1.ANALYSES OF THE EFFECTS OF LYOPHILIZATION ON RBC RECOVERY

Lyophilized storage of red blood cells(RBC) may provide a useful storage alternative Both shelf life and transportation logistics can be improved over those for existing refrigerated or frozen storage. The present study was designed to evaluate the effects of lyophilization process on mean RBC recovery. Blood was drawn from healthy adult having no history of either hemoglobinophathy or abnormal RBC metabolism. Blood stored in plastic transfer bags were lyophilized for 16 hours, then it was rehydrated . In group 1, samples were stored at 4℃ for 6 hours, while in group 2 were stored at 4℃ for 5 hours, then 25℃ for 1 hour. The mean RBCs recovery in group 2 was higher than group 1(81 4?7 6% vs 62?10 2%,respectively). Samples were lyophilized using buffer containing different concentrations of glucose, and studies showed that there was no significant difference in mean RBCs recovery between samples in lyophilization buffer containing high and medium concentration of glucose, but low concentration of glucose in buffer resulted in lower mean RBCs recovery. The mean RBCs recovery was 30% in low concentration of glucose buffer and 45 1%, 47 2% in medium and high concentrations of glucose buffer, respectively. We also observed effects of different rehydration buffer with different osmotic pressure, and it showed that high osmotic pressure buffer resulted in higher mean RBCs recovery than iso-osmotic buffer. There was little change in the cell size and cell volume. The results presented in this report clearly demonstrate that human RBCs can be successfully lyophilized. Lyophilization buffer and rehydration buffer may influence the mean RBCs recovery.
2.Clinical Study on 38 Cases of Primary Sjogren's Syndrome Patients Treated with Xuanfei Bujin Particles

Objective To observe clinical efficiency of Xuanfei Bujin particles in the treatment of primary Sjogren's syndrome (PSS) and its impact on symptom score, ESR, immunoglobulin, as well as soluble intercellular adhesion molecule (sICAM-1). Methods Fifty eight cases of PSS patients were randomly divided into 38 cases of traditional Chinese medicine treatment group and 20 cases of western medicine treatment group. The symptom score, clinical efficiency, ophthalmic examination, sICAM-1, sIL-2R, ESR, immunoglobulin and other changes were observed. Results Total effective rate of Xuanfei Bujin particles treatment group was 84.21%, better than the control group. Xuanfei Bujin particles can significantly reduce dry mouth, dry eyes, and other symptoms of the main points, and improve the inspection of ophthalmology, sICAM-1. Conclusion Xuanfei Bujin particles is safe and efficient, and has certain degree in immune regulation.
3.Clinical application of interventional therapy for avascular necrosis of the femeral head

Objective To evaluate the possibility of interventional therapy for avascular necrosis of the femeral head, with different curing methods, way of introduction and cultural heritages. Methods Vasodilator, thrombolytics and promoting microcirculatory drug were directly injected into the feeding arteries of the avascular necrotic femoral heads, under the condition of applying the blood stoppage belt at the root of thigh with pressure before the drug injection into the femoral pronator and extension arteries. The treatment was repeated 45 d later. Urokinase 10 5 unite/d ?10 were administrated with venous infusion ipsilaterally on the 15th day after the beginning of the therapy. Imaging features and clinical symptoms were recorded and correlatively studied before and after the treament. Results Symptoms relief especially the pain reached 100% after the treatment with various degrees of bony repair and new bone formation. Furthermore, increase caliber of feeding small arteries for femoral head and multiplicity of microvasculature, shortening of opacification time were revealed by DSA. IV stage bony change showed mild or inconspicuous. Conclusions Interventional catheterization treatment for avascular femoral head necrosis, especially the patients of fore Ⅲ stage, is safe and effective.
4.Considerations on Unlicensed and Off-Label Drug Uses

OBJECTIVE: To provide reference for dealing with the problems arose from unlicensed and off-label drug uses.METHODS: The rationality of unlicensed and off-label drug uses and the possible problems induced by which were analyzed.RESULTS: The existence of unlicensed and off-label drug used has its rationality,and it also in line with the international practices,but it may cause some problems related to policy,laws and regulations and medication safety etc.CONCLUSION: The concerned department and trade association should attach great importance to unlicensed and off-label drug uses and establish a generally accepted principle and strategies to tackle the problems.
5.Clinical Evaluation on Ratio of Neutrophil-to-Lymphocyte in Different Pathogens

Objective To investigate the clinical value of neutrophil-to-lymphocytc ratio(NLR) in identifying blood stream infection caused by different pathogens and for differentiating coagulase negative staphylococcus(CNS)bloodstream infection and contamination.Methods Medical records of 500 patients who underwent blood culture test and routine blood test at the same time were retrospectively analyzed,blood culture negative group 356 patients,blood culture positive group 144 patients,which included Gram-negative group,Gram positive group,fungi group,CNS bloodstream contamination and mingled group.Collected the results and calculated the NLR at the same time.NLR were applied by t test of each group.ROC curve was used to determine the cut off value of NLR.Results ①Mean values of NLR in negative blood culture,blood stream infection group,CNS bloodstream infection and contamination were 6.12,13.15,10.11 and 6.24.NLR had statistical difference between negative blood culture and blood stream infection group,CNS bloodstream infection group and contamination group (P<0.05).②Mean values of NLR in and fungi group were 15.33,11.63 and 10.58,respectively.NLR had statistical difference between Gram-positive bacteria group and Gram-negative bacteria group (P<0.05).NLR had no differences among Gram-positive bacteria (15.33) and Gram negative bacteria (11.63) compared with (10.58) fungi respectively (P>0.05).③The area under the curve of NLR predicting a positive blood culture,distinguishing Gram-positive bacteria and Gram-negative bacteria,differentiating CNS bloodstream infection and contamination were 0.86,0.60 and 0.75,respectively.The optimal cut off values of NLR for predicting a positive blood culture,distinguishing Gram-positive bacteria and Gramnegative bacteria,differentiating CNS bloodstream infection and contamination were 10.45,7.50 and 8.10 respectively.Conclusion NLR is highly effective in distinguishing blood stream infection and differentiating CNS bloodstream infection and contamination.
6.Distribution and Resistance of Pathogens Isolated from Cerebrospinal Fluid: Result of Specimens in 3 Years

ObjectiveTo analyze the distribution and antibiotic resistance of bacteria isolated from cerebrospinal fluid. Methods569 cerebrospinal fluild specimens were analyzed. ResultsPathogens were isolated from 93 specimens (16.3%), in which 52 were Gram positive bacteria and 38 were negative, 3 were fungi. All the Gram positive cocci were sensitive to vancomycin and linezolid, while the Gram negative bacteria were resistant to routine antibiotics such as cefotaxime and ceftriaxone, but sensitive to carbapenems. ConclusionThe prevailing pathogens in intracranial infection are Gram positive cocci, especially Staphylococcus epidermidis and Staphylococcus aureu.
7.Urotensin Ⅱ promotes collagen expression in rats cardiac tissues and proliferation of cardiac myofibroblasts in new-born rats

Objective To investigate the effect of urotensin Ⅱ on myocardial fibrosis in rats.Methods The pressure overload animal model was established in rats by abdominal aorta coarctation.The rats were divided into sham operation group,modeled for 4,8 and 12 weeks group.The expression of U Ⅱ,GPR14,col-Ⅰ,col-Ⅲ,and PKA in cardiac tissues was detected by Western blot.Isolated and cultured cardiac myofibroblasts (CFs) from new-born SD rats were treated with U Ⅱ,KT5720 or SB-611812,and then the proliferation of CFs was observed by micro scope and CKK-8.Results The expression of U Ⅱ,GPR14,col-Ⅰ,col-Ⅲand PKA increased markedly in cardiac tissues of model rat,which were time-dependent.U Ⅱ promoted the proliferation of CFs (P<0.05),which could be inhibited by KT5720 or SB-611812.Conclusions U Ⅱ/UT system promotes the occurring and development of myocardial fibrosis.
8.Epidermal growth factor receptor mutations in non-small cell lung cancer of tissue and peripheral blood by fluorescence quantitive polymerase chain reaction

Objective To investigate gene mutations of the epidermal growth factor receptor of tumor tissue and peripheral blood in non-small-cell lung cancer, and to compare the consistency of EGFR mutations in tumor tissue and peripheral blood, fluorescence quantitive polymerase chain reaction (FQ-PCR) was performed according to standard procedure. Methods 112 peripheral blood and 87 tumor tissue of NSCLC were detected by FQ-PCR, among which 45 cases were analyzed using peripheral blood-tumor tissue matched samples. The relationship between clinical efficacy and EGFR mutations Was analyzed. Results Our results showed that there is 32 mutations in EGFR in 112(28.6%)of the patients'peripheral blood samples and 27 mutations in EGFR in 87(31.O%)tumor tissue. EGFR mutations were mainly found in adenocarcinoma and non-smokers in NSCLC (P<0.05). The consistency of EGFR mutation between tumor tissue and peripheal blood reached 71.4%. Objective To examine serum lipid levels and to investigate the risk factors of dyslipidemia in children and to study the indices of screening test for dyslipidemia of children in Beijing.Methods Serum total cholesterol(TC),triglyceride(TG),high density lipoprotein-cholesterol(HDL-c) and low density lipoprotein-cholesterol(LDL-c) were detected by HITACHI 7060 automatic biochemical analyzer in Beijing students.Body height, weight,upper body height,waist circumference,upper arm circumference and blood pressure were measured.Body mass index(BMI) was calculated according to the measurement of body height and weight.Waist-to-height ratio(WHR) was also calculated.Then a Logistic regression analysis and ROC study were conducted to examine the relationship between the above indices and dyslipidemia.Results Of 962 students,107 were found dyslipidemia.The prevalence rate was 11.12%.The upper body height,upper arm circumference,BMI,WHR,systolic pressure and diastolic pressure in children with dyslipidemia were higher than those of normal subjects(P
10.Characteristic and Resistance of Fungi Isolated from Hemopathy Patients

OBJECTIVE To investigate the distribution and resistance of fungi isolated from hemopathy patients,and to provide the laboratorial data in order to prevent and treat infection caused by fungi more effectively.METHODS Strains were isolated from the upper respiratory tract,lower respiratory tract,genitourinary tract,alimentary tract;and perianal region,perineum,and the soft tissue and skin wounds.Antimicrobial susceptibility of clinical isolates was tested by broth microdilution susceptibility test.RESULTS Candida albicans,Aspergillus,tropicalis,C.krusei,and C.glabrata were the main fungi of total 3104 detected isolates.The resistance rates of C.albicans and C.tropicalis to fluconazole,itraconazole and 5-flucytosine were no more than 1.4%.The MIC50-MIC90 range of C.tropicalis for itraconazole was narrow(0.25-0.5 mg/L),but that was wide for fluconazole(0.25-4.0 mg/L).CONCLUSIONS C.albicans,Aspergillus,C.tropicalis,C.krusei,and C.glabrata are possibly the most common pathogens causing endogenous and exogenous fungi infection.The main infection sites of Aspergillus are the upper respiratory tract and lower respiratory tract,which may be associated possibly with ward's air quality.The MIC50to MIC90 range of C.tropicalis is wider for fluconazole than for itraconazole,which is possibly correlated with overexpression of CtMDR1 in C.tropicalis caused by the widely application of fluconazole in therapy.
