The pharmacokinetics of domestic or imported nimesulide tablet was determined following a single oral dose of 200 mg given to 10 volunteers in randomized crossover study. The plasma concentration of nimesulide was assayed by HPLC method. The concentration -time curve of nimesulide conformed to a one-compartment model and the main parameters of domestic nimesulide were as follows: T1/2Ke 3. 61?1.43 h; Tpeak 2. 07?0. 63 h; Cmax=9. 46?2. 06 mg?L-1 ; AUC = 76. 39?17. 62 mg?L-1?h-1 respectively. The relative bioavailability of domestic tablet was 92.2%. The results of three factors analysis of variance and Bayesian method showed that two formulation were bioequiva-lent.
3.Use of Psychotropic Substances in Outpatients of Our Hospital

OBJECTIVE: To investigate the use of psychotropic substances for outpatients in our hospital.METHODS: The psychiatric substances used in the outpatients between June and December in 2006 were analyzed statistically in respect of the defined daily dose,DDDs and average daily expense etc.RESULTS: Of the total 7 286 prescriptions analyzed,the psychotropic substances prescribed totaled 12 kinds,of which,2 were typeⅠ,and 10 type Ⅱ.Estazolam,Alprazolam and Diazepam ranked at the first three places in terms of numbers of prescriptions;Estazolam,Alprazolam and Clonazepam ranked at the first three places in terms of DDDs;Zolpidem,Midazolam and Alprazolam ranked at the first three places in terms of consumption sum;Nitrazepam,Estazolam and Phenobarbital ranked at the lowest places in terms of average daily expense.CONCLUSION: Of the total psychiatric substances consumed in the outpatients of our hospital,benzodiazepines were used predominantly for their merits of low cost but proved efficacy.

In light of the medical safety control standards for general hospital appraisal in Zhejiang and the principles followed in designing the standards, the authors put forward some suggestions for medical institutions to improve their medical safety control. ①Medical safety occupies a key position in hospital appraisal standards. Some points are presented for discussion with regard to the criteria for malpractice-caused deduction of marks. ②The focal point of medical safety control lies in prevention. Medical institutions and their staff ought to have a correct understanding of malpractice and try to remove hidden dangers. The directive function of plans for preventing and handling malpractice ought to be brought into full play. In addition, nationally uniform and comprehensive criteria and routines for diagnostic, treatment and nursing procedures ought to be set up speedily. ③ It is imperative to make use of advanced means so as to ensure the smooth, timely and efficient reporting and feedback of medical safety information. ④The idea of service ought to be reinforced and a risk assessment mechanism ought to be set up.
5.Changes in plasma B-type natriuretic peptide and carbohydrate antigen 125 levels in elderly chronic heart failure

Objective To investigate the correlations of plasma B-type natriuretic peptide (BNP) and carbohydrate antigen 125 (CA125) levels in elderly patients with chronic heart failure (CHF) and to evaluate the clinical value.Methods A total of 124 elderly patients diagnosed as chronic heart failure were recruited.Patients with CHF were divided into the stage Ⅱ group,stage Ⅲ group and stage Ⅳ group according to the New York Heart Association (NYHA) classification.35 health subjects were randomly enrolled as the control group.The plasma levels of BNP and CA125 between different groups were compared.The correlations of the plasma BNP and CA125 levels with chronic heart failure were analyzed.60 cases with clinical improvement by aggressive treatment were followed up and the plasma levels of CA125 and BNP in them were tested.The changes in heart failure indicators were analyzed before and after treatment.Results The plasma BNP and CA125 levels were higher in patients with CHF than in control subjects (all P<0.01).The plasma BNP and CA125 levels were gradually increased in the stage Ⅱ,Ⅲ and Ⅳ group (all P<0.01).The plasma levels of BNP and CA125 were negatively correlated with left ventricular ejection fraction and positively correlated with left ventricular end-diastolic diameter (r=-0.75,-0.73,0.72,0.63,respectively,both P<0.01).There were no significant differences in the plasma levels of BNP and CA125 between patients with or without atrial fibrillation (both P >0.05).Conclusions Plasma BNP and CA125 levels are correlated with NYHA classification in CHF,which can be used as indexes for the diagnosis,treatment and prognosis of CHF.

Objective To investigate the value of quantitative TB‐DNA test ,interferon gamma release test and the detection of tuberculosis antibodies for the diagnosis of active pulmonary tuberculosis .Methods 51 patients were diagnosed as tuberculosis from 2013 July to 2014 June in the hospital ,whose sputum smear microscopy for acid fast bacilli were positive .Then TB‐DNA quantitative test ,interferon gamma release test (T‐SPOT .TB)and tuberculosis antibody detection were performed for those pa‐tients .All the result were retrospectively analysed .Results The positive rate of T‐SPOT .TB was 90 .1% ,the positive rate of quan‐titative TB‐DNA test was 74 .5% and the positive rate of tuberculosis antibody detection was 37 .3% .Conclusion Because the re‐sult of T‐SPOT .TB is not affected by the process of specimen collection ,it is the most sensitive test of the three tests at present , and has higher value in the auxiliary diagnosis of active pulmonary tuberculosis than the other two .

10.Effects of 4-aminosalicylic acid on migration, activation and apoptosis of polymorphonuclear neutrophil in rats with experimental colitis

Objective To investigate the effects of 4-aminosalicylic acid (4-ASA) on rats with 2,4,6-trinitrobenzenesulfonic acid-induced colitis in order to understand its mechanisms in treatment of inflammatory bowel disease (IBD). Methods Thirty SD rats were given 2,4,6-trinitrobenzenesulfonic acid to induce colitis and were divided into model group, 5-ASA (200 mg/kg) treated group and 4-ASA (200 mg/kg) treated group with 10 each. Another 10 rats were severed as normal control. Seven days later,all animals were sacraficed for estimation of colonic tissues. The iNOS and serum level of interleukin (IL)-8 were detected by immunohistochemistry and enzyme-linked immunosorbent assay (ELISA) ,respectively. And the apoptosis of polymorphonuclear neutrophil (PMN) was examined by flow cytometry. Results In comparison with model group, the body weight was increased in rats treated with 4-ASA (t= 14.09,P<0.01), whereas the macroscopic and histological scores and MPO activity were decreased (t=7.87,18.37,6.66 and 19.60,respectively, all P values <0.01), which were similar to 5-ASA treated group (all P values > 0. 05). The expression of tissue iNOS was 73.55%±5.15% in model group, which was higher than that in control group [(5.95±1.45)% ,t=39.93,P<0.01)],but was lower than that in 5-ASA treated group [(37.80±3.82)%,t = 17.62,P<0.01] and 4-ASA treated group [(42.27±3.52) %, t= 15.76 ,P<0.01]. The serum level of IL-8 in model group was significantly higher than that in 5-ASA treated group and 4-ASA treated group (P<0. 01). The apoptosis of PMN in model group was lower than that in control group (t= 11.48,P<0.01), but higher than that in 5-ASA treated group (t= 7.51, P<0.01) and 4-ASA treated group (t= 10.47,P<0.01). Conclusions The efficacy of 4-ASA in treatment of IBD may be related to the mechanisms of reducing the migration and the activities of PMN, up-regulating PMN apoptosis and scavenging reactive oxygen radicals produced by PMN.
