1.Efficacy and Safety of Ornidazole in the Treatment of Trichomonal Vaginitis

Objective:To explore the efficacy and safety of ornidazole in the treatment of trichomonal vaginitis. Methods:Totally 112 cases of trichomonal vaginitis from February 2011 to December 2012 in our hospital were selected and randomly divided into the ob-servation group and the control group with 56 cases in each. The control group was treated with metronidazole tablets 200mg, po,tid, and the observation group was treated with ornidazole tablets 500mg, po,bid. The treatment course was 7 days. The clinical effect, score of clinical symptoms and incidence of adverse reactions of the two groups were compared. Results:The total effective rate of the observation group was 96. 4%, which was higher than that of the control group (82. 1%), and the difference was statistically signifi-cant (P<0. 05). The symptom scores of the two groups were significantly improved after the treatment (P<0. 05), however, the score between the two groups showed no significant difference (P>0. 05). The incidence rate of adverse reactions in the observation group was 10. 71%, which was significantly lower than that in the control group (37. 5%), and the difference was statistically signifi-cant (P<0. 05). Conclusion:The clinical curative effect of ornidazole in the treatment of trichomonal vaginitis is notable with faster onset, shorter treatment course, lower adverse reaction rate and better patient tolerance,which is valuable in the clinical promotion.
2.Retrospective Analysis of Clinic and Epidemiology of Respiratory Syncytial Virus Pneumonia in 309 Neonates

Objective To explore the characteristics of clinic and epidemiology of neonatal respiratory syncytial virus(RSV) pneumonia in newborn infants.Methods Three hundred and nine neonates(male 196,female 113) who were admitted to children's hospital of Fudan University and identified as having RSV pneumonia from Jan.2004 to Dec.2008 were enrolled.RSV antigen was detected in exfoliated respiratory cells by direct immunofluorescence.All the clinical data were collected and subjects and they were divided into different groups based on path of infection,gestational age and RSV status.Results From case notes over the 5 years,309 eligible neonates with RSV infection were identified.Male term neonates were more likely to acquire infection,as compared to female,1.71.0.The incidence of RSV infection peaked in winter and spring.Compared with community acquired infection group,hospital acquired infection group had more premature infants(23.2% vs 8.7%,P=0.002),lower birth weight[(3 010.8?852.8) g vs(3 153.2?943.4) g,P=0.026],much longer mean length of hospital stay[(19.0?8.0) d vs(12.2?4.5) d,P=0],more expensive cost[(5 646.4?3632.2) RMB vs(4 175.8?2 879.2) RMB,P=0] and later occurrence day[(21.3?8.6) d vs(15.8?6.0) d,P=0].Compared with simple RSV infection group,mixed infection group had more expensive cost[(6 063.1?3 085.4) RMB vs(4 513.2?3 860.8) RMB,P=0.047] and more oxygen use(40.0% vs 25.7%,P=0.006).The clinical characteristics of patients with RSV pneumonia varied.Preterm group compared with term group more frequently exhibited apnea and cyanosis(20.0% vs 0.7%,42.9% vs 22.7%),respectively.Forty-two percent of them had bacterial infection,while 13.3% of them had infection of other part of body.The prognosis of most RSV(99.0%) infections was good.Conclusions RSV was an important cause of respiratory tract infections during the last five years.Clinical characteristics of RSV infections were atypical.Hospital acquired RSV infection was more severe than the community acquired infection in neonates.There are no specific,effective interventions for treating RSV infections,so preventive measures are most important.

4.Expression and clinical significance of selenium binding protein 1 in gastric cancer

Objective To detect the expression of selenium binding protein 1 ( SBP1 ) in gastric cancer cell line SGC7901, BGC823, normal gastric epithelial cell line GES-1, gastric cancer tissues and normal gastric tissues, explore the relationship between SBP1 and pathologic features, and discuss the feasibility of SBP1 as an diagnostic marker of gastric cancer. Methods The clinical data of 135 patients with gastric cancer who were admitted to Zhongnan Hospital of Wuhan University from 2006 to 2007 were retrospectively analyzed. The expression of SBP1 in the gastric cancer tissues and 16 cases of normal gastric tissues were detected by immunohistochemistry. The mRNA and protein expressions of SBP1 of SGC7901, BGC823 and GES-1 were determined by Western blot and RT-PCR. All data were analyzed by using chi-square test and one-way analysis of variance.Results The mRNA expressions of SBP1 in BGC823 and SGC7091 were 0. 120 ± 0. 020 and 0. 133 ± 0. 015,respectively, which were significantly lower than 0. 907 ± 0. 015 in GES-1 ( F = 2106. 462, P < 0.05 ). The protein expression of SBP1 in BGC823 and SGC7901 were 0.253 ±0.015 and 0.273 ±0.015 ,respectively, which were significantly lower than 0.877 ±0.025 in GES-1 ( F = 1026. 758, P <0.05 ). A strong positive reaction of SBP1 was observed in 3 cases of gastric cancer tissues and 16 cases of normal gastric tissues. The decrease of the protein expression of SBP1 was correlated with clinical stages of the patients ( x2 = 12. 629, P < 0.05 ), rather than the sexes, ages, tumor histological types, tumor differentiation, infiltration depths and lymph node metastasis (x2 =2. 142, 0.860, 1.838, 5.001,4.858, 1.994, P>0. 05). Conclusions The decrease of SBP1 expression could be used as a marker in diagnosing gastric cancer. Down-regulation of SBP1 expression may play an important role in the genesis and prognosis of gastric cancer.
5.Transdermal fentanyl in the management of cancer pain

Objective To evaluate the pain control, general condition and well-being of the patients with advanced cancer treated with transdermal fentanyl as well as the effects of transdermal fentanyl on heart rate variability (HRV) and endocrine function. Methods This open prospective study included 40 patients with advanced cancer. The direct conversion ratio from oral morphine to transdermal fentanyl system (TFS) was 100:1 daily. In patients who had never received any opioid before the initial dose of TFS was 25?g/h. The transdermal system was changed every 72h and the dosage was adjusted to the need of the patients according to the VAS scores and the requirement of morphine. 8 hospitalized patients were monitored by 24h Holter for changes in HRV. Blood samples were taken before transdermal fentanyl patch when pain was severe and 24h after TFS for determination of ACTH,?-endorphin, growth hormone(GH) and insulin levels.Results Pain relief during treatment with transdermal fentanyl was significant accoring to VAS scores. Well-being scores showed that patients' quality of life including appetite, sleep, mental state and daily activities was significantly improved after two weeks treatment. ObjectiveTo observe the effect of telmisartan on the level of serum adiponectin and the expression of cardiac adiponectin (APN) receptor 1 in spontaneously hypertensive rats (SHR) in order to explore the role of APN in ventricular remodeling caused by hypertension and the new possible mechanism that telmisartan protects the heart against ventricular remodeling.Methods The sixteen 12-week-old SHR were randomly divided into SHR control group (SHR group, n=8) and SHR+telmisartan group (TEL group, n=8), and eight age-matched male Wistar Kyoto rats were regarded as control group (WKY group). The TEL group was treated with telmisartan 5 mg-1 · kg-1 ·d-1 in a small amount of distilled water and the other two groups were given equal amount of distilled water by gavage.After 8 weeks, the left ventricular end diastolic diameters (LVEDD),thickness of interventricular septal thickness (IVST), left ventricular post wall thickness (LVPWT) and mitral valve blood flow spectrums were recorded by echoeardiograph.The E/A ratio was calculated by velocity of E peak divided by A peak. The left ventricular weight index (LVWI) was calculated. HE staining and Masson staining were used to detect the pathology changes of cardiac tissue and collagen volume fraction (CVF).Serum APN concentration of SHR was detected by enzyme-linked immunosorbent assay (ELISA). The myocardial protein expression of AdipoR1 was detected by in munohistochemical staining.Reverse transcription polymerase chain reactions (RTPCR) method was used to detect the mRNA expression of AdipoRl in myocardium.ResultsThe values of IVST, LVPWT and LVWI were higher, LVEDD and E/A ratio were lower in SHR group than in WKY group.The disordered arrangement of myocardialcells,interstitialfibroblast hypertrophy and hyperplasia were observed in SHR group. The CVF increased in SHR group than in WKY group [(6.22±0.16)% vs. (3. 18±0. 17)%, P<0.05]. The concentration of serum APN was significantly lower in SHR group [( 10.96±2. 15)μg/ml vs. (24.32±2. 20)μg/ml, P<0. 01].The expressions of myocardial AdipoR1 mRNA and protein were significantly lower in SHR group.The values of IVST, LVPWT and LVWI were lower, LVEDD and E/A ratio were higher in TEL group than in SHR group. The arrangement of myocardial cells were relatively ordered and interstitial fibroblasts hypertrophy and hyperplasia were not significant, CVF was decreased in TEL group versus SHR group. The concentration of serum APN was significantly higher in TEL group [(17.71 ± 5.82)μg/ml, P<0. 01]. The expressions of myocardial AdipoR1 mRNA and protein were significantly higher in TEL group than in SHR group. Conclusions When SHR undergoes ventricular remodeling, the level of serum APN is lower, the mRNA and protein expressions of myocardial AdipoRl are significantly reduced. Telmisartan can reverse the ventricular remodeling and play the role in protecting heart in SHR, which may be due to the serum APN levels increasing, cardiac APN receptor 1 and its mRNA up-regulation.
10.Expressions and Significances of Caveolin-1 and Tight Junction Proteins in Schistosomiasis Colitis in Mice

Intestinal schistosomiasis is a kind of intravascular parasitic diseases, and chronic inflammation of colon is one of the basic pathological changes of the sickness.However, the mechanism of caveolin-1 and tight junction proteins in the pathogenesis of intestinal schistosomiasis is still unclear.Aims: To study the expressions and significances of caveolin-1 and tight junction protein occludin, claudin-1 in schistosomiasis colitis in mice.Methods: Forty BALB/c male mice were randomly divided into control group and infection group.Schistosomiasis colitis model was established by placing 40 Schistosoma Japonicum cercarie on the abdomen.Mice were sacrificed after 8 weeks.HE staining was performed.The permeability of intestinal vascular endothelium was detected by Evans blue method.The leukocyte counts in peritoneal lavage fluid were measured.qPCR was used to determine the mRNA expressions of caveolin-1, occludin, claudin-1 and eNOS in colon tissue.Western blotting and immunohistochemistry were used to detect the protein expressions of caveolin-1 and occludin.Results: Large number of egg granuloma was observed in colon submucosa and accompanied by extensive inflammatory cells infiltration in infection group.Compared with control group, content of Evans blue and leukocyte counts in peritoneal lavage fluid were significantly increased (P<0.05);mRNA expressions of caveolin-1, occludin, claudin-1 were significantly decreased (P<0.01);protein expressions and positivity rates of caveolin-1 and occludin were significantly decreased in infection group (P<0.05).Conclusions: Down-regulation of expressions of caveolin-1, occludin and claudin-1 can induce leukocyte accumulation via increasing the permeability of intestinal vascular endothelial cells, thereby involving in the development of schistosomiasis colitis.
