1.Prevention of chemotherapy-induced oral mucositis in patients with acute leukemia by the two oral care protocols : the comparisons of Sodium Bicarbonate-normal saline gargling and Chlorhexidine gargling.

Oral mucositis continues to be a major complaint of patients who have chemotherapy for the acute leukemia. An innovative and inexpensive remedy which produces favorable results for those afficted is not yet introduced. So we tried to develop two oral care protocols for reducing the level of oral mucositis during cytotoxic therapy through literature review and our clinical experience. The one is sodium bicarbonate-normal saline gargling, and the other consists of chlorhexidine gargling. This quasi-experimental study was performed to compare the efficacy of these two different oral care protocols. Twenty subjects were assigned to one of the two specific diagnosis of leukemia, aim of the chemotherapy. The Oral Assessment Guide(OAG), the Beck's perception of oral comfort, WHO Grading system fot mucositis and the discomfort of oral gargling solution were used to assess oral status and subject's oral discomfort during chemotherapy. Each subjects were observed daily from the start of the chemotherapy until Absolute Neutrophil Count(ANC) reached 1,000. It continued about 2-4 weeks. The data analyzed by Mann-Whittney U test and ANCOVA. The result was follows as: The patient who used sodium bicarbonate-normal saline gargling showed significantly higher mean score of the discomfort of oral gargling solution than chlorhexidine gargling. The other scores were not significantly different between two groups. However the subjects using the sodium bicarbonate-normal saline gargling showed a lower level of oral mucositis. We concluded that oral using sodium bicarbonate-normal saline gargling was between to reduce the level of oral mucositis during chemotherapy and nursing assessments of the oral cavity seemed to promote patient's compliance with the oral care regimen.

2.A Case of Cutaneous Focal Mucinosis.

We report a case of cutaneous focal mucinosis in a 60 year-old male, who had asymptomatic localized small flesh colored papule and nodule on the left side of the neck. The histopathologic finding revealed fibroblastic proliferatior in the dermis and an increased prodact on of hyaluronic acid by these fibroblasts. The lesions were treated by surgical excision.

3.An experimental study on effect of bladder reflex arc with sensory afferent pathway in the treatment of atonic bladder after spinal cord injury

Objective To establish bladder reflex arc with sensory afferent pathway using SD rats,and evaluate its effect in the treatment of atonic bladder after spinal cord injury.Methods Twenty-four male SD rats were used in the study.For each rat,the right side was the experimental side,and the left side was the control side.In the right side,the L5 ventral root (VR) was anastomosed to S2 VR,and the distal end of S2 dorsal root (DR) was anastomosed to the proximal end of L5 peripheral process of dorsal ganglion.In the left side,no treatment was done.In order to evaluate the validity of the bladder reflex arc,general observation,neuro-electrophysiological test and wheat germ agglutinin horseradish peroxidase (WGA-HRP) method were used before and after the spinal cord destruction between L6 and S2 level at 3 months postoperatively.Results Twenty-one rats survived 3 months after the operation,and anastomotic nerves were separated successfully only in seven rats.Compound action potentials (CAPs) of plexus vesica and compound muscle action potentials (CMAPs) of bladder smooth muscle were found by electrical stimulation in distal end of the anastomotic stoma of the right S2 DR.There was no statistically significant difference in action potential before and after paraplegia.No action potential was detected in control sides after paraplegia.The curves of CAPs and CMAPs in the right side were similar to those in the control side,and the mean maximum amplitude reached respectively 71.9% and 82.4% of those in the left side before paraplegia.In addition,WGAHRP labeled cells were observed in L5 anterior horn and posterior horn in the experimental side after WGAHRP injection.Conclusion Reconstruction of bladder reflex arc with sensory afferent pathway can promote axonal regeneration of motor and sensory nerves,and then the regenerated axon could contact with cells in anterior and posterior horn of spinal cord through parasympathetic nerves,ultimately the capability of axoplasmic transportation could be reestablished.Therefore,this method can be used for treating atonic bladder.
4.Application of operational simulation training system in the training of ophthalmic students

To assess the impact of the operational simulation training system on the cultivation of ophthalmic students.
●METHODS: Four hundred and eighty ophthalmic students in China Medical University from 2009 to 2011 were chosen as objects and divided into two groups at random. The students in the experimental group used operation simulation training system combined with traditional teaching methods, while the students in the control group only used traditional teaching methods. We examined the operation skills and the confidence of all the students half a year later.
●RESULTS: The improvement of the operation skills during the suture technique examination, and the confidence for operation technology were significantly different in two groups. The basic skills of the students in the examination groups improved apparently during the operational simulation training and they were full of confidence to their operational technique.
●CONCLUSlON: The operational simulation training is useful for promoting the clinical ability for ophthalmic students and should be widely popularized.
6.Protein C genetic variation was associated with the susceptibility to acute respiratory distress syndrome in Chinese Han population

Objective To investigate the genetic variants in the protein C (PC) and endothelial protein C receptor (EPCR) genes associated with the risk and outcome of acute respiratory distress syndrome (ARDS) patients in Chinese Han race.Methods Five tagSNPs (single nucleotide polymorphism,SNP) in the PC and EPCR genes were genotyped in patients with ARDS (n =275) and non-ARDS (n =337) in order to find the association between them in this case-control study.The SNPs were genotyped by SNPstream Beckman platform.Then,the correlation between the associated SNPs and plasma levels of activated protein C (APC) in patients with ARDS was investigated.The APC levels were measured using enzyme linked immunosorbent assay (ELISA) method.Results Association analysis rcvealed that two PC SNPs in perfect linkage disequilibrium,rs1799809 and rs1158867,were significantly associated with susceptibility to ARDS.T allele frequency of rs1799809 in ARDS patients was significantly higher than that in non-ARDS patients (OR =1.569,95% CI:1.192-2.066).And the genotype frequencies of rs1799809 were also significantly different between these two groups (P =0.007).The association remained significant after adjustment for multiple comparisons.Haplotype consisting of three SNPs in the PC gene was also associated with susceptibility to ARDS.The frequency of haplotype CCC in the ARDS samples was significantly lower than that in the non-ARDS group (P < 0.01).Moreover,ARDS patients canrying rs1799809 TT genotype showed lower serum levels of APC than patients with TC and CC genotypes (Padj =0.02).However,genotype and allele analyses of EPCR did not show any significant difference between ARDS and non-ARDS patients.Conclusions These findings indicated that common genetic variation in the PC gene was significantly associated with susceptibility to ARDS in Chinese Han race.The PC genetic variation influenced plasma concentration of APC in patients with ARDS.
7.Application of Ergonomics for Design of Operation Field in Sanitary Train

Objective To investigate the specific application of ergonomics for design of operation field in sanitary train. Methods The use of ergonomics theory, the layout of operation field, personnel operating capacity, environmental comfort and operation security were analyzed, surveyed and discussed. Results The principles and guidelines for design of opera- tion field in sanitary train were proposed in order to make full development the efficiency of "operator-equipment-environ- ment" mode. Conclusion The application of ergonomics for design of operation field in sanitary train can be improved the rationality of the layout of field of operation, environmental comfort and work efficiency.

10.The clinical characteristics and prognosis of subgaleal hemorrhage in newborn.

PURPOSE: Subgaleal hemorrhage (SGH) is a rare but potentially fatal condition in newborns; however, few studies have reported on this condition. We aimed to identify the clinical characteristics and prognostic factors of SGH. METHODS: We retrospectively reviewed the medical records of 20 neonates diagnosed with SGH between January 2000 and June 2017. Enrolled neonates were clinically diagnosed when they had tender fluctuant scalp swelling that crossed the suture lines. RESULTS: Among 20 neonates with SGH, 12 were boys and 7 were girls; median hospitalization duration was 9.7±6.9 days. Fourteen neonates (70%) were born via vacuum-assisted vaginal delivery, and 4 via vacuum-assisted cesarean section. Of the neonates enrolled, half of them initially showed unstable vital signs, including apnea, desaturation, and cyanosis. Ten neonates had acidosis and 3 had asphyxia (pH < 7.0). Intracranial lesions associated with SGH were observed in 15 neonates (75%), including subdural hemorrhage (50%), subarachnoid hemorrhage (15%), intraventricular hemorrhage (5%), cerebral infarct (15%), skull fracture (30%), and cephalohematoma (20%). Twelve neonates (60%) required transfusion, 5 (25%) had seizures, and 3 (15%) died. Eight neonates (40%) had hyperbilirubinemia (mean total bilirubin, 13.1±7.4). The mean follow-up period was 8.4±7.5 months. At follow-up, 10 neonates (58.8%) were healthy with normal development, whereas 7 (41.2%) had neurological deficits. CONCLUSION: The morbidity rate was 41.2% due to severe metabolic acidosis. Anemia, hyperbilirubinemia, low Apgar scores, and subdural hemorrhage did not affect the prognosis. The long-term outcomes of neonates with SGH are generally good. Only arterial blood pH was significantly associated with death.
