OBJECTIVE: To summarize and analyze the characteristics of bone metabolism with the growing of age, and explore the skeletal change with the increase of age especially in the elderly people and the females of postmenopause so as to provide scientific evidence for the prevention and treatment of osteoporosis.DATA SOURCES: Literatures about age growth and bone metabolism were computer-searched in Medline database published from January 1993 to December 2003, with the key words of "The bone metabolism,The age growth" in English. Meanwhile, it was also manually searched in library published between January 1993 and December 2003 for related articles in Japanese.STUDY SELECTION: After the primary search, inclusive criteria: ①Physiological characteristics, mechanism and influenced factors of the bone metabolism with the growing of age. ②Analysis of retrospective study was done in clinical cases. Repetitive experimental studies were excluded.DATA EXTRACTION: Totally 21 papers were accorded with the abovementioned requirements, and 14 repetitive studies were excluded. Seven articles were referred to the inclusive criteria, in which 3 articles were researchful; 2 articles were diagnostic criterions; 2 articles were textbook.DATA SYNTHESIS: Bone metabolism changed greatly; skeleton became mature gradually and bone calcium increased with the age growth. Bone metabolism reached homeostasis after 20 years, and then appeared negative growth after 40 years and bone calcium decreased. Bone metabolism was the most intense and variable parameter with the growing of age. The rate of osteoporosis increased greatly in elderly people, especially in the females of postemenopause, which might be related to endocrine disorder and so on, except degeneration in elderly people. Studying these could provide scientific evidence for the prevention and treatment of osteoporosis.CONCLUSION: On the basis of understanding the relationship between age growth and bone metabolism, we realize that osteoporosis may be prevented mainly by means of establishing healthy life-style, full nutrition and high calcium ingestion.

Objective To investigate the association of Lys656Asn polymorphism in leptin receptor gene with serum triglyceride level.Methods The genotypes of Lys656Asn polymorphism in LepR gene were maped by polymerase chain reaction-single strand conformation polymorphism(PCR-SSCP) assay in 232 unrelated subjects of Shanghai Chinese population.The clinical data were also analyzed.Results No significant difference of genotype frequency of Lys656Asn polymorphism in LepR gene was observed between obese and non-obese groups.In obese group,"G" Allele was associated with increased triglyceride level(1.74?0.91 vs 0.95?0.32)(P

Objective To provide strong evidence for sterilization standardization by comparing the results of bacteria culture after conjunctiva sac irrigation by gentamicin and povidone-iodine (PVI) and analyzing the sterilization effect of these two common disinfectant.Methods Six randomized control trials comparing the sterilization effect between gentamicin and PVI in conjunctiva sac irrigation were retrieved through the following databases:Pubmed,Embase,Cochrane Library,WANFANG DATA,CNKI and VIP database.Relative Risk (RR) was set as the effect indexes to evaluating the conjunctiva sac bacterial-culture results before and after irritation.The data of these articles were processed with Meta analysis by random model.The side effects of both dilutions were also compared using the descriptive analysis.Results Before irrigation,bacteria in the sac were mostly gram-positive coccus,among which Staphylococcus epidermidis and Staphylococcus aureus were most common.The detection rate of these two bacteria were 30.5％ (226/740) and 2.4％ (18/740),and the constituent ratio were 65.5％ and 5.2％ respectively.After irrigation,the number of the bacteria was decreased dramatically,but could not be totally eliminated.There was no significant difference in sterilization effect between gentamicin and PVI after conjunctiva sac irrigation.The weighted average value of RR was 0.51 with 95％ confidence interval (CI) in (0.25,1.06),P =0.07.Comparing to gentamicin,PVI was less comfortable after conjunctiva sac irrigation,but when the concentration was regulated,it would be acceptable by patients and didn't affect surgeries.Conclusions There was no significant difference in sterilization effect between gentamicin and PVI in conjunctiva sac irrigation.The side effect of PVI was minor in low concentration.PVI is more specific to the bacteria species than gentamicin.Using PVI was recommended in preoperation sterilization.

Objective To develop a physical in vitro model of the human carotid artery(CA) and analyze the factors that influence the embolus-capturing efficiency of the retrievable artery filters. Methods The flow model closely simulated the physiological parameters of the human carotid artery. The influence of the following experimental parameters on the capturing rate was analyzed with a multiple logistic regression model: embolus diameter and length, CA diameter, flow quantity, type of filter, and type of testing. Results At the 5% level,except for flow quantity,a significant influence on the capturing rate could be demonstrated for the 6 factors mentioned above, the embolus diameter was the strongest influencing factor. In CA with diameters of 4,5,6,7 mm, the 8-wire filter captured (90?7)%, (91?8)%, (81?8)%,and (68?9)% of 0.5 mm emboli and (93?4)%, (95?6)%, (95?6)%,and (82?11)% of 1 mm emboli. In all, the smaller, the interlacing opening of the filter, the higher the capturing rate, and the presence of an embolus in a filter would decrease the capturing rate for the second embolus. Conclusion Retrievable artery filter is a promising cerebral protection device. The physical in vitro model is an effective alternative in studying the influence factors of the capturing rate of the filters.

Objective:To investigate the correlation between the microsatellite polymorphisms in ob gene 3′flanking region with obesity in Chinese Han ethnicity in Shanghai.Methods: The genotypes of polymorphisms in ob gene 3′flanking region were determined by PCR in 232 Chinese Han subjects(including 130 obese patients and 102 normal controls).The clinical data,including height,weight,waist circumference,hip circumference,body mass index,waist-to hip ratio(WHR),blood glucose,serum insulin,blood lipids,and uric acid,were compared between the 2 groups.Results: No significant difference of genotype frequency(Ⅰ/Ⅰ 0.115,Ⅰ/Ⅱ 0.331,Ⅱ/Ⅱ 0.554 vs Ⅰ/Ⅰ 0.059,Ⅰ/Ⅱ 0.333,Ⅱ/Ⅱ 0.608) and allele frequency(Ⅰ 0.281,Ⅱ 0.719 vs Ⅰ 0.225,Ⅱ 0.775) of ob gene 3′flanking region was observed between the 2 groups.In obese group,patients with Ⅰ+Ⅱ genotype was associated with increased WHR compared with those with Ⅱ/Ⅱ genotype(0.91?0.05 vs 0.86?0.03,P=0.047).Logistic regression analysis showed that WHR was independently correlated with ob gene polymorphisms(P=0.042).Conclusion: Polymorphisms in ob gene 3′ flanking region are correlated with central obesity in obese patients of Chinese Han ethnicity in Shanghai.

目的探讨康复训练配合针刺治疗脑卒中患者吞咽功能障碍的疗效。方法 87例脑卒中伴吞咽功能障碍者分为综合治疗组(56例)和对照组(31例)。综合治疗组进行针灸和康复训练 ,对照组仅进行内科常规治疗。结果综合治疗组与对照组在统计学上有非常显著性差异(P<0.01)。结论脑卒中吞咽功能障碍患者早期进行综合康复能促进功能恢复、减少鼻饲并发症。

2-Amino-3-hydroxylpyridinE-H2O2-horseradish peroxidase (HRP) voltammetric enzymE-linked immunoassay based on N-heterocyclic substrate has been successfully applied for the detection of carcionembryonic antigen(CEA) in human serum. 2-Amino-3-hydroxylpyridine is oxidized with H2O2 catalyzed by HRP, and the resulting electroactive product produces a sensitive voltammetric peak at the potential of 0.36 V(vs. SCE) in Britton-Robinson (B-R) buffer solution. By this voltammetric peak, free HRP can be measured and immunoassay of HRP label can be developed. The enzymE-catalyzed reaction conditions and voltammetric detection conditions have been optimized, and the electrode procedure of the enzymatic product was investigated. The selected optimum reaction conditions were that the reaction medium was pH 6.0 B-R buffer solution for 10 mL reaction solution containing 1.0 mL of 0.2 mol/L B-R buffer solution, 3.0 mL of 8.0 mmol/L 2-amino-3-hydroxylpyridine solution and 1.5 mL of 0.5 mmol/L H2O2 solution, and the reaction time was 30 min at 37 ℃. The optimum detection conditions were that the supporting electrolyte was pH 7.0 B-R buffer solution for 10 mL of the overall detection solution containing 5 mL of reaction solution and 1.0 mL of 0.2 mol/L B-R buffer solution. The optimum instrumental conditions for the detection were chosen as follows: the initial potential, 0.00 V; the final potential, -0.80 V; the potential scanning rate, 400 mV/s; the mercury drop standing time, 7 s. Under the optimum conditions, the linear range for detection of free HRP was 4.0×10-4-1.0 μg/L with a detection limit of 0.12 μg/L. Based on the new immunoassay system, the linear range of the detection to CEA was 0.50-80 μg/L and the detection limit was 0.50 μg/L, which is 10 times lower than that of traditional spectrophotometric enzymE-linked immunosorbent assay method. The 2-Amino-3-hydroxylpyridinE-H2O2-HRP voltammetric enzymE-linked immunoassay new system exhibits excellent performance having wider linear range and lower detection limit. The new method is inexpensive and simple. It has a great potential in clinical diagnosis.
