1.Determination of Prednisolone and Prednisolone-hemisuccinate in Biological Media by RP-HPLC

OBJECTIVE:To establish an analytical method for the determination of prednisolone and prednisolone-hemisuccinate in biological media METHODS:Using RP-HPLC,biological media were extracted with a mixture of n-pentane and methyl tert-butyl ether(2∶3) The mobile phase consisted of acetonitrile and trisodium citrate buffer (33∶67) The sample was eluted on C18 column and detected at 254nm RESULTS:The extraction recoveries of two drugs were over 80% and the RSDs of within-day and day-to-day were below 3% CONCLUSION:The method is simple,rapid,sensitive,accurate and good enough to be applied to the pharmacokinetic study of new preparation of prednisolone and can also be applied to other similar glucocorticoids
2.Observing the effect of Xiaoyukang capsule on the treatment of limb fracture healing

Objective To observe the effect of Xiaoyukang capsule on the treatment of limb fracture healing.Methods65 patients according to randomized double-blind were divided into two groups (n=33) and control group (n=32 patients), postoperative observation group Xiaoyukang capsule treatment, control group were not special treatment.At different time points in all patients visual analog scale (VAS score), fracture healing fracture line score and grade the degree of X-ray, using Elisa method of total osteocalcin and collagen type I amino-terminal extension of a peptide levels (TPINP) was measured analysis of two sets of data differences.ResultsThe patients in the observation group 3 days, 7 days after, 10 days after VAS scores were lower than the control group[(3.49±0.31) points vs.4.69±0.46 points](P<0.05);[(2.40±0.25) points vs.(3.54±0.35)points](P<0.05);(1.33±0.14) points than (2.42±0.27) points, P<0.05).Patients observed Group 8 weeks, 12 weeks, 16 weeks after the fracture line rates were higher (1.59±0.27 points than (1.01±0.21) points(P<0.05);(2.65±0.40) points than (1.96 ± 0.36)min, P<0.05;(3.45±0.46) points than (2.65 ± 0.41) points(P<0.05).③ patients after 8 weeks of observation group and TPINP water osteocalcin were higher (65.37±8.07)μg/L ratio(39.95±7.18)μg/L(P<0.05);[(81.05±6.89)ng /mL ratio of (39.95±7.18) μg/L] (P<0.05).④ observation group 8 weeks after fracture healing X-ray grade level 69.70% than the control group 37.50%, and the difference was significant (P<0.05).Conclusionlimb fracture patients after taking the capsules Xiaoyu relieve pain, regulate the body and osteocalcin levels TPINP to promote fracture healing.
4.Blockade of renin-angiotensin system decreases the incidence of streptozotocin-induced diabetes in rats with long-term high fat diet via inhibiting endoplasmic reticulum stress

AIM: To investigate the effects and mechanisms of renin-angiotensin system (RAS) blockade on pancreatic islet β-cell function in rats with injection of streptozotocin (STZ) and long-term high-fat diet. METHODS: Normal male Wistar rats fed with 16 weeks-long high calorie, and high-fat diet were treated with telmisartan (TI, n=15), and injected with STZ after 24 weeks. One week later, islet function was evaluated by intravenous insulin releasing test (IVIRT). The immunoglobulin binding protein (BIP)/glucose-regulated protein (GRP78), C/EBP-homologous protein (CHOP)/growth arrest and DNA-damage-inducible gene 153 (GADD153) mRNA expression levels in the islets were detected by RT-PCR. The expression levels of insulin and Bax protein in the islets were examined by immunohistochemistry. RESULTS: The incidence of diabetes was 80%(12/15)in the high-fat diet+STZ group (HS) and 33% (5/15) in the high-fat diet+STZ+ telmisartan group (TS). Compared to HS group, maximum of insulin secretion in TS group was increased 56.9%. Early insulin secretion index (EISI) and acute insulin response (AIR) were increased by 1.98 times and 0.88 times, respectively. The expression of insulin and insulin positive cell density (PCD) were increased obviously in β-cells. The expression levels of BIP, CHOP and Bax in the islets were decreased significantly. CONCLUSION: Blockade of RAS increases the resistance to streptozotocin-induced diabetes in rats with long-term high-fat diet, and the expression of apoptosis-related molecules is downregulated in endoplasmic reticulum. The mechanism that RAS blockade improves pancreatic islets function and reduces diabetes incidence to some extent may be via attenuating endoplasmic reticulum stress.
5.Effect of captopril on AGS nude mouse model of gastric cancer

AIM:To observe the effect of captopril on the genesis and development of gastric cancer , and to explore its clinical treatment feasibility for gastric cancer .METHODS:The human gastric cancer cell line AGS was used to establish a tumor model in nude mice , and the model mice were randomly divided into 3 groups: positive control ( 5-fluorouracil) group, normal control (saline) group and experimental (captopril) group.After intraperitoneal injection or intragastric administration of the drugs , the tumor growth curve was determined , and the tumor tissues were also sampled to detect the expression of Ki-67, STAT3, Bax and Bcl-2 by real-time quantitative PCR and immunohistochemistry .The apop-tosis was detected by TUNEL +DAPI staining .RESULTS: The tumor growth curve showed that the tumor model in the nude mice was successfully established .The tumor volumes among groups showed significantly different after 14 d growth. The increase in the tumor volume in normal control group was significantly faster than that in the other two groups , and that in positive control group was the slowest .The expression of Bax in captopril group increased , and the expression of STAT3, Ki-67 and Bcl-2 was reduced as compared with normal control group and positive control group .Compared with normal con-trol group, the apoptotic rate increased significantly , and the protein expression of p-STAT3 and STAT3 decreased obviously in positive control group and captopril group .CONCLUSION:With better feasibility , angiotensin-converting enzyme in-hibitor captopril has a significant effect on treating gastric cancer in the AGS nude mouse model by regulating the expression of STAT3, Bax, Bcl-2 and Ki-67 to accelerate the apoptosis of cancer cells , thus inhibiting tumor growth .
6.Advances in diagnosis and treatment of small bowel adenocarcinoma

Small bowel adenocarcinoma (SBA) is a relatively uncommon neoplasm with poor prognosis. However, the incidence rate of this condition increases. SBA is usually diagnosed at the late stages, and the majority of patients present with the advanced stage. Data are limited when making decisions for treatment because of the lack of randomized trials for SBA. Radical surgery is considered necessary when possible. Adjuvant chemotherapy is predicted to be beneficial, but this procedure has not yet been investigated through randomized trials. Platinum-based chemotherapy is apparently the most effective treatment regimen used in retrospective tri-als for advanced SBA. Targeted therapies, such as those against the angiogenetic pathway or the epidermal growth factor receptor pathway, have not yet been established for this type of cancer. This article reviews the progress in the diagnosis and treatment of SBA.

10.Effect of prostaglandin E1 lipid microsphere on cardiac haemodynamics and oxygen metabolism during the perioperative period in the infants with ventricular septal defect and severe pulmonary artery hypertension

Objective To measure the effect of prostaglandin E1 (PGE1) lipid microsphere on cardiac haemodynamics and oxygen metabolism during the perioperative period in the infants with ventricular septal defect(VSD)and severe pulmonary artery hypertension (PAH).Methods Forty infants [(7.1 ± 3.3) years old] with VSD and severe PAH who underwent surgery under cardiopulmonary bypass were involved in the study.They were divided into 2 groups averagely:control group (20 cases) and experimental group (20 cases).All the patients were continuously intravenous pumping of nitroglycerin or PGE1 during the perioperative period.The effect of PGE1 on cardiac haemodynamics and oxygen metabolism between the 2 groups were measured during 72 hours postoperatively.Results The statistical analysis demonstrated that the values trend of mean arterial blood pressure (mABP),mean pulmonary arterial pressure (mPAP),mPAP/mABP,pulmonary vascular resistance index (PVRI),left ventricular stroke work index (LVSWI) were affected during 72 h postoperative period (P <0.05).The mABP at 48 h,LVSWI at 48 h,72 h in experimental group were significantly higher than those in control group (all P <0.05).The mPAP at 8 h,48 h,PVRI at 72 h and pulmonary arterial wedge pressure (PAWP)at 12-48 h in experimental group were significantly lower than that in control group (all P < 0.05).Compared to postoperative period,mABP at 12 h,72 h,mPAP at 4-12 h,48 h were increased significantly in control group (P < 0.05) ; mABP and LVSWI at 8-72 h,right ventricular stroke work index at 48 h,72 h and cardiac index at 72 h were significantly increased (P <0.05),while PVRI and PAWP at 72 h,mPAP/mABP at 24-72 h were significantly decreased in experimental group (P < 0.05).There were no significant differences in the values of oxygen metabolism between both groups (P >0.05).Conclusions LipoPGE1 can significantly decrease the pulmonary arterial pressure,which can enhance cardiac function and decrease the duration of intubation after surgery.
