Objective:To study the mechanisms of neutrophil elastase (NE) induced expression of respiratory mucin MUC5AC. Methods:Using gene recombination techniques,four luciferase reporter gene plasmids containing different length of human MUC5AC gene promoter were constructed.Site-directed mutagenesis technique was used to establish mutants of Sp-l and NF-?B site in MUC5AC gene promoter; the relative luciferase activities were detected in the transfected human pulmonary A549 cells. Results:Series of luciferase reporter gene containing different sequences of human MUC5AC promotor were constructed successfully.NE could increase the expression of luciferase reporter gene plasmid containing -1300bp,-689bp and-324bp version of MUC5AC promoter in the transfected A549 cells (P

Standardized training of residents is a necessary way for clinical medical personnel train-ing. Residents graduating from medical colleges and universities should accept the standardized, scientific, strict pre-service training, in order to realize role conversion from medical students to hospital physicians. This paper summarizes and analyzes the pre-service training, focusing on the training object, course design, organization implementation, evaluation, questionnaire survey, and puts forward some thoughts and sugges-tions on strengthening and improving pre-service training form, expanding the content of training, and quan-tifying the evaluation of training effect.
3.The express and research of chemerin and CRP in 45 patients with early-onset preeclampsia

Objective The purpose of this study was to compare chemerin and CRP levels between women with and without ear‐ly‐onset preeclampsia based on maternal body mass index (BMI) .Methods 45 patients with early onset preeclampsia (gestation week is less than 34 weeks) ,30 normal pregnancy women were enrolled in the study .The serums levels of chemerin and CRP were measured in these pregnancy women .Results The chemerin [(379 .18 ± 52 .09)ng/L ,(481 .27 ± 63 .12)ng/L] and CRP [(7 .59 ± 1 .41)mg/L ,(11 .74 ± 1 .61)mg/L] levels in MPE and SPE were significantly higher than those [(316 .08 ± 51 .90)ng/L ,(3 .47 ± 1 .27)mg/L]in normal pregnancy women(P<0 .05) .While the chemerin(424 .64 ± 46 .61)ng/L and CRP (8 .32 ± 3 .82)mg/L levels increased with maternal BMI in patients overweight and obese women levels compared with normal weight women [(331 .66 ± 51 .77)ng/L ,(5 .93 ± 2 .95)mg/L] ,the difference are significant(P<0 .05) .The correlation analysis showed that the chemerin were positively correlated with BMI ,systolic blood pressure ,diastolic blood pressure ,triglycerides ,and CRP were positively correlated with BMI .Conclusion The expression of chemerin and CRP were associated with severity of preeclampsia and BMI .
4.Stem cell therapy for degenerative disc diseases:present status and prospects

BACKGROUND:None of the current treatment strategies has been focused on relieving or reversing the disk degeneration process after degenerative disc diseases. In recent years, more and more scientists try to treat degenerative disc diseases using stem cel therapy. OBJECTIVE:To explore the research status and prospects of stem cel therapy for degenerative disc diseases. METHODS: A computer-based online search of PubMed database between January 2004 and December 2014was performed to search related articles with the key words of “stem cel, intervertebral disk” in English. Literatures related to stem cel therapy for degenerative disc diseases were selected; in the same field, the articles published lately in authoritative journals were preferred. RESULTS AND CONCLUSION: A total of 342 articles were primarily selected, and 43 articles were involved in result analysis according to inclusion criteria. Stem cel therapy is a newly treatment for degenerative disc diseases. Cels appropriate for stem cel therapy include embryonic stem cels, induced pluripotent stem cels, mesenchymal stem cels, human umbilical cord mesenchymal stem cels and chondrocytes or nucleus pulposus cels. Although cel leakage, intervertebral disc infection and tumorigenesis are the main chalenges, stem cel therapy for degenerative disc diseases is promising in the future.
5.Lumbar disc degeneration:tissue engineering repair and biomechanical evaluation

BACKGROUND:At present, the clinical treatment of lumbar disc degeneration mainly includes conservative treatment, traditional surgery and minimal y invasive surgery. The therapeutic purpose is to relieve symptoms, but the long-term effect is not very satisfactory. Therapeutic methods focusing on biological functional recovery have been concerned gradual y, but the clinical application is far in sight. OBJECTIVE:To review the advances in the treatment of lumbar disc degeneration regarding tissue-engineered repair and biomechanics. METHODS:PubMed database was searched by the first author for relevant articles published before December 2014 using the keywords of“intervertebral disc degeneration, clinical treatment, biological treatment, tissue engineering, biomechanics, repair, progress”in English. A total of 100 articles were searched initial y and final y, 40 articles were included in result analysis. RESULTS AND CONCLUSION:Although the therapeutic schemes are varied, the treatment of intervertebral disc degeneration is a great chal enge for clinicians and basic researchers. Currently there is no perfect clinical treatment, and indications corresponding to various therapies should be paid attention as wel as long-term fol ow-up evaluation. For various reasons, the biological treatment for intervertebral disc degenerative disease is becoming more and more popular, providing a promising prospect for the treatment of intervertebral disc degeneration. So far, large amounts of data have been obtained from animal experiments, but there are stil many problems to be solved. Other chal enges also involve the al aspects of general tissue engineering methods, such as cel s, cytokines and scaffolds. In these studies, the nucleus pulposus tissue engineering based on the combination of heparin-functionalized chitosan hydrogel, cytokines and stem cel s exhibits a promising prospect.
6.Antibiotic resistance and multilocus sequence typing of Enterococcus faecium strains isolated from ;children under 3 years old in Sui county, Henan province

Objective To investigate the antibiotic resistance, virulence genes, and sequence types of Enterococcus faecium ( E. faecium) strains isolated from children under 3 years old in Sui county, Henan province. Methods Kirby-Bauer antibiotic testing was performed to analyze the antibiotic sensitivi-ties of E. faecium strains to 15 common antibiotics. PCR analysis was used to detect the virulence genes car-ried by the E. faecium strains. Multilocus sequence typing ( MLST) was performed for the typing of E. faeci-um strains. Results Forty-seven E. faecium strains were isolated from 120 stool samples collected from chil-dren under 3 years old in Sui county, Henan province, of which 95. 7% were antibiotic-resistant strains. Most of the isolated E. faecium strains were resistant to rifampicin, accounting for 91. 5% (43/47) of all isolates, followed by those resistant to erythromycin and tetracycline, which accounted for 68. 1% (32/47). Moreover, high resistance rates to those antibiotics commonly used in clinical treatments of E. faecium infec-tion including β-lactam and aminoglycosides antibiotics were observed. Those strains resistant to more than three kinds of antibiotics belonged to the same clonal complex including 12 strains of clonal complex 17 ( CC17) harboring the virulence gene of hyl. All of the isolated E. faecium strains were susceptible to vanco-mycin, linezolid, chloromycetin and nitrofurantoin. Conclusion The E. faecium strains isolated from chil-dren under 3 years old in Sui county, Henan province were multi-antibiotic resistant. There were drug-resist-ance strains belonging to the CC17 and carrying the virulence gene of hyl.
8.The value of multipoint and multidirectional ultrasound-guided fine-needle aspiration cytology in preoperative diagnosis of papillary thyroid microcarcinoma

Objective To investigate the value of multipoint and multidirectional ultrasound-guided fineneedle aspiration cytology (US-FNAC) in preoperative diagnosis of papillary thyroid microcarcinoma (PTMC).Methods From Jan.2012 to Jan.2013,multipoint and multidirectional US-FNAC were performed in 177 patients with 233 nodules suspected as PTMC,and the cytopathological findings were compared with surgical pathological results and clinical follow-up results.Results Of the 233 biopsies,the cytology were classified as malignant for 146 lesions,suspicious for malignancy for 43 lesions,benign for 24 lesions,follicular lesions for 4 lesions and inadequate sample for 16 lesions.After the second multipoint and multidirectional US-FNAC,only 1 case did not have sufficient cells for diagnosis,and the puncture failure rate was 0.43%.Actually there were 189 malignant and 44 benign lesions confirmed by clinical follow-up and pathological results.The sensitivity,specificity,and accuracy of multipoint and multidirectional US-FNAC was 99.5%,81.8% and 96.1% respectively.Conclusions Multipoint and multidirectional US-FNAC has definite value in increasing puncture qualified rate and positive diagnosis rate before operation.
9.Evaluation of cardioprotective effect of ischemic preconditioning on ischemic myocardium using 99Tcm-Syt I-C2A in the myocardial ischemia-reperfusion rat model

Objective Precondition is an approach to myocardial protection during ischemia-reper-fusion by inhibiting myocardial cell apoptosis.The purpose of this study was to evaluate the cardioprotective effect using 99Tcm-syuaptotagmin I (Syt I)-C2A to detect myocardial cell apoptosis in the myocardial is-chemia-repedusion rat model.Methods (1) The C2A domain of Syt I was labeled with 99Tcm using 2-iminothiophene hydrochloride (IT) method.Radiochemical purity was determined with thin layer chroma-tography.The binding activity of radiolabeled protein was assessed using eamptothecin-treated Jurket cells.(2) One group of 6 rats was prepared for myocardial ischemia-reperfusion model(A group),and another group of 6 rats was prepared for myocardial ischemia precondition model(B group).99Tcm-Syt I-C2A was injected via the tail vein at a dosage of about 7.4 MBq.At 1h after injection,the rat was sacrificed,and the heart was removed to rinse with saline and dye with triphenyl tetrazolium eoride (TTC).According to the resdt of myocardial dye,theischemic myoeardium was separated from the viable myocardium and weight was measured,and then its radioactivity was determined by gamma counting.The difference of radioactive uptake in the ischemic myocardium between these two group models was compared using percentage activity of injection dose per gram of tissue(%ID/g)±standard deviation(x±s).SPSS 12.0 was used for data analy-sis,and t-test was used to compare data.Results (1) The radiochemical purity of 99Tcm-Syt I-C2A was (98.90±0.43)%,and the radioactivity in the camptothecin-treated group was (10.99±0.55) folds higher than that of non-treated viable control group.(2)In the ischemia-reperfusion model,the radioactive uptake of 99Tcm-Syt,I-C2A was(2.41±0.32)%ID/g in the ischemic myocardium,and(0.16±O.02)%ID/g in the nomud myocardiunm.However,in the myocardial ischemia precondition model,(0.46±0.05)%ID/g in the isehemic myocardium was measured,and(0.20±0.05)%ID/g in the normal myocardium.Uptake of 99Tcm-Syt I-C2A in ischemic myocrdium showed statistically significant difference (t=8.52,P
10.Urodynamic tests contribute to the choice of therapies for type-III B prostatitis.

OBJECTIVETo analyze the parameters of urodynamic tests for patients with type-III B prostatitis and evaluate the significance of the results of urodynamic tests in the choice of therapies for this disease.
METHODSUrodynamic tests were performed for 87 type-III B prostatitis patients aged 22-45 (30.7 ± 8.5) years, who had moderate or severe lower urinary tract symptoms (LUTS) and failed to respond to routine therapy. Different treatments were administered according to the results of urodynamic tests followed by observation of the therapeutic effects.
RESULTSUrodynamic abnormalities were found in 70 of the 87 patients, bladder outlet obstruction in 28 (32.2%), detrusor overactivity in 25 (28.7%), bladder hyperesthesia in 18 (20.7%), low compliance in 10 (11.5%), detrusor-external urethral sphincter dyssynergia in 1 (1.1%), and impaired detrusor contractile function in 1 (1.1%). Treatments achieved obvious effectiveness in 26 cases (29.9%), effectiveness in 51 (58.6%), and no effectiveness in 10 (11.5%).
CONCLUSIONUrodynamic tests contribute significantly to the choice of therapies for type-III B prostatitis patients with moderate or severe LUTS.
