1.Comparison of transesophageal arterial blood oxygen saturation monitoring and finger pulse oximeter

Objective To investigate the sensitivity and accuracy of monitoring oxygen saturation of arterial blood flowing through descending aorta with a probe placed in the lower segment of esophagus ( SeO2). Methods This study was approved by the ethics committee of our hospital and written consent was obtained. Forty-five ASA Ⅰ-Ⅲ patients (34 males, 11 females) aged 14-79, weighing 40-90 kg undergoing elective (36 patients) or emergency (9 patients) surgery were studied. After induction of anesthesia a probe we made (patent number ZL 200320115080.2) was placed in the lower segment of esophagus and connected to a monitor (Philips-150A) . The depth of insertion and the direction of the probe were adjusted until the same waveform as SpO2 was obtained. Once the best waveform was obtained, 10-15 min stabilization was allowed. The ventilator was then disconnected. When SeO, decreased from 100% to 90% mechanical ventilation was started again. The following parameters were recorded:(1) the onset time (the time from disconnection of ventilator to the time when SeO2 and SpO2 started to decrease), (2) the time needed for SeO2 and SpO2 to decrease from 100% to 95% (T95) and 90% (T90), and (3) the recovery time (the time needed for SeO2 and SpDO2 to return to 100% after mechanical ventilation was started again. Arterial blood samples were taken for blood gas analysis when Se02 and SpO2 were 90% , 95% and 100% . Results SeO2 , SpO, and SaO2 were 100% when the patients were being ventilated with 100% oxygen. When the ventilator was disconnected SeO, dropped much faster than SpO2. It took (286?15)s for SeO2 to drop from 100% to 95% while for SpO2 it took (398 ?28)s. The most readings of SeO2 were in the range of 2 standard deviation from SaO2. SeO2 correlated well with SaO2 and SpO2. r was 0.9754 and 0.9656 respectively(P
2.Metabolic Flux Analysis of L-Arginine Fermentation in Corynebacterium glutamicum

In this paper, metabolic networks of the Corynebacterium glutamicum GWY020 and the two de-rivatives carrying additional mutations HUI821and GUI089 were established and modified. The concentra-tions of extra-cellular metabolites were determined under sub-steady-state (50 h~52 h) of the batch culture. The metabolic flux distribution maps of the three strains were obtained, compared and analyzed. These re-sults indicate that the introduction of analog supersensitive marker or analog resistant marker skew the metabolic flux towards the formation of L-Arginine. This study revealed the usefulness of the metabolic flux analysis as a tool for verification of existing production strains. The analysis may play an important role in helping us to rationally re-design metabolism for further improvement of fermentation process.
3.Receptor activator of nuclear factor kappa-B ligand induces osteoclast precursor culture and differentiation

BACKGROUND:Previous studies have applied long-bone mechanical separation method to obtain osteoclasts, which are terminal y differentiated cells and cannot further proliferate. Therefore a large number of osteoclasts can be harvested with bone marrow cells inducing culture method and RAW264.7 cells inducing culture method to meet clinical requirements.
OBJECTIVE:To investigate the optimal method of receptor activator of nuclear factor kappa-B ligand (RANKL) induced osteoclast precursors to differentiate into mature osteoclasts.
METHODS:After bone marrow cells were isolated from mouse, RANKL and macrophage colony stimulating factor were added into the medium together, or RAW264.7 cells were cultured with RANKL to induce osteoclasts. The osteoclast precursors were treated with different concentrations of RANKL to observe the number of generated osteoclasts and evaluate the dose-effect relationship between RANKL and osteoclastogenesis. Annexin V-FITC and propidium iodide staining were used for flow cytometry to analyze the mononuclear-macrophage apoptosis during osteoclastogenesis.
RESULTS AND CONCLUSION:When 10μg/L RANKL was used, the peak of osteoclastogenesis appeared at days 6-7;when the concentration of RANKL was up to 100μg/L, the peak appeared at days 4-5. The number of new osteoclasts was dose-dependent on the RANKL concentration. 50μg/L of RANKL was the turning point in the fitted curve from osteoclastogenesis and RANKL concentration. After the RANKL concentration was more than 150μg/L, the number of osteoclasts slowed down obviously. RANKL can induce monocyte-macrophage to form osteoclasts and promote monocyte-macrophage apoptosis. The highest number of osteoclasts would be obtained to each unit of RANKL when monocyte-macrophage cells were cultured with 30μg/L of RANKL in the same vaccination density (104/cm2). Experimental findings indicate that, RAW264.7 cells or bone marrow cells inducing culture methods are simple and feasible, the optimum cellseeding density was 104/cm2;the appropriate stimulation concentration of RANKL was 30-50μg/L.

Objective To investigate the effect of Clopidogrel on the plasma endothelin(ET)and calcitonin gene related peptide(CGRP)in patients with unstable angina pectoris(UAP).Methods Patients with UAP were divided into two groups,the regular treated group(n =50)was treated with Isoket and the Dansben group(n =50)was treated with combined Isoket and Clopidogrel.The plasma ET and CGRP were measured before and after treatment by radioimmunoreactive assay.Data were compared between the two groups and also compared with those measured in 20healthy subjects for control.Results ET level was significantly higher and CGRP level was lower in UAP patients than that in healthy subjects(P < 0.05).After Clopidogrel treatment,ET was significantly decreased(P < 0.01)and CGRP increased(P <0.05)in combined treatment group compared with patients received lsoket only.Conclusion Clopidogrel was an effective drug for UAP treatment,which could efficiently regulate the ET and CGRP metabolism.

We herein overviewed the history,achievement and plan of the twelfth-five program for the development of key medical disciplines in Zhejiang Province.We summarized the practice and characteristics of establishing of the key medical discipline in the Zhejiang Province,and a systemic survey was conducted on the effectiveness of the program.We then proposed several improvements strategies including academic training,multidiscipline coordination,and fund managements to eventually establish the new pattern of discipline development.
6.Clinicopathologic characteristics of intestinal T-cell lymphoma

Objective To study the clinicopathologic characteristics and differential diagnosis of T-cell immunophenotype in intestinal non-Hodgkin's lymphoma(NHL).Methods The clinicopathologic characteristics of 13 cases with intestinal T-cell lymphoma were analyzed by light microscopy and immunohistochemistry(Envision detection method).Results The lesions of 8 cases with T-cell lymphoma were found on the small intestine and 5 on the colon.Grossly,8 cases showed ulcer pattern,3 polypoid pattern and 2 presented as a regional thickening of intestinal wall.The tumor cells were medium to large size with pleomorphic nuclei and inflammatory background.The neoplastic lesions expressed the immunophenotype of peripheral T cells.The neoplastic cells of 13 cases(100%)expressed leukocyte common antigen(LCA);10(76.9%)cases expressed CD3;9(69.2%)CD45RO;5(38.5%)EB virus(EBV);3(23.1%)CD56 and 2(15.4%)vimentin(VIM).All the cases were negative for CD20,CD79a,CK,CDX2,NSE,CgA and CD117.ConclusionIntestinal T-cell lymphoma is a rare,aggressive neoplasm with poor prognosis and should be distinguished from other malignant tumors of intestine.

AIM: To establish the quality standard for Gubiling Capsules (Penis Et Testis Canis, Radix Noto ginseng, Radix Achyranthis Bidentatae, Herba Epimedii, Radix Aucklandiae, etc.). METHODS: Radix Achyranthis Bidentatae, Herba Epimedii, Radix Aucklandiae were identified by TLC, and the ginsenoside Rg 1 content was determined by HPLC. RESULTS: Ginsenoside Rg 1 showed a good linear relationship in the range of 1.2-6.0 ?g ( r = 1.00 0 ) and average recovery was 96.9%. RSD was 0.4%. CONCLUSION: These methods are simple, accurate and specific and can be used for the quality control for Gubiling Capsules.

10.Three dimensional conformal radiotherapy with late course accelerated hyper-fractionation for ad- vanced esophageal carcinoma

Objective To evaluate the results and toxic effects in three dimensional radiotherapy (3DCRT)with late course accelerated hyper-fractionation for advanced esophageal carcinoma.Methods From December 2001 to December 2003,115 patients were randomly divided into two groups:in 3DCRT group,55 patients were given late course accelerated hyper-fractionation radiotherapy to a total close of 66 Gy (2.0 Gy per fraction,5 times per week;to a dose of 36 Gy were changed into 1.5 Gy/f,2 f/d).In CF group,60 patients were given late course accelerated hyper-fractionation radiotherapy to a total dose of 66 Gy.Results The 1-, 2-,3-,4-year survival rates in 3DCRT group were 63.6 %, 50.9 %, 38.2 %, 30.9 % compared to 58.3 %, 38.3 %,33.3 %,23.3 % in CF group(X~2=4.44,P=0.031),and local control rates in 1-,2-,3-,and 4- years in 3DCRT group were 72.7 %,58.1%,47.2 % and 36.3 %,compared to 53.3 %,40 %,28.3 %,and 20 %(X~2=5.33,P=0.013)in CF group.However,in the 3DCRT group,the incidence of acute esophagitis was similar to CF group; hematogenous toxic and acute bronchitis were similar between the two groups. Late course lung injury in CF group was higher than that in 3DCRT group.Conclusion 3DCRT is able to improve the local control rate and survival rate,and unable to improve acute and late radiation toxie effects.
