1.Clinical analysis of diagnosis and surgical treatment of pulmonary sequestration

Objective To summarize the experience in the diagnosis and surgical treatment of pulmonary sequestration. Method The clinical data from 15 patients with pulmonary sequestration underwent surgical operation from July 2000 to January 2009 were reviewed and analyzed retrospectively. Results Pulmonary sequestration was diagnosed in 14 patients before the operation with the diagnosis rate of 93.33% (14 /15).Preoperation plane and contrast chest CT scan were performed in all cases,and 9 patients performed angiography reconstruction. Twelve patients with intralobar sequestration underwent lobectomy and 3 patients with extralobular sequestration underwent local lesion resection. Abnormal supply arteries were intraoperatively found in 14 patients originating from the thoracic main artery, 1 patient from the eeliac artery. Smooth recovery was achieved in all patients. No perioperarive death occurred. Conclusions Pulmonary sequestration is a rare disease with high misdiagnosis rate in some patients. Contrast enhanced CT scan and angiography may improve the diagnosis of pulmonary sequestration. Operation is a safe and effective method for the treatment of pulmonary sequestration.
3.Application of transanus ilues tube in the case of left-semicolon cancer with intestinal obstruction

Objective To investigate the case of left-semicolon cancer with intestinal obstruction for the methods of one-stage resection and anastomosis.Methods The clinical data of ten patients with left-semicolon can- cer with intestinal obstruction treated by transanus ilues tube,were restrospectively analyzed from October 2004 to December 2006.Results No postoperative anastormotic leakage was found and the patients were clinically cured. Conclusion The technical problem that left-semicolon cancer with obstruction for one-stage resection and anastomo- sis was resolved by the application of transanus ilues tube.
4.Dynamic Change of Cerebral Blood Flow of Newborns with Hypoxic-Ischemic Encephalopathy

Objective To observe the dynamic change of cerebral blood flow of newborns with hypoxic-ischemic encephalopathy(HIE).Methods Cerebral blood flow of middle cerebral artery and pulsatility index(PI) on 75 newborns with HIE and 50 normal infants were examined with transcranial doppler sonography at different time points,and the relations between cerebral blood flow and clinic indexes were analyzed.Results The blood velocity of normal infants increased gradually, and PI decreased from 2 to 5 days.The velocities were lower than that of normal infants,and PI was higher at 12th hour and 1st day, but during 2-5 days,the velocities got higher and PI got lower, in which the decrease of velocities correlated positively with Apgar scores and the increase of velocities were negatively correlative to Apgar scores.Compared with the neonates who had poor prognosis retrospectively with those had good prognosis, the velocity changes were found to be more significant.Conclusion The change of cerebral blood flow can show the pathophysiology of HIE and prognosticate the prognosis of neonates with HIE.
5.Preparation, characterization and performance of gamma-polyglutamic acid/carboxymethyl chitosan-calcium phosphate cement

BACKGROUND: Nowadays complex bone defects have become a great challenge to orthopedists. A synergistic contribution of various growth factors and a crosstalk between their signaling pathways have been suggested as determinatives for the overall osteogenic outcome.OBJECTIVE: To develop calcium phosphate cement (CPC) incorporated with γ-polyglutamic acid/carboxymethyl chitosan (PGA/CMCS), and to evaluate its physical and chemical properties and sustained-release function. METHODS: The γ-PGA/CMCS polymer composites were prepared by graft copolymerization and spray freeze drying methods, and then loaded with recombinant human bone morphogenetic protein 2 (rhBMP-2) growth factor. CPC served as control group, and γ-PGA/CMCS-CPC containing different contents of rhBMP-2 as experimental groups. A γ-PGA/CMCS-CPC scaffold with regular blade-like crystalline structure was fabricated by injection compression molding. Before mixed with the liquid phase, the solid additives were properly mixed by wet method of CPC solid and the γ-PGA/CMCS carrier, then the pre-blended mix was freeze-dried. The setting time and compressive strength of bone cement in each group were detected, and the microstructure of the material surface was observed under scanning electron microscopy. In vitro release of rhBMP-2 was investigated. The effect of bone cement extracts on cell proliferation was determined through MTS assay.RESULTS AND CONCLUSION: γ-PGA/CMCS-CPC had the same physicochemical properties to the CPC. Initial and final setting time, compressive strength of bone cement had no significant differences among groups. The scanning electron microscope results showed that the γ-PGA/CMCS-CPC scaffold was covered by regular blade-like crystalline structure and the γ-PGA/CMCS particles were uniformly dispersed in the CPC crystals. A sustained release of rhBMP-2 was observed from the γ-PGA/CMCS-CPC. The cell experiments exhibited that the samples with regular blade-like crystalline structure had better cell response compared to CPC control groups with irregular crystalline structure. These findings indicate that γ-PGA/CMCS-CPC can maintain good physicochemical properties, and release growth factor or drug to promote bone formation.
6.Effect of 36 item neuro-motor examination on early diagnosis of children with cerebral palsy within 1 year-old

Objective To evaluate the effect of 36-neuro-motorial examination on early diagnosis of children with cerebral palsy within 1 year-old.Methods36-neuro-motorial examination was analysed in 210 children with cerebral palsy from 2 to 12 months. Results4-29 items reflecting the abnormity of reflex, muscle tone, posture and motion were observeed in patients with cerebral palsy.If these abnormities were laid according as frequency among 2-6 months,reflex abnormities are knee tendon reflex, palmar grasp reflex,Babinski sign,asymmetrical tonic neck reflex,ankle clonus,sitting equilibrium reflex,stepping reflex,crossed extension reflex,tonic labyrinthine reflex,trunk incurvation reflex,Moro reflex; posture abnormities are head to back≥15°,from supine to the side position, axillar suspension reflex,posterior neck fovea≥1cm in the supine position,landau reflex,Vojta reflex,traction reflex,tonic torticollis,abnormal spontaneous posture;muscle tone abnormities are foot dorsiflexion angle,scarf sign,tonic palmar grasp,to up the foot heel≥30° in the stand position,thumb decussation to the palm,poples angle,adductor angle,heel-ear angle; others are head cycle≤-2s, squint, active movement attenuation or abnormity, auditory abnormity, visual abnormity, ophthalmodonesis, epilepsy.Conclusions 36-neuro-motorial examination is effective for early diagnosis of cerebral palsy within 1 year.

8.Relationship between expression of VEGF and COX-2 in breast cancer

ObjectiveTo discuss the expression of VEGF and COX-2 in breast cancer and their correlation with Clinical relationships.MethodsWith Elivision immunohistochemistry,the expression of VEGF and COX-2 was evaluated in 60 samples with primary breast cancer,and it was determined that the correlation of their expression and the clinical features including age,tumor size,clinical TNM staging,pathological grading,armpit lymph node metastasis and prognosis. ResultsThe expression of VEGF had close correlation with tumor size, the clinical TNM staging, pathological grading, and armpit lymph node metastasis (P <0.05),but had no correlation with age.The expression of COX-2 had close correlation with the clinical TNM staging pathological grading, and armpit lymph node metastasis (P <0.05), but had no correlation with age and tumor size. The expression of VEGF had positive correlation with that of COX-2 (r =0.2615, P <0.05);In the death group, the expression of VEGF and COX-2 was significantly higher than the survival group (P <0.05).ConclusionThe up-regulated expression of VEGF and COX-2 closely correlated with tumor take place, evolution, metastasis and soake of breast carcinoma. The expression of COX-2 had positive correlation with that of VEGF.Combined detection would be helpful in screening the patients of breast cancer with high risks of recurrence and metastasis, judge the prognosis, so it is good for further treatment.

Objective To study the expression of phosphorylated myosin light chain kinase(P-MLCK) in human pulmonary arterial endothelial cell(HPAEC) induced by lipopolysaccharide(LPS).
