1.Construction and thinking of data element standard directory of traditional Chinese medicine clinical pharmacy information.

The aim of this study was to develop the data element standard directory of traditional Chinese medicine (TCM) clinical pharmacy information, to provide application standards and models of TCM clinical pharmacy for the electronic medical record (EMR). The developed line of work is as follows: initially establish research through four forms: literature analysis, questionnaires, discussion groups, expert advice. The research range from the Chinese herbal medicine research, herbal origin, harvesting, processing, identification of traits, physical and chemical identification, modern research, character, taste, Indications, clinical application, processing, dispensing medicine, Chinese medicine specifications, usage, dosage, caution, efficacy indications to small packaging applications, drug research, management and other related issues, including traditional Chinese medicine theory, application and hospital management information; according to the general and part 16 content of the national "Health Information Data Element Standards", and the basic method of extracting data element to study and develop the data element of TCM clinical pharmacy information from the defining content. Correspondingly propose the ideas and methods of construction of the "Data Element Standard Directory of TCM Clinical Pharmacy Information", sort out medicine clinical information data element standard catalog, divided into basic categories, clinical application class, management class three parts, and set norms and standards of identifying data elements, definitions, allowable value of traditional Chinese medicine clinical information, and discuss the sources and standards of information collection, leaving the interface, standardized and scientific terminology, docking with the existing standards, maintenance and management program and oter issues.

2.Correlation of chemokines and chemokine receptors with immunological abnormality in newly diagnosed systemic lupus erythemtosus patients

Objective To explore the association of chemokines and their receptors with immunologi- cal abnormality in newly diagnosed systemic lupus erythematosus(SLE) patients.Methods The serum con- centration of MIP-1?,MIP-1?,RANTES,IFN-?IL-4 were measured by enzyme-linked immunoabsorbent assay (ELISA) in 37 newly diagnosed.SLE patients and 20 normal controls.The expression rate of CCR1, CCR3,CCR5 on CD4~+T cells were detected by flow cytometry in 18 SLE patients and 10 normal controls.Re- suits Serum MIP-1?,MIP-1?concentrations were significantly higher in SLE patients than in normal control group (P<0.01),the concentration of MIP-1?positively correlated with MIP-1?(r=0.609,P<0.01);the per- centage of CD4~+CCR1~+ and CD4~+CCR5~+ cell were significantly lower in newly diagnosed SLE patients than in normal control group (both P<0.01),the percentage of CD4~+CCRI~+ cells correlated negatively with the level of serum MIP-1?and IFN-?r=-0.525,P=-0.017;r=-0.442,P=0.045);the percentage of CD4~+CCR5~+ cell corre- lated negatively with the level of serum IFN-?(r=-0.645,P=0.001);the ratios of CD4~+CCR3~+/CD4~+CCR5~+ was significantly higher in newly diagnosed SLE patients than in the normal control group (P<0.01).Conclusion Abnormal change and interaction of chemokines and their receptors with cytokines lead to immunologic dys- function and may participate in the initiation of SLE.

Cariprazine,a potent dopamine D3 and D2 receptor partial agonist with preferential binding to D3 receptors,is approved by the US Food and Drug Administration (FDA) for the treatment of schizophrenia and manic or mixed episodes associated with bipolar Ⅰ disorder in September 2015.A large number of clinical researches have confirmed that cariprazine has good efficacy,safety and tolerability in the treatment of schizophrenia or bipolar Ⅰ disorder in adults.The aim of this article is to provide an up-to-date review about the mechanism of action,pharmacokinetics,drug-drug interaction,clinical application,safety and tolerance of cariprazine.
4.Influences of peroxisome proliferator-activated receptor gamma 2 genetic polymorphism on the effects of dietary intervention to the blood lipids abnormalities.

OBJECTIVETo find out the effects of peroxisome proliferator-activated receptor gamma 2 (PPARgamma2) genetic polymorphism and nutrition intervention to blood lipid abnormal population.
METHODS412 hyperlipemia residents of Han group were screened from 3 main districts in Nanjing by multistage stratified cluster random sampling, and separated into nutritional intervention and control group by simple random method. The intervention group (221 individuals) were provided with coarse good grain and health education while only health education was provided for the control group (191 individuals). Medical examinations (including body mass index (BMI), waist-to-hip ratio (WHR); total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C) and fasting blood glucose (FBG)) were taken every 6 months between March 2007 and March 2008, and PPARgamma2 genetic polymorphism was also detected later.
RESULTSAfter intervention, TC levels of intervention group and control group were (4.90 +/- 0.86) and (5.16 +/- 0.94) mmol/L respectively; TG levels were (1.68 +/- 0.97) and (2.29 +/- 1.10) mmol/L respectively; HDL-C levels were (1.35 +/- 0.36) and (1.16 +/- 0.33) mmol/L respectively, all of the differences were significant in statistics (t values were -2.95, -6.01, 5.55 respectively, P < 0.01). The levels of BMI ((24.81 +/- 3.21) kg/m(2)), WHR (0.88 +/- 0.07), FBG ((5.40 +/- 1.17) mmol/L), TC ((4.92 +/- 0.87) mmol/L) and TG ((1.68 +/- 1.01) mmol/L) decreased significantly (t values were 19.06, 16.43, 1.98, 8.86, -14.32 respectively, P < 0.01) compared to the levels before intervention (BMI (25.39 +/- 3.30) kg/m(2), WHR (0.92 +/- 0.07), FBG (6.07 +/- 2.17) mmol/L, TC (5.28 +/- 0.94) mmol/L and TG (2.70 +/- 1.86) mmol/L), while HDL-C (1.37 +/- 0.36) mmol/L increased significantly compared to the level before intervention (1.13 +/- 0.42) mmol/L (t = -7.68, P < 0.01) in the individuals with Pro/Pro of intervention group. WHR (0.90 +/- 0.06) and TG ((1.71 +/- 0.59) mmol/L) decreased significantly compared to the levels before intervention (WHR (0.95 +/- 0.06) and TG (2.58 +/- 1.12) mmol/L) (t values were -3.53 and -8.05 respectively, P < 0.01) in the ones with Pro/Ala. Moreover, susceptibility of change for BMI in Pro/Pro genotype carriers ((-1.21 +/- 1.02) kg/m(2)) was significantly greater than that in Pro/Ala genotype carriers ((-0.58 +/- 1.85) kg/m(2), t = -6.29, P < 0.01).
CONCLUSIONSeveral indices of individuals with Pro/Pro improved obviously after nutrition intervention, which showed that effects of intervention to these people were better than those with Pro/Ala and Ala/Ala.

5.Studies on fluoride concentration and cholinesterase activity in rat hippocampus.

OBJECTIVETo study the accumulation of fluoride in rat hippocampus and its effect on cholinesterase activity.
METHODSRats were subchronically exposed to NaF, and fluoride concentration and cholinesterase activity in rat hippocampus were determined.
RESULTSFluoride concentration in rat hippocampus was significantly correlated with the dosage of fluoride, and there were significant differences among high dosage group [(13.03 +/- 1.79) micro g/g], low dosage group [(9.83 +/- 0.92) micro g/g] and control [(8.27 +/- 1.11) micro g/g], P < 0.01. Acetylcholinesterase activities among three groups [(0.111 +/- 0.031) micro mol/mg, (0.143 +/- 0.025) micro mol/mg, (0.183 +/- 0.027) micro mol/mg] were also significantly different (P < 0.01), which was negatively correlated with fluoride concentration in rat hippocampus (r = -0.700, P < 0.01). The activity of butylcholinesterase in high dosage group [(0.041 +/- 0.010) micro mol/mg] was different from that of control [(0.067 +/- 0.025) micro mol/mg, P < 0.05], but the activity was not significantly related with fluoride concentration in rat hippocampus (r = -0.317, P = 0.094).
CONCLUSIONFluoride may go through the blood-brain barrier and accumulate in rat hippocampus, and inhibit the activity of cholinesterase.

6.Epitope Tagging of the rpoS gene of Y. pestis by Recombineering Technique

Objective: To facilitate the functional analysis of chromosomal genes and their products, the recombineering technique to epitope tagging of chromosomal genes of Y. pestis was adapted. Methods: The epitope tag was generated by primer annealing and then fused with resistance gene by fusion PCR. The epitope-resistance cassette was inserted into pBluecript, resulted in the template plasmid, pBS-MH. The tagging cassette for rpoS was obtained by PCR amplification from pBS-MH with primers containing homology specific to the target gene. PCR products were transformed into recombination competent cells and recombinants were selected. PCR and DNA sequencing were used to confirm the correct tagging event. The expression of the tagged protein was detected with Western blot by using monoclonal antibody to the epitope. Results: The template plasmid containing fusion of epitope and resistance gene was successfully constructed. The sigma factor gene, rpoS, was tagged with a myc-his tag at the COOH terminus. Expression of the tagged rpoS was successfully detected indirectly by the antibody against His tag. Conclusion: The chromosomal gene tagging by recombineering technique represents a powerful tool in the functional study of bacterial genes and their products.
7.Analysis of iodine nutritional status of pregnant women and level of neonatal heel blood thyroid stimulating hormone in Zhoupu and Kangqiao districts of Pudong New Area of Shanghai

ObjectiveTo investigate the iodine nutritional status of pregnant women,newborn heel blood thyroid stimulating hormone(TSH) level and their relationship with urinary iodine(UI) level during pregnancy in Zhoupu and Kangqiao districts of Pudong New Area of Shanghai.Methods A total of 993 urinary samples(the first,second and third trimesters of pregnancy were 200 people,respectively),breast feeding(193 people) and non-pregnant women (200 people) in Zhoupu and Kangqiao districts of Pudong New area were collected from Apr 2009 to Dec 2010.Two hundred copies of neonatal heel blood samples were collected.Median of UI was measured by arsenic-cerium catalysis.TSH in neonatal heel blood was analyzed 72 h after birth by time resolved fluoroisnmunoassay(TRFIA).ResultsMedian UI of all pregnant women was 161.35 μg/L,and that in third trimesters of pregnancy( 126.35 μg/L) was lower than that of the first,the second,the breast feeding and non-pregnant women (178.80,180.50,167.90,163.40 μg/L,all P< 0.05).The percentage of UI level less than 150 μg/L in the third trimester[57.5%(115/200) ] was higher than that of the first[39.0%(78/200) ],the second[39.5%(79/200) ],the breast feeding [ 16.6% (32/193) ] and non-pregnant women [ 23.0% (46/200) ],respectively (all P < 0.05).The percentage of UI level higher than 300 μg/L in the first [9.0%(18/200)],the second[8.0%(16/200) ] and the third trimester [ 5.0% ( 10/200 ) ] of pregnancy was lower than that of the breast feeding [ 20.2% (39/193) ] and nonpregnant [20.5%(41/200) ] women,respectively(all P < 0.05).The level of neonatal heel blood TSH was(2.92 ± 1.83)mU/L,the range was 0.01 - 9.76 mU/L,11.0%(22/200) of the neonates heel blood TSH level(5 mU/L)exceeded the ratio of World Health Organization (WHO) standard ( < 3% ) suitable for iodine nutrition.Conclusions The overall level of iodine nutrition among pregnant women in Zhoupu and Kangqiao districts of Pudong New Area of Shanghai is in the appropriate range,but the pregnant women in the third trimester is in mild iodine deficiencies,and the neonates in these districts may be prone to iodine deficiency.Monitoring of iodine nutrition of pregnant women should be strengthened and iodine supplementation should be done scientifically.
8.Correlational research between neutrophil/lymphocyte ratio and chronic heart failure

Objective:To explore the influencing factors of neutrophil to lymphocyte ratio (NLR) in patients with chronic heart failure (CHF) and the relationship between NLR and cardiac contractile function.Methods:From September 2016 to May 2018, 135 CHF patients were enrolled in the cardiovascular department of Fenyang hospital, Shanxi Province for prospective research.According to the level of NLR, the patients were divided into three groups: low NLR group (<2.3, 46 cases), middle NLR group (≥2.3~≤4.3, 45 cases), high NLR group (>4.3, 44 cases). The basic clinical data, laboratory examination data and noninvasive cardiac hemodynamic indexes of the three groups were compared.Results:(1) By comparing the data of different NLR groups, we found that there were significant differences in (N-terminal pro-brain natriuretic pepfide, NT-proBNP)( F=4.485, P=0.013), total bilirubin( F=6.085, P=0.003), albumin( F=3.695, P=0.027). (2)NLR was correlated with NT proBNP, total bilirubin, albumin( r=0.267, 0.256, -0.243, P=0.002, 0.003, 0.005, respectively). (3)Multiple linear regression analysis showed that NLR of CHF patients was correlated with left ventricular ejection fraction (LVEF), NT proBNP, total bilirubin and albumin (standard regression coefficients were -0.239, 0.223, 0.247 and -0.213, respectively, P<0.05). (4) Pearson correlation analysis showed that NLR and cardiac output ( r=-0.173, P=0.045), cardiac index ( r=-0.175, P=0.042), LVEF ( r=-0.278, P=0.001), maximum ejection velocity ( r=-0.207, P=0.016), systolic index ( r=-0.214, P=0.013), hearther index(HI) ( r=-0.179, P=0.038), cardiac work (CW) ( r=-0.235, P=0.006), cardiac workindex (CWI)=( r=-0.244, P=0.004) were negatively correlated. Conclusion:NT-proBNP, total bilirubin, albumin and LVEF are the factors that affect the NLR of patients with CHF.NLR has a certain value in the evaluation of CHF, therapeutic effect and prognosis prediction.
9.Application of ultracision-harmonic scalpel for tonsillectomy

Objective To investigate the advantages and disadvantages of ultracision-harmonic scalpel assisted tonsillectomy by compared with conventional tonsillectomy.Methods Eighty-eight patients were randomly divided into ultrasonic scalpel group (group A,42 cases) and control group (group B,46 cases).The tonsillectomy in group A was performed with ultracision-harmonic scalpel,and in group B,the tonsillectomy was performed by routine method.The surgical time (complete removal of tonsils),blood loss,and postoperative sore throat situation were recorded.Results Surgical time in group A [( 14.7 ± 4.0 )min] was shorter than that in group B [(28.9 ±7.6) min],t =- 10.691,P <0.05.Blood loss in group A [(3.1 ±1.1) ml] was less than that in group B[(19.0 ±5.2) ml],t =- 19.544,P<0.05.Postoperative sore throat was less painful in group A than that in group B in 10 hours after surgery,but much painful than group B 3 days after surgery and most patients lasted longer.There were statistical differences (P < 0.05 ).The average peel off time for the tunica albuginea was 8 days after the operation by using traditional method,by compared with the ultrasonic scalpel method,average time was 11 days,the difference showed statistical significance (t =5.115,P < 0.05).Conclusions Compared with traditional tonsillectomy,ultracision-harmonic scalpel tonsillectomy had shorter operative time,less blood loss and so on,but the sore throat symptoms persisted longer.In addition,the tunica albuginea peeled off later,so avoidance of secondary bleeding caused by improper diet was mandatory.
10.Combined implantation of intracranial Solitaire AB stent with endovascular coil in treatment of wide-necked posterior communicating artery aneurysms

Objective To summarize the technique and the clinical value of Solitaire AB stents combined with Axium detachable coil in treatment of wide-necked posterior communicating artery aneurysms.Methods Twenty-eight patients with wide-necked posterior communicating artery aneurysms,admitted to our hospital from March 2010 to September 2012,were performed endovascular procedures by intracranial Solitaire AB stent and Axium detachable coil.Their clinical data and treatment efficacy were retrospectively analyzed.Results All patients were identified as having wide-necked intracranial aneurysms suitable for stent-assisted Axium treatment.Total occlusion in 24 patients and partial occlusion in 4 patients were achieved.All patients recovered well.All patients were angiographically followed-up 3 months after the procedure; among them,no filling was shown for the 23 aneurysms which were densely packed,1 case of parent artery occlusion and 4 aneurysms with neck remnant did not change.Conclusion The Solitaire AB stent is safe intracranial stent-assisted coil for embolization of wide-necked intracranial aneurysms; it is very suitable for treating the wide-necked posterior communicating artery aneurysms with severely tortuous intracranial artery,enjoying few complications.
