1.Non-traditional antiarrhythmic medicines for atrial fibrillation

Atrial fibrillation is the most common arrhythmia in clinical practice.However,the therapeutic effects of traditional antiarrhythmic medicines are far from satisfactory,because of the high rate of arrhythmia recurrence and the potential proarrhythmia effect.Recently,with the further investigation of the mechanisms that initiate and perpetuate atrial fibrillation,and the support of extensive data from clinical trials and animal experiments,thera-peutics based on anti-inflammation,antioxidation,regulation on renin-angiotensin-aldosterone system(RAAS)and gap junction may be new therapeutic targets for the treatment of arrhythmia.In this paper,the potential molecular mechanisms involved in these processes and several clinical trials about some non-traditional antiarrhythmic medi-cines were reviewed,in order to propose a new idea about the development of antiarrhythmic medicines.

3.Water Supply and Latrine Improvement and Prevention of Diarrheal Diseases in Rural Areas in Sichuan Province

Objective To survey the effect of innovation of rural water supply and latrine improvement to the control of the incidence of diarrheal disease in Sichuan Province,to provide the scientific basis to the prevention and treatment of diarrheal disease.Methods Research and analyze the condition of the rural water supply innovation,latrine improvement and the incidence of diarrheal disease on 12 administrative villages in Luojiang and Danling County from Dec.2006 to Sep.2007.Results Among the 1 659 houses under research,the main type of water supply is non-central water supply,accounting for 92.65%.The main origin of central water supply is underground water,accounting for 83.6%.Only 716 houses have sanitary latrine,accounting for 43.16%,the main type of sanitary latrine is marsh gas pool,accounting for 96.79%.The main type of non-sanitary latrine is dry latrine without leak,accounting for 92.79%.Mongzi,Longtan,Shihe and Meiwan are both water supply and latrine innovated countries,Yujiaan,Minghui are only latrine innovated countries,Mingyue,Wuying and Sanyan are noninnovated countries.During the 20 551 persons under research for four times,192 persons have diarrheal symptom.The annual incidence is 0.93%.The diarrheal disease for both water supply and latrine innovated country,either water supply or latrine innovated country,non-innovated country is 0.70%(48/6 872),0.91%(77/8 506),1.30%(67/5 173).There is significant discrepancy(?2=11.486,P0.05).The incidence of diarrheal disease of latrine improvement country is lower than none-latrine improvement country.There is significant discrepancy(?2=15.061,P
4.Safety and effectiveness of 6% hydroxyethyl starch 130/0.4 versus crystalloid solution for patients undergoing elective surgery:a meta-analysis

Objective To systematically review the safety and effectiveness of 6% hydroxyethyl starch (HES) 130/0.4 and crystalloid solution for the patients undergoing elective surgery .Methods We searched the Medline , EMBASE , CENTRAL , CNKI and Wanfang databases for randomized controlled trials involving comparison of the safety and effectiveness of 6% HES 130/0.4 and crystalloid solution for the patients undergoing elective surgery . The major evaluation indexes included intraoperative blood loss , postoperative incidence of dialysis and intraoperative incidence of hypotension . The secondary evaluation indexes included mortality , perioperative incidence of nausea and vomiting , coagulation function , renal function and incidence of adverse events .The two reviewers independently screened ,evaluated and extracted the data .Meta-analysis was performed using the Cochrane Collaboration’s RevMan 5.2 software .Results Ten trials involving 693 patients were included in our meta-analysis .Eight trials were in English and 2 trials were in Chinese .Comparison of the safety and effectiveness of 6% HES 130/0.4 and balanced solution was conducted in 7 trials (498 patients in total) ,and the results showed that 6% HES 130/0.4 reduced the intraoperative incidence of hypotension (OR 0.31 ,95% CI 0.13-0.75 ) and perioperative incidence of nausea and vomiting , (OR 0.08 , 95% CI 0.01 to 0.67 ) in the patients undergoing spinal anesthesia ( P< 0.05 );there was no significant difference in the intraoperative incidence of hypotension ,perioperative incidence of nausea and vomiting ,mortality ,and intraoperative blood loss under general anesthesia or combined spinal-epidural anesthesia between the two solution groups ( P> 0.05 );heterogeneity was found in the urine output under general anesthesia ( P<0.000 01 ,I2 =93% ) .The safety and effectiveness were compared between 6% HES 130/0.4 and normal saline in 3 trials (195 patients in total) and the results showed that there was no significant difference in the intraoperative blood loss ,intraoperative incidence of hypotension ,perioperative incidence of nausea and vomiting ,coagulation function ,renal function ,and incidence of adverse events between the two solution groups ( P> 0.05 ) .Conclusion 6% HES 130/0.4 is safe and effective when applied for the patients undergoing elective surgery .
5.Stromal cell derived factor-1/chemokine receptor 4 signaling pathway is involved in bone morphogenetic protein 2-induced migration of bone marrow mesenchymal stem cells

BACKGROUND:Stromal cel derived factor-1 (SDF-1)/chemokine receptor 4 (CXCR-4) biological axis plays a chemotactic role in a variety of cel s, making it possible to regulate the regeneration of a variety of tissues. Whether the bidogical is involved in bone morphogenetic protein-2 (BMP-2)-induced homing of bone marrow mesenchymal stem cel s, however, is stil unclear.
OBJECTIVE:To study the role of SDF-1/CXCR4 signaling pathway in BMP-2-induced migration of mouse bone marrow mesenchymal stem cel s.
METHODS:Bone marrow mesenchymal stem cel s in logarithmic growth were selected and intervened with SDF-1 (0, 50, 100 and 200μg/L), BMP-2 (0, 50, 100 and 200μg/L) and AMD3100 (50μg/L) to induce cel migration detected by Transwel method.
RESULTS AND CONCLUSION:The migration of bone marrow mesenchymal stem cel s was closely related to SDF-1 and BMP-2, and proportional to the concentration of both SDF-1 and BMP-2. When SDF-1 and BMP-2 were used jointly, the number of migrated cel s was increased significantly, and highest number of migrated cel s was obtained at 200μg/L. Moreover, these migrated cel s showed a nest-like distribution under microscopy. AMD3100 as an inhibitor markedly suppressed the migration of bone marrow mesencnymal stem cel s induced by BMP-2, but the number of migrated cel s was likely to increase with the increasing concentration of BMP-2 that exceeded a specific value. Overal , our findings show that SDF-1/CXCR4 signaling pathway is an important pathway in BMP-2-induced migration of bone marrow mesenchymal stem cel s.
6.Kidney cancer stem cells and epigenetic regulation

Targeted therapeutic strategy for cancer stem cells (CSCs) is the key to prevent tumor relapse and metastasis. The im-portant roles of epigenetic regulation on the development of stem cells and gene reprogram of somatic cells suggest that this process may remarkably affect the occurrence and development of CSCs. The epigenome, which comprises DNA methylation, histone modifica-tions, chromatin structures, and non-coding RNAs, controls gene expression patterns. In renal cell carcinoma (RCC), aberrant changes occur in the epigenome. To date, cells with CSC properties from RCC have been successfully isolated using different methods, such as sorting using the Hoechst 33342 side population, forming tumor spheroid, and sorting CD105 cell surface biomarker. According to the progress in genetic studies on RCC, in addition to DNA sequence, the abnormality in the regulatory mechanism has considerable func-tions in tumor progression. Epigenetic changes may be integral to the behavior of cancer progenitor cells and their progeny. Knowledge on epigenetics in renal tumorigenesis process is beneficial in the development of new therapeutic modalities and may deliver new prog-nostic and early diagnostic markers. This paper reviews the latest development in the study of RCC stem cells and the underlying mech-anisms of epigenetic regulation on the development of CSCs in RCC.
7.Prokaryotic expression and preliminary identification of protein CrdS of Helicobacter pylori.

The CrdS protein responding to the acidic adaptation was prokaryotic-expressed in our Laboratory to explore the regulatory mechanism in the acidic adaptation of Helicobacter pylori (H. pylori). The whole genomic DNA of H. pylori strain 26695 was abstracted and set as the template firstly. And then the hp1364 gene coding CrdS protein was amplified via the PCR technique. Then the clonal recombinant plasmid pUCm-T-hp1364 and the prokaryotic expression plasmid pQE30-hp1364 were built and identified by the methods of PCR, cutting with two enzymes and sequencing. After that, the plasmid pQE30-hp1364 was transferred into the E. coli XL1 blue and induced with IPTG. Using western blot and SDS-PAGE, it can be analyzed that the expressed recombinant protein existed mainly in the form of the inclusion bodies and its relative molecular mass was about 46 kDa. The successfully attained recombinant protein CrdS will provide the material to explore the regulatory mechanism in the acidic adaptation of H. pylori and the new way to resist the infection of H. pylori.

8.Core-crosslinked poly(N-isopropylacrylamide-co-N,N-dimethylacrylamide)-b-poly(ε-caprolactone)micelles for paclitaxel thermo-sensitive controlled release behaviors

BACKGROUND: Polymer micelles is a new type of drug carriers developed in recent years,with a wide range of carrying drugs,structural stability,excellent tissue permeability,long residence of drugs in vivo,and effective reaching the target.The performances of intelligent targeting and decreasing the initial burst release have become the focus of recent researches.OBJECTIVE: To obtain an intelligent targeting drug carrier of low critical solution temperature(LCST)at 40℃,to change drug release behavior through the changes of temperature,and to further improve the stability and drug release behavior of the micelles by core-crosslinking.METHODS: By radical polymerization of N-isopropylacrylamide(NIPAAm)and N,N-dimethylacrylamide(DMAAm),hydroxyl terminated poly(N-isopropylacrylamide-co-N,N-dimethyl acrylamide)[P(NIPAAm-co-DMAAm)]was synthesized.Molecular weight and LCST of P(NIPAAm-co-DMAAm)were regulated by adjusting the mercaptoethanol and monomer ratio,as well as the ratio of NIPAAm and DMAAm,Amphiphilic block copolymer P(NIPAAm-CO-DMAAm)-b-PCL was prepared via bulk ring-opening polymerization of ε-caprolactone by using the end hydroxyl group of P(NIPAAm-co-DMAAm)as initiator and stannous octoate as catalyst.The block copolymer reacted with acryloyl chloride to obtain amphiphilic block copolymers with unsaturated double bonds at the terminal.Drug loaded nano-micelles with different nuclear cross-linked degrees were prepared by dialysis method,and its release behavior was investigated.RESULTS AND CONCLUSION: Amphiphilic block copolymers,with the LCST of 42℃,were obtained with hydroxyl or acryloyl endgroup.By blending them at different ratios,thermo-sensitive drug-loaded nano-micelles with different core-cresslinking degrees were prepared.The drug release rate was faster at 43℃ than at 37℃.With the core-crosslinking degrees increasing,the release of paclitaxel gradually slowed.The results suggest that the drug release rate from micelles prepared from thermo-sensitive P(NIPAAm-co-DMAAm)-b-PCL can be regulated by the degree of cross-linking.
9.Research on In-vitro Percutaneous Absorption and Pharmacodynamics of Chinese Herbal Compound Cremor for Eczema with Different Mass Concentrations of Synthetic Borneol

Objective To compare in-vitro percutaneous absorption and pharmacodynamic actions of anti-inflammation and inhibiting delayed-type hypersensitivity of Chinese herbal compound cremor for eczema (CHCCE) with different mass concentrations of synthetic borneol. Methods By adopting modified Franz diffusion device add with isolated BALB/cnude mice skin as a barrier, in vitro percutaneous absorption effectiveness of CHCCE with different mass concentrations of borneol was compared by in vitro percutaneous test after the content of matrine was determined with high performance liquid chromatography(HPLC). Meanwhile, the effects of CHCCE with different mass concentrations of synthetic borneol on reducing dimethylbenzene-induced auricular edema and suppressing delayed-type hypersensitivity induced by 2,4-dinitrochlorobenzene(DNCB) in mice were compared. Results Cumulative permeation amount of matrine in CHCCE with synthetic borneol was higher than that in CHCCE without synthetic borneol 2~ 48 h after administration (P < 0.05). There was no significant difference of cumulative permeation amount (P>0.05) among CHCCE groups with different mass concentrations of synthetic borneol after 48 h. In vitro percutaneous absorption behavior of matrine arrived to the steady state and the cumulative permeation amount of matrine presented a decreasing trend in all medication groups 12 h after administration. Within 12 h of the medication, the permeation rate of CHCCE with different mass concentrations of borneol was in the sequence of 3% borneol > 1% borneol > 2% borneol > 0.5% borneol > no borneol. The content of matrine was decreased with the increase of mass concentration of synthetic borneol after 12 h. The results of pharmacodynamic actions of CHCCE showed that compared with the blank control group, CHCCE with 1%, 3% synthetic borneol could significantly suppress the acute inflammation induced by dimethylbenzene and inhibit contact dermatitis induced by dinitrochlorobenzene (DNCB) in mice(P < 0.05). Compared with Triamcinolone Acetonide Acetate and Miconazole Nitrate and Neomycin Sulfate Cream, CHCCE with 1%, 3%synthetic borneol could decrease the serum level of IL-4, but the difference was insignificant(P>0.05). Conclusion CHCCE with 1% synthetic borneol has good effects on in vitro transdermal absorption, and can suppress inflammation and delayed-type hypersensitivity effectively.
10.ADR Induced by Traditional Chinese Medicine:Analysis of 85 Cases

OBJECTIVE:To analyze the manifestations and characteristics of adverse drug reactions (ADR) induced by Traditional Chinese medicine.METHODS:85 ADR reports collected from the spontaneous reporting system in our hospital from 2005 to 2008 were analyzed retrospectively.RESULTS:ADR almost involved all the body system and 31.63% were nervous system.Main dosage form inducing ADR was injection (51.76%).CONCLUSION:ADR induced by Traditional Chinese Medicine can not be disregarded.Clinical pharmacists should pay more attention to the drug therapy by intravenous administration to reduce the incidence of ADR.
