Objective To investigate the retinal toxicity and verify the safe dose of intravitreal injection of nonsteroid anti inflamatory drug, diclofenac sodium. Methods Twenty eight healthy adult white rabbits were divided at random into 7 groups and received in every right eye the intravitreal injection of a single dose of diclofenac sodium solution ranging from 0.4～1.0 mg/0.1ml respectively,the left eyes were regarded as control ones. Before injection and on the 1st, 3rd, 7th, 14th, 21st, and 28th day after injection the electroretinography on both eyes was examined. On the 28th day after injection the retinas of two rabbits of every group were examined by using light microscopy. On the 10th and 30th day after injection the retinal tissues around the optic nerve disk of two eyes from every group at random were tested by using transmission electron microscopy. Results The ratio of amplitude of b wave of electroretinography in 0.4 mg and 0 5 mg groups had no significant difference from groups before injection,the retinal tissues showed no structural changes in light and electron microscopy examination. Objective Investigation and analysis of intelligence and psychomotor function in children born after implementingt universal salt iodization(USI). Methods Historical serious illness areas of water iodine below 10μg/L were selected as study sites, water iodine in 50 - 100 μg/L in the historical non-endemic areas were as control points in Henan, 2008. Cluster sampling was used to select children aged 6 - 15 years as observing subjects,IQ were measured with CRT- Man Test(CRT-C2). A "Tianjin Medical psychomotor test battery" (JPB) was carry out to test psychomotor function. Results In IDD regions 230 children were surveyed post-USI and 1284 children preUSI. The IQs post and pre USI were 99.3 and 99.9, respectively, and the proportion of IQ ≤69 were 2.17%(6/230) and 2.80%(36/1284), respectively. In non-IDD regions 650 children were surveyed post-USI and 2079children pre-USI. The IQs post and pre USI were 95.3 and 93.8, respectively, and the proportion of IQ ≤ 69 were 2.31%(15/650) and 3.37%(70/2079), respectively. In IDD regions, the abnormal rate of T scores and damage index post USI were 3.6%(2/56), 5.3%(3/56), respectively, significantly lower than pre USI [18.1%(29/160),18.1%(29/160), x2 = 7.54, 6.86, all P ＜ 0.01]. Conclusions USI could increase the IQs of children and decrease the positive rate of JPB, and significantly improve the quality of whole nation and persistently eliminating IDD.

At present, MRI is the main means in discriminating benign and malignant compression fracture of vertebrae. Sometimes the change of MRI signal in them is overlap, therefore, it is difficult to diagnose accurately. Modified new drugs R&D in China should be based on clinical demand and focused on the differentiation research in order to obtain new preparations with signif-icant clinical advantages to better serve the clinical practise. This paper states the concept and characteristics of modified new drugs in China,makes case analysis and shares the development history of blockbuster modified new drugs in the world,clarifies the opportuni-ties and challenges and provides ideas for the future development of modified new drugs in China.

Objective To understand the fairness of health resources of endemic diseases institutions in Henan Province,to provide evidence for endemic allocation of health resources for the government.Methods Health-related data resources in endemic disease prevention and control institutions were collected in Henan Province,including personnel,business space area,the value of equipment,and health funding,etc.The percentages of population distribution and geographical distribution of health resources were calculated; Lorenz curves of demographic fairness and geographic fairness were drawn.According to Lorenz curve (Gini coefficients of the population and the geographical distribution were calculated based on Lorenz curve),Gini coefficients were used to evaluate the fairness of the endemic disease prevention institutions for health resource allocation.Results As can be seen from Lorenz curve,the population fairness of endemic disease prevention and health personnel was the best,but the number of senior officers was less and the total value of equipment owned was low.The geographical fairness of all the indicators of health resources was poorer,and geographic fairness among senior officers was the worst.The Gini coefficient of health technical personnel,senior personnel,undergraduate or above personnel,value of equipments,area of business room and health funding allocation determined by the population distribution was less than 0.4.The health resources were relatively fair and reasonable according to the distribution of population.The Gini coefficients of all the indicators determined by the distribution of geography were higher than those determined by the distribution of population.Furthermore,the geographic Gini coefficient of senior personnel was above 0.4 which was in the vigilance state.Conclusions The equality of health resources determined by population distribution in the institutions for endemic diseases control in Henan is superior to those determined by geographic distribution.The allocations of health resources of endemic diseases control in different areas are unbalanced.

Objective To evaluate the value of high resolution MR imaging (MRI) with diffusion weighted imaging (DWI) in the preoperative diagnosis of rectal cancer. Methods 55 patients with rectal cancer may underwent high resolution MRI examinations before surgical operation, and with DWI (b=600, 1000 s/mm2). The apparent diffusion coefficient (ADC) of the tumors were measured. The image findings were compared with pathology. Select 15 patients without rectal lesion, ADC value of the tumors and normal rectal wall were measured. Results High resolution MRI diagnosis rectal cancer sensitivity, specific degree and coincidence rate were 95.7% (45/47), 50.0% (4/8), 89.1% (49/55), high resolution MRI with DWI diagnosis colorectal cancer sensitivity, specific degree and coincidence rate were 100.0 % (47/47), 75.0 % (6/8), 96.4 % (53/55). The high resolution MRI tumor T-staging agreed with the histological stage that was 88.9% (40/45), the diagnosis of T2 stage was 87.5 % (14/16), the diagnosis of T3 stage was 89.6 % (26/29). The same b-valued under the ADC values of rectal cancer tumors and the normal rectal were different (P ＜0.05). No correlation can be pointed out between ADC and pathological classification of each tumor.Conclusion High resolution MRI with DWI for the rectal cancer preoperative diagnosis and T-staging has higher diagnostic accuracy, DWI can serve as an important supplement sequence of colorectal cancer diagnosis.

Objective To create a stable and reliable model for cerebral ischemia.Methods (1)Distal middle cerebral artery occlusion (dMCAO) model: SD rats of 270-350 g in weight were anesthetized using isoflurane.Both common carotid arteries (CCA) were exposed and occluded for 30 min.Via a bone window between the left eye and ear, the exposed left middle cerebral artery was cauterized and cut during bilateral CCA occlusion.(2) Evaluation of the model: microPET study was performed at 10 h after surgery.microMRI scan were done at 24 h.TTC staining were done at 48 h.Behavioral tests, including vibrissaeelicited forelimb placement test, were done from day 2 to 60 after surgery.Tissue damage was evaluated using cresyl-violet staining Mortality was also observed.Results Infarction areas were 54.50% ± 3.15%(95% CI:49.49-59.51 ) using microPET scanning, 45.30% ± 2.35% (95% CI:42.94-47.86) using microMRI scanning, 43.39% ± 2.33% (95% CI:40.94-45.84)using TTC staining, and 30.10% ±2.22% (95% CI:28.05-32.15) using CV staining.The behavioral test scores were lower in the ischemic group than in the sham control group.This dMCAO model was successfully performed in all rats, and the mortality rate was 0.Conclusions Our results suggest that permanent dMCAO plus bilateral CCA occlusion for 30 min can produce a stable and reliable model suitable for research on cerebral ischemia.

Objective To investigate sedation of dexmedetomidine versus midazolam on me-chanically ventilated patients.Methods Seventy-six mechanically ventilated patients were randomly divided into midazolam group (group M)and dexmedetomidine group (group D).The loading dose and maintenance dose of midazolam was 0.06 mg/kg and 0.04-0.2 mg·kg-1 ·h-1 ,and 1 μg/kg and 0.2-0.7 μg·kg-1 ·h-1 for dexmedetomidine.The drug dose was adjusted according to Ramsay score scale.Time when Ramsay reach 2-4 score,the length of ICU stay,mechanical ventilation time,drug adverse reactions,the levels of inflammation factors were recorded and compared between two groups.Results The length of ICU stay and arousing time in group D was significantly lower than that of group M (P <0.05).The levels of TNF-α,hs-CRP in group D were lower after 24 hr treat-ment than that of group M(P <0.05).The levels of hs-CRP in group D were lower after 48 hr treat-ment than that of group M (P <0.01).Conclusion Dexmedetomidine treatment reduced the lengh of ICU stay and levels of inflammatory factors of mechanical ventilated patients.
