Graves disease occur in association with myasthenia gravis is rare. We report a case of Graves disease and myasthenia gravis treated by bilateral subtotal thyroidectomy and total thymectomy simultaneously. A 37 year old woman was admitted with anterior neck mass and ptosis. Various examinations were compatible with combined Graves disease and myasthenia gravis. The bilateral subtotal thyroidectomy and total thymectomy were done simultaneously. The pathologic diagnosis was Graves disease and thymic hyperplasia. The patients postoperative course was uneventful. The thyroid function of patient became euthyroid and the clinical symptoms related with myastenia gravis resolved during follow up period.

PURPOSE: This study aims to investigate gender differences in the association between depressive symptoms and weight, weight perception, and body satisfaction among Korean adolescents. METHODS: A secondary data analysis was performed on data from 33,374 adolescents who participated in the 2015 Adolescent Health Behavior Online Survey. They were classified as underweight, normal weight, or overweight/obese; weight perception was classified into perception of being underweight, normal weight, or overweight/obese; and weight satisfaction into desire to gain weight, satisfied, and desire to lose weight. RESULTS: Among boys, perception of being underweight (adjusted odds ratio [AOR]: 1.20, 95% confidence interval [CI]: 1.07~1.35) and desire to gain weight (AOR: 1.45, 95% CI: 1.30~1.62) were associated with depression. Among girls, perception of being overweight or obese (AOR: 1.18, 95% CI: 1.07~1.29) and a desire to lose weight (AOR: 1.30, 95% CI: 1.18~1.42) were associated with depression. CONCLUSION: Gender differences were observed in the association between weight perception and depression in adolescents. The perception of being underweight among boys and the perception of being overweight/overweight among girls were associated with depression. Thus, gender-specific intervention programs to correct weight perception and weight satisfaction are needed in order to relieve depressive symptoms in adolescents.

PURPOSE: The purpose of this study is to identify the correlation that exists between quality of life and self-efficacy of schizophrenic patients, focusing on the influence of related factors on these varables. METHOD: The subjects of this study were 151 schizophrenic patients. The data were collected using questionnaires. The instruments used for this study were the general self-efficacy scale developed by Sherer & Maddex (1982), the specific self-efficacy scale modified and complemented by these co-researchers on the basis of the past studies and modified SIP by Voruganti (1996). The period of data collection was from July. 2000 to Jan. 2001. Data analysis was done by SPSS, t-test, ANOVA and the Pearson Correlation Coefficient. RESULT: 1. The level of self-efficacy showed a mean score of 60.0 and the level of quality of life, a mean score of 47.0. 2. The general characteristics affecting the self-efficacy of schizophrenic patients were staying with family (p=0.05) and employment (p=0.00). 3. The general characteristics affecting the quality of life of schizophrenic patients were staying with family (p=0.04), employment (p=0.05) and duration of illness (0.03). 4. A positive correlation was identified between self-efficacy and quality of life (r=-0.469, p=0.000). CONCLUSION: The study suggests that nursing intervention strategy should be worked out to develop a psychiatric rehabilitation program that can promote self-efficacy and thus enhance the quality of life of schizophrenic patients.

The present study was aimed at investigationg the usefulness of clinical stage, degree of histopatphological malignancy, p53 protein and PCNA labelded indices in deciding the prognosis, proliferation and infiltration of oral squamous cell carcinoma (SCC). Twenty-seven cases of SCC (seven were recurrent cases) were analyzed and 10 cases of non-specific inflammatory lesion served as control. Malignant scores were obtained with the Anneroth's classification. Proliferative activity of SCC in oral cavity was examined by means of proliferaling cell nuclear antigen (PCNA) and p53 protein immunostaining using DO7, PC10 (Novostra Co., U.K.) as primary antibody and biotinylated linked antibody (LSAB kit, Dako Co., U.S.A.) as secondary antibody. The labeled indices of PCNA and p53 were calculated by computer image analysis system in 400 multiplication microscopic fields. Results obtained were as follows : 1. The mean age of SCC group was 61.2 years. Seven of 20 cases had recurrence in 4 months after the operation, in which no significant correlation were noted between pre-and postoperative clinical stage, malignant scores, p53 protein and PCNA indices. 2. Positive immunostaining was noted in 65% (13 of 20 cases) on p53 protein and in 80% (16 of 20 cases) on PCNA. 3. Mean p53 protein and PCNA labeled indices were 24.71+/-27.86% and 41.49+/-2.51% in SCC group. They were high in SCC group than those in control group, although there was no signficant correlation between the two indices. 4. Mean malignant score was 15.22 0.64 in SCC group. There was no correlation between malignant score and clinical stage, and between malignant score and p53 protein or PCNA indices. 5. There was no signficant correlation between SCC and recurrent SCC group in malignant scores, p53 protein or PCNA indices. These results suggest that p53 and PCNA labeled indices have only limited usefulness in deciding the prognosis, proliferation and infiltration of oral squamous cell carcinoma.

Spinal epidural abscesses were evaluated using Gadolinium-enhanced magnetic resonance imaging (MRI) in 19 surgically and clinically confirmed cases, including 13 with tuberculous spondylitis and 6 with pyogenic spondylitis. We observed rim enhancement patterns in 10 patients and diffuse enhancement patterns in 9 patients. The spread patterns were localized to ventral or dorsal epidural space and encircled entire epidural space. We conclude that rim enhancement indicates abscess and diffuse enhancement indicates granulation tissue and fibrosis. Tuberculous epidural abscess spreads to ventral epidural space and pyogenic epidural abscess encircles entire epidural space.

Coronary artery fistula is an unusual anomaly in elderly patients that consists of a communication bet- ween one of the coronary arteries and other cardiac chambers or veins. It causes many cardiac compli- cations due to hemodynamic changes, and thus has recognized its clinical significance. It occurs usually in congenital origin, but also occurs secondarily in traumatic or neoplastic or artherosclerotic coronary diseases. Congenital coronary artery fistula is a very rare congenital anomaly that results in multiple hemodynamic complications. It has reported rarely in elderly patients, that induces myocardial ischemia due to coronary steal syndrome and heart failure that caused by diastolic volume overload via a left to left shunt. We observed a 68-year-old male patient with exertional chest pain and dyspnea, in whom selective coronary angiography revealed abnormal reticular communication between left anterior descending artery and left circumflex artery and drained to left ventricle through multiple microfistulous channels. We report this patient case who complicated by myocardial ischemia and left ventricular failure those caused by hemodynamic complication due to multiple coronary artery-left ventricular fistulae.

PURPOSE: All procedures for the removal of retained intrahepatic stones were performed on inpatient basis. We evaluated the advantage of the procedures performed on inpatient basis compared with outpatient basis in terms of the success rate, causes of failure and the complication. MATERIALS AND METHODS: Percutaneous removal of retained intrahepatic stones was performed in 58 patients through a T-tube tract on inpatient basis from April 1990 to December 4992. Stones were exclusively intrahepatic in 28 patients, whereas 30 patients had combined stones in common bile duct. Preshaped catheters, baskets, and balloon catheters were used to remove the stones and dilate the strictures of the ducts. Extracorporeal shock wave lithotripsy and choledochoscopy with electrohydraulic lithotripsy were used to crush the impacted stones. RESULTS: The stones were completely removed in 34 (58.6%) of 58 patients and most of the stones were removed in 17 patients (29.3%). The overall success rate was 87.9%. The complications (cholangitis in 13% and pancreatitis in 0.8% per session) were found and successfully managed by appropriate care. CONCLUSION: Better success rate could be achieved by more aggressive appraoch on inpatient basis when compared with previous reports on outpatient basis. We suggest that the procedures should be performed on inpatient basis especially in a case with severe strictures or impacted stones.
