2.Risk factors for lymph node metastasis of early colorectal cancer and indices of endoscopic resection

Objective To analyse the risk factors for lymph node metastasis of early colorectal cancer and the therapeutic indication for endoscopy.Methods The clinical data of the 269 early colorectal cancer patients who underwent surgical treatments between January 2009 and December 2013 in the West China hospital of Sichuan University were analyzed retrospectively.Pathologic features were compared between different histological types and investigated by univariate and multivariate analysis of their possible risk factors for lymph node metastasis.Results Univariate analysis showed that tumor size (P =0.029),depth of tumor invasion (P =0.006),histological type (P =0.000) and lymphatic involvement (P =0.035) were correlated with lymph node metastasis.Multivariate analysis revealed that tumor size (OR =5.385,95% CI:1.156-25.075,P =0.032)and histological type (OR =5.145,95% CI:1.553-17.053,P =0.007) were independent risk factorsfor lymph node metastasis.Comprehensive analysis showed that lymph node metastasis could not be found in patients with tumor invading the mucous layer.However,lymph node metastasis occurred if the tumor invaded the submucous layer.The larger and less differentiated tumor was,the higher occurrence of lymph node matastasis would be (P < 0.05).Conclusion Endoscopic resection is recommended for those with early colorectal cancer localized in the mucous layer and without lymph node metastasis.It is recommended that patients with submucosal carcinomas undergo a preoperative endoscopic ultrasonography or pathological screening for the most appropriate surgical treatment,as the larger and the lower differentiated the tumor is,the higher chance the lymphatic metastasis is.

Objective:To study the effects of Taoist cognitive therapy on type A behavior of patients with coronary heart disease.Methods:206 patients with coronary heart disease were randomized divided into two groups. 5.Expression of P16,CyclinD1 and P53 in hydatidiform mole and its significance

Objective To study the relationship between P16,CyclinD1 and P53 anti-oncogene and the gen- esis of hydatidiform mole.Methods 30 samples of hydatidiform moles and normal early pregnant aborted placenta villi respectively were obtained to detect the P16,CyclinD1 and P53 anti-oncogene expression in two kinds of tissues by using SP immunohistochemical staining.Results Compared with that of normal villi,the expressions of P16,P53 and CyclinD1 anti-oncogene were quite different in hydatidiform moles.The expression of P16 was all positive,while CyclinD1 and P53 were all negative in the chorion of early gestation.A descending tendency of P16 expression was found,while the expression of CyclinD1 showed an ascending tendency.The positive rate of P16,CyclinD1 and P53 expression was significantly different between the groups.It was also observed that there was significant difference between the P16 and the proliferation trophocyte.Conclusion P16,CyclinD1 and P53 anti-oncogene have a close relationship with the genesis of human hydatidiform mole.
6.Effects of supracondylar fractures of humerus treated by three different surgical methods.

OBJECTIVESTo research the effects of supracondylar fractures of humerus treated by three different surgical methods.
METHODSThere were 103 patients with supracondylar fractures of humerus including 58 male and 45 female. The age was from 3 to 12 years old with the average of 7.4 years. To divide the cases into three groups by the operative approach in the humerus, the inside approach were in 37 cases (group A), the outside in 35 cases (group B) and the rear in 31 cases (group C). Comparing the reduction of the stretching and bending function of elbow joint and incidence of elbow varus.
RESULTSAll patients were followed-up for 1.5 to 8 years after operation with the average of 32 months. The reduction of stretching and bending function of elbow joint occurrenced in 6 cases in group A, 6 cases in group B and 25 cases in group C, the incidenc of group C were obvious more than group A and B (P < 0.01). The elbow varus occurred in 8 cases in group A, 17 cases in group B and 7 cases in group C, the incidence of group B were obviously more than group A and C (P < 0.05).
CONCLUSIONThere are different effects between three kinds of operative approaches in the humerus. The incidence of outside group is higher on the varus of elbow. The incidence of rear group is higher on the ability of elbow joint stretching and bending. The effect of inside group is sure on the operative approach in the humerus.

7.Estimation on ethanol content measured in vivo.

Recently the cases after drinking are increasing, but the systematic studys on ethanol content in vivo and correlative problems are still absent. According to the measured results of ethanol content in vivo, ethanol metabolic distributed rules, mechanisms of ethanol toxicological effect and its production in vivo, this study analysed systematically the time after drinking, total quantity of absorbed ethanol, psychological situations, behavioral dominated ability, death causes and manners in order to find out the implied forensic medical information and provide the reference for colleague.

8.Study on establishment of spastic cerebral palsy model of macaque by partial resection of motor cortex

Objective To establish spastic cerebral palsy model of macaque by partial resection of motor cortex and explore its evaluation method.Methods Four individuals of 3-month-old macaques were divided into healthy control group and operation model group according to random number table.Partial resection of the motor cortex was carried out in operation model group,in which precentral gyrus cortex from above the right lateral cerebral fissure to the inter-hemicerebral fissure,together with the posterior-superior frontal gyrus (about 0.3 to 0.5 cm in front of the anterior median sulcus) cortex were removed with the depth of about 0.5 to 0.6 cm.After the operation,the continuous camera shooting was used to record whether left limb motor dysfunction and abnormal posture existed or not.Muscle tension was assessed by manual examination of muscle tone with reference to the modified Ashworth scale.The quantitative indexes of the two groups were detected by using the gross motor and fine motor assessment scale.9.4T magnetic resonance imaging (MRI) was used to detect the brain imaging changes.Results After operation,the macaque in the operation model group immediately showed left hemiparesis,left upper limb abnormal lifting,left lower limb paralysis,left limb claudication,and eating mainly relied on the right side of the body.After 6 weeks of operation,left limb activity of the operation model group was significantly lower than that of the healthy control group,and the gross motor scores and fine motor scores were significantly lower than those of the healthy control group(Friedman test:χ2=33.939,P<0.05;χ2=37.526,P<0.05).The macaque in the operation model group showed some symptoms that abnormal posture mainly tilted to the left for the rest,sitting in a corner of the monkey cage,left arm was put on the cage to maintain postural balance,and movement was left slightly inclined,which had simulated the typical clinical manifestations of human spastic hemiplegic cerebral palsy.Muscle tension was checked by hand,and the left limb paralysis and muscle tension decreased after operation in the model group,and the left muscle tension increased gradually after 5 weeks,and gradually increased to score 4 points and the score remained 3 after 10 weeks.Brain MRI of 3 weeks postoperatively suggested scar tissue formation after right motor cortex resection,which supported the pathological changes of the hemiplegic cerebral palsy models.Conclusions Through the partial resection of the motor cortex,the model of spastic cerebral palsy was established successfully.The results of behavioral evaluation and MRI showed that the model was consis-tent with spastic hemiplegia.
9.Application of spiral CT 3D reconstruction of rib fracture in clinical forensic practice.

OBJECTIVE:
To assess the clinical application of spiral CT 3D reconstruction in forensic diagnosis of rib fracture.
METHODS:
Thirty-five cases of rib fractures were collectively studied by X-ray, regular CT scan, and adoptive 3D reconstruction with spiral CT.
RESULTS:
Clear and stereoscopic images of rib fractures without interference of the surrounding soft tissue were obtained in all of 35 cases examined.
CONCLUSION
Spiral CT 3D reconstruction could be a valuable means in forensic diagnosis of rib fracture.
