Objective To observe the expressions of grouth-related oncogen (GRO)α, epithelial neutrophil activating protein-78 (ENA-78) and neutrophil-activating peptide-2 (NAP-2) of rat asthma. And to investigate the role of neutrophil in the pathogenesis of asthma exacerbation. Methods In this experi-ment, the rat model of asthma were randomly divided into two groups on average, including asthma group and control group. Levels of ENA-78 at blood neutrophil were detected by flow cytometry method. The ex-pressions of GROα protein at bronchial wall and NAP-2 protein at blood neutrophil were detected by immuno-histochemieal method. Results Levels of GROα, ENA-78 and NAP-2 proteins in asthma group [0.138 ±0.009(A value), 97.65±13.99(MFI), 0.198±0.016(A value), respectively]were significantly higher than those in control group[0.077±0.010(A value), 50.79±8.66(MFI), 0.079±0.015(A value), re-spectively], all P < 0.01. Conclusion Levels of GROα, ENA-78 and NAP-2 were increased at rat asth-ma. They may be participate in inflammation of asthma exacerbation. Neutrophil may promote inflammatory cells influxing into airway wall via increasing synthesis of CXC chemotactic factors.

Objective To investigate curative efficacy and safety of Epirubicin in combination with Capecitabine in the treatment of triple-negative breast cancer and its effects on level of serum vascular endothelial growth factor C ( VEGF-C).Methods 86 patients of triple-negative breast cancer who received therapy from January 2013 to December 2014 were selected as research objects.According to therapeutic schemes,those patients were divided into control group(n=42)and observation group(n=44).Control group was treated with epirubicin,while observation group was treated with Epirubicin in combination with Capecitabine.Then, the short-term curative efficacy, adverse events effects,level of VEGF-C were observed and compared.Results After the course of treatment, the short-term curative efficacy ratio in control group and observation group was 59.5% and 65.9% respectively with no statistical difference.During the treatment, the occurrence of neutropenia, hand-foot syndrome, gastrointestinal reaction, cardiac damage and liver function damage in the two groups was statistically same respectively.After post-treatment, in comparison with control group, level of VEGF-C in observation group was statistically lower ( P <0.05 ).Conclusion Epirubicin in combination with Capecitabine is effective for triple-negative breast cancer,which has high short-term curative efficacy ratio and significantly decrease level of VEGF-C with not increasing occurrence of toxic side effects.

Objective To summarize the experiences in using Bio-Gide and Bio-Oss for guided bone regener- ation in dental implantation.Methods In 28 cases of bone deficiency,Bio-Gide membranes were applied to cover alveolar defects filled with the Bio-Oss bone powder.In postoperative periodic follow-.up,the bone regeneration effect was observed by successive clinical and X-ray examination.Results 38 implants were inserted in the 28 patients and Bio-Gide membranes were used in the sites of the 38 implants.Alveolar bone defects were filled with new bone in 27 patients,1 implant loosed because of inflammation.37 implants had ideal osseointegration at stageⅡsurgery and were prosthetic rcconstructed successfully.No implant loosed during the observed period of 15 months to 4 years. Conclusion Bio-Gide and Bio-Oss have ideal effect of guided bone regeneration in dental implantation.

OBJECTIVETo typing of tibial plateau fracture based on spiral CT reconstruction and to explore effect of the typing method for treatment.

METHODSA hundred and twenty-six cases with tibial plateau fracture (male 95, female 31, age from 23 to 58 years old), the fractures were classified based on reconstruction image of spiral CT. Including central compression type in 13 cases, split type in 8, split compression type in 79, comminution type in 26. According to the different typing the suitable incision of operation and fixed method for fracture were select.

RESULTSA hundred and twenty-six cases were followed up for 0.5-4 years with an average of 1.2 years. According to Hohl system score to knee joint function, there were statistical significance in the pain,active movement,active range of motion between before and after operation (P < 0.01) and there were no statistical significance in stability and self-evaluation (P < 0.01).

CONCLUSIONTyping of tibial plateau fracture based on spiral CT reconstruction helpful to choose operative approach, reduction and fixed method and obviously improve clinical effect.

OBJECTIVETo evaluate the efficacy of bevacizumab in combination of irinotecan,fluorouracil and leucovorin for metastatic colorectal cancer treated by failed prior oxaliplatin -based regiment.

METHODSSixty-two patients were randomly divided into two groups, group A of 30 patients received bevacizumab plus irinotecan, fluorouracil and leucovorin, group B of 32 patients received irinotecan, fluorouracil and leucovorin. The response rate,change of tumor markers,one year survival rate and safety were observed.

RESULTSTumor response rate was 30% in group A, 21.8% in group B respectively. Disease control rate(CR+PR+SD) was 80% in group A, 50% in group B. The obvious change of concentration of tumor markers was observed between pre-treatment and post-treatment, which was significantly different in group A(P<0.05). One year survival rate, median of time to progression and median duration of survival between group A and group B were 26.7% vs 18.8%, 5.9 months vs 3.9 months, 10.9 months vs 8.9 months(P<0.05). The adverse effect in group A was the same as group B. Bevacizumab was associated with hypertension and bradycardia.

CONCLUSIONSThe chemotherapy of bevacizumab combined with irinotecan, fluorouracil and leucovorin results in better efficacy in patients with progressive metastatic colorectal cancer.

Objective To investigate the dynamic change of nutritional risk and malnutrition before op-eration and chemotherapy beginning and at the chemotherapy end in the patients with gastrointestinal malig-nant tumors.Methods The patients receiving gastrointestinal malignant tumors operation in Zhejiang Provin-cial Tongde Hospital from January 2017 to December 2022 were selected as the study subjects.The anthropo-metric measurement data,Karnofsky functional status(KPS)score change were recorded before operation and chemotherapy beginning and at the chemotherapeutic end.The nutritional risk screening 2002(NRS2002)and Global Leadership Initiative Consensus on Diagnostic Criteria for Malnutrition(GLIM)were adopted to evalu-ate the nutritional status of the patients.Results A total of 239 patients were included,there were 221 cases(92.47％)of nutritional risk before the chemotherapeutic beginning,206 cases(86.19％)of GLIM standard malnutrition,71 cases(29.70％)of moderate malnutrition and 135 cases(56.49％)of severe malnutrition in GLIM classification evaluation.At the chemotherapeutic end,there were 182 cases(76.15％)of nutritional risk,181 cases(73.75％)of GLIM standard malnutrition,52 cases(21.76％)of moderate malnutrition and 129 cases(53.97％)of severe malnutrition in GLIM classification evaluation.The screening rate of nutritional risk and malnutrition at the chemotherapeutic end were significant higher than those before operation and the difference was statistically significant(P＜0.001).The levels of total protein,albumin,leukocyte count,neu-trophile granulocyte,hemoglobin and platelet count at the chemotherapeutic end were decreased compared with those before operation with statistical difference(P＜0.05).Conclusion The nutritional risk and malnu-trition incidence rate in the patients with gastrointestinal malignant tumors are high,moreover which at the chemotherapeutic end are higher than those before operation.

OBJECTIVETo improve the diagnosis and treatment of coincident vesical transitional cell carcinoma (VTCC) and prostate cancer.

METHODSWe analyzed the clinical data of 5 cases of coincident VTCC and prostate cancer.

RESULTSThe 5 patients, at the mean age of 66.2 years, were diagnosed as having grade II - III VTCC by cystoscopy and biopsy, 1 with a history of prostate cancer, and the other 4 with prostate cancer confirmed by postoperative pathological examination. Two of the patients were treated by radical cystoprostatectomy, 1 by radical cystoprostatectomy and ileum conduit surgery, 1 by transurethral resection of bladder tumor, and the other 1 by palliative ureterocutaneostomy due to cardiopulmonary problems. The follow-up lasted 8 -26 months. One of them died of diffused metastasis 20 months after surgery, 1 survived with the tumor untreated, and the other 3 remained tumor free.

CONCLUSIONCoincident VTCC and prostate cancer is easy to be missed in diagnosis. PSA detection, rectal palpation, transrectal ultrasonography, biopsy, and cystoscopy are the main diagnostic options for this disease. Its treatment should be based on the classification and clinical staging of the two cancers. Coincident VTCC and prostate cancer does not suggest poor prognosis.

OBJECTIVETo study the effects of ulinastatin on coagulation in children who underwent open-heart surgery with cardiopulmonary bypass (CPB).

METHODSFifty children who underwent open-heart surgery for ventricular septal defect were randomly divided into two groups: ulinastatin treatment and control. Before CPB, ulinastatin (1.0×10(4) U/kg) was added to CPB priming fluid only in the ulinastatin treatment group. Activated partial thromboplasin time (APTT), prothrombin time (PT), thrombin time (TT), fibrinogen and international normalized ratio (INR) were measured both before and at 1 hr, 6 hrs and 24 hrs after CPB.

RESULTSThe PT in the ulinastatin group was more prolonged than in the control group at 1 hr after CPB (18.7 ± 0.7 s vs 15.5 ± 0.5 s) and 6 hrs after CPB (17.5 ± 0.6 s vs 15.0 ± 0.6 s). The APTT in the ulinatatin group was also significantly more prolonged than in the control group at 6 hrs after CPB (38.7 ± 3.1 s vs 35.3 ± 3.1 s) and 24 hrs after CPB (34.2 ± 3.0 s vs 31.1 ± 2.6 s).

CONCLUSIONSUlinastatin may prolong PT and APTT after CPB, and thus affects coagulation in children.

OBJECTIVETo evaluate the influence of stenting main vessel on side branches and to compare the different effects on side branches between BMS and DES (Taxus and Cypher).

METHODSWe reviewed the angiograms and the medical records of 183 patients who had received stent implantation in coronary main vessels and had follow-up angiograms. Any side branch was jailed by stent was evaluated.

RESULTSSide branch occlusion occurred in 8.9% in all branches (10.5% in Cypher DES group, 11.1% in Taxus DES group and 7.8% in BMS group). Spontaneous recanalization of side branches was observed in 72% (90.9% in Cypher DES, 66.7% in Taxus stent group and 66.7% in BMS). The ostial side branch stenosis before stenting and the involvement of the side branch origin within the lesion of the parent vessel are the major independent predictors for side branch occlusion.

CONCLUSIONSThe influence of different DES implantations in coronary main vessels on side branches were similar and there was no difference between DES and BMS. Side branch occlusion had relatively benign clinical course. Most occluded side branches had late spontaneous reperfusion.
