Colorectal cancer is one of the most common malignancies in the world, and rectal cancer accounts for a large percentage of this disease in Chinese patients. Surgical extirpation is the mainstay of therapy, Intraluminal ultrasound is highly accurate in detecting depth of invasion and lymph node metastasis, and is also helpful in preoperative staging of rectal cancer. Preoperative adjuvant therapy could down-stage tumors, improve the radical resection rates of advanced lower rectal cancer, increase the rate of sphincter-saving surgery, and decrease the local recurrence rate. Total mesorectal excision could signifi-candy decrease the local metastasis of rectal cancer, and rectal cancer resection with preservation of pelvic autonomic nerve is effective in preventing urinary or reproductive dysfunction. Recently, minimally invasive surgery has been introduced for the management of rectal cancer, and transanal endoscopic microsurgery is a new method with advantages in rectal cancer treatment.

Objeetive To analyze the risk factors on anastomotic leakage after low anterior resection with the technique of total mesorectal excision(TME)for rectal carcinoma. Methods Our retrospective study included 498 patients of rectal cancer who underwent anterior resection with TME technique. Results 7.2%patients(36/498)developed clinical anastomotic leakage.Sex,distance of tumor from anal verge,preoperative radiotherapy were found related to the anastomotic leakage ( P＜0.05 or0.01),while age,diabetes,hypertensive disease,vesse]emboli,diversion stoma construction,anastomotic technique were not.Among the 37 patients with diversion stoma construction simultaneously,3 patients developed leakage who recovered conservatively.Among the other 33 leakages 10 were cured conservatively.The other 23 leakages necessitated a laparotomy and proximal colostomy. Conclusion Sex,distance of tumor from anal verge,preoperative radiotherapy were risk factors for anastomotic leakage after TME.

Objective To investigate the expression and prognostic significance of p27kip1 in patients with stage I and Ⅱ colorectal carcinoma ( CRC). Methods We retrospectively analyzed the expression of p27kip1 by immunohistochemistry in paraffin-embedded specimens from 114 consecutive patients operated for stage I and Ⅱ CRC. Statistical analysis was done with SPSS10. 0 package. Results The rate of p27kip1 over-expression in stage I and ⅡCRC was 85. 1 % (97/114). The expression of p27kip1 was statistically associated with TNM stage and histologic grade, while there was no significant relationship between p27kip1 expression and patient's age, tumor location. This paper has reported the effect of preservation myceh'al fungus by the methods of slant and rubber plug. The mycelial fungus preserved include 29 genera, 69 species, 128 strains. The preservation time is from 2 years to 17 years. By slant inoculation, their survival conditions are still good keep their original charaters. The result shows that it is simple and effective preserving some mycelial fungus by the methods of slant and rubber plug.

Objective To investigate the expression and clinical significance of p27 protein in stage Ⅱ colon cancer.Methods The expression of p27 protein in 57 samples of stage Ⅱ colon cancer tissue from BeijingCancer Hospital was detected by immunohistochemistry and tissue microarray technique.Survival was analyzed by the Kaplan-Meier method,and the correlation between the expression of p27 protein and the prognosis of patients was analyzed by Pearson chi-square test.Multivariate analysis was done by using Cox proportional hazard regression model.Results The rate of high expression of p27 protein in stage Ⅱ colon cancer was 60%(34/57).The differentiation.location and distal metastasis of the tumor were correlated with the expression of p27 protein(X2=5.97,5.93,5.05,P＜0.05);while age,tumor size,growth patterns of tumor were not correlated with the expression of p27 protein(X2=0.64,0.49,0.33,P＞0.05).The 5-year survival rate of the patients with high expression of p27 protein was 94%(32/34),which was significantly higher than the 78%(18/23)of the patients with low expression of p27 protein(X2=3.86,P＜0.05).Multivariate analysis showed that low expression of p27 protein and tumor penetrating serous membrane are the independent adverse prognostic factors for stage Ⅱ coloR cancer.Conclusions Low expression of p27 protein is one of the independent adverse prognostic factors for stage Ⅱ colon cancer,and the expression of p27 protein should be consulted when screening out the patients who are in the high risk group of stage Ⅱ colon cancer.

Objective To screen the plasma differential expressed proteins in patients with Crohn's disease(CD)and ulcerative colitis(UC)using surface-enhanced laser desorption ionizationtime of flight mass spectrometry,and to establish decision trees algorithms.Methods The plasma samples from 24 UC patients,25 CD patients and 25 healthy controls were analyzed with CM10 protein chip.The proteomic spectra of CD,UC and inflammatory bowel disease(IBD)were compared with controls respectively.The differential proteins that significantly altered expression levels were selected to establish decision trees algorithms of CD.UC and IBD and then blind validations were tested.Results In the range of m/z 2000-30 000,differential expressed proteins that changed at least 2-fold between CD and controls were 9.between UC and controls were 5,and between IBD and controls were 11(P＜0.05).The software automatically picked up the m/z 8208 and 8837 as decision trees algorithms for differentiating CD from controls and m/z 6985 for differentiating UC from controls as well as m/z 8208,1752,28840 and 1702 for differentiating IBD from controls.The sensitivities of decision trees algorithms for CD,UC and IBD were 96%,82%and 91%respectively,and the specificities were 100%,85%and 100%respectively.Conclusions The protein of m/z 8208 which has high sensitivity in differentiating CD from controls is worthy of further study.

Objective To compare the rehabilitation effect of free rehabilitation, the needle therapy and the hand function recovery machine combined with free rehabilitation on movement function, the feeling function as well as the movement pattern of newly stroke patients with hemiplegia. Methods 49 newly stroke patients were divided into the free rehabilitation group, the acupuncture group and the hand function recovery machinegroup, each group received corresponding intervention. The movement function, the feeling function as well as the movement pattern were compared among the three groups using blind method. Results Brunnstrom evaluation for hands feeling-movement function, the Fugl- Meyer evaluation score showed obvious progress compared to before the treatment, score of fine hand - function assessment of Carrshepherd showed no change. Two weeks after treatment,Brunnstrom evaluation for hands feeling-movement function and the Fugl - Meyer evaluation score showed no difference between the free rehabilitation group and the acupuncture group, but the hand function recovery machine group showed better results than the other two groups. One months after treatment,Brunnstrom evaluation for hands feeling-movement function was different among these groups, but not the Fugl-Meyer evaluation score, the hand function recovery machine group showed better results than the other two groups, still score of fine hand-function assessment of Carrshepherd showed no change. Conclusions Acupuncture, free rehabilitation, hand function recov - ery machine all make progress in hand function recovery for newly stroke patients with hemiplegia, but hand function recovery machine combined with free rehabilitation proved to be the best method. Its effect for hand intricate function needs to be enhanced.

Objective To explore the relationship between homocysteine(HCY),glycosylated hemoglobin(HbA1c),lipids and posterior circulation ischemia and the clinical significance of their levels in the posterior circulation ischemic (PCI)diseases. Methods Difference between PCI were diagnosed in 140 examinees and a healthy control group,with fasting serum,HCY, HbA1c,total cholesterol (TC),triglyceride (TG),high density lipoprotein cholesterol (HDL-C)and low-density lipoprotein cholesterol (LDL-C)were detected.Results HCY and HbA1c were significantly higher,HDL-C was significantly lower in the patients with PCI,two independent sample t-test showed a significant difference between the test group and control group (P <0.001 and P =0.001),the remaining lipid indicators was not statistically significant.Spearman rank correlation analysis showed that HCY levels had significant positive correlation (P <0.001)and TG levels had significant negative cor-relation (P =0.013)with the subjects age,HCY levels had significant negative correlation with TG levels(P = 0.028), HbA1c levels had significant positive correlation with TG levels and LDL-C levels(P =0.001 and P =0.027).Conclusion High levels of HCY and HbA1c were closely associated with PCI,which HCY and HbA1c should be attached great impor-tance to the effective prevention and treatment and improve of PCI.
