miR-181, and then Real time q-PCR was used to verify transfection efficiency. Flow cytometry was used to detect the change of cell apoptosis rate.
? RESULTS: Compared with control group, the expression of miR-181 was significantly higher in both the anterior lens capsules of age-related cataract and human lens epithelial cell apoptosis model; the relative expression of miR-181 in lens epithelial cells transfected with miR-181 mimic was increased, whereas decreased in cells transfected with miR-181 inhibitor;the apoptosis rate of cells transfected with miR - 181 mimic was increased, while reduced in miR-181 inhibitor group. Each result was statistically significant (P<0. 01).
?CONCLUSlON:High expression of miR-181 is detected in anterior lens capsule of age-related cataract. miR-181 might play a certain role in the pathogenesis of cataract via promoting human lens epithelial cell apoptosis. miR-181 probably becomes a new approach for the nonoperative treatment of cataract, but the concrete mechanism still needs to be further studied.
2.Expression of aquaporin 2 in rat kidney under hypoxia at an altitude of 4600 m

Objective To investigate the change of aquaporin 2 (AQP2) mRNA and protein levels in renal collecting duct of SD rats after hypoxin caused by rising of the altitude to 4600 m. Methods Forty male SD rats were randomly divided into 4 groups (24 h, 48 h, 72 h and 1 week group), and 10 rats in Xining city were used as control group. All the 40 SD rats were transported to Kekexili Natural Reservation areas (4600 m) in Qinghai province. Rats of four experimental groups were sacrificed and renal tissue samples were harvested at different time point respectively, the control group rats were treated in Xining city (2260 m) as well. The concentration of plasma antidiuretic hormone (ADH) was measured by radioimmunity method. The expression of AQP2 mRNA and proteins was evaluated by real-time fluorescent quantitative-PCR, Western blot and immunofluorescence assay. Results The concentration of plasma ADH was decreased at 24 h and was only 28.5% of that of control group, reaching the lowest concentration at 48 h [(86.94±6.49) μg/L vs (302.5±310.48) μg/L], then it increased gradually and was similar to the control group at 7 d [(306.46±11.14) μg/L vs (302.53±10.48)μg/L, P> 0.05]. There were significant differences of the control group with 24 h, 48 h and 72 h groups, respectively[(302.53± 10.48) μg/L vs (142.46±10.57)μg/L, (86.94±6.49)μg/L, (169.65±11.15) μg/L respectively, P<0.01]. The change of AQP2 gene expression level was consistent with the change of ADH. It was decreased at the begining when exposure to altitude and it reached its lowest level at 48 h. It was then returned to high level similarly to that of the control group at 7 d (0.09±0.01 vs 0.09± 0.008, P>0.05 ). There were significant differences of the control group with 24 h, 48 h and 72 h group, respectively (0.09±0.008 vs 0.04±0.005, 0.03±0.002, 0.04±0.003 respectively, P<0.01 ). Conclusions AQP2 expression in the renal collecting duct of SD rats is altered over the period exposed to altitude. It is decreased in the early hypoxia period, and is increased in later period. This change may be related to the intensity of hypoxia, which is mediated by a potential adaptation mechanisms against hypoxia caused by high altitude.
3.Analysis of risk factors and bacterial drug resistance in severe pneumonia caused by multi-drug resistant Klebsiella pneumonia

Objective To explore the risk factors of severe pneumonia caused by multi-drug resistant Klebsiella pneumonia,and antimicrobial drug resistance among these isolates.It may help to prevent and control the disease and promote to rational use of antibiotics.Methods We conducted the case-control study in our PICU.It included 89 patients with severe pneumonia caused by multi-drug resistant Klebsiella pneumonia as case group and 68 patients with severe pneumonia caused by Klebsiella pneumonia as control group during the same period.To compare the two groups on irrationality use of antibiotics (especial for third generation cephalosporin),length of stay,tracheal cannula,time of mechanical ventilation and underlying conditions (malnutrition,congenital heart disease,heredity and metabolic disease).Antimicrobial susceptibilities among 89 multi-drug resistant Klebsiella pneumonia isolates were analyzed.Results There were 63 cases (70.79%) for irrationality use antibiotics in case group,while there were 27 cases (39.70%) in control group (P < 0.01).The cases for length of stay over 7 days in case group (48 cases,53.93%) were more than those cases with the same situation in control group (12 cases,17.65 %) (P < 0.01).Thirty-eight cases (42.69 %) needed mechanical ventilation therapy in case group,while 16 cases (23.53%) needed mechanical ventilation therapy in control group (P < 0.01).The cases for duration of mechanical ventilation over 5 days in case group (18 cases,20.22%) were more than those cases in control group (5 cases,7.35%) (P <0.05).The cases with underlying disease in case group (13 cases,14.61%) were more than those cases in control group (2 cases,2.94%) (P < 0.05).Multi-drug resistant Klebsiella pneumonia isolates demonstrated that high-level resistance for penicillins,cephalosporins,aminoglycosides and quinolones,but still susceptible to carbapenems.Conclusion Several risk factors are associated with severe pneumonia caused by multi-drug resistant Klebsiella pneumonia,including irrational use of antibiotics (especial for third generation cephalosporin),long term of length of stay,endotracheal intubation,long term of mechanical ventilation,and having underlying disease (malnutrition,congenital heart disease,heredity and metabolic disease).Multi-drug resistant Klebsiella pneumonia isolates demonstrated that high-level resistance for penicillins,cephalosporins,aminoglycosides and quinolones,but still susceptible to Carbapenems.Carbapenems should be used as first-line drugs for severe pneumonia caused by multi-drug resistant Klebsiella pneumonia.

5.Spiral CT reconstruction for typing of tibial plateau fracture to guide surgical therapy.

OBJECTIVETo typing of tibial plateau fracture based on spiral CT reconstruction and to explore effect of the typing method for treatment.
METHODSA hundred and twenty-six cases with tibial plateau fracture (male 95, female 31, age from 23 to 58 years old), the fractures were classified based on reconstruction image of spiral CT. Including central compression type in 13 cases, split type in 8, split compression type in 79, comminution type in 26. According to the different typing the suitable incision of operation and fixed method for fracture were select.
RESULTSA hundred and twenty-six cases were followed up for 0.5-4 years with an average of 1.2 years. According to Hohl system score to knee joint function, there were statistical significance in the pain,active movement,active range of motion between before and after operation (P < 0.01) and there were no statistical significance in stability and self-evaluation (P < 0.01).
CONCLUSIONTyping of tibial plateau fracture based on spiral CT reconstruction helpful to choose operative approach, reduction and fixed method and obviously improve clinical effect.

6.Aolymer-encapsulated microbubble contrast agent: preparation and in vitro echogenic characteristics.

OBJECTIVETo prepare an ultrasound microbubble contrast agent using PLLA-PEG-PLLA copolymer as the shell material, and test their acoustic characteristics in vitro.
METHODSPLLA-PEG-PLLA tri-block co-polymers were synthesized by ring-opening polymerization. Microbubbles were prepared by using double emulsion technique. Light microscope and scanning electron microscope were applied to observe the morphology of the microbubbles. Their size distribution was analyzed using MASTERSIZER 2000. The acoustic characteristics of microbubbles were tested by using color Doppler Ultrasonography.
RESULTSMicrobubbles were well dispersed in water. Contrast enhancement was stronger and longer lasting at low mechanical index.
CONCLUSIONThe polymer-encapsulated microbubbles prepared by using double emulsion technique with PLLA-PEG-PLLA as the shell material can be used as an ultrasound contrast agent.

7.Association between gestational weight gain and birth weight (2001-2009) :a cohort study carried outin Kunshan city, Jiangsu province, China

Objective To identify the association between gestational weight gain and birth weight over the past 9 years in Kunshan city,Jiangsu province,China.Methods This population-based study was conducted between 2001 to 2009.Data were retrieved from Perinatal Monitoring System of Maternal and Child Health Care Hospital of Kunshan.The study population consisted of 33 631 women and singleton live fetus.Gestational weight gain was defined as the total weight gain during the last and first prenatal care program and divided by the interval weeks.Results From 2001 to 2009,the average incidence of low birth weight was 1.86%,while the average incidence of macrosomia was a bit higher,fluctuating around 8.47%.On those underweight mothers,after adjustment for potential confounders,and stratified by the BMI levels,which were evaluated at the first prenatal care program,we found that weight gain in the 3rd and 4th intervals,could reduce the risk of low birth weight (less than 2500 g).With those mothers with normal-weight,weight gain in the 2 rid,3 rd and 4th intervals,would reduce the risk of low birth weight.Risks in the 4th quantile among underweight and normal-weight group were prevalence odds radio (POR) 95%CI:0.51 (0.32-0.80) and 0.58 (0.42-0.79),respectively.The risks showed a significant downward trend in underweight and normal- weight groups with increased gestational weight gain.As for macrosomia (≥4000 g),the risks increased (POR 95%CI) 4.69(2.82-7.81 ) in underweight,4.15 (3.43-5.03) in normal-weight,in overweight,3.64 (2.62-5.06) and 1.96 (1.48-2.60) in obese mothers with increased levels of gestational weight gain.Trend tests indicated that the risks of marcosomia increased in all levels of BMI,with the increase of gestational weight gain.Conclusion Findings from this population-based study suggested that gestational weight gain could reduce the risks of low birth weight among underweight and normal-weight groups,while increase the risks of macrosomia in all parturients,as compared with lowest levels of gestational weight gain.

Through a review of the current state and risks of adolescent health in the world and in China, the main health issues faced by adolescents are summarized. The importance of promoting adolescent health and the importance of effective adolescent health services are highlighted. The past 30 years important documents of international adolescent health and development have been sorted out. The history of the development of adolescent health care in China has been reviewed. It was pointed out that the health and development of adolescents has become a hot spot and focus of international attention, and it has received more and more attention in China. The adolescent health care has ushered in a new opportunity for development, which will help the health and development of adolescents in China.
9.The influence of coronary main vessel stenting on side branches.

OBJECTIVETo evaluate the influence of stenting main vessel on side branches and to compare the different effects on side branches between BMS and DES (Taxus and Cypher).
METHODSWe reviewed the angiograms and the medical records of 183 patients who had received stent implantation in coronary main vessels and had follow-up angiograms. Any side branch was jailed by stent was evaluated.
RESULTSSide branch occlusion occurred in 8.9% in all branches (10.5% in Cypher DES group, 11.1% in Taxus DES group and 7.8% in BMS group). Spontaneous recanalization of side branches was observed in 72% (90.9% in Cypher DES, 66.7% in Taxus stent group and 66.7% in BMS). The ostial side branch stenosis before stenting and the involvement of the side branch origin within the lesion of the parent vessel are the major independent predictors for side branch occlusion.
CONCLUSIONSThe influence of different DES implantations in coronary main vessels on side branches were similar and there was no difference between DES and BMS. Side branch occlusion had relatively benign clinical course. Most occluded side branches had late spontaneous reperfusion.

10.Genetic analysis of a juvenile with maturity-onset diabetes of the young type 12.

OBJECTIVE:
To explore the genetic basis for a juvenile with maturity-onset diabetes of the young type 12(MODY12).
METHODS:
High-throughput sequencing was carried out to screen for the variants. Candidate variant was verified by Sanger sequencing. Pathogenity of the variant was predicted by searching the genetic databases and analysis by using bioinformatic software.
RESULTS:
Genetic testing indicated that the patient and his mother have both carried a heterozygous c.3976G>A variant (p.Glu1326Lys) in exon 32 of the ABCC8 gene. Prediction of the protein structure suggested the variant to be deleterious. Based on the guidelines of the American College of Medical Genetics and Genomics, the variant was predicted to be uncertain significance.
CONCLUSION
Whether the c.3976G>A variant of the ABCC8 gene is the cause of the disease in this patient or not depends on the functional studies and more case data. Above finding has enriched the spectrum of ABCC8 gene variants.
