Journal of the Korean Society of Plastic and Reconstructive Surgeons 2010;37(4):400-408
PURPOSE: Complete septal extension grafts have been widely used in rhinoplasty for effective projection of the short retruded columella in Asian patients. Autologous septal cartilages and porous high-density polyethylene sheets are frequently used as septal extension grafts. This study was conducted to compare the postoperative results of porous polyethylene sheets and septal cartilages used for correction of unilateral cleft lip nasal deformities by using photogrammetric analysis. METHODS: This study investigated a total of 49 patients with cleft lip nasal deformities who underwent corrective surgery, and were followed up for at least 6 months. Septal cartilages were used in 39 patients, and porous polyethylene sheets were used in 10 patients. In all patients, through the open rhinoplasty, complete septal extension grafts were sutured to the caudal margin of the septal cartilage, and the alar cartilage was sutured with suspension. The cleft side alar cartilage was overcorrected by approximately 3 - 5 mm. Postoperative outcomes were evaluated by using photogrammetric analysis. Five indices and 4 angles were measured on their photographs taken before and after the surgery. In patients with unilateral cleft lip nasal deformities, symmetry was also evaluated by means of columellar length index. RESULTS: The postoperative values obtained in photogrammetric analysis showed improvements in comparison with the preoperative ones. The polyethylene group produced more improved outcomes than the septal cartilage group but also resulted in more complications at the same time. CONCLUSION: The results of this study indicates that complete septal extension grafts are efficient for the correction of unilateral cleft lip nasal deformities. However, since postoperative complications occur more frequently in the polyethylene group than in the septal cartilage group, caution is advised in using porous high-density polyethylene sheets in patients with cleft lip nasal deformities.

Journal of the Korean Academy of Family Medicine 2004;25(6):455-461
BACKGROUND: Nowadays the addictive use of internet is becoming a serious social and health problem. The authors performed a study in PC bang users to evaluate the prevalence of addictive internet use and the association between addictive internet use and behavioral and psychological factors. METHODS: A face-to-face interview survey of 900 Korean internet users at six PC-bangs was performed between 11th and 19th of December, 2002, using standardized questionnaires composed of Young's internet addiction scale, scale of unwillingness to communicate, patient problem questionnaire for screening depression and anxiety, and questions asking internet using patterns and socio- demographic factors. RESULTS: Of the final 888 participants, 3.4% were identified as addictive users and 41.3% as over-users of internet service. The subjects who were male, unemployed, less educated, using internet mainly at PC bang, using internet more frequently, starting or finishing off internet use usually in the morning, using internet service usually for doing games tended to be more addictive users of internet (P<0.05). In proportional odds model analysis, increasing duration of internet use (OR=1.22; 95% CI, 1.16~1.30), daily use of internet (OR=2.06; 95% CI, 1.21~3.51), finishing off internet use between midnight and 6 a.m. (OR=2.00; 95% CI, 1.30~3.07), anxiety (OR=5.38; 95% CI, 2.86~10.1), and increasing approach avoidance (OR=1.05; 95% CI, 1.03~1.08) were significantly associated with a higher degree of internet addiction. CONCLUSION: Addictive use of internet should be suspected in a person who uses internet for long time, daily, and over midnight. An active evaluation and management for anxiety are needed among addictive internet users.

Chinese Journal of Contemporary Pediatrics 2015;17(3):213-216
OBJECTIVETo investigate the clinical efficacy and safety of preferred use of high-frequency oscillatory ventilation (HFOV) in the treatment of neonatal pulmonary hemorrhage.
METHODSThe clinical efficacy of preferred use of HFOV (preferred use group) and rescue use of HFOV after conventional mechanical ventilation proved ineffective (rescue use group) in the treatment of 26 cases of neonatal pulmonary hemorrhage was retrospectively analyzed. The oxygenation index (OI), pulmonary hemorrhage time, hospitalization time, ventilation time, oxygen therapy time, complications, and outcome of the two groups were compared.
RESULTSCompared with the rescue use group, the preferred use group had significantly lower IO values at 1, 6, 12, 24, 48, and 72 hours after treatment (P<0.05). Compared with the rescue use group, the preferred use group had a significantly lower incidence of ventilator associated pneumonia (VAP) (P<0.05) and a significantly higher cure rate (P<0.05). There were no statistically significant differences in the incidences of pneumothorax, intracranial hemorrhage, and digestive tract hemorrhage between the two groups (P>0.05). Compared with those in the rescue use group, children who survived in the preferred use group had significantly shorter pulmonary hemorrhage time, hospitalization time, ventilation time, and oxygen therapy time (P<0.05).
CONCLUSIONSCompared with the rescue use of HFOV, preferred use of HFOV can better improve oxygenation function, reduce the incidence of VAP, shorten the course of disease, and increase cure rate while not increasing the incidence of adverse effects.
