OBJECTIVE： A RP－ HPLC method was established for determination of serum concentration of propofol in man METHODS： The mobile phase consisted of methanol－ water（ 68∶ 32v/v） The detection was carried out at wave length 258nm and flow rate 1ml/min with carbamazepine as internal standard RESULTS： The retention times of propofol and carbamazepine were 9 20 and 5 16 min， respectively The mean recovery of propofol was 99％  The within－ day and inter－ day variations were all less than 10％  Propofol and carbamazepine were seperated well The assay linearity was obtained in the range of 1～ 16μ g/ml in serum（ r=0 9 994）  CONCLUSION： The method is sensitive， simple and reliable for the determination of propofol concentration

The paper expounds the connotation of HIS troubles, the methods for determining HIS troubles and the necessity of establishing a contingency plan for clearing such troubles. It also puts forward some specific measures for managing the contingency plan: ①defining the accountability and division of work in clearing various kinds of troubles; ②doing a good job of coordination in clearing troubles; ③carrying out strict training for the relevant personnel; and ④conducting checks and supervision all the time. Establishment of the plan makes it clear to HIS managers that enough attention ought to be paid to the prevention of troubles.

Purpose:To investigate the effects and side- ef fects of thalidomide in the treatment of relapsed and refractory multiple myelom a (MM).Methods:There were thirteen men and two women aged 33～7 6 (median 58) years in thalidomide alone group. Two cases were newly diagnosed my eloma; twelve of them were refractory myeloma, including three relapsed cases wh o had experienced autologous peripheral blood stem cell transplantation (AutoPBS CT); another one is primary plasma cell leukemia. The starting dose was 100mg/d, escalating up to a maximum of 800mg/d. The combination group included twenty me n and seven women aged 40～81 (median 56) years of which case was newly diagnose d myeloma; twenty-five cases were refractory myeloma, two of whom relapsed after AutoPBSCT; another one was primary plasma cell leukemia. Dexamethasone 40mg/d w as used at d 1-4 , d 9-12 , d 17-20 when the dose of thalidomide increased to 400mg/d. Each cycle lasted for one month.Results: The total response rate was 42.9% and 57.7% in thalidomide alone group and in co mbination group, respectively. The response rate in refractory patients was 33.3 % and 44.0% in thalidomide alone group and in combination group, respectively. T here was no difference between the two groups in response rates. Both of the two groups had side effects of constipation and skin rash, etc. in different degree s but usually tolerable. Conclusions:Both thalidomide alone and thalidomide combined with dexamethasone was effective in the treatment of relap sed and refractory MM.

Objective To investigate the changes of androgen receptor (AR) and AR mRNA of the ventral lobe, lateral lobe part 1(LP1) and lateral lobe part 2 (LP2) in the Wistar rat prostate after androgen manipulation. Methods Healthy adult male Wistar rats were randomly divided into 3 groups: control group(N), castration group(C) and androgen replacement group (T). In castration group, orchiectomy was performed through the scrotum. In androgen replacement group, testosterone propionate (2 5 mg/d) was injected subcutaneously for 14 d after castration. Expressions of AR and AR mRNA in the separate lobes of the adult Wistar rat prostate after androgen ablation and replacement were determined by immunohistochemistry and semi quantitative PCR. Results Different autoregulation of AR protein and mRNA was observed in the ventral lobe, LP1 and LP2 of rat prostate after androgen manipulation. In the ventral lobe and LP1, immunoreactive nuclear AR markedly decreased in staining intensity in the glandular epithelium 2 d after castration compared to that in the intact rats. Epithelial immunostaining continued to decline and was absent at day 7 and returned to normal level 3 d after testosterone replacement. There was a transient increase in AR mRNA demonstrated by RT PCR within 3 d and returned to control level within 7 d after castration in the ventral lobe and LP1 of rat prostate. In contrast, in LP2, the epithelial cells showed continued expressions of both AR protein and mRNA 14 d following androgen withdrawal. Conclusion These results suggest that there might be a different regulation of AR mediated by androgen in ventral lobe, LP1 and LP2 of rat prostate.

Objective To evaluate the effect of dexmedetomidine on myocardial injury in infants undergoing repair of ventricular septal defect.Methods Forty ASA grade Ⅱ or Ⅲ infants,aged 3-6 months,weighing 4-6 kg,scheduled for elective repair of ventricular septal defect,were randomly divided into 2 groups (n =20each):control group (group C) and dexmedetomidine group (group D).Anesthesia was induced with midazolam,etomidate,cisatracurium and sufenanil.The infants were mechanically ventilated after nasotracheal intubation.PET CO2 was maintained at 30-40 mm Hg,Anesthesia was maintained with cisatracurium,sufenanil and sevoflurane.In group D,dexmedetomidine was infused at 0.5 μg· kg-1 · h-1 until the end of operation.While in group C normal saline was given at the same rate until the end of operation.BP and HR were recorded at 10 min before operation (T1),skin incision (T2),chest opening (T3),10 min after aortic unclamping (T4) and the end of operation (T5).Blood samples were taken from the right internal jugular vein for determination of plasma creatine kinase MB (CK-MB) activity and cardiac troponin T (cTnT) concentrations at T1,T5 and 24 h after operation (T6).Results Compared with the baseline value at T1,no significant changes were found in HR and BP at different time points in group D,HR and BP were significanfly increased at T2-T5 in group C,and the plasma CK-MB ctivity and cTnT concentration were significantly increased at T5 and T6 in the two groups (P ＜ 0.05).HR and BP at T2-5 and plasma CK-MB activity and cTnT concentration at T5 and T6 were significantly lower in group D than in group C (P ＜0.05).Conclusion Dexmedetomidine infused at 0.5 μg· kg-1 · h-1 after induction can reduce myocardial injury in infants undergoing repair of ventricular septal defect.

Objective To study the cause of death and mechanism after(TACE)in China during the past 14 years.Methods Related repots in Chinese Medical Current Content(CBM)and National Knowledge lnfrastruc ture(CNKI)from January 1994 to June 2008 were retrieved.The cause of death and mechainsm after TACE wer e analyzed.Results A total of 150 patients who died after TACE were reposed in China during the past 14 ye ar s.84%eases were caused by liver lunction failure,upper gastrointestinal bleeding and rupture of liver cancer. 78.7%cases died one month postoperation.Conclusion Liver function failure.upper gastrointestinal bleeding and rupture of liver cancer are the main complications which Can cause death and the majority cases died early.
