2.Dysphagia Caused by Osteochondroma of the Cervical Vertebral Body: A Case Report.

A 56 year-old female presented with dysphagia 8 weeks ago and newly developed dysphonia 2 weeks ago. The radiology study and swallowing difficulty evaluation study revealed the esophagus and the posterior wall of the laryngopharynx to be severely compressed by the mass of the anterior 4th and 5th vertebral body of cervical spine. En bloc excisional biopsy of the bony mass was performed, which completely resolved the clinical symptoms. We report a rare case of osteochondroma occurring at the anterior portion of cervical spine leading to dysphasia and dysphonia with a review of relevant literature.

3.Ossification of Spinal Ligaments.

This authors reviewed the radiographs of 20,057 patients with spinl radiculomyelopathy from April 1983 August 1990. The incidence and distribtion of the ossification of the spinal ligaments were analized as following. 1) The overall incidence of spinal ligaments ossification was 3.6% among 20,057 patients. 2) The most frequently occurs at lumbar area(67.1%), and then cervical(27.5%) thoracic(0.4%) area as in orders. 3) The OPLL was most common type(2.19%) of these spinal ligaments ossification, and then OALL(0.7%), OLF(0.24%) as in orders. 4) OPLL (1) OPLL was most frequent at lumbar area(58.6%) and then cervical(37.0%), thoracic(0.4%) area as in orders. (2) The cervical OPLL was commonly found at C5 & C6 area and morphologically, the multiple segmented type was most common then continuous type, mixed type and single segmented type in orders. (3) The multiple segmented type was more frequently found at upper cervical area, but the continuous type was more frequent at lower cervical area. 5) OALL (1) OALL was most frequent at lumbar area(86.0%) but thoracic(1.4%) and cervecal area(5.3%) were relatively low in frequency. (2) Of the lumbar OALL, the L4/5 area was most frequent site(37.8%) but the L3/4(25.3%) and L5/S1 area(24.5%) also had relatively high occurance rate. 6) OLF (1) OLF was most frequent at lumbar area(92.7%), but the incidence of OLF was relativerly rare at cervical(2.1%) and thoracic area(5.2%). (2) Of the lumbar OLF, the L4/5 area was most common site(33.1), but L5/S1 area had also high occurance rate(27.2%).

5.A Case of Hidrotic Ectodermal Dysplasia.

Congenital ectodermal dysplasia are a group of familial disorders that affect tissues and organs of ectodermal origin to varying degrees. It devided hidrotic type and anhidrotic type by geneologic origin and distinct clinical manifestationa. The hidrotic type usually trait by autosomal dominant gene and manifested dystrofic nails, defect of hair and eyebrow and palmoplantar hyperkeratosis. This patient is 16 year oId male. He had been dystrophic nails, alopecia since at birth, thereafter palmoplantar hyperkeratosis, clubbing fingers noted. C.R.C. Urinalysis, stool examination, chest x-ray were within normal limit. Biopsy specimen from palm revealed hyperkeratosis, hypergranulosis, acanthosis and, mild inflamrnatory cell infiltrate on upper dermis. Biopsy speeimen from the scalp revealed norrriaI findings except slight hypoplasia of ebaceaus gland.

6.A Mycological Study on the Distribution of Candida Species in Normal Human Skin.

This study was done on 171 volunteers in order to investigate the present status of cutaneous normal flora of Candida species and the distribution of Candida species on the normal human skin in dermatologic clinic of Korea University Hospital from January, ]984 to December, 1984. Among the 171 volunteers, 115 strains of Candida species were identified by culture on the Sabouraud's dextrose agar media, germ tube test, sugar assimilation test, sugar fermentaion test and culture on corn meal with Tween 80 media, The results were summarized as follows: 1. The incidence of cutaneous normal flora of Candida species was 13, 5,5 (115 species among 855 sites) 2. Candida species were distributed in oral cavity(45 species), 4th interdigital toe web(39 species), groin(12 species), axilla(11 species) and 4th interdigital finger web(8 species) in order of frequency. 3, Of ll5 species, 45 species were cultured in summer, 27 species in autumn, 18 species in winter and 14 species in spring. 4 Of 115 Candida species, C. albicans(88 species), C. parapsilosis(12 species), C, tropicalis(8 species), C. guilliermondii(3 species) and C. stellatoidea(2 species) were identified. Two species could not be identified.

7.Clinical Application of the Argon Laser in Dermatology.

The argon laser has been used in the treatment of a variety of cutaneous vascular and pigmented lesions. In 1983, authors reported on the therapeutic effect from argon laser treatment of nevus flammeus. This study was undertaken to evaluate the therapeutic effect and complication of the argon laser on many cutaneous lesions except nevus flammeus. The data of the 67 patients who were treated between November, 1984 and March, 1986 and could be followed up over 3 months at the Department of Dermatology, Seoul National University Hospital were analyzed. Patients were treated with Coherent Radiation Model No. 920 Laser with a spot size of 1 mm in diatemer. The pawer ranges were between 1.6 and 2.6 W using pulsed wave or continuous wave according to the size and the shape of the lesions. The argon laser is an effective modality in the treatment of superficial vascular lesions and especially when telangiectasia is prominent. In pigmented lesions, argon laser is effective for pigmented nevi excluding intradermal nevi and has been found effective in small sized nevus spilus and lentigines.

8.A Study on Comparison of the Physical Growth between the Korean and the Japanese Young Adults(18-29 yrs.).

The physical conditions of the Korean and the Japanese young adults (aged 18-29) were analyzed for the 10 important body dimensions such as stature, body weight, sitting height, chest circumference, and etc.. The Korean data published in 1979 and 1992 were compared with the Japanese data published in 1984 and 1994, respectively. Also, the physical developments and the sex differences of the Korean in past 13 years were compared with those of Japanese. The results are as follows. 1) The stature, sitting height and perineum height of the Korean young male and female are significantly smaller than those of the Japanese. 2) In the past, the chest circumference, waist circumference, hip circumference, body weight of the Korean are larger values than those of the Japanese. But, there are no significant differences in recent surveys. 3) Most body dimensions of male show significantly larger than those of female in the two countries. 4) There is no significant difference in thigh circumference between the Korean male and female in 1992. 5) There are no significant differences in thigh circumference between the '84 Japanese male and female and in hip circumference between the '94 Japanese male and female. 6) Compared with the results of the past surveys, the relative body weight, perineum height, and chest circumference of female to male are decreased in the two countries. But, the relative hip circumference of female to male are increased in both countries.

9.Changes of Ranges of Motion According to Ages and Manifestation Frequency of Impingement Sign in Shoulder Impingement Syndrome.

To analyze the changes of range of motion and manifestation frequency of impingement sign according to ages in shoulder impingement syndrome after excluding complete rotator tears by MRI, the authors reviewed 74 patients whose impingement injection tests to the subacromial space were positive in the Dankook university hospital. We used paired student t-test for comparison with the contralateral shoulder. l. Average forward elevation, abduction, external rotation and internal rotation of the shoulders were not significantly different in stage I and II, but significantly different statistically in stage II over 40 years old. 2. Average external rotation at 90degrees abductions of the shoulders was not significantly different statistically in stage I, II and stage II over 40 years old. 3. Frequencies of the painful arc were 83% in stage I, 71% in stage II and 58 % in stage II over 40 years old. Physical examinations of painful arc in 1 case of stage II, 7 cases of stage II over 40 years old were impossible because of limitation of motion. 4. Frequencies of lateral impingement sign were 100% in stage I, 86% in stage II, 100% in stage II over 40 years old. Frequencies of Neer's impingement sign were 75% in stage I, 67% in stage II, 88% in stage II over 40 years old. Frequencies of Hawkins impingement sign were 67% in stage I, 62% in stage II and 82% in stage over 40 years old. Except external rotation at 90degrees abduction, range of motion of the shoulder may be decreased in impingement syndrome over 40 years old. Because of difference of frequencies, more than two impingement signs should be tested for the diagnosis of shoulder impingement syndrome.

10.A Clinical Study of Soft Tissue Sarcoma in Orthopedic Surgery

The soft tissue sarcoma ih the Orthopedic surgery is not common, occuring in less than one percent of all malignant tumors. Whille the sarcoma is comparatively highly malignant, its treatment has been varied, and the effect of the each treatment has been controversial and generally poor on statisties. We studied 58 cases of soft tissue sarcoma in the Orthopedic department of Pusan Gospei Hospital in the 6 years period from March, 1975 to March, 1981. We confirmed the sarcoma diagnosis by biopsy, and measured the size, the degree of local infilteration and occurrence of metastasis with the help of plain X-ray, angiography, lung and bone scan, and computerized tomograms. We made treatment plans according to above results. We classified the soft tissue sarcoma into 8 histologic types and obtained following conclusions: l. Among the 58 cases of soft tissue sarcoma, and majority is consisted of fibrosarcoma, liposarcoma and rhabdomyosarcoma. 2. There were 41 cases occurred in the lower extremity and 17 cases in the upper extremity. Thus soft tissue sarcoma are more frequent in the lower extremity, especially in thigh (2 patients). 3. We found 19 cases between the age of 41 to 50 years. In general, the soft tissue sarcoma are more frequent between the age of 31 to 50 years. 4. Among the 58 cases, 10 patient came to our hospital because of recurrence of soft tissue sarcoma, after receiving local excision in other hospitals. In other words 17 percent of our cases showed recurred sarcoma and duration of recurrence was within 16 months. 5. Metastasis of the soft tissue sarcoma had taken place in 17 patients (29%), most frequently metastasis to the lung in 12 cases (70%). 6. We treated 30 cases by chemotherapy and radiotherapy after excision or amputation. In 15 sarcomas that could not be excised, but the medical treatment with chemotherapy and radiotherapy. 7. The follow-up study revealed better result obtained in the group of aggresive treatment with anticancer chemotherapy and radiotherapy after excision or amputation.
