1.Effect Evaluation of Comprehensive Intervention with Low-salt Diet in Elderly Patients with High Blood Pressure

Objective To investigate the role of the extended-care in improving the compliance with low-salt diet in elderly patients with hypertension.Methods We selected 100 cases of elderly patients with hypertension in hospital.The 100 cases were divided into control group (50 cases) and observation group (50 cases) randomly.Observation group was given comprehensive intervention in low-salt diet education and control group was given traditional low-salt diet education.Tow groups were divided into three stages (on the day of admission,seven days after admission,one month after discharge).We analyzed low-salt cognitive scores,low-salt behavioral scores and salt-intake/d comparably.Results On the day of admission,tow groups had no statistically significant difference (P>0.05) in low-salt cognitive scores,low-salt behavioral scores and salt-intake/d;seven days after admission,tow groups had statistically significant difforence (P<0.05) in low-salt cognitive scores,low-salt behavioral scores and salt-intake/d;one month after discharge,tow groups had statistically significant difference (P<0.05) in low-salt cognitive scores,low-salt behavioral scores and salt-intake/d.Conclusion We use comprehensive intervention method to elderly patients with hypertension in low-salt diet education,through personalized low-salt diet education following evidence-based nursing,we can increase the compliance with low-salt diet in elderly patients with hypertension after discharge and improve blood pressure control effectively and to promote the image of the hospital and achieve the higher satisfaction from patients and families.
2.Analysis on the clinical effect of Yangxue Zhitong pills combined with Shuangbaisan on the treatment of children with hip synovitis

To observe the clinical effect of Yangxue Zhitong pills combined with Shuangbaisan on the treatment of children with hip synovitis in.Methods 60 children with hip synovitis from November 2013 to May 2015 were randomly divided into the observation group and the control group,30 cases in each group.The observation group were given conventional treatment and the control group was given Yangxuezhitong pills combined with Shuangbaisan.Hip VAS and blood flow index in the two groups were followed-up and compared.Results (5 days after treatment,hip VAS in the observation group(3.50±0.46),was lower than that of the control group(4.51±0.55)(P<0.05).②CRP,TNF-α,IL-6,IL-1 level in the observation group were(3.01±1.73mg/L,15.58±5.46pg/mlL,74.9±19.4pg/mL,22.57±4.01ng/L),which were lower than those in the control group(5.69±2.05mg/L,32.47±4.16pg/mL,97.6±24.2pg/mL,32.17±4.38ng/L)(P<0.05).③After treatment,total effective rate in the observation group(93.3%)was higher than that in the control group 66.7%(P<0.05).Conclusion The clinical therapeutic effect of Yangxue Zhitong pill combined with Shuangbaisan on the treatment of children with hip synovitis is exact,which is worthy of further clinical research and application.
3.Simultaneous determination of four anthraquinones in rat plasma by HPLC-FLD method and its pharmacokinetic study.

This paper aims to develop a method for the determination of aloe-emodin, rhein, chrysophanol and physcion and study the pharmacokinetic properties of four anthraquinones in rat plasma after oral administration of gardenia and rhubarb decoction. The plasma concentrations at different time points of four anthraquinones were determined by HPLC-FLD method. Plasma samples were extracted with liquid-liquid extraction procedure. Plasma samples were separated on a C18 column (4.6 mm x 150 mm, 5 μm), using 0.2% acetic acid and methanol as mobile phase at a flow rate of 1.0 mL min(-1) with gradient elution. The excitation and emission wavelengths were set at 430, 525 nm, respectively. DAS 2.0 software was applied to calculate the pharmacokinetic parameters. The results showed four anthraquinones can be absorbed. The main parameters of aloe-emodin, rhein, chrysophanol and physcion were as follows: C(max) for aloe-emodin was (0.085 ± 0.058), (3.772 ± 1.152), (0.464 ± 0.267), (0.028 ± 0.008) mg x L(-1) respectively; t(max) for rhein was (1.042 ± 0.510), (0.805 ± 0.307), (1.167 ± 0.283), (0.616 ± 0.162) h respectively; t½ for chrysophanol was (3.557 ± 1.250), (6.879 ± 1.126), (5.196 ± 2.032), (4.337 ± 1.816) h; AUC(0-t) for physcion was (0.504 ± 0.130), (9.558 ± 1.106), (2.545 ± 1.554), (0.052 ± 0.018) mg x h x L(-1). This paper developed a selective, accurate and sensitive HPLC-FLD method for the simultaneous determination of four anthraquiones in rat plasma.

4.Development of ABCC4 on various tumors and chemotherapy drugs

ATP binding cassette C4(ABCC4,MRP4)plays an important role in transshipment physio-logic,endogenous or exogenous substances. The over expression of ABCC4 gene has been found in many kinds of solid tumors and hematological malignancies. The target gene also influences metastasis and recurrence process. ABCC4 can reduce the intracellular concentration and the sensitivity of various chemotherapy drugs, which is bad for prognosis.

Objective To evaluate the pattern of energy metabolism and nutrients intake in patients with chronic viral hepatitis and posthepatitic cirrhosis to effectively direct their nutrition therapy.Methods Resting energy expenditure (REE) was measured with open-circuit indirect Jorimetry in 60 patients with chronic viral hepatitis and 60 patients with posthepatitic cirrhosis.Their normal basal energy expenditure (BEE) was predicted by Harris-Benedict equation and energy intake (EI) was determined by diet recall. Correlation between REE and indicators for nutrition assessment was analyzed.Results REE was (77? 21) kJ?kg~(-1)?d~(-1) in 60 patients with pusthepatitic cirrhosis,significantly lower than BEE[(95?16) kJ? kg~(-1)?d~(-1)(P0.05,and their EI was (127?34) kJ?kg~(-1)?d~(-1),1.41?0.43 times as REE,in which PROI was (1.02?0.29) g?kg~(-1)?d~(-1),1.31?0.61 times as PROE (0.87?0.34) g?kg~(-1)?d~(-1),also indicating a negative nitrogen balance (-2.02?4.07).REE,EI and intake of three nutrients,serum level of albumin and prealbumin (PA) and body weight significantly decreased in patients with posthepatitic cirrhosis,as compared to those in patients with chronic viral hepatitis (P
7.Progress of study on neuritin.

Neuritin is a new member of the neurotrophic factor family, whose gene is named cpg15 (candidate plasticity-related gene 15) and can be activated by neural activity or neurotrophins (NTs). Experiments show that neuritin is able to promote the growth and branching of neurites, and plays an important role in neuronal plasticity and neuronal regeneration. Recent studies have proved that neuritin is not only involved in the regulation of various physiological functions in the nervous system, but also related in angiogenesis and tumorigenesis. Here we review the mechanisms involved in cpg15 expression and regulation, biological effects of neuritin, and how neuritin plays its biological activities. The hot issues and difficulties in the study of neuritin are also discussed.

8.Genetic characteristics of breakthrough varicella-zoster virus infection during the varicella outbreaks in Minhang District of Shanghai

Objective To analyze the genetic characteristics of breakthrough varicella-zoster virus infection during the varicella outbreaks in Minhang District of Shanghai in 2013-2014. Methods Samples of the varicella-zoster liquid were collected from patients with chickenpox in Minhang District in 2013-2014 and used for the extraction of genomic DNA. The open reading frame ( ORF) of 22 and 62 regions were se-quenced and further analyzed by using bioinformatics methods. Results A total of 24 samples were success-fully collected and sequenced, and all of them were wild strains. Among the 24 varicella-zoster virus strains, 23 strains were highly homologous to the parental strain ( P-Oka) and the vaccine strain ( V-Oka) , indica-ting that they belonged to J genotype. Only one strain, whose genotype was between M and E, was highly ho-mologous to the mosaic( M) CA123 strain ( M1 genotype) , indicating that it might belong to M1 genotype. Conclusion The epidemic strains of varicella-zoster virus in Minhang District were mainly J genotype. Lo-cal epidemic of M and other genotypes of varicella-zoster virus also existed. There were some gene variations in different strains of J genotype. These varicella-zoster virus strains of non-vaccine genotypes might be one of the reasons causing the breakthrough cases of chicken pox.
9.Efficacy of fascia iliaca compartment block with dexmedetomidine combined with ropivacaine for analgesia in patients suffering from proximal femoral fractures

Objective To evaluate the efficacy of fascia iliaca compartment block with dexmedetomidine combined with ropivacaine for analgesia in the patients suffering from proximal femoral fractures.Methods Eighty emergency patients with proximal femoral fractures,aged 25-70yr,weighing 55-82 kg,of ASA physical status Ⅰ-[Ⅲ,were equally and randomly divided into 2 groups using a random number table:ropivacaine group (group R) and dexmedetomidine mixed with ropivacaine group (group DR).All the patients underwent fascia iliaca compartment block described by Dalens.0.4% ropivacaine 30 ml was injected in group R,and 1 μg/kg dexmedetomidine 30 ml containing 0.4 % ropivacaine was injected in group DR.The severity of pain was assessed by VAS scores,and the level of sedation was assessed by Ramsay scores.At 30,60,90 and 120 min after injection (T1-4),VAS scores at rest and during activity were recorded,the effective analgesia (VAS scores at rest and during activity≤6) and satisfactory sedation (Ramsay scores 2-4) in group DR were also recorded,and the development of hemorrhage or hematoma at the puncture site,local anesthetic poisoning,adverse cardiovascular events and over-sedation was also recorded.Results Compared with group R,the rate of effective analgesia during activity was significantly increased at T2-4,and no significant change was found in the rate of effective analgesia at rest in group DR.In group DR,the rate of satisfactory sedation was 73 %,88%,95% and 95% at T1-4,respectively,and no over-sedation occurred.No patients developed hemorrhage or hematoma at the puncture site,or local anesthetic poisoning in the two groups.Conclusion Fascia iliaca compartment block with 1 μg/kg dexmedetomidine combined with 4% ropivacaine 30 ml can alleviate the early pain caused by passive activity without inducing obvious adverse reactions in the patients suffering from proximal femoral fractures.
