1.Clinical observation of breast-conserving operation combined with radiotherapy for 43 patients with breast cancer

Objective To investigate the efficacy,side effects and cosmetic effects of breast-conserving operation combined with radiotherapy for patients with breast cancer. Methods Forty-three patients with early stage of breast cancer(n=30 for stage I and n=13 for stage II) were performed extended resection plus axillary lymph node dissection or quadrant resection plus axillary lymph node dissection.After operation,tangential field irradiation was conducted with 50 Gy of X ray,tumor bed was irradiated with additional 10 Gy of electron beam,and supraclavicular field irradiation was also administered with 30 Gy of X ray and 22 Gy of electron beam. Results There was no recurrence during the follow up for 14 to 50 months.The acute side effects of radiotherapy included breast complaints(n=10,grade Ⅰ,23.26%),skin erythema and hyperpigmentation(n=19,grade Ⅰ,44.19%),moist desquamation(n=5,grade Ⅱ,11.63%) and radiation esophagitis(n=4,grade Ⅰ,9.30%),while one patient experienced radiation esophagitis one month after radiotherapy.No such complications as skin fibrosis and necrosis,angiotelectasis,breast and upper extremity edema occurred.The short-term cosmetic effects were satisfactory to 93.02% of all the patients. Conclusion For early stage of breast cancer(stage I and stage Ⅱ),breast-conserving operation combined with radiotherapy may yield satisfactory survival rate and short-term cosmetic effects,as well as low incidences of side effects.
2.Establishment of HepG2~(Tet-on) cell line controlled by the Tet-on regulatory system

Objective To establish tetracycline-controlled inducible system(Tet-on) in HepG2 cell for further research of the function of related gens.Methods The HepG2 cells were transfected with pWHE146 vector by using liposome transfection reagent.The transfected cells were selected in medium containing G418,and G418-resistant clones were isolated.All individual G418-resistant clones were screened by transient transfection with plasmid pTRE-hyg-luc for clones with low background and high induction of luciferase in response to Dox.Results One HepG2 cell line,which exhibited high levels of induction(154,106 times)and high gene expression levels,was obtained.Conclusions The HepG2 cell line can be used to highly express eukaryotic gene and this Tet-on system is available for use in eukaryotic gene function studies.
3.Reduction mammoplasty by central pedicle flap transposition

Objective To explore a surgical procedure of reduction mammary plasty to assure the viability and sensory of the nipple-areolar complex (NAC) after transposition and to keep the lactiferous function partly. Methods Reduction was accomplished by using perforating vascular branches from the pectoralis major muscle and its fascia supplying the nipple and breast parenchyma instead of the subdermal plexus. The central vascular pedicle supplying the nipple-areola complex was preserved. And the sensory nerve of the NAC was not injured. The erection of the nipple could be observed in operation. Lactation was possible because of preservation of the lactiferous ducts. The technique had been used in 17 patients since 1996. Results The nipple-areola complex was safely transposed with great freedom, and the amount of resection was accurately adjusted for symmetry. No cases of nipple-areola complex sensory change occurred postoperatively in all 17 patients. Breast-feeding was possible at least in one patient after operation. Conclusion This method is ideal for the patients, which keeps the viability and sensory of the nipple-areolar complex and the function of lactation. In another case, the amount of resection is easily adjusted for symmetry.
4.Advanced alcoholic osteonecrosis of femoral head

[Objective]To discuss the relation-ship between AAONFH(advanced alcoholic osteonecrosis of femoral head)and extended overdose drinking.[Method]Reviewed 165 cases of total hip arthroplasty,to analyzed the clinical data among 27 AAONFH cases,reviewed the literatures,discussed the pathogenesis of AAONFH and the relation-ship between blood biochemistry and blood cloting funtion and compared the experimental results among literatures.[Result]Theres various factors resulted in osteonecrosis of the femoral head associated with alcohol long-term over intake.Individuals consuming more than 750 ml of alcohol daily were at greater risk of double or half ONFH,Theres no change in biochemical test,but blood clotting funtion in this case would degrade,osteoarthritis of hip would synchronize.[Conclusion]Individuals consuming more than 750 ml of alcohol daily have the potentiality of ONFH;Its no specificity significance in biochemical test,the necrosis extent of the femoral head is relative to the capacity and times of alcohol intake.THR(total hip replacement)is the best treatment for AAONFH,but theres more staxis potentiality in this cases.
5.AN EXPERIMENTAL STUDY ON CHRONIC SPORTS INJURIES IN QUADRICEPS AND CALF MUSCLES AND THEIR INSERTION AREAS

The experiment was designed to simulate active training with animal model. A dynamic and static trai-ning program for adult rats was carried out 30-60'/day, lasted from 25 to 232 days. The rats were killedperiodical1y in batches and the sections were prepared for exploring histological changes in knee and ankle areas on L/M. Degenerative cbanges in quadriceps and gastrocanemius muscles appeated by degrees aftet a period of in-tensive training. The nuclei proliferated in chains or clusters arranged in the center and the edge of thefibers. The number of nuclei for each chain or cluster could be as much as 20 or 30. Meanwhile, therewere also ischemic changes in small vessels. All these were the characteristic changes induced by mictotraumain muscles under repeated active and strenous training. In calcaneal and patellar ateas, there were changes of cartilaginous degeneration, necrosis and prolifera-tion (cartilaginous exostoses and osteophytes) too. The typical changes of enthesiopatby---degeneration, necrosis and proliferation (osteophyte) could beobserved in calcaneal tendon and its insertion area and in superior insertion atea of lower patella. The occur-rence of enthesiopathy in this experiment migbt be related to tke stress of traction force within muscle andcartilage during running in upward treadmill.
6.External fixators for treatment of pelvic fractures

Objective To investigate the advantages of the external fixators used in the treatment of pelvic fracture. Methods From April 2002 to July 2008, 55 patients with unstable pelvic fracture were treated respectively with simple external fixators, external fixators plus traction and external fixators plus in-ternul fixation according to their different Tile classifications. Results Of this group, 51 cases were followed up for an average of 11 months, 2 cases died because of multi-injuries, and 2 cases were lost in the follow-up. All of the 51 patients obtained bony union. On average, it took 11 (6 to 14) weeks to achieve weight-bearing walking, and 12 (8 to 14) weeks to remove the external fixators for the patients. By Liu Li-ming's evaluation system, the good to excellent rate was up to 88.2%. Conclusions The pelvic fractures of Tile B1 and B2 can be treated with simple external fixators and those of Tile C with external fix-ators plus internal fixation or traction to achieve fracture reduction and stability of the pelvic ring. For the patients with multi-injuries, external fixators used in the emergency treatment can stabilize the pelvic ring, reduce blood loss and improve the success rate.
8.Establishment of migraine rheumatism stasis syndrome animal model.

OBJECTIVETo establish the migraine rheumatism stasis syndrome animal model.
METHODThe rat migraine rheumatism stasis syndrome animal model was established through rheumatism stimulation with manual climate box, 5-HT reduction caused by reserpine and local cerebral vasospasm. General vital signs (activity, weight, eye gum, hair, feeding, excrement), head scratch frequency and image collection were observed to analyze the changes in biological signs of stasis syndrome (tongue image RGB), thrombin and serotonin of model rats.
RESULTThe reserpine group and the reserpine plus rheumatism model group showed significant reduction in blood coagulation time, pain threshold and 5-HT content in blood and brain (P < 0.01); the reserpine plus rheumatism model group showed an increase in eye gum and decreases in activity, feeding, with thin sloppy stool. According to the tough RGB values, the control group showed light red toughs, the reserpine group showed dark purple toughs, the reserpine plus rheumatism model group showed gray toughs, with notable differences in tough RGB values in all three group.
CONCLUSIONThe rheumatism stimulation with manual climate box, 5-HT reduction caused by reserpine and local cerebral vasospasm can be used to induce the migraine rheumatism stasis syndrome animal model, but its modeling assessment method and process shall be further improved.

9.Effect of concurrent or sequential exemestane combined with radiation on radiosensitivity of MCF-7 cells

Background and purpose:Radiotherapy and endocrine therapy are both important parts of adjuvant therapy for breast cancer, yet few studies have been conducted focusing on the interaction between radiation and endocrine therapy. Up to now, no conclusion has been drawn on the timing sequence of adjuvant radiation and endocrine therapy, which is indeed crucial in clinical practice. This study intended primarily to investigate the effect of concurrent or sequential exemestane combined with radiation on radiosensitivity of MCF-7 cells and its possible mechanism,and further to provide rationale for optimal clinical treatment modality.Methods:MCF-7 cells were arranged into three trial groups: the radiation group, exemestane sequenced with radiation group and exemestane followed radiation group. Radiosensitivity was evaluated by clonogenic assay, cell proliferation was measured by MTT assay, the ability to induce cell apoptosis was evaluated by DAPI staining assay, the changes of Bcl-2 and Bax were detected by Western blot.Results:Sensitive enhancement ratios (SER) were 1.51 and 1.37 in the exemestane sequenced with radiation group and exemestane followed radiation group, respectively. Compared with the radiation group, the percentage of cellular proliferation inhibition and apoptosis increased obviously in the exemestane sequenced with radiation group and exemestane followed radiation group. Exemestane combined with radiation made the Bax protein increase obviously and the Bcl-2 protein lowered significantly.Conclusion:Exemestane can enhance the radiosensitivity of MCF-7 cells, whose mechanism might be relevant to the promotion of cellular apoptosis. However,the treatment sequence does not affect the outcome.
