2.Clinical complication understanding of early postoperative enteral nutrition in gastrocolonic surgery

Objective To evaluate if the enteral nutrition have effect on the immune function and in- flammatory reaction after operating about gastric carcinoma.Methods 58 postoperative patients suffering from stomach cancer and colon cancer were randomly divided into the EN group and the TPN group.On the first postoperative day,nutrition fibre were given via nasal intestinal tube,increasing the capacity and drop- ping speed day by day until patients can eat liquid diet.While patients in the TPN group didn't eat anything until enterokinesia completely recovered.Observing on preoperative day 1 and on postoperative day 3 and day 8 respectively to check IgA,IgG,IgM,C3,C4,CRP,LYM,LYM%,TP,ALB,PA.Results The results showed that on the postoperative day 8,the target ascension extent was higher than that in the PN group.The statistical significance was very obvious(P
3.Efficacy of nursing intervention for improving the quality of sexual life for patients with cervical intraepithelial neoplasia

ObjectiveTo discuss effective measures to improve postoperative sexual life quality of patients with cervical intraepithelial neoplasia (CIN).MethodsOne hundred and twenty patients with CIN were randomly assigned to the blue-ribbon club group, the friendship-support group and the control group. The blue-ribbon club patients were regarded as club members and received special education every two months. The friendship-support group patients received education one by one in the hospital setting and the control group patients received the same education as before participated into the trial. Female sexual function index (FSFI)was used to evaluate sexual function before and 6 months after intervention.ResultsExcept sexual pain, the total score of FSFI and the score of sub-domain in the blueribbon club were significantly higher than that in the control group. The total score of FSFI and score of sub-domain were significantly higher than that in the control group. There were no significant differences between both intervention groups.ConclusionsBoth education intervention modes of the blue-ribbon club group and the friendship- support group could effectively improve sexual function for post-operation patients with CIN.
4.Expressions of ER, PR and CerbB-2 in papillary thyroid carcinoma and their clinical significances

Objective To explore the expressions of ER,PR and CerbB-2 in human papillary thyroid carcinoma,and their correlation with the clinical features.Methods The expressions of ER,PR and CerbB-2 were detected by immunohistochemistry in 89 papillary thyroid carcinoma samples and 13 nodular goiter samples.Results In papillary thyroid carcinoma samples,ER,PR and CerbB-2 were detected for 50.56 %(45/89),43.82 % (39/89),24.72 % (22/89),respectively.In nodular goiter,ER,PR and CerbB-2 were detected for 15.38 % (2/13),7.69 % (1/13),0,respectively,there were significant differences between the two groups (x2 =5.649,P=0.017,x2 =6.211,P=0.013,x2 =4.097,P=0.043).The positive rates of PR were 56.26 %(26/46) in without lymph node metastasis,compared with those lymph node metastasis 30.2 3% (13/43) were significantly higher,the difference was statistically significant (x2 =6.239,P =0.012).Conclusion The expressions of ER,PR and CerbB-2 are significant higher in papillary thyroid carcinoma.The detection of ER,PR and CerbB-2 might be helpful for early diagnosis and prognosis of papillary thyroid carcinoma.

6.Relationship between methylation status of Rap1 GTPase activating protein and colon cancer

Objective To explore the methylation status of Rap1 GTPase activating protein (Rap1GAP) promoter in colon cancer, and to provide the oretical basis and research direction for the early diagnosis, targeted therapy, anti-multidrug resistance of colon cancer and so on. Methods The paraffin embedded specimens of 33 patients with colonic adenocarcinoma diagnosed by pathology were analyzed from Department of Pathology of Xinzhou City People′s Hospital from January 2010 to September 2014, including 19 males and 14 females, and aged 41-72 years old. The paraffin embedded specimens of 16 patients with colonic adenoma were enrolled, including 9 males and 7 females, and aged 34-58 years old. 13 normal tissues from the tumor distal margin (from the tumor > 15 cm) were selected. Quantitative methylation specific PCR (q-MSP) was applied to detect methylation level of Rap1GAP gene promoter. The methylation level differences of Rap1GAP gene promoter region among 3 groups or between different clinicopathologic factor subgroups were compared. Results The methylation rates [median (interquartile range)] of Rap1GAP promoter were 65.43 % (50.35 %), 21.37 % (8.39 %) and 17.43 % (15.71 %) in colonic adenocarcinoma group, colonic adenoma group and adjacent normal tissue group, respectively. The methylation rate of colonic adenocarcinoma group was significantly higher than that of colon adenoma group or that of adjacent normal tissue group (P< 0.05). The methylation rates of Rap1GAP promoter in colonic adenocarcinoma were not correlation with age, sex, differentiation and the stage of TNM [ male vs. female: 42.74 % (70.44 %) vs. 21.98%(80.00%);≤60yearsoldvs.>60yearsold:36.26%(62.62%)and26.23%(76.42 %);well-differentiated vs. moderately/poorly-differentiated: 21.98 % (40.32 %) vs. 42.74 % (74.20 %); TNM Ⅰ-Ⅱ vsⅢ-Ⅳ: 25.31 % (48.27 %) vs. 36.26 % (75.55 %); all P> 0.05]. Conclusion The methylation status of RAP1GAP promoter maybe associate with genesis and development of colon cancer, which might be used as a target for early diagnose of colon cancer.
7.The value of studying liver function reserve in hepatic carcinoma by 13C-methacetin breath test

Objective To investigate the characteristic of 13C-methacetin breath test (13C-MBT) as a tool to monitor the hepatic function of patients with hepatic carcinoma by comparing with Child-Pugh classification and general liver function. Methods Thirty-nine patients with primary liver cancer, 16 patients with hepatic metastasis and 14 healthy volunteers serving as controls were included in this study. According to Child-Pugh classification, the primary liver cancer patients were divided into A, B and C subgroups. All subjects received 13C-MBT and routine liver function tests after an overnight fast. The three major parameters of 13C-MBT i.e. maximum excretion rate before 40 min (Mwnax40), 13CO2 cumulative excretion of 40 min(CUM40) and that of 120 min(CUM120) were recorded and the two metabolism curves (DOB curve, MV curve) were made. Results (1) In the control, hepatic metastasis and primary liver cancer groups, both the DOB curve and MV curve were similar in shape; the peak time occurred at about 20 min after administration and then the curves lowered progressively. There were significant differences between the primary liver cancer group and the other two groups, but it was not statistically different between the hepatic metastasis group and the controls. The shape was obviously distinct in the groups A, B and C of primary liver cancer. The group A had a single sharp peak curve, group B a relatively flat peak curve with a lower level for a long time after the ascending phase and group C no clear excretion peak or even a negative curve. (2) As to the three parameters of 13C-MBT, there were statistical difference between the primary liver cancer group and the other two groups(P <0. 05). Between hepatic metastasis group and controls,there was statistical difference about CUM120 (P <0. 05), but no statistical difference about Mvmax40 and CUM40. While in the three groups with primary liver cancer groups, there was statistical difference between group A and B in Mvmax40 and CUM40 ( P < 0. 05 ), but no statistical difference between group B and C. As to CUM120, there was statistical difference only between group A and C. (3) Comparing the three parameters of 13C-MBT with routine liver function tests, there was negative correlation with TBA, positive correlation with Alb, PA, ChE and no correlation with ALT, AST, TBil, γ-GT, ALP and PT. (4) There was a good consistency between 13C-MBT and Child-Pugh classification in the evaluation of liver function of patients with liver cancer ( Kappa = 0. 647, P < 0. 05 ). Conclusion The value of the three parameters of 13C-MBT is decreased with severity of the liver disease and 13C-MBT may be used to evaluate the reserved hepatic function in patients with primary liver cancer with a diagnostic value equivalent to Child-Pugh classification. The study further confirms that 13C-MBT has correlation with TBA,AIb, PA and ChE.
8.Construction of the quantitative structure retention relationship of cefdinir related substances.

The molecular descriptors of impurities with known structure in cefdinir were calculated, selected and associated with the chromatographic retention behavior to establish a model. This quantitative structure retention relationships (QSRR) model for the related substances of cefdinir was established under specific chromatographic condition and verified by other impurities. 12 molecular descriptors were used to establish the QSRR model, F_AFRBWF, Blbn_J, SsCH3, SssCH2, SsNH2, SssNH, SssS, SHdCH2, EEM_AFc, EEM_AFpl, EEM_XFpl and Pi_MaxQ. The relativity between true values and predictions in QSRR of cefdinir is R2 = 0.9836 (n = 18), ΔRRT is no more than 0.154, as 10.17% in RRT. The results indicate that the QSRR model for the related substances of cefdinir can be used to evaluate the analysis methods for related substances and predict the chromatographic behavior of new impurities, which will provide a new way for the evaluation of the effectiveness for drug quality control.
9.Effects of human lung cancer A549 cell line by FasL gene transduction

Purpose:To study the effect of FasL gene on human lung adenocarcinoma cell lines and possibility of ex- ogenous FasL gene for gene therapy of lung cancer.Methods:An adenoviral expression vector with full length cDNA of FasL gene insert was constructed(Ad-FasL) and transfected into A549 cells.The effect of exogenous FasL gene on the growth of A549 cells was examined in vitro and in vivo.Results:Expression of FasL gene in A549 cells was confirmed by FCM and RT-PCR.The in vitro growth of the Ad-FasL transfected A549 cells was significantly inhibited (inhibition rate: 84%) as compared to mock (Ad-LacZ) transfected A549 cells.Colony-forming activity in vitro of the Ad-FasL transfected A549 cells was completely inhibited.The Ad-FasL transfected cells became apoptotic which was confirmed by the appear- ance of pre-G_1 on flow cytometry (FCM).The growth of A549 xenografts in nude mice was retarded by intra-tumol injection of Ad-FasL.Conclusions:FasL gene participates in the induction of cell apoptosis.Its use in gene therapy of cancer is promising.
10.Expression of Rap1 GTPase-activating protein 1, matrix metalloproteinase 2 and matrix metalloproteinase 9 in colorectal carcinoma and their significance

Objective To investigate the expression of Rap1 GTPase-activating protein 1 (Rap1GAP),matrix metalloproteinase 2 (MMP-2) and matrix metalloproteinase 9 (MMP-9),and their relation with clinical patterns in colorectal carcinoma.Methods Immunohistochemistry was used to detect the expression of Rap1GAP,MMP-2 and MMP-9 in colorectal carcinoma,villous adenoma,tubular adenoma and normal colorectal tissue,and their relationship with clinicopathological parameters was analyzed.Results The positive rate of Rap1GAP expression was 30.4 % (14/46),77.8 % (14/18),69.6 % (16/23) and 95.2 % (20/21) in colorectal carcinoma,villous adenoma,tubular adenoma,and normal colorectal tissue,respectively (x2 =30.659,P=0.000).The figures were 71.7 % (33/46),55.6 % (10/18),52.2 % (12/16) and 9.5 % (2/21) for the positive rate of MMP-2 expression (x2 =22.459,P =0.000),as well as for 76.1% (35/46),61.1% (11/18),56.5 % (13/23) and 14.3 % (3/21) for the positive rate of MMP-9 expression,respectively (x2 =22.643,P =0.000).In patients with colorectal carcinoma,the expression of Rap1GAP was correlated with tumor differentiation (x2 =5.275,P =0.022),but not sex,age,or lymphatic metastasis (all P > 0.05).The expression of MMP-2 and MMP-9 were correlated with lymphatic metastasis (x2 =6.661,P =0.010;x2 =8.475,P =0.040),but not sex,age or tumor differentiation(all P > 0.05).There was a negative correlation between expression of Rap1GAP and MMP-2,MMP-9 in colorectal carcinoma,respectively (r =-0.424,P =0.003;r =-0.294,P =0.048),but no correlation between the expression of MMP-2 and MMP-9 (r =0.101,P =0.505).Conclusions Rap1GAP,MMP-2 and MMP-9 play important roles in the malignant biological behavior of colorectal carcinoma,and the expression of Rap1GAP is negatively correlated with MMP-2 and MMP-9.The interactions among the three affect the occurrence and development of colorectal carcinoma.
