2.Application of the glycated haemoglobin in screening undiagnosed diabetes in hospitalised patients

Objective To assess the economic efficiency of glycated haemoglobin(HbA1C)determination in screening the undiagnosed diabetes among hospitalised patients. Methods Exclusive criteria were made based on the information of the electronic patient history,including age<18 years,hospitalization for diabetes treatment, and having received a transfusion within half a year. Pathology samples from participants were collected for blood routine analysis and HbA1C screening test. Screening the undiagnosed diabetes was based on the level of HbA1C. Results In this study ,1012 patients were enrolled ,78 patients with diabetes ,and 934 patients haven′t been diagnosed before. Among the 934 patients ,HbA1C level of 51 patients was over 6.5%(48 mmol/mol). These 51 patients (5.46%) were determined to have previously unknown diabetes. The prevalence of undiagnosed diabetes was 5.46% during the study period. The cost of HbA1C test was ¥1098 per new diagnosis of diabetes. Conclusions HbA1C is a simple ,inexpensive screening test for diabetes ,which can significantly improve the diagnostic efficiency,and the early detection of diabetes can slow the progression of complication and reduce the medical care expenditures.
3.Vascular supply of intrinsic muscles of foot and anatomic basis for muscular flaps design.

OBJECTIVETo investigate the vascular supply of intrinsic muscles of foot and anatomic basis for muscular flap design.
METHODSA radiopaque injectate (lead oxide-gelatin mixtures, 26 ml/kg) was injected into 10 fresh cadavers. The dissected regions were photographed and each intrinsic muscles on the foot was removed and radiographed. The number, type, diameter of vascular branches of muscles and their distributions were observed. The area of the vascular territory supplied by each source vessel was calculated using Scion Image for Windows software.
RESULTSThere were significant architectural differences among the intrinsic muscles. The muscles length varied from 22.5mm to 116.2mm [average, (66.1 +/- 23.2)mm]. The measured fiber length were relatively consistent, ranging from 14.2 mm to 27.5 mm [average, (20.2 +/- 4.5)mm]. There are 63 vascular branches into the 23 foot muscles, each muscle having average branches of 3.2 +/- 0.8. The average diameter of branches, the length and width of each vascular territorial area is (0.8 +/- 0.3) mm, (2.2 +/- 0.8) cm, and (0.9 +/- 0.4) cm, respectively. Other findings included that some muscles were not present in some cadavers.
CONCLUSIONSThe blood supply of intrinsic muscles of foot is abundant with different diameter and distributions of branches. There is an anatomic basis for muscular or musculoosseous flap design. There are 7 intrinsic muscles with large and reliable vascular supply which can be chosen as muscular flaps.

4.Evaluation of dynamic morphology of atrial septal defect by real-time three-dimensional transesophageal echocardiography

Objective To determine whether real-time three-dimensional transesophageal echocardiography(RT-3D TEE) is an accurate non-invasive technique for defining the morphology of atrial septal defects(ASD). Methods In 20 patients with secundum ASD, mean age 42.2 years (7 male, 13 female) ,live three dimensional surgical views of ASD were acquired,qualitative morphology of ASD such as the shape, presence of fenestrations and the defect margins were noted during the cardiac cycle. The measurements obtained from 2-dimensional transesophageal echocardiography(2D-TEE) and RT-3D TEE were compared to those obtained from stretched balloon diameter(SBD) or surgery. Measurements of the size and area change of ASD were validated during the cardiac cycle by RT-3D TEE,stepwise multiple linear regression analysis was performed to test the correlation between the maximum change ratios of area and diameter of ASD and age, atrial septal length, ASD shunting velocity, the maximum diameter of ASD and right ventricular systolic pressure. Results ① The morphology of ASD was circle-like or elliptical in systole,irregular or elliptical in diastole. ②The area and diameter of ASD measured by RT-3D TEE was minimal in isovolumetric contraction phase, maximal in isovolumetric diastole phase. The correlation coefficient of maximum diameter measured by 3D with SBD was 0.962 , greater than that by 2D and SBD (0.820). ③The change ratio of area and maximum diameter of ASD acquired by RT-3D-TEE was 11.48%-71.12% and 2.80%-43.87% respectively,and the correlation coefficient of them was 0.921. Conclusions RT-3D TEE using live 3D-Zoom mode accurately displayed the varying morphology,dimensions and spatial relations of ASD. RT-3D TEE can offer visualization morphological changing of ASD in different periods of cardiac cycle,providing more information for percutanous catheter intervention and open heart surgery.
5.Evaluation of left and right atrial volumes and functions of fetuses in second and late trimester of pregnancy by atrial volume tracking

Objective To evaluate the left atrial(LA) and right atrial(RA) volumes and functions of fetuses in second and late trimester by using atrial volume tracking (AVT).Methods One hundred and seventy-six fetuses were enrolled in this study.Using AVT,the maximal and minimal volumes of RA and LA were measured from time-atrial volume tracking curves.RA and LA ejection fraction (RAEF and LAEF) derived from the value of atrial maximal volumes(LAVmax,RAVmax) subtracted atrial minimal volumes (LAVmin,RAVmin) and divided by LAVmax or RAVmax.Pearson correlation analysis was used to analyze the relationships between LAVmax,RAVmax,LAVmin,RAVmin,LAEF,RAEF and gestational ages.Results For all normal cases,the volumes of LA and RA were closely correlated with gestational weeks(r =0.80,0.85,0.83,0.82,P <0.001).The volume of LA was significantly smaller than that of RA (P <0.01).The RAEF and LAEF remained stable with advancing gestational age and there was no significantly difference between RAEF and LAEF (P >0.05).Conclusions This study establishes normal reference values for fetal RA and LA volumes during the second and third trimesters of pregnancy.The volumes of RA and LA are correlated with gestational weeks,and the volume of RA is bigger than that of LA.RAEF and LAEF are uncorrelated with gestational weeks,and there's no difference between RVEF and LAEF.
6.Measles-Mumps-Rubella vaccine for children with egg allergy: Is admission for inpatient vaccination necessary?

Introduction: Children who develop any hypersensitivity
reaction to eggs are routinely referred to hospital for
Measles-Mumps-Rubella (MMR) vaccination as inpatients to
prevent anaphylaxis. We aimed to study the association
between hypersensitivity reactions after egg exposure and
similar reactions after MMR immunisation; and examine the
necessity of hospital admission for vaccination.
Methods: A prospective observational study was conducted
in Paediatric Department in Bukit Mertajam Hospital,
Penang, between March and December 2014. Children
referred from local polyclinics for inpatient MMR vaccination
because of a history of egg allergy were recruited. The
children were observed in the ward for post vaccination
allergic reactions. Concurrently, a group of children without
egg allergy was recruited from those admitted for other
illnesses but had recent MMR vaccination at polyclinics.
Parents of these children were interviewed and asked if they
had observed any reactions post vaccination. In both
groups, sociodemographics, medical history and family
history of atopy were collected.
Results: Eighty-seven subjects were recruited in this study.
Fifty-four infants with egg allergy had previous mild allergic
reactions after exposure to eggs or egg-related products.
They were associated with a family history of egg
hypersensitivity, personal history of acute gastroenteritis
and upper respiratory tract infections. Two of them
developed cutaneous rashes post vaccination during
observation, but none developed anaphylactic or
anaphylactoid reactions. Two infants among those without
egg allergy had post vaccination fever. There was no
association between egg allergy and hypersensitivity
reactions to MMR vaccine (p=0.632).
Conclusions: MMR vaccine can be safely administered to
children with mild egg allergy, hence admission for
vaccination in the hospital is not warranted. Risk
stratification is required to ensure only infants with severe
reactions will be admitted for vaccination.

7.Clinical effect of vitrectomy with intravitreal ranibizumab for diabetic retinopathy

AlM: To observe clinical effect of vitrectomy with intravitreal ranibizumab for the treatment of diabetic retinopathy ( DR) .
METHODS:From February 2011 to February 2013, there were 90 cases in our hospital diabetic retinopathy patients withvitrectomy surgery. lt was randomly divided into observation group ( 45 cases ) and control group ( 45 cases ) . Two groups of patients were performed vitrectomy. Patients in observation group were injected intravitreal ranibizumab before surgery, then vitrectomy was underwent for diabetic retinopathy. Vitrectomy was only underwent in control group.
RESULTS:The patients in observation group with good effect accounting for 71% (32/45) and good rate was 89%(40/45 ), which were significantly higher than that in control group 51% ( 23/45 ) , 71% ( 32/45 ) . Differences were statistically significant ( P < 0. 05 ). Amount of bleeding was 9. 64 ± 2. 27mL, intraocular pressure was 13. 64 ± 3. 27mmHg. lt was significantly less than that in control group 10. 21 ± 3. 14mL, 16. 00 ± 3. 14mmHg. Differences were statistically significant (P<0. 05).
CONCLUSlON: The vitrectomy with intravitreal ranibizumab treatment of diabetic retinopathy can not only reduce blood loss, but also reduce edema and improve effectiveness and safety. lt's worth recommending for clinical practice.
8.Progress of experimental researches on Chinese herbal compounds for inducing tumor cell apoptosis.

Cell apoptosis is an initiative process of cell death regulated by genes. Inducing cell apoptosis is a new way of cancer treatment. In this paper, the progress in experimental studies on the effect of Chinese herbal compounds (CHC) on the induction of tumor cell apoptosis will be reviewed in terms of major mechanisms (gene expression regulation, cell morphology change, telomerase activity change, and immunity enhancement, etc.). The study on disassembled CHC, as well as the synergistic effect when in combined use with chemotherapy for inducing apoptosis, is also reviewed here.

9.Progress in research on oocytes parthenogenetic activation.

Parthenogenetic activation is a procedure that an oocyte at meiosis II stage is activated into mitosis by some chemical or physical stimulation other than a sperm and the embryo is formed in the absence of any contribution from a male gamete. The activation of oocyte is the result of calcium ion oscillations and deactivation of some cytokines such as maturation promoting factor, mitogen-activated protein kinase and cytostatic factor. Parthenogenetic activation is artificially induced by various kinds of physical and/or chemical methods. The main activation method of human oocyte is chemical methods. The rates of activation and cleavage depend on the age, origin,and culture conditions of the oocyte.

10.Multi-epitope DNA vaccines against avian influenza in chickens.

Multiple epitopes from one or more viruses can be lined up and co-expressed in one vector to generate multi-epitopes DNA vaccines. In the study, four recombinant plasmids were constructed based on HA and NP gene of avian influenza virus (AIV) (H5N1): (1) pIRES/HA, carrying the complete HA gene; (2) pIRES/tHA, carrying a truncated HA gene fragment of major neutralizing antigenic epitopes; (3) pIRES/tHA-NPep, in which three CTL epitopes of NP gene of AIV were fused to the truncated HA from the C-terminal; and (4) pIRES/tHA-NPep-IFN-gamma, which was constructed by replacing neo gene in pIRES/ tHA-NPep with IFN-y of chicken. Fifty five SPF chickens were randomly divided into five groups and immunized with the above four constructs and control plasmid. Each chicken was intramuscally immunized with 200 microg plasmid DNA three times in a two-week interval. Two weeks after the third immunization, chickens were injected with H5N1 subtype avian influenza virus. Before the virus loading no detectable antibodies to HA were found in the chicken serum; but high levels of HI antibodies were detected in the serum of the survived chickens. The percentages of CD4+ and CD8+ T lymphocyte in peripheral blood of immunized chickens increased steadily after the vaccination. After virus loading all chickens in the control group died within three to eight days, and the survival rates of the four DNA vaccine groups were as follows: pIRES/HA, 54.5%; pIRES/tHA, 30%, pIRES/ tHA-NPep, 36.3%, pIRES/tHA-NPep-IFN-gamma, 50%. These results indicated that multi-epitopes DNA immunization can induce immune response and protect chickens from homologous virus loading.
