BACKGROUND: To investigate the possibie difference, if any, between office blood pressure(BP) and home BP may be important in the diagnosis and treatment of hypersensive patients. This report deails the difference between the two BP's and the usefulness of digital electronic sphygmomanometer(DES) for self-monitoring of home BP. METHODS: The BP's of 14 patients with essential hypertension were measured with mercury sphygmomanometers at outpatient department by physician and with DES at home(twice a day) by the patients. Patients were followed up every 2 weeks for 4 weeks and previous 2 weeks' average home BP's were compared with the office BP's of each 2 weeks' end. RESULTS: There was a significant difference between office and home BP(both systolic and diastolic) ; office average BP(151/95mmHg) was higher than home average BP(136/86mmHg). CONCLUSION: Caution may be needed in the interpretation of office BP unless it is measured several times after adequate rest.

6.A Comparision of Adenosine and Verapamil for the Treatment of Paroxysmal Supraventricular Tachycardia.

BACKGROUND: According to the 1992 version of ACLS guideline, adenosine is recommended as the first line drug far the treatment of paroxysmal supraventricular tachycardia(PSYT). But adenosine is not used frequently in our country, despite currency proven effect and safety. Therefore we tried to compare the efficacy and safety of adenosine with verapamil for the treatment far PSVT. MATERIALS AND METHODS: We prospectively reviewed charles of PSVT patients admitted to YongDong Severance Hospital from Jan. 1995 to Dec. 1996. These patients were randomly divided into two groups. The flat group was given 6mg of adenosine initially, and another 12mg was given within 5 minutes if fast dose failed. The other group was given 5mg of verapamil initially, and if failed, 10mg was given within 10 minutes. The results were analyzed by Chi-square test and student-t method. RESULTS: In converting PSVT to normal rhythm, adenosine and verapamil showed similar results and there was no difference between the two Groups in frequency of side elects, but serious arrhythmia such as ventricular fibrillation and ventricular tachycardia was not seen in the adenosine group. CONCLUSION: Adenosine not only showed comparable effect and safety, but also had shorter action time than verapamil. Therefore we recommand adenosine, as a safe and effective fort line drug for PSVT.

7.A Case of Pilomatrixoma after Split Thickness Skin Graft.

PURPOSE: Pilomatrixoma is a benign, usually asymptomatic tumor. It presents clinically as a solitary superficial subcutaneous nodule measuring between 0.5 cm and 5 cm in diameter on the head or upper extremeties and has not been reported after skin graft. The objective of this article is to report our experience in treating pilomatrixoma which occurred after split thickness skin graft on the lower extremity. METHODS: A 56-year-old female was treated in August 2005 with a 0.5 X 0.5 cm firm subcutaneous nodule at recipient site of split thickness skin graft on the left medial thigh. The tumor was successfully removed by complete excision and histologic examination was followed. RESULTS: The diagnosis was pilomatrixoma which was characterized by a dual population of proliferating basophilic cells and diagnostic shadow cells. CONCLUSION: The tumor was successfully treated by complete resection. The authors report this very rare case of pilomatrixoma which occurred at recipient site of split thickness skin graft.

9.The Proportion of misunderstanding about health knowledge in medical and nonmedical group.

BACKGROUND: There is much harmful influence for health promotion when general publics and medical group misunderstand common health knowledge. Therefore we investigated the proportion of misunderstanding about health knowledge of general publics and medical group. METHODS: The data were collected via questionnaire which consisted of 30 questions (ll health behavior, 5 symptom, 14 treatment knowledge) quoted or revised on the basis of the book ('Misunderstood common health knowledge 100' published by humanism practice medical doctor association, 1994, hanwol company, South korea). The subjects were nonmedical group (third grade elementary school teachers and parents of students in Kyong ju city) and medical group (doctors-specialist, resident, intern, oriental doctor, medical student, nurse, assistant nurse in Kyong-Ju, Pohang city). The score was given 0 for false answer or no answer and 1 for true answer. RESULTS: The dominant subclasses of study subjects were as follows .Sex; female(352:64.5%)/age; 30-40 years old(318 persons : 58.5%) education ; high school graduate group(232: 42.8%)/job; nonmedical group(housewives, 168:36.2%), rnedical group(nurse, 67:36.2%). Medical group acquired higher total average score than non-medical group. There were no differences in the total scores about treatment, symptom, and health behavior in nonmedical group by job. In the medical group, doctors earned significantly higher score than other jobs. The average score per question was 0.53 in nonmedical group and 0.69 in medical group. Non-medical group received average score below 0.5 in 14 questions among 30 questions, while medical group received only in 5 items. CONCLUSIONS: Medical group acquired higher total average score than nonmedical group. Average score per question was 0.53 in non-medical group and 0.69 in medical group.

10.Erdheim Chester Disease (ECD): A Case Report.

Erdheim Chester disease (ECD) is very rare non-Langerhans cell histiocytosis (LCH) which occurs in the skeletal system and multiple organs. As it is progressive, sometimes it causes fatal results. However, it is often misdiagnosed as LCH or multiple bone metastasis and, thus, is very difficult to diagnose. In Korea, only 10 cases were first reported in 1999. In particular, there have been a few orthopedic approaches or reports in English-speaking literatures, and no report has been issued in Korea. The authors performed bone biopsy in patients with knee and lower extremity pain who were referred for the integrated treatment. We attempts to report this diagnosis experience with literature review.
