2.The therapeutic effect of Pingyangmycin lipiodol emulsion embolization for hepatic cavernous hemangioma

Objective To evaluate the therapeutic effect of Pingyangmycin lipiodol emulsion (PLE) embolization for cavernous hemangioma of liver (CHL). Methods Seldinger technique was adopted and the catheter was super-selectively sent into the supplying artery of tumors in 18 CHL patients, 5-10 ml of Lidocaine and 5-20 ml of PLE were slowly injected. Before the embolization, 50-100 mg of pethidine was routinely injected. The embolization was conducted in several times in patients with large tumors, with multiple supplying arteries, and whose age was above 60 years. During the 3-48 months'follow-up after the operation,the change of the tumor diameter before and after the embolization was compared by using CT and ultrasound. The clinical symptomatic relief and the complications were also observed. Results The CHL of all the 18 cases were filled up by PLE. The foci were completely vanished in 9 cases, reduced by over 50% in 7 cases,and reduced by 25%-50% in 2 cases,respectively. The follow-up angiographic examination was performed in six cases, the tumor staining was completely vanished and the tumor-supplying artery was closed up. The improving rate of the clinical symtoms reached 89% . There were no serious complications. Conclusion The PLE embolization in hepatic cavernous hemangioma proves to be reliable, safe, minimally invasive, and with little side effect. This therapy, therefore, can be the first method of choice in treating the cavernous hemangioma of the liver.

Objective:To determine the expression of Mcm2(a member of MCMs family) in colorectal neoplasm and to assess the relationship between Mcm2 expression and clinicopathological characteristics of colorectal neoplasm.Methods: Immunohistochemical staining was used to detect the expression of Mcm2 in normal colorectal mucosa(n=23),adenoma(n=33),and cancer(n=69) tissues,and the results were quantified by calculating the labelling index(LI).Results: Mcm2 expression in normal colorectal mucosa was confined to the 1/3-1/2 of colonic crypts basal;there was no expression in coloncytes surface.In colorectal adenoma and cancer,Mcm2 expression was seen throughout epithelium,including the coloncytes surface.The median(range) LI of Mcm2 in these 3 tissues were 18%(5%-37%),35%(15%-75%),66%(41%-90%),respectively.The differences between normal mucosa,adenoma and cancer were statistically significant(P
4.Establishment of a assessment model for the clinical efficacy of neoadjuvant chemotherapy in nasopharyngeal carcinoma

Objective To establish a model to predict the clinical response of neoadjuvant chemotherapy for nasopharyngeal car‐cinoma ,and provide basis for the individual treatment .Methods The clinical data of 63 cases of advanced nasopharyngeal carcinoma patients who have received neoadjuvant chemotherapy in the past 2 years were analyzed retrospectively .Univariate and multivariate analyses were performed using the Logistic analyses to identify efficacy factors .Results The response rate in nasopharyngeal tumor and lymph node metastasis were 39 .7% and 50 .8% ,respectively .Single factor analysis showed that patients with no distant metas‐tasis ,cranial nerve inviolated ,EBV negative and high expression of Ki67 were more sensitive to therapy .Logistic analysis showed that the influencing factors for the effect of the new chemotherapy include :distant metastasis ,cranial nerve inviolated and EBV . Thus ,the prediction model would be:Logit= -0 .470 -2 .863 × distant metastasis + 1 .328 × cranial nerve invasion+ 3 .639 × EBV ,its sensitivity ,specificity ,positive predictive value and negative predictive value were 79 .4% ,82 .8% ,84 .4% and 77 .4% . Conclusion The distant metastasis ,cranial nerve invasion and EBV infection were important predictive factors for neoadjuvant chemotherapy of nasopharyngeal carcinoma .This model could be used to predict the response of patients with nasopharyngeal carci‐noma .
5.The interventional therapy of the metal foreign matter in the stomach

Objective To investigate the interventional therapy and its value in the metal foreign matter in the stomach. Methods Eight patients with metal foreign matter in the stomach was studied. All patients were male, and their age ranged from 28 to 46 years with the mean age of 32.2 years. All patients had the medical history of swallowing metal foreign matter in compulsory detoxification or imprisonment. The catheter was inserted into the stomach lead by guide wire lubricated by paraffine. Then the guide wire was withdrawn and a 2.6 m long guide wire was folded in the middle and was inserted into the sromach through the catheter. A loop was made on the guide wire, and the loop was controlled to to hitch the forigen mater, then the guide wire was drawn out slowly . Results A total of 12 metal foreign matters in the stomach in all 8 patients were taken out safely, and no comqlications occurred. Conclusion The interventional therapy for the metal foreign matter in the stomach is simply, minimal invasive, cheap, effective, and with little complication. This therapy is a clinic treatment, the patient is glad to accept, and is the ideal therapy for foreign matter in the stomach.

7.Endoscopic treatment of choledocholithiasis in patients with decomipensated cirrhosis

ObjectiveTo explore the safety and efficacy of endoscopic treatment for choledocholithiasis in patients with decompensated cirrhosis.MethodsA total of 104 cases of choledocholithiasis with decompensated cirrhosis were treated by ERCP in our hospital between January 2001 and March 2011.ChildPugh grading and model for end stage liver diseases (MELD) were investigated pre-ERCP and 2 weeks post-ERCP.Complication and mortality during the follow-up were recorded.ResultsThe success rates of complete stone retrieval was 92.3% (96/104),and plastic stent was placed in other 8.Major complications ineluded post-ERCP bleeding in 9 cases (8.7%) and pancreatitis in 8 (7.7% ).MELD score valuated 2weeks after ERCP ( 10.1 ± 6.3 ) was significantly lower than that of pre-ERCP ( 1 1.9 ± 6.2,t =2.22,P <0.05).The Child-Pugh grading before ERCP of 104 patients were 28 in grade A (26.9% ),52 in grade B (50.0% ) and 24 in grade C (23.1% ),which was significantly different from those of 2 weeks after ERCP (40/52/12).No death was recorded during hospital stay for choledocholithiasis,and 2 patients of Child-Pugh grade C died in 3 months after discharge.ConclusionERCP is an effective method for choledocholi-thiasis in patients with decompensated cirrhosis.The main severe complication was post-ERCP bleeding.Those patients with decompensated cirrhosis would benefit from ERCP if we took effective measures.
8.Primary observation of clinical curative effect of superselective uterine artery embolization for treatment of placenta increta

Objective To explore the curative effect on superselective uterine artery embolization for treatment of placenta inereta.Methods Pelvic arteriography was performed to confirm bleeding vessels.Then a 5 F Cobra catheter was inserted superselectively into uterine artery ipsilateral to bleeding,through which methotrexatum(MTX)and gelatin sponge were injected for embolization.After the procedure,bleeding,blood pressure,dischargement of placenta tissue,uterine recuperation,and plasma β-HCG were monitored.Results Bleeding vessels were confirmed in all of the 5 cases of placenta increta.Uterine artery embolization was successful at sole procedure.The operation time was 25.0 to 60.0 min.with the mean time (37.4±5.8)min.Vaginal bleeding stoped in 3.0 to 12.0 minutes after embolization and the mean time was(5.7±2.4)min.Blood pressure returned to normal after operation and vital signs were stable.Placenta tissue discharged on the 5th day to the 4th week after embolization and the mean time was 17 d.The uterus recuperated and blood β-HCG recovered simutaneously.The menstruation and ovulation during follow-up returned to normal.Conclusion Superseleetive uterine artery embolization for treatment of placenta increta has advantages such as short operation time,minimal invasion,definite curative effect and reservation of uterus,which is worthy in clinical application.
9.Clinical effect of San'ao Pian on treating subacute cough induced by airway inflammation

Objective To investigate the clinical effect and safety of San' ao Pian's treatment on subacute cough induced by airway inflammation.Methods Ninety cases with subacute cough induced by airway inflammation were selected as our subjects.They were randomly divided into control group(n =45) and San'ao Pian group(n =45).Patients in control group were given regular treatment,and in San'ao Pian group were received San'ao Pian's(1.0 g/time oral,3 times/day) for 2 weeks plus regular treatment.The changes of clinical symptoms such as cough,sputum,and wheeze were observed and recorded at the 3th,7th,14th day.Results (1) The main symptoms regarding of cough were improved in two groups(P < 0.01).The day cough symptoms score in San'ao Pian group were (2.18 ±0.62),(1.22 ±0.46),(0.83 ±0.45) at the 3th,7th,14th days after treatment,lower than that of control group ((2.78 ± 0.55),(2.05 ± 0.41),(1.86 ± 0.68)) and the differences were significant(F within group =10.23,P < 0.05 ; F between =8.46,P < 0.05 ; F intercross group =12.05,P < 0.05).The same trend was seen in night cough symptoms score ((2.12 ± 0.51) vs.(2.38 ±0.38),(1.18±0.44) vs.(1.85±0.49),(1.01 ±0.32) vs.(1.24±0.37) ;Fwithingroup=6.38,P < 0.05 ; F between group =7.59,P < 0.05 ; F intercross group =8.13,P < 0.05).(2) The total efficacy rate of San'ao Pian's was 86.7%,including 22 cases were cured,17 cases markedly effective,higher than that of control group (68.9%),including 14 cases were cured and 17 cases markedly effective,and the difference was significant.Conclusion San'ao Pian's is proved with better clinical effect in terms of treating subacute cough induced by airway inflammation.
10.Safety and effect of femtosecond laser-assisted cataract surgery combined with Cionni capsular tension ring implantation in the management of traumatic lens subluxation

AIM:To investigate the safety and effect of femtosecond laser-assisted cataract surgery with Cionni modified capsular tension ring (MCTR) implantation in the management of traumatic lens subluxation.METHODS: Totally 11 patients (11 eyes) with traumatic lens subluxation were divided into three groups according to the severity of lens dislocation, ranging from 90° to 120° (4 eyes), 120° to 180° (5 eyes) and 180° to 270° (2 eyes).The contact LenSx femtosecond laser cataract surgery platform was applied to create the capsulotomy, prepare nuclear fragmentation and make corneal wound creation.Anterior vitrectomy was performed in some patients during the surgery.After capsular retractors insertion and phacoemulsification, the MCTR was inserted to the capsular bag and fixed to the sclera.Finally, the IOL was implanted into the capsular bag.Postoperative visual acuity, intra-and post-operative complications, anterior capsular opening, IOL and MCTR position and intraocular pressure (IOP) were assessed.RESULTS:The duration of follow-up was 2mo.All the operations were completed successfully.Five eyes underwent cataract surgery combined with anterior vitrectomy.Four eyes had been inserted with 2-eyelet MCTR and seven eyes with 1-eyelet MCTR.The best corrected visual acuity (BCVA) after operation was better than 0.5 in 4 eyes, between 0.3 and 0.5 in 3 eyes, between 0.1 and 0.3 in 3 eyes, and less than 0.1 in 1 eye.Compared with preoperative BCVA, the difference was statistically significant (P<0.05).All the IOLs were stably centered and the eyelet of MCTR was fixated steadily between the iris and the anterior capsule.The common intra-and post-operative complications were subconjunctival hemorrhage, incomplete capsulotomy, residual cortex, secondary glaucoma and posterior capsular opacification.CONCLUSION:Femtosecond laser-assisted cataract surgery can improve the success rate of capsulorhexis, and reduce the difficulty of nuclear fragmentation.Femtosecond laser-assisted cataract surgery combined with MCTR implantation is an ideal surgical method for traumatic lens subluxation.
