1. The effect of neoadjuvant chemotherapy on microvessel density and lymphatic microvessel density in uterine cervical cancer and its clinical impact

Objective: To investigate the effect of neoadjuvant chemotherapy (NACT) on microvessel density (MVD)/ lymphatic microvessel density (LMVD) in locally advanced uterine cervical cancer and its clinical impact. Methods: Seventy patients with IB2-IIB cervical cancer were recruited, and the surgical specimens of cervical cancer tissues were collected. The MVD and LMVD of cervical cancer were examined by immunohistochemistry, and the relationship between clinical response of NACT and the clinicopathological characteristics was analyzed. Results: The levels of MVD and LMVD in cervical cancer tissues were significantly higher than those in the normal cervical tissues (P<0.001; P=0.028). The MVD was related to the depth of cancer invasion (P=0.039), and the LMVD was related to the level of serum squamous cell carcinoma antigen (SCCAg) and the lymph node metastasis (P=0.017; P=0.049). The level of MVD was significantly decreased after NACT as compared with that before NACT (P=0.019). As compared with the levels of MVD and LMVD before NACT, these levels were both decreased after NACT (P=0.019; P=0.176). The overall response rate of NACT was 70.0% (49/70), and the complete response rate was 21.4% (15/70). Multivariate analysis displayed that the depth of cancer invasion was an independent factor related to the response of NACT (P=0.018, odds ratio = 0.169). Conclusion: NACT can be helpful to reduce the level of MVD and the volume of tumor in locally advanced cervical cancer. Copyright© 2011 by TUMOR.
2.Practice and Experience of Medication Consultation Wechat Platform in Our Hospital

OBJECTIVE:To provide reference for medication consultation Wechat platform in the hospital. METHODS:The medication consultation Wechat platform of our hospital was introduced,including main method,content and effect. The consulting pharmacist team was set up,and Wechat public number was applied and operated experimentally;information pharmacist was re-sponsible for its daily maintenance. We released pharmacy news,accumulated consumers and expanded influence through other me-dia at regular intervals. RESULTS & CONCLUSIONS:According to the medical practice,two modules“medication consultation”and“pharmacy news”were set up. From Dec. 2014 to Jul. 30th 2015,medication consultation system had accumulated 1 121 con-sumers,with an increase of 5 to 8 ones everyday;average number of consultation records was 10 everyday;we also had published 22 batch of medication news(79 messages),including 31 original messages and 3 video messages. Several messages gained much attention from network media and offline media. As new pharmaceutical care mode,Wechat platform contribute to realize a individ-ual,convenient and mobile medication consultation service,medication health science popularization,and improves medication compliance of patients and the awareness of medication safety in field of pharmaceutical administration Wechat platform,so as to improve pharmaceutical care effect of“taking patients as the center”and promote the propaganda of hospital brand. Thus it has a good prospect of application and popularization.
3.Magnetic resonance Propeller technique in reducing motion or magnetic susceptibility caused artifacts

BACKGROUND: Motion artifacts and magnetic susceptibility artifacts have serious impacts on magnetic resonance (MR) image quality. Periodically rotated overlapping parallel lines with enhanced reconstruction (Propeller) technique, collecting data spirally using radial blades by filling pattern, greatly reduce or eliminate these artifacts.OBJECTIVE: To discuss the clinical application of MR Propeller technique in eliminating the artifacts of brain images.DESIGN, TIME AND SETTING: The comparative observation was performed at Kunming General Hospital of Chengdu Military Area Command of Chinese PLA from July 2006 to April 2007.PARTICIPANTS: Twenty-one cases showed dysphoria or unable to control oneself in the routine cranial MRI examination, and 5 presented artifacts because of magnetic-susceptibility.METHODS: Twenty-one cases, who had dysphoria and could not control themselves in routine cranial MRI examination, underwent Propeller T2 weighted imaging (WI), and the images were compared with routine T2 fast-recovery fast spin echo sequence; 5 cases, who had artifacts because of magnetic-susceptibility, received Propeller diffusion weighted imaging (DWI), and the images were compared with routine DWI.MAIN OUTCOME MEASURES: The changes of motion artifacts and magnetic susceptibility artifacts resulted from Propeller technique.RESULTS: The artifacts caused by motion in 21 cases were significantly reduced or eliminated by Propeller T2WI scanning, and those caused by magnetic susceptibility in the other 5 cases were also significantly eliminated by Propeller DWI. Satisfactory MR images were obtained.CONCLUSION: Propeller T2WI or DWI technique can obviously reduce or eliminate the artifacts caused by motion or magnetic-susceptibility, and can significantly improve the image quality for clinical diagnosis.
4.Association of VTQ value of spleen with staging of liver fibrosis in hepatitis B

Objective To investigate the feasibility of VTQ value of spleen in noninvasive assessment of staging of liver fibrosis in patients with hepatitis B.Methods VTQ values of spleen were measured in 56 patients with hepatitis B by using ARFI technique and liver biopsy was performed.The patients were then grouped according to the stages of liver fibrosis stage.The results were analyzed by the intergroup comparison,Pearson Correlation analysis,and receiver operating characteristic curve (ROC curve).Results There was positive association of the VTQ value with the stages of liver fibrosis,whose correlation coefficient was 0.83 (P < 0.05);There was no significant difference between the spleen VTQ value and the liver fibrosis S1 and SO (P > 0.05),whereas there were significant differences among other groups (P < 0.05).The area under the ROC curve was 0.95,the cut-off value was 2.79m/s,and the specificity and sensitivity were 81.8% and 91.7% respectively.Conclusions The VTQ value measured by ARFI technique has a better value in noninvasive diagnosis of stages of hepatitis B liver fibrosis.

Designing of natural product-like compounds using natural products as template structures is an important strategy for the discovery of new drugs. Gambogic acid (GA), which is a Garcinia natural product with a unique caged xanthone scaffold, inhibits potent antitumor activity both in vitro and in vivo. This review summarized the researches on the identification of the antitumor pharmacophore of GA, and the design, structural optimization and structure-activity relationship (SAR) of natural product-like caged xanthones based on it.

6.Correlation of acquisition time of C-arm cone-beam CT with image quality and radiation doseduring cerebral angiography using an anthropomorphic head phantom

Objective To investigate the influence of acquisition time of C-arm cone-beam CT on image quality and radiation dose of cerebral angiography.Methods C-arm cone-beam CT of cerebral angiography was performed on the male anthropomorphic head phantom,with DynaCT imaging mode and the acquisition time of 5 s,8 s and 20 s were used.Scanning was performed with each acquisition time for three times,and VR,MIP and MPR images were reconstructed.The attenuation values and their standard deviations of intracranial segment of the internal carotid artery (ICA),middle cerebral artery(MCA),anterior cerebral artery(ACA) and uniformed brain tissues were measured to calculate the image noise,signal to noise ratio (SNR) and contrast to noise ratio (CNR).We used the image noise,SNR and CNR as the objective standard to evaluate the image quality,and One-way ANOVA analysis of variance was used to assess the difference among them.A scale with scores 1 to 5 was used to rate the quality of the reconstructed image of ICA,MCA,ACA as a subjective evaluation,the difference among the evaluation scores were analyzed using Kruskal-wallis.We recorded the dose area product (DAP) of each acquisition time and the effective dose(ED) was calculated to assess the radiation dose,the difference among them were analyzed using One-way ANOVA analysis of variance.Results In C-arm cone beam CT 20 s imaging,the SNR of intracranial segment of the ICA,M CA,ACA were 22.29± 1.41,29.36 ±0.11 and 23.13 ±2.10 respectively,in 5 s imaging13.83 ±0.61,14.65 ±0.16 and 12.79±0.19 respectively,in 8 s imaging 14.92±0.96,18.97 ± 1.24 and 16.65 ±0.46 respectively,all the results showed a significant difference (F valued 58.19,327.29,52.74 respectively,all P valued<0.01),the CNR of 20 s imaging were higher than that of 5 s and 8 s imaging,the Noise of ICA,MCA,ACA and the uniformed brain tissues of 20 s imaging were lower than that of 5 s and 8 s imaging,all the results showed significant difference (all P valued<0.01).The subjective evaluation scores of VR imaging of 5 s,8 s and 20 s were 3.61 ±0.49,4.06±0.53,4.72±0.45 respectively,the scores of MIP imaging were 3.42±0.50,3.83±0.65 and 4.50±0.51 respectively,the scores of MPR imaging were 2.83±0.45,3.14±0.35 and 3.67±0.49 respectively,all the results showed significant difference (x2 valued 51.29,42.25 and 43.56 respectively,all P valued <0.01).The DAP values in C-arm cone beam CT 5 s,8 s and 20 s imaging were (9.11 ±0.18),(13.81±0.75)and(58.62±0.62)Gy· cm2,ED values were (0.91 ±0.02),(1.38±0.75)and(5.84± 0.11) mSv,all the results showed significant difference (F valued 3 720.30 and 3 654.85 respectively,all P valued<0.01).Conclusion The image quality and radiation dose of C-arm cone-beam computed tomography are closely correlated with acquisition time.
7.Protective effect of ciliary neurotrophic factor on retinal ganglion cell in acute ocular hypertension rat

BackgroundGlaucoma associates with apoptosis of retinal ganglion cells ( RGCs).Research showed that ciliary neurotrophic factor (CNTF) can repair optic nerve trauma,but it has not reported whether CNTF has a protective effect on glaucomatous optic neuropathy.ObjectiveThis experimental study was to explore the protective effect of CNTF on RGCs in rat eyes with acute ocular hypertension.MethodsOcular acute hypertension models were induced in bilateral eyes of 24 clean Wistar rats by forced perfusion of a balanced salt solution into the anterior chamber.Two days before molding,0.5 μg recombinant human CNTF (5 μl) was injected intravitreously in the left eyes,and 5 mmol/L sodium phosphate solution (5 μl) was injected in the same way as the control group.Three other Wistar rats were used as the normal control group.The animals were sacrificed by excessive anesthesia and the retinal sections were prepared 1,3,7,14 days after molding.The morphology of the retina was examined and RGCs counting was performed by hematoxylin & eosin staining and observed under a light microscope.The expression of glutamic acid in RGCs was assessed by immunochemistry.ResultsRegular retinal structure with clearly defined cell layers were observed in normal control rats.Changes in vacuoles in RGCs were seen in the model control group.The number of degenerative RGCs decreased in the CNTF group.Compared with the model control group,number of normal RGCs increased from 1 day to 14 days after molding with a significant difference between the two groups (all P=0.000).Immunochemistry assay showed that the numbers of positive cells for glutamic acid were 5.50±1.04 and 6.00±1.41 for the average of 3 fields at 3 days or 7 days in the CNTF group,and those in the model control group were 9.00±2.91 and 10.83 ± 1.94,respectively,showing significant differences between them ( all P =0.000 ).However,no comparable differences were found in the numbers of positive cells for glutamic acid at 1 day and 14 days between the two groups( P=0.578,0.180).ConclusionsCNTF down-regulates the expression of glutamic acid in RGCs and offers neuroprotection for RGCs in an acute glaucoma rat model.
8.Comparison of pulmonary nodule detection rate and accuracy in low-dose chest CT between iterative reconstruction algorithm and filtered back proj ection algorithm

Objective To compare the differences of pulmonary nodule detection rates between iterative reconstruction (sinogram affirmed iterative reconstruction,SAFIRE)algorithm and filtered back projection (FBP) algorithm in chest CT, and to evaluate the detection accuracy.Methods Three groups of tube voltage values of 80,100,and 120 kV were defaulted on the new dual-source CT,with automatic mAs care dose 4D technology, the chest phantom with simulated pulmonary nodules was scanned, then the images were reconstructed with FBP and SAFIRE (grade 1-5 ),respectively. The detection rates of simulated pulmonary nodules in the chest CT images reconstructed of SAFIRE (grade 1-5 )and FBP were compared, and their diameters and CT values were measured.Results With the same tube voltage, no significant difference was found in the detection rate of simulated pulmonary nodules between SAFIRE (grade 1-5 )and FBP (P>0.05 ), the diameter deviation of simulated nodules of SAFIRE (grade 3 )was less than FBP, and the difference in the average CT value of the simulated nodules between SAFIRE (grade 3)and FBP was not statistically significant (P>0.05);the simulated nodule detection rate of 100 kV was equivalent to the detection rate of 120 kV,the simulated nodule (-800 HU and 3 mm )detection rate of 80 kV was less than that of 120 kV;as the tube voltage reduced,or simulated nodule diameter decreased,or the density of simulated nodule reduced,the nodule’s diameter deviation was increased. Conclusion Compared with FBP,the capabilities of SAFIRE in pulmonary nodule detection in different densities and different sizes are same,and SAFIRE algorithm is helpful for accurate displaying of pulmonary nodules,and it can be used for low-dose CT lung cancer screening program.
9.Construction and identification of RNAi lentiviral vector targeting rat CD86 gene

Objective To construct an RNAi lentiviral vector targeting rat CD86 gene and detect its effect of gene silencing in dendritic cells. Methods The effective sequence of siRNA targeting rat CD86 gene was confirmed in our previous work. DNA oligo containing short hairpin frame was synthesized and re-annealed, and then cloned into pGCL-GFP lentivind expression vector. PCR and sequencing analysis were made for verifying the positive clones. The virus packaging plasmids were transfected into 293T cells to har-vest shRNA lentivirus. After infection in dendritic cells, real-time PCR and Western blot were performed to determine the expression level of CD86 at mRNA and protein of NC( negative control virus). Results PCR and sequencing analysis revealed that shRNA plasmid was correctly constructed. Virus with a titer of 2×108 TU/ml was successfully packaged. CD86 expression in dendritic cells can be knockdown at both mRNA and protein level by virus infection as characterized by 90.6% decrease of mRNA and significant inhibition of protein compared with NC. Conclusion The recombinant lentiviral shRNA expressing vector targeting rat CD86 gene has been successfully constructed and packaged. CD86 mRNA and protein can be effectively down-regulated in dendritic cells.
10.Application of MR fast scanning techniques in fetal congenital diaphragmatic hernia

Objective To investigate the diagnostic value of MR fast scanning techniques in fetal congenital diaphragmatic hernia. Methods 12 cases diagnosed or suspected fetal congenital diaphragmatic hernia by ultrasound with gestational age (>20 weeks) were underwent MR examination.The fast scan sequences were as follows:Half-Fourier acquisition single-shot fast spinecho se-quence(HASTE)、True fast imaging steady-state precession sequence (True FISP)、Turbo FLASH T1 WI sequence.The axial,co-ronal and sagittal scanning were focused on fetal head and body.The results of prenatal MR examination was compared with neonatal surgical findings or autopsy results after inducing labor.Results All of 12 cases,9 cases were left diaphragmatic hernia,2 cases were right diaphragmatic hernia and 1 case was hiatal hernia.2 cases of them were complicated with other congenital malformations. The features of MRI were as follows:Some portions of bowels and stomach crossed into the thorax were visible in the HASTE and TRUE FISP sequence.The herniated stomach and bowels showed high or scattered high signals.The intestinal types were visible as well.The lung tissue intruded at the apex of the lung and the heart deviated to the opposite side were demonstrated.T1 weighted im-ages obtained from ultra-fast FLASH (Turbo FLASH)T1WI sequence showed characteristic high signals in the meconium,which delineated clearly the shape of the colon and small intestine.The prenatal MRI results of all cases were consistent with surgical (10 cases)and autopsy (2 cases)findings.Conclusion MR fast scanning techniques can clearly show fetal congenital diaphragmatic her-nia.Multiple sequences scanning can comprehensively map the organizational structure and anatomical relationship of fetal congenital diaphragmatic hernia with the surrounding tissues.MR fast scanning techniques play an important role in the diagnosis of fetal con-genital diaphragmatic hernia.
