Authors retrospectively analyzed ultrasonographic findings of 12 cases of breast carcinomas which were proven pathologically at Yeungnam University Hospital from March 1992 to August 1992. Classically, breast carcinomas were described as irregular and lobulated hypoechoic solid masses with inhomogeneous internal echoes and frequent attenuation of the sound beam. And other additional ultrasonographic findings were echogenic rim, disruptions of superficial layer, microcalcification, skin thickening and so on. In our studies, not all of these findings of breast carcinomas were found in each case, but most of these findings were noted. However, several studies have demonstrated considerable overlap in the ultrasonographic appearance of benign lesions and carcinoma. Thus, accurate sonographic determination of the type of solid mass is not possible with current ultrasonographic imaging techniques and criteria. For more accurate diagnosis of breast lesions, sonographic and other imaging techniques are interpreted together.

Authors performed the immunohistochemical study with avidin biotin-peroxi- dase complex staining on the nail unit of the human fetus for determining the presence of keratin and prekeratin. Seven fetuses, ranging from 12 to 27 weeks in age, were examined. In all cases, the keratin and prekeratin were found in the epidermis of nail units, but not found in stratum corneum and nail plate. Keratin was located predominantly in suprabasal cells whereas prekeratin was located diffusely in the epidermis. Interestingly keratin and prekeratin were found as early as 12 week-old gestational period. There was no significant difference in presence of keratin and prekeratin.

INTRODUCTION: Percutaneous transluminal angioplasty(PTA) has been widely accepted for the treatment of arterial stenosis. However, acute thrombosis, localized dissection or recoiling may occur as complication, resulting in acute occlusion. Restenosis is also a problem in long term follow up. Recent advance of metallic stent is considered as a good relief for the problems of PTA. PURPOSE: We analyzed 11 patients with metallic stent in the treatment of the iliac artery stenosis. METHOD: Subjects were male patients aged from 53 to 79 years old. All were suffered from claudication of lower extremities, pain and discolorization and diagnosed as atherosclerosis. Pelvic arteriography was performed to localize the stenotic lesion. Through the ipsilateral arterial puncture, metallic stent was inserted after PTA. The metallic stents were Wall stent in 7, Hanaro stent in 2, Palmaz stent 2 and Memotherm stent in 2 lesions. RESULT: The indications for metallic stent were long segment patients, in all cases, the stent was successfully inserted. The pressure gradient measured improved significantly after procedure. For the 3 to 24 months follow up, symptoms has not relapsed clinically. CONCLUSION: On the basis of our priliminary experience, Metallic stent is safe and effective for the treatment of iliac arterial stenosis in conjunction with PTA and surgery.

PURPOSE: This study was done to evaluate the efficacy and safety of percutaneous sclerotherapy or endovascular treatment of hemangioma and venous malformation. METHODS: 9 interventional procedures were performed in 7 patients whose age ranged from 16 years to 52 years. Five of these patients had arteriovenous malformation, the remaining had hemangioma and venous malformation. N-buty1-2-cyanoacrylate was injected percutaneously in 4 procedures and intraarterially in one procedure. Ethanol with lipiodol(10%) was injected percutaneously in 2 procedures. 1valon particle was embolized intraarterial in 1 procedure. RESULTS: N-buty1-2-cyanoacrylate injection, Contour arterial embolozation, Etanole with lipiodo1, sclerotherapy followed by physician and was judged beneficial in 7 of 7 patients. One pateint had minor complication(skin ulcer) but no long term sequelae. CONCLUSIONS: Percutaneous sclerotherapy and endovascular treatment, either or before surgery is a safe and effective method of managing hemangioma and arteriovenous malformations.

A 28-year-old woman presented with multiple, asymptomatic, erythematous to bluish papules located on the chest. Histopathologically, three round, well defined cystic structures were seen on the upper and lower dermis. The first cyst was milia, the second was apocrine hidrocystoma and the other, largest cyst was an eruptive vellus hair cyst (EVHC). A diagnosis of multiple pilosebaceous cysts combined with apocrine hidrocystoma was made. Since the milia and EVHC originate from the pilosebaceous unit, and the apocrine duct opens to the pilosebaceous orifice, we suggest that they can occur simultaneously in the same unit.

Pseudoxanthoma elasticum is a rare, heritable disorder of elastic fibers characterized by yellowish, coalescing papules on the loose and wrinkled flexural skin. An 18-year-old man presented cutis laxa-like marked wrinkling on the axillae, trunk and thighs for about 10 years. There was no similar skin lesion on his family members and no other systemic involvement was found. Histopathological examination of the abdomen revealed fragmentation and calcification of elastic fibers in the middle and lower dermis. Von Kossa staining showed calcium deposition along the altered elastic fibers. Herein, we report a rare case of autosomal recessive pseudoxanthoma elasticum presenting generalized cutaneous laxity.

The authors report a rare case of right distal posterior inferior cerebellar artery(PICA) aneurysm arising from just distal to its vermian brance. A 36-year-old female patient presented with a sudden onset of headache, nausea, dizziness and ataxia without any other neurologic deficit. On brain computerized tomographic(CT) scan and magnetic resonance imaging(MRI), a well-defined round mass was detected just behind the fourth ventricle. On transfemoral vertebral angiogram(TFVAG), it was confirmed as a saccular aneurysm arising from the vermian brance of the right PICA. The aneurysm was initially directly clipped through bilateral suboccipital approach. Postoperative follow up angiogram after two weeks revealed a more enlarged aneurysm and an incompletely clipped neck. After making sufficient exposure of the aneurysm, the neck was reclipped and the sac removed. Postoperative results were uneventful.

Localized scleroderma is a disease of unknown origin and is characterized by circumscribed sclerotic skin changes. When occurring in a linear pattern, it is called linear scleroderma. 'En coup de sabre' is a subtype of linear scleroderma which is characterized by band-like, ivory-colored depressions on the frontoparietal scalp region. Several treatment modalities have been used. However, they have not been effective or safe. We report our experience of two patients with 'en coup de sabre' on the forehead, who were treated successfully with polymethylmethacrylate microspheres (PMMA-NewPlastic(R)).
