1.Study on the intervention of caffeine citrate in the treatment of neonatal apnea

Objective To investigate the effect of caffeine citrate in the treatment of neonatal apnea and the corresponding intervention measures. Methods A total of 88 children with apnea were enrolled in this study from December 2015 to February 2017, and were randomly divided into control group and study group, 44 cases in each group.The study group on the basis of conventional therapy plus caffeine citrate, the control group on the basis of conventional therapy plus aminophylline group, two newborns with apnea were duration of treatment should be 7 for 7 days, record the treatment effect and the incidence of adverse reactions. Results The total effective rate was 88.64% in the study group and 72.73% in the control group (P<0.05). The incidence of adverse reactions was 11.36% in the study group and 40.91% in the control group (P<0.05). Conclusion The application of caffeine citrate treatment of apnea with clinical efficacy and safety of the ideal of the newborn, in the course of treatment given targeted clinical nursing intervention is conducive to the protection of newborns with apnea of quality of life and life safety.
2.Effect of CT scanning parameters on CT number

Objective To study the effects on tissue CT number caused by scan protocols.Methods The phantom was repeatedly scanned in different protocols by changing only one of parameters,such as X-ray tube voltage,mAs and recon kernel,while other parameters were ketp unchanged.The CT number of different materials in phantom were measured and analyzed.Results The CT numbers of tissues changed remarkably with the tube voltage and had different relativity for different tissues.The CT numbers had positive correlation with kV for such maierials as polyethyle,lexan,perspex,but for teflon the correlation was negative.The mAs and recon kernel had no effects on CT number.Conclusions The CT number of tissue changes with scanning X-ray tube voltage,so the setting of scan parameters should be taken into account in image diagnosis and radiotherapy.
3.Analysis of clinical effects of iliolumbar fixation in treating sacrum fracture of Denis type II.

OBJECTIVETo evaluate the clinical effects of iliolumbar fixation for the sacrum fractures of Denis type II.
METHODSThe clinical data of 86 patients with sacrum fracture of Denis type II treated by iliolumbar fixation from January 2008 to January 2012 were retrospectively analyzed. There were 55 males and 31 females, aged from 17 to 55 years old with an average of 39.1 years. Among them, 73 cases complicated with pelvis fracture and 13 cases with acetabular fracture; 37 cases with sacral neurological symptoms and 49 cases without sacral neurological symptoms. Fracture healing time, nerve function, clinical function and complications were observed in the patients.
RESULTSIn 86 cases, 6 cases were out of followed-up and 80 cases were followed up from 24 to 71 months with an average of 36 months. The mean fracture healing time was 13 weeks (ranged, 10 to 38 weeks). According to Gibbons scoring to evaluate the neurological function, preoperative nerve rehabilitation, lower limbs feeling, lower limbs activity,bladder and rectum function,total score respectively were 0.62 +/- 0.04, 1.54 +/- 0.35, 1.12 +/- 0.18, 0.23 +/- 0.01, 3.46 +/- 0.47 and postoperative respectively were 0.82 +/- 0.12, 0.36 +/- 0.04, 0.05 +/- 0.01, 0.03 +/- 0.01, 1.25 +/- 0.22, there were statistically significant differences between preoperative and postoperative (P < 0.05). According to Majeed scoring to evaluate the clinical function, postoperative pain, standing, sitting, sexual life, work ability, total score respectively were 22.54 +/- 4.02, 27.93 +/- 5.46, 8.47 +/- 3.61, 2.54 +/- 1.33, 16.46 +/- 4.34, 81.32 +/- 8.73, 60 cases got excellent results, 17 good, 3 fair. The main complications including fracture nonunion of 5 cases,deep incision infection of 1 case, and screw prominence resulting uncomfortable of 8 cases.
CONCLUSIONIliolumbar fixation has the advantages of stable fixation, satisfactory functional rehabilitation, less complications, and is a good method in treating sacrum fracture of Denis type II.

4.The influence of different nifedipine types on the blood pressure variability

Objective To observe the influence of the short effect antihypertension drugs- nifedipine and medial effect antihypertension drugs- extended release nifedipine on the blood pressure variability (BPV) in essential hypertension(EH). Methods Twenty-five EH patients were underwent 24-hour noninvasive ambulatory blood pressure monitoring (ABPM) and observed their BPV respectively before taking drugs, after taking nifedipine and extended release nifedipine. Meantime,25 normotensive controls (NC) were observed. Results (1)BPV in EH group was higher than that in controlled group and the severer the rise of blood pressure, the more obvious the increase of BPV (P 0.05). Conclusions Nifedipine could increase BPV but extended release nifedipine did not change BPV while they decreased blood pressure. Effect of extended release nifedipine was better than nifedipine in decreasing blood pressure.
5.The evaluation of obstructive sleep apnea syndrome using multi-slice spiral CT and 3D airway reconstruction technology

Objective To analyze the application of multi‐slice spiral CT and 3D airway reconstruction technique on obstruc‐tive sleep apnea syndrome (OSAS) of pharyngeal soft tissue and upper airway stenosis .Methods Selected 92 patients with OSAS in our hospital as the observation group and another 92 healthy persons were selected as control group ,all patients achieved multi‐slice CT and 3D airway reconstruction technical inspections ,counted and compared their pharyngeal soft tissue and upper airway stenosis .Results Airway stenosis of tongue back ,and hypopharynx retro‐palatal of control group were more narrow compared with the control group(P<0 .05);the observation group′s pharyngeal soft tissue was more thick than the control group ,so as the length and width(P< 0 .05) .Conclusion Multi‐slice spiral CT and 3D airway reconstruction technique can effectively share pharyngeal soft tissue and upper airway stenosis of patients with OSAS .
6.Ultra-high performance liquid chromatography tandem mass spectrometry method for detection of α-hydroxybutyrate

Objective To establish an ultra high performance liquid chromatography-tandem mass spectrometry (UHPLC-MS/MS) method for detecting α-hydroxybutyrate (α-HB) in serum.Methods Electrospray ionization negative ion and multiple reaction monitoring mode were used to detect serum α-HB.The linearity,low limits of quantification,precision,recovery and interference of UHPLC-MS/MS were evaluated.The reference interval of this method was established in 130 serum samples (62 males and 68 females) from Shanghai East Hospital.Dixon method was used to judge the outliers and K-S test was used to analyze the data normality.The standard curve was scored by linear regression analysis.Results The total run time was 4 min of UHPLC-MS/MS method for the determination of α-HB.It has a good linear relationship in the range of 0.5-40.0 mg/L(r=0.999 4);the low limit of quantification was 0.5 mg/L;the in-batch and inter-batch coefficient of variation precision were less than 4.1% and 6.3%,respectively;the recovery ranged between 95.8% and 103.8%.Hemolytic samples (about 5 g/L hemoglobin),lipemic samples (about 12 mmol/L triglyceride),icteric samples (about 150 μmol/L total bilirubin) had no significant interference to the detection.The reference range of the apparent healthy population was 1.46-6.48 mg/L.Conclusions A method for the determination of serum α-HB by UHPLC-MS/MS was established.The method was simple,rapid,and could be used for the detection of clinical samples.
7.Effects of external use of jiuyi dan for one month on blood and urine mercury levels and liver and kidney functions of rabbits.

OBJECTIVETo observe the changes of the blood and urine mercury (Hg) levels and liver & kidney functions of rabbits after administration of Jiuyi Dan (calcined gypsum-Sheng Dan 9: 1) for 1 month and the recovery of rabbits after the drug withdrawal.
METHODThe rabbits were randomly divided into 2 groups: the calcined gypsum group and the Jiuyi Dan group. After 36 mg of calcined gypsum and 40 mg of Jiuyi Dan were used on the surface of wound (5 cm x 5 cm) on one side of rabbit back for 4 h, the surfaces of wound were washed by saline. The bloods were taken from the rabbit hearts before and after the drug administration for 14 and 28 days, and after the drug withdrawal for 7, 40, 71, and 92 days for determining Hg level in blood, and liver & kidney function indicators (ALT, AST, CREAT and BUN). The Hg level in urine collected from bladders was examined while rabbits were dissected after the drug withdrawal for 1, 40, 71, and 92 days.
RESULTThe Hg level in blood was significantly increased (P < 0.01) after the rabbits were administrated with drugs for 14 and 28 days and after the drug treatment was stopped for 7 and 40 days. The Hg level in urine was significantly enhanced after the drug withdrawal for 1, 40, 71 days. However, the liver & kidney indicators were not influenced.
CONCLUSIONThe Hg level in rabbit blood and urine was significantly increased after the consecutive administration of double-dose Jiuyi Dan for 1 month. However, the blood Hg level and urine Hg level recover after the drug withdrawal for 71 days and 3 months, respectively. The liver & kidney indicators do not significantly change with the dose.

9.The effect of all-trans retinoid acid and sodium selenite (Na2SeO3) on VEGF and its receptor expression in HL-60 cells.

In order to investigate the effect of non-medullar toxicity drug - all trans retinoid acid (ATRA) and cancer preventive trace element-selenium compound - sodium selenite (Na(2)SeO(3)) on the expression of vascular endothelial growth factor (VEGF) and its receptor in HL-60 cells, the expression of VEGF and its receptor in HL-60 cells were detected by ELISA technique and flow cytometry before and after treatment with two drugs. The results showed that the mean VEGF concentrations in the cultural supernatant of 5 and 10 micro mol/L ATRA-treated HL-60 cells for 48 and 72 hours were lower than those of the control group without adding ATRA. The differences between the ATRA-treated groups and the control group were statistically significant (P = 0.001, P = 0.000, P < 0.01, respectively). The levels of VEGF-R on the surface of HL-60 cells also decreased after treatment with ATRA of 5 and 10 micro mol/L for 72 hours, but at 48 hours the expression rates of VEGF-R on HL-60 cells of the two ATRA treated groups were not significantly decreased. At 48 and 72 hours, Na(2)SeO(3) of 5 and 10 micro mol/L had no obvious effect on HL-60 secreting VEGF, but notablely inhibited the expression of VEGF-R. In conclusion, ATRA could inhibit the expression of VEGF and its receptor in HL-60 cell. Na(2)SeO(3) could not inhibit the expression of VEGF in HL-60 cell, but could decrease the receptor expression of VEGF, which mechanism should be further studied. ATRA and Na(2)SeO(3) had not obvious medullar-inhibition, but anti-angiogenesis activity. It is suggested that combination of two drugs with conventional therapy may enhance the effect of radiotherapy and chemotherapy, and reduce the dose and thus toxicity of chemotherapeutic agents.
