OBJECTIVETo study the results of arthroscopic partial meniscectomy for medial meniscal tear in late middle-aged adults.

METHODSFrom October 2010 to October 2012,49 patients meeting inclusion criteria were treated with arthroscopic partial meniscectomy for medial meniscal tear. There were 31 males and 18 females,with an average age of (55.3±2.8) years old. There were 22 left knees and 27 right knees. The average flexion angle was (116.01±12.03)°. The pre-operative HSS Knee Score was 48.73±8.43, and the Lysholm score was 63.95±5.45. The patients were followed up and evaluated. All the patients received a standard surgery, in which the torn tissue was removed, and the anterior part of the meniscus was left in situ.

RESULTSAll the surgeries were successful with no serious complications. All the patients were followed up,and the duration ranged from 12 to 36 months. All the patients had 5 degrees of muscle strength. The average flexion angle was (136.77±18.56)°. There was significant difference between the two angles before operation and after operation. The pre-operative HSS Knee Score was 90.17±4.10, and the Lysholm score was 87.84±5.16. Statistically, the difference between preoperative score and the postoperative score was significant.

CONCLUSIONArthroscopic partial meniscectomy is a good option for medial meniscal tear in late middle-aged adults. Operative indication is crucial and the excellent surgical technique is also critical for the good clinical outcome. All the patients could get good clinical results, although there are some patients with motion restrictions in the early stage after operation.

Objective To investigate the difference in expression profiles of micro-RNA (miR-NA) between human epidermal stem cells and epidermal keratinocytes.Methods (1) Human primary epidermal stem cells and keratinocytes were obtained with enzyme digestion method and type Ⅳ collagen coated chosen method.Growth of cells cultured in vitro was observed by inverted microscope.Monoclonal antibody of integrinβ1,keratin 1 (CK1),CK10,and CK19 were detected by immunocytochemical staining.(2)Total RNA was respectively isolated from epidermal stem cells and epidermal keratinocytes by Trizol-based single-step procedure,detected by formaldehyde denaturing gel electrophoresis,purified by mirVanaTM miRNA isolation kit,and then labeled and hybridized by miRNA labeling and hybridization kit.Images of hybridization were analyzed using the Feature Extraction (Version 10.7).Data normalization and difference analysis were performed with GeneSpring (GX 10.0).Moreover,miRNA microarray results were confirmed by RT-PCR.(3) Target genes of differently expressed miRNA were predicted.Results Epidermal stem cells exhibited rapid adherence to type Ⅳ collagen and formed distinct clones after 3 days of culture; expressions of integrinβ1 and CK19 were positive.Keratinocytes could not adhere rapidly to type Ⅳ collagen and formed few clones after 3 days of culture ; expressions of CK1 and CK10 were positive.(2) Of the epidermal stem cells,31 miRNAs were up-regulated and 153 down-regulated.Besides,significantly up-regulated miRNAs were hsa-miRNA-125b-3p,hsa-miRNA-197-5p,and hsa-miRNA-376a-3p,while significantly down-regulated miRNAs were hsa-miRNA-203,hsa-miRNA-29b-3p,and hsa-miRNA-34a-3p.Findings of RT-PCR on hsa-miRNA-197-5p and hsa-miR-29b-3p revealed high concordance with the microarray results.(3) Some miRNAs target genes indicated that miRNA related to cell proliferation,differentiation,apoptosis,aging and other biological characteristics.Conclusion Distinct differences in miRNA expression profiles are detected between human epidermal stem cells and keratinocytes and this may correlate closely with their different biological characteristics such as proliferation and differentiation ability.

Objective To investigate the clinicopathological features ,diagnosis and differential diagnosis of primary leiomyo‐sarcoma of the thyroid gland .Methods The pathological characteristics and immunohistochemical features in a case of primary leio‐myosarcoma of the thyroid gland were analyzed and the relevant literatures were reviewed .Results Grossly ,the mass measured 5 .5 cm × 4 .0 cm × 3 .5 cm ,gray and yellow in cut surface and partial hard quality .Histologically ,the tumor was composed of spin‐dle cells ,arranging in bundles with obvious dysplasia ,nuclear mitosis was easily found .The tumor infiltrated thyroid follicles and in‐vaded surrounding tissues .The immunohistochemical study showed that Vimentin ,SMA ,Caldesmon and Actin were positive ,and CK ,CK8 ,EMA ,TTF‐1 ,P53 and LCA were negative .Conclusion Primary leiomyosarcoma of thyroid gland is extremely rare ,its diagnosis should be combined with clinical and imageologic data ,histological characteristics and immunohistochemistry .

Objective To investigate the relationship between muscarinic receptor subtypes and detrusor hyperreflexia. Methods Spinal cord was transected in male Wistar rats at L 1～2 level, changes of urodynamics were monitored 3 weeks later by cystometric studies.Spontaneous contraction frequency and strength of detrusor strip mustarded by carbachol (muscarinic receptor agonist), atropine (nonselective M antagonist), methoctramin (M 2 receptor selected antagonist) and 4 DAMP (M 3 receptor selected antagonist) were measured. Results Bladders after SCI showed littler capacity, lower compliance and hyperreflexia. Detrusor of all groups contracted at a definite tension. The contraction frequency in the hyperreflexia group (4.797?0.257 times/min) was higher than in the control's(3.227?0.173 times/min).There were no obvious changes while detrusor strips were mustarded by different muscarinic receptor antagonists.Carbachol could increase detrusor contraction frequency( 6.316 ?0.409 times/min) and the effect could be blocked by atropine(5.304?0.095 times/min) and 4 DAMP(5.046?0.089 times/min). Conclusions Detrusor is a rythmatic tissue at a difinite tension.Excitability rise of detrusor is the main cause of hyperrefexia. M 3 receptor can enhance the excitability of detrusor causing direct contraction.The function of M 2 receptor is still unclear.

OBJECTIVE:To establish the GC-MS fingerprint spectrum of volatile components in Danqi piantan capsule. METH-ODS:GC-MS was performed. HPLC conditions:The column was BPX 5,temperature of injection was 250 ℃,respectively;carri-er gas was He,flow rate was 1 ml/min,split injection,split ratio was 50:1 and volume injection was 0.02 μl. MS conditions:ion source was EI,bombarding energy was 70 eV,temperature of ion source and transmission line was 200 ℃ and 250 ℃,respective-ly;scan range was 40-500 amu,solvent delay of 2 min. The peak with largest chromatographic peaks areas was reference peak,com-mon peaks of 10 batches of Danqi piantan capsule were analyzed. Similarity evaluation system for chromatographic figerprint of TCM was adapted for the similarity analysis and NIST standard MS database was used to retrieve and identify the common peak. RESULTS:There were totally 13 common peaks in the 10 batches of Danqi piantan capsule with similarity degree>0.92;accord-ing to the verification,the fingerprint spectrum was a dapted for the similarity analysis and reference fingerprint of 10 batches of Danqi piantan capsule showed good consistency. The No.3 peak was Angelica sinensis unique,the No.2,4,5,7,8,12 peak was Acorus tatarinowii unique. CONCLUSIONS:The established method is specific and stable,and can provide basis for the quality as-sessment for Danqi piantan capsule.

Objective To evaluate clinical significance of detrusor leak point pressure (DLPP) in the damage of the upper urinary tract secondary to neurogenic voiding dysfunction. Methods 38 cases of voiding dysfunction received the urodynamic test,especially detecting of detrusor leak point pressure. Results 38 cases were divided into 2 groups by DLPP of 4.0 kPa,26 in high DLPP group and 12 in low DLPP group.The bladder capacity of the high pressure group is (422.95?183.27)ml,significantly less than the low pressure group (464.83?106.43)ml,and its compliance is lower as well as with DSD.Clinical results of B ultrasound,IVU,BUN,Cr show that the high pressure group has more serious than that in the low pressure group. Conclusions DLPP is valuable in evaluating the damage of upper urinary tract secondary to neurogenic voiding dysfunction.

Objective To explore the therapy for herpes zoster (HZ) after renal transplantation. Methods 12 renal transplantation recipients with herpes zoster were treated with acyclovir, ultraviolet rays and nerve block. The pain was assessed with visual analogue scale (VAS). Results The scores of VAS decreased from 9.3 to 2.1 after treatment (P<0.01). The total effective rate was 100%. Conclusion Acyclovir combined with ultraviolet rays and nerve block is a more safe and effective therapy for the herpes zoster in renal transplantation recipients.

Objective:To explore the levels of serum adiponectin,insulin growth factor Ⅰ (IGF-Ⅰ) and prealbumin in preterm infants,and analyze their correlation with preterm EUGR infant.Methods:321 cases of preterm infants who were born in our hospital and transferred to neonatal department in 24 hours from January,2014 to January,2016 were enrolled in the present study.According to the weight at discharge,they were divided into the EUGR group and the non EUGR group,the growth and development as well as the levels of serum adiponectin,IGF-Ⅰ and PA in the two groups were compared.Results:The occurrence rate of EUGR was 55.76％ (179/321) in the 321 cases ofpreterm infants;the weight of EUGR group was significantly lower than that of the non EUGR group at 42 th days and three months after birth(p＜0.05).The SDS of both groups were all negative,and that of EUGR group was significantly lower than non EUGR group (p＜0.05).The levels of serum adiponectin,IGF-Ⅰ and PA of both groups were significantly lower than the control group at seventh days of birth,and the PA level of EUGR group was significantly lower than non EUGR group (p＜0.05).There was no significant difference in the serum adiponectin and PA levels between 7 days and 14 days in EUGR group (P ＞ 0.05),however,the serum adiponectin and PA levels in the non EUGR group was obviously increased and significantly higher than that of the EUGR group (p ＜ 0.05).The level of IGF-Ⅰ in both groups keep unchanged during two weeks (P＞0.05).Conclusion:The lower levels of serum adiponectin,IGF-Ⅰ and PA were closely related to the EUGR of preterm infants,which could be used as biochemical indexes to early diagnosis of EUGR.

The cerebral blood flow of infant is effected by physiological and pathological factors.As the cerebrovascular autoregulation of neonates is poor,in pathological cases,especially when hypoxemia and hypercapnia impaired regulation of its own,lead to changes in cerebral blood flow,then resulting in severe brain injury.It has made enormous progress in the research on the changes of cerebral blood flow in newborns in recent years.In normal infants,cerebral blood flow velocities is positively correlated to gestational age and body weight,and increase gradually with day-age in the first week after birth.The cerebral blood flow of newborn with brain injury can present as insufficiency,over-perfusion or low speed high-resistance earlier and high speed low-resistance later.Different results may be related to the duration and severity of asphyxia,but all of those are signs of damage of self-regulatory function of cerebral blood flow.Cerebral hemodynamic change is the important pathogenesis mechanism of brain injury.
