1.Primary malignant pericardial mesothelioma:clinicopathological and immunohistochemical analysis

Objective To explore the clinicopathological and immunohistochemical features of primary malignant pericardial mesothe- lioma.Methods Seven cases of primary malignant pericardial mesothelioma,in 4 females and 3 males,with the age ranged from 22 to 51 and a mean of 40 years old,were studied with clinicopathological,immunohistochemical and histochemical techniques,The pertinent litera- ture was also comprehensively reviewed.Results 3 biopsy specimens and 4 surgical specimens,including 2 autopsy specimens,were stud- led.The main clinical symptoms were palpitation and short breath,which were aggravated after movement.The longest course was 4 years,and the mean disease course was 12 months.The chest films demonstrated an obviously increase in heart shadow,and the pericardi- um was thickened in some cases.CT clearly showed tumorous shadow in 2 cases.The histopathologic picture showed tubular type in 2 ca- ses,papillary in 2,signet ring cell type in 1 and myofibroblast types in 2.In 2 autopsy specimens it was found that pericardium was dif- fusely thickened.In one case the tumor was metastasized to the right lung,mediastinum,and pulmonary hilar lymph node.In another case there was metastasis to right kidney.Immunohistochemical staining showed that the neoplastic cells of all cases were positive for CK5/6, calretinin,CK.vimentin and EMA,but negative for CEA.In the histochemical staining seven cases,the malignancy was positive for CI to different extent,but negative for HCL Conclusion Immunohistochemistry and histochemistry play an important role in diagnosis and dif- ferential diagnosis for primary malignant mesothelioma and other tumors in pericardium.
2.Reverse hybridization applied in detection on human papillomavirus infection of twenty-three subfamilies

Objective To establish a method of reverse hybridization to detect five subfamilies of low risk Human Papillomaviruses(HPV6,11,42,43 and 44)and eighteen subfamilies of high risk HPV (HPV16,18,31,33,35,39,45,51,52,53,56,58,59,66,68,73,83 and MM4)in one reaction.Methods Special probes for twenty-three HPV subfamilies were fixed on nylon membrane bars,biotin labeled general primers mediated polymerase chain reaction(GP-PCR)were applied in HPV DNA amplification.PCR amplified DNA fragments were reversely hybridized with special probes that were fixed on the membranes. All samples(136)detected by reverse hybridization method were paralleled with the methods of Hybridization Capture Ⅱ(HC-Ⅱ)and sequencing.Results Positive rate of the 136 samples detected by reverse hybridization was 41.9%,while HC-Ⅱ 42.6% and sequencing 40.4%.Reverse hybridization detection indicated coherence with the other two methods(Kappa 0.8644 and 0.9089,respectively).While sequencing was lab standard for DNA test,the sensitivity was 96.36%,specificity was 95.06%,accuracy was 95.59%.Conclusions Method of reverse hybridization is adaptable to 23 kinds of HPV subfamilies, which can confirm the exactly subfamilies of HPV infection.This method is adaptable in clinical detection of HPV,with high sensitivity,high specificity,simply and convenient operation and the results are easily to be read.
3.Application of percutaneous transhepatic biliary drainage and percutaneous transhepatic insertion of biliary stent in treating malignant biliary obstruction

Objective To evaluate the clinical efficacy of percutaneous transhepatic biliary drainage (PTBD) and pcrcutaneous transhcpatic insertion of biliary stent (PTIBS) for malignant biliary obstruction.Methods PTBD or PTIBS were performed in 56 patients with malignant biliary obstruction, which were aused by hepatic carcinoma (n = 14), biliary duct carcinoma (n = 11), gallbladder carcinoma (n = 5),stomach carcinoma accompanied with metastasis of lymph node (n = 14), carcinoma of ampulla (n = 1 ) or carcinoma of pancreatic head (n = 11 ). The diagnosis was confirmed by ultrasonography, CT or MRI in all patients. The obstructed site was well identified, including high obstruction in 19 patients and lower obstruction in 37 patients. Based on the imaging findings, suitable interventional procedure was employed.Results PTBD or PTIBS were performed successfully in all 56 patients, of them PTBD was adopted in 11,PTIBS in 40 and both PTBD and PTIBS in 5. The serum total bilirubin decreased from (295.65±152.86)μmol/L before the procedure to (151.05 ± 107.36) μmol/L after the procedure, (P < 0.01 ). Postoperative infection could affect the fading of jaundice (P < 0.01 ), but the location of the obstruction carried no relationship with the fading of jaundice (P = 0.063). Conclusion Both PTBD and PTIBS are safe and effective palliative therapies for malignant biliary obstruction, which can markedly relieve patient of jaundice,improve the quality of life and elongate the survival period.
4.Enzymatic degradation of marine polysaccharide YCP and analysis of YCP-derived fragments

Aim: To explore the relationship between structure and immunological activity of marine polysaccha-ride YCP,the physicochemical property and immunological activity of the YCP-derived fragment were studied.Methods: YCP was hydrolyzed by α-amylase from human saliva.The hydrolysate was purified to obtain an polysaccharide fragment by gel filtration chromatography.The physicochemical properties of this YCP-derived fragment was characterized by HPLC,FT-IR and TLC.In addition,changes of phagocytic activity,production of reactive nitrogen and macrophage binding were investigated.Results: The relative molecular weight of YCP-de-rived fragment was approximately 6.6 × 10~3.The monosaccharide composition and FT-IR of the YCP-derived frag-ment were identical to YCP.No significant effect of the YCP-derived fragment on NO production and murine mac-rophage phagocyte were observed.And this fragment was not able to compete the binding between YCP and mac-rophages.Conclusion: The remarkable decrease of immunological activity of YCP-derived fragments degraded byα-amylase of human saliva suggests that the complete structure and high molecule weight of YCP are essential for its immuno-modulatory activity.
5.Diagnosis value of spiral CT for lymph node metastasis in thoracic esophageal carcinoma

Objective To analysis the spiral CT scan diagnosis value of lymph node metastasis in thoracic esophageal carcinoma and investigate the rules of lymph node metastasis distributions.Methods 117 patients with surgically and pathologically proved esophageal carcinoma underwent CT scans of the chest before the surgical operation to identify the presence of lymph node metastasis and evaluate the diagnosis value of spiral CT in lymph node metastasis.CT imaging which was detected by CT and histopathological analysis performed with attention to the distribution characteristics.Results In 117 cases of esophageal patients,the diagnosis of sensitivity,specific and accuracy degree on lymph node metastasis were 86.0 %,80.6 %,84.6 %.The most lymph node metastasis in the upper thoracic esophagus is the mediastinum group[8/14(57.1%)]and the trachea group[4/14(28.6 %)].the middle is on the trachea surrounding group[21/41(51.2 %)],the main pulmonary artery window group[17/41(41.5 %)]and subcarinal group[15/41(36.6 %)].the lower thoracic esophagus is around the side of preventriculus[7/19(36.8 %)]and the left gastric artery[5/19(26.3 %)]respectively.Conclusion Spiral CT imaging can accurately evaluate the lymph node metastasis of esophageal and distribution characteristics.
6.Analysis of intestinal flora in patients with hyperuricemia in Qindao District

Objective To investigate the variations of intestinal flora of common carp in patients with hyperuricemia in Qingdao District.Methods The fecal flora was analyzed by gradient dilution method.The levels of uric acid in blood and feces were detected by enzyme colorimetry method and phosphortungstic acid method,respectively.Results E.coli (7.58 ± 0.34,P < 0.05),the total count of aerobian (7.76 ± 0.67,P < 0.05),and bacteroides (2.75 ± 0.31,P < 0.05) were significantly increased in hyperuricemia patients compared to controls.Bifidobacterium (5.38 ± 0.34,P < 0.05) and lactobacillus (2.69 ± 1.48,P < 0.05) were sig-nificantly decreased compared to controls.Concentrations of uric acid in blood and feces were both significantly higher in the patients relative to healthy controls.Decomposition capability was similar to healthy controls.Decomposition capability of uric acid revealed a close correlation with bifidobacterium and lactobacillus,respectively (r =0.565,0.328,P < 0.05).Conclusions Intestinal dysbacteriosis was found by the analysis of fecal flora in patient with hyperuricemia in Qingdao district.Dysbacteriosis might participate in the process of hyperuricemia onset.
7.Study on the expression of LGT proteinic fingerprint in the change of TSH,FT3,FT4

Objective Exploration of TSH, FT3, FT4 changes in critical condition prior to death.Methods 84 patients that were anti-tumor therapy acceptable and not associated with the treatment of thyroid disease in patients with terminal tumors. Age from 24 to 85-year-old, male 56, female 28. The use of SELDI technology (surface-enhanced laser description / ionization time-of-flight mass spectrometry protein chip technology) checks to M/Z for the 11 100 + H~11 900 + H peak of the cluster, divided into the positive group for critically ill patients with early warning protein (LGT) and the negative group for it. All patients were given TSH, FT3, FT4 inspection. The results were statistically analyzed. Results TSH, FT3, FT4 in the LGT positive group was significantly lower than in the negative LGT group. There was no correlation between TSH,FT3 and FT4 in the LGT positive group;In the negative LGT group FT3 and FT4 had a positive correlation,and FT3 and TSH no relevance. Conclusion Thyroid function is normal in the stage of cancer patients stable condition. Possible pituitary-hypothyroidism may occur as the disease advances to the end or before death.This may be an important factor causing the acceleration to the death of the patients. However, the cause and effect between this phenomenon and the early warning of protein fingerprint group(LGT) is still not clear. In critical condition checking thyroid function and render supplementary amount of thyroid hormone in accordance with the checking results may be beneficial.
8.Anterolateral thigh flap transferred with iliotibial tract for coverage of complicated forearm wounds

Objective To introduce the transfer of anterolateral thigh flap with iliotibial tract in repair of massive skin and soft tissue defects of the forearm and reconstruction of extension or flexion of the forearm. Methods Eight cases of complicated raw wounds of the forearm were repaired with transfer of anterolateral thigh flap with iliotibial tract. Flexor tendons of 3 cases and extensor tendons of 5 cases were repaired with iliotibial tract. The axial vessel of the flap was used to rebuild blood supply of the hand. Results All the flaps survived completely. A follow-up ranging from 3 months to 2.5 years re- vealed that the reconstructed forearms were good in appearance and soft in texture and restored protective sensation. Affected limbs could perform extension or flexion. Total range of motion (TRM) of the hands was excellent in 6 cases and poor in 2. Conclusion Anterolateral thigh flap with iliotibial tract can not only repair soft tissue defects of the forearm, but also reconstruct the main extension or flexion of the forearm simultaneously.
9.Treatment of multiple level noncontiguous thoracolumbar fractures with posterior pedicle screw

Objective To explore the clinical effect of posterior pedicle screw internal fixation in the treatment of multiple level noncontiguous thoracolumbar fractures.Methods Thirteen patients with multiple level noncontiguous thoracolumbar fractures were treated by posterior pedicle screw internal fixation.The Frankel score,percentage of vertebral compression and Cobb angle of the injured vertebral segment were analyzed to evaluate the surgery efficacy.Results All patients were followed up from 12 to 24 months ( averaged 15 months ).All cases achieved bone fusion with no implant failure.The Cobb angle of the injured vertebral segment was corrected from preoperative(22.2 ±5.3) degree to postoperative(5.3 ±3.5) degree and(6.2 ±3.6) degree at the last follow up.The percentage of vertebral compression was corrected from preoperative (45.7 ± 14.1 )% to postoperative ( 6.1 ± 3.8 ) % and ( 7.2 ± 3.9 ) % at the last follow up.All improvements showed significant differences when compared statistically( t =15.03,t =12.05,Ps <0.05 ).The spinal cord function was improved 1 to 2 degree in all patients except 2 patients of grade A.Conclusion The posterior fixation with pedicle screw is a secure,safe and effective method in treating multiple level noncontiguous thoracolumbar fracture.
