1.Intractable facial paralysis treated with different acupuncture and acupoint embedding therapies: a randomized controlled trial.

OBJECTIVETo compare the clinical therapeutic effects on intractable facial paralysis among acupuncture, acupoint catgut embedding and PDS embedding.
METHODSTwo hundred and seventy-nine patients of intractable facial paralysis were randomized into an acupuncture group, a catgut embedding group and a PDS embedding group, 93 cases in each one. Cuanzhu (BL 2), Yuyao (EX-HN 4), Taiyang (EX-HN 5), Xuanlu (GB 5), Dicang (ST 4), Jiache (ST 6), Quanliao (SI 18), Qianzheng (Extra), Xiaguan (ST 7) and Fengchi (GB 20) on the affected side and bilateral Hegu (LI 4) and Zusanli (ST 36) were selected in the three groups. In the acupuncture group, acupuncture was applied to those acupoints, once every two days; 10 treatments made one session; the successive three sessions were required. In the catgut embedding group and the PDS embedding group, catgut and PDS were embedded separately, once every 10 days; 3 treatments made one session and the successive two sessions were required. The therapeutic cycle was 2 months in the three groups. The therapeutic results were analyzed statistically 2 months later. The scores of Sunnybrook facial nerve grading system and the facial nerve electroneurography (ENoG) were used for the efficacy assessment before and after treatment in the three groups.
RESULTSAfter treatment, the score of Sunnybrook facial nerve grading system was improved apparently after treatment as compared with that before treatment in any of the three groups (all P < 0.05). The score in either of the embedding groups was better than that in the acupuncture group (both P < 0.05). The difference was not significant between the catgut embedding group and the PDS embedding group (P > 0.05). After treatment, ENoG wave ampli tude values in the frontal muscle and orbicularisoris muscle on the affected side were improved as compared with those before treatment in the three groups (all P < 0.05). Those in the catgut embedding group and the PDS embedding group were better than those in the acupuncture group (all P < 0.05), and had no significant difference as compared with those on the healthy side (P > 0.05). In the acupuncture group, ENoG wave amplitudes on the affected side were lower than those on the healthy side (both P < 0.05).
CONCLUSIONAcupuncture, catgut embedding and PDS embedding at acupoints all achieve the therapeutic effects on intractable facial paralysis. However, the acupoint embedding therapy presents more advantages on the recovery of facial nerve function. The efficacy is similar between the catgut embedding and the PDS embedding.

2.The experimental study of the microbial contamination after the puncture needle downward and ex-posed in the air of infusion bottle stopper

Objective To study the relationship between the exposure time of puncture needle of infusion bottle stopper and microbial contamination during clinical intravenous transfusion. Methods A total of 600 cases from November 1, 2014 to January 31, 2015 who have received the clinical intravenous transfusion for investigation were selected.When replacing the infusion bottle (bag), inserting the puncture needle slowly across the bottle stopper and making the needle tip be canted to the transfusion bottle mouth (bag) of the rubber plug, gently squeezing the Murphy's tube until solution was not dripping, recording down the exposure time in the air of the needle tip from medicine droplet to the end. To dip the lower part with sterile swabs and culture the swabs in nutrient broth medium. Meanwhile, to replace the next bottle of medicine and get the remaining 2 ml of liquids into the culture broth medium, after 48 h, both of which medium were switched to blood plate culture cultivation for observing the general situation of the bacteria growth. Results Among the 600 cases of clinical transfusion, 24 cases were positive for sterile swabs microorganisms culture, positive rate was 4.0%, among which microorganisms, 15 cases were gram-positive coccus, 3 cases were gram-negative bacillus, 3 cases were gram-positive bacillus and 3 cases were fungi. Correspondingly, 3 cases were positive for liquid broth culture, positive rate was 0.5%as the gram-positive coccus. The exposure time and broth microbial culture result was statistically significant, while the exposure time and medicinal broth microorganisms culture result possesses had no statistical significance. Conclusions Inserting the puncture needle across the bottle stopper could successfully reduce the liquid drug residues in the infusion bottle (bag), however, which might also cause time-dependent microbial contamination during the exposure process in the air.

4.Prognostic value of 18F-FDG PET-CT in patients with peripheral T-cell lymphoma

Objective To investigate the prognostic value of interim and post-therapy 18F-FDG PET-CT in patients with peripheral T-cell lymphoma.Methods A retrospective analysis was conducted on data from 44 patients with newly diagnosed peripheral T-cell lymphoma who underwent interim (after 3 cycles of chemotherapy,27 cases) or posttherapy PET-CT (after the completion of first-line therapy,35 cases).Interim and posttherapy PET-CT status (positive vs negative) was visually interpreted according to criteria of the International Harmonization Project,and PET-CT status was assessed for its ability to predict progression-free survival (PFS) and overall survival (OS).Results Interim 18F-FDG PET-CT results were positive in 16 cases and negative in 11 cases.The median PFS and OS in the patients with positive results were 8 months and 14 months,respectively,while those in patients with negative results were 30 months and 39 months,respectively.The 2-year PFS and 3-year OS rates in patients with positive results at interim PET-CT were 18.8 % (3/16) and 12.5 % (2/16),respectively,while those in patients with negative results were 90.0 % (10/11) and 63.6 % (7/11),respectively (x2 =13.092,P =0.000; x2 =7.386,P =0.007,respectively).Posttherapy 18F-FDG PET-CT results were positive in 14 cases and negative in 21 cases.The median PFS and OS in patients with positive results were 10 months and 22 months,respectively,while those in patients with negative results were 26 months and 38 months,respectively.The 2-year PFS and 3-year OS rates in patients with positive results at posttherapy PET-CT were 7.1% (1/14) and 14.3 % (2/14),respectively,while those in patients with negative results were 76.2 % (16/21) and 57.1% (12/21),respectively (x2 =15.574,P =0.000;x2 =6.245,P =0.012,respectively).Conclusion Both interim PET-CT status and posttherapy PET-CT status have significant value in monitoring response to therapy and predicting prognosis for patients with peripheral T-cell lymphoma.
5.Study on HPLC fingerprint characteristics of Selaginella plants.

OBJECTIVETo establish a method to analyze HPLC fingerprint characteristics of 10 plants from Selaginella.
METHODHPLC was applied for establishment of fingerprints, which were used to evaluate and distinguish the different species of Selaginella.
RESULTThe different species from Selaginella showed different HPLC fingerprint characteristic. The samples of the same species but collected in different period, different environment or different locations showed certain difference in fingerprints
CONCLUSION2 important mutual fingerprint peaks were found in the 10 plants of Selaginella species and 5 peaks can be used as "main fingerprint peaks". The dates of these peaks can used for assessment of phylogenetic relation among species and evaluation of quality.

6.Epidemiological characters of Yunnan sudden death syndrome and its research progress.

Yunnan sudden death syndrome (YSDS) is an abruptly fatal disease of unknown etiology, found mostly in central or northwestern mountain area (with altitude between 1,815 and 2,225 meters) of Yunnan province from June to September every year. It occurs mostly in young female adults, with high incidences in Lisu, Yi and Miao ethnics and high familial aggregation. The clinical manifestation of YSDS is changeful and the pathological characteristic is lack of specificity. The pathogenesis may be attributed to several factors including poor hygiene and lower socioeconomic conditions, lack of Selenium or Chromium, infection of Coxsackie B virus, mushroom consumption and special geological conditions. This article reviews the epidemiologic features, clinical manifestations, pathological features, etiology and hypothesis in order to provide clues for the research of YSDS.

7.Levels of serum HMGB2 and HMGB3 and clinical significance in non-small cell lung cancer patients

Objective:To investigate the levels of serum high mobility group box (HMGB) 2 and HMGB3 and clinical significance in patients with non-small cell lung cancer (NSCLC) .Methods:A total of 137 NSCLC patients admitted to the First Affiliated Hospital of Xi'an Medical University from January 2020 to January 2022 were selected as the NSCLC group, and another 90 cases who underwent healthy medical checkups during the same period were selected as the healthy group. Serum HMGB2 and HMGB3 levels were compared between the two groups. The relationship between serum HMGB2 and HMGB3 levels and the clinical and pathological characteristics of NSCLC patients was analyzed. NSCLC patients were divided into a good prognosis group ( n=86) and a poor prognosis group ( n=51) according to the prognosis, and the clinical data of the two groups were compared. Multivariate logistic regression was used to analyze the influencing factors of the prognosis of NSCLC patients. Receiver operator characteristic (ROC) curve was used to analyze the predictive value of serum HMGB2 and HMGB3 on the prognosis of NSCLC patients. Results:Serum HMGB2 [ (6.35±1.66) ng/ml vs. (2.58±0.76) ng/ml, t=20.19, P<0.001] and HMGB3 [ (2.48±0.56) ng/ml vs. (1.09±0.13) ng/ml, t=23.13, P<0.001] levels in NSCLC group were higher than those in healthy group. Serum HMGB2 and HMGB3 levels of NSCLC patients with a history of smoking ( t=2.80, P=0.006; t=5.04, P<0.001), lymph node metastasis ( t=3.53, P=0.001; t=4.02, P<0.001), and TNM stage Ⅲ-Ⅳ ( t=2.58, P=0.011; t=3.82, P<0.001) were significantly higher than those of patients with no history of smoking, no lymph node metastasis, and TNM stage Ⅰ-Ⅱ. The serum levels of HMGB2 [ (7.80±1.83) ng/ml vs. (5.49±1.56) ng/ml, t=7.85, P<0.001] and HMGB3 [ (2.91±0.78) ng/ml vs. (2.23±0.43) ng/ml, t=6.58, P<0.001) ] in the poor prognosis group were higher than those in the good prognosis group, and the proportion of patients with lymph node metastasis ( χ2=4.81, P=0.028), history of smoking ( χ2=11.67, P=0.001), and TNM stage Ⅲ-Ⅳ ( χ2=6.18, P=0.013) was significantly higher than that in the good prognosis group. Multivariate logistic regression analysis showed that lymph node metastasis ( OR=1.96, 95% CI: 1.14-3.36, P=0.015), smoking history ( OR=2.02, 95% CI: 1.33-3.06, P=0.001), TNM stage ( OR=2.28, 95% CI: 1.35-3.86, P=0.002), HMGB2 ( OR=2.01, 95% CI: 1.40-2.91, P<0.001), and HMGB3 ( OR=1.99, 95% CI: 1.25-3.15, P=0.003) levels were independent influencing factors of prognosis of NSCLC patients. ROC curve analysis showed that the area under the curve (AUC) of serum HMGB2 and HMGB3 alone and in combination to predict the prognosis of NSCLC patients were 0.833, 0.862 and 0.922, respectively, and the AUC predicted by the combination was significantly higher than that predicted by serum HMGB2 ( Z=2.44, P=0.015) and HMGB3 ( Z=2.54, P=0.011) alone. Conclusion:Serum HMGB2 and HMGB3 levels are up-regulated in NSCLC patients and are closely associated with poor prognosis, and the combined detection of the two has certain predictive efficacy for prognosis of NSCLC patients.
8.Expression and purification of IFNbeta-HSA fusion protein in Pichia pastoris.

In order to obtain enough fusion protein for developing preclinical studies of IFNbeta-HAS, we screened Pichia pastoris transformants expressing high-level protein by immunology method. The yield of IFNbeta-HSA was about 500 mg/L by fed-batch fermentation. The purity of IFNbeta-HSA reached 96% through the steps of ultrafiltration, Blue Sepharose FF, Ni2+-IMAC and DEAE Sepharose FF. Analysis of Western blotting showed that IFNbeta-HSA had the antigenicity of IFNbeta and HSA. The specific activity was about 1.96 x 10(7) IU/mg by standard survival activity test on WISH cells challenged with VSV virus. This study provided a method to produce IFNbeta-HSA.

9.Clinical analysis of 1 423 cases of root canal therapy.

OBJECTIVETo evaluate the clinical quality of root canal therapy (RCT) in West China Dental Hospital of Sichuan University.
METHODS1 423 RCT teeth were finished from Mar. 2001 to Feb. 2002 in West China Dental Hospital. Root canal filling quality and treatment period of these teeth were evaluated. 695 teeth of the total were revisited 2 years later and 2-year success rate were evaluated.
RESULTSThe ratios of adequate filling, underfilling, and overfilling were 79.97%, 14.62% and 5.41%, receptively. Full canal RCT ratio of molar was 89.44%. Average RCT treatment period was 2.8 weeks. 2-year success rate of RCT was 94.39%.
CONCLUSIONClinical RCT level of West China Dental Hospital was satisfactory from 2001 to 2002.

10.Effect of early enteral nutrition on prognosis of patients during veno-arterial extracorporeal membrane oxygenation

Objective To observe the feasibility of early enteral nutrition (EEN) in adult patients during veno-arterial extracorporeal membrane oxygenation (VA-ECMO) and its related clinical results. Methods Thirty-four patients admitted to the Intensive Care Unit (ICU) of Ningbo Medical Center Li Huili Hospital from January 2012 to December 2017 to receive VA-ECMO treatment were selected as the study objects. All patients received EEN after exclusion of contraindications; the target calories were calculated by using 104.65 kJ·kg-1·d-1(25 kcal·kg-1·d-1) and according to 1.2 g·kg-1·d-1, the target protein requirements were calculated. Within one week of EN energetic treatment, if the feeding amount reached 80% or over of the above calculated targets, the EN administration was fulfilling the standard, thus the patients were divided into a standard group and a non-standard group, After ICU admission, the acute physiology and chronic health evaluation Ⅱ (APACHEⅡ) score, multiple organ dysfunction syndrome (MODS) score, the time from the beginning of ECMO to the start of EN, time reaching feeding standard, VA-ECMO persistent days, ICU days of stay, total hospitalization days, hospital mortality, and reasons for feeding interruptions were recorded, and the effects of EEN on prognosis of patients during VA-ECMO were analyzed in the two groups. Results The APACHE Ⅱ score and MODS score of the non-standard group were higher than those of the standard group on admission of ICU (APACHE Ⅱscore: 25.50±5.62 vs. 19.91±8.53, MODS score: 11.08±3.26 vs. 6.73±2.05, both P < 0.05), and the hospital mortality was lower in the standard group than that in non-standard group [40.9% (9/22) vs. 83.3% (10/12), P < 0.05]. The comparisons of differences in time from the beginning of ECMO to the start of EN, time of reaching feeding standard, VA-ECMO treatment days, ICU days of stay, hospitalization days between the two groups were of no statistical significance (all P > 0.05). The most common reason for interruption of feeding was high gastric residual volume (GRV). Conclusion Whether a critically ill patient receiving VA-ECMO can reach the target feeding amount in a week or not is related to the degree of disease severity; it is difficult for a seriously ill patient to reach the target amount of feeding, that may affect their prognosis.
