Objective To observe the changes of plasma lipoprotein concentrations after an oral glucose tolerance test and research their relation to insulin resistance in Type 2 diabetic patients.Methods The age,duration of diabetes,sex,waist circumference,hip circumference,waist-hip ratio,height,body weight,body mass index ( BMI ),systolic blood pressure,diastolic blood pressure,fasting plasma glucose,HbAl C,fasting insulin,and fasting and 60-,120-,180-minute post-load plasma glucose,plasma insulin,triglycerides ( TG),high-density lipoprotein cholesterol (HDL-C),low-density lipoprotein cholesterol(LDL-C) and total cholesterol(TC) after a 75-g oral glucose tolerance test were measured in 26 Type 2 diabetic patients.Comparison of plasma fasting lipoprotein concentrations and postload lipoprotein concentrations was made,and the relation between fasting and post-load lipoprotein concentrations and Homa-IR was researched.Stepwise multiple regression analysis was applied to investigate the areas under the lipoproteins' curves over 180 minutes.Results The declination of post-load lipoprotein concentrations after the 75-g glucose tolerance test was statistically significant( all P ＜0.01 ).Plasma TG concentration both in fasting and post-load state was positively related to Homa-IR( all P ＜ 0.05 ),and plasma HDL-C concentration in fasting and post-load state was negatively related to Homa-IR( all P ＜ 0.05 ).However,the correlation between both fasting and post-load state LDL-C and TC concentration and Homa-IR had no statistical significance.Moreover,stepwise multiple regression analysis revealed Homa-IR was the independent factor capable of modulating the area of HDL-C curve over 180 minutes after the 75-g glucose tolernce test ( P ＜ 0.05 ).Conclusion There was a significant declination of plasma lipoproteins after a 75-g glucose tolerance test,and plasma fasting and post-load TG and HDL-C concentrations were related to insulin resistance.Insulin resistance should play a significant role in the post-load HDL-C concentration after 75-goral glucose tolerance test.

Objective To observe the features of hypocalcemic myopathy secondary to hypoparathyroidism (HP). Methods The symp-toms and signs, laboratory findings and treatments of 13 patients with hypocalcemic myopathy secondary to HP were analyzed retrospective-ly. Results 13 cases of HP were classified into idiopathic HP (8 cases), secondary HP (3 cases) and pseudo-HP (2 cases). The symptoms of hypocalcemic myopathy secondary to HP included tetany, episodic carpopedal spasm, muscle cramps and myalgias in the forearms and calves. It presented blepharospasm, trunk spasm and laryngospasm. Some patients demonstrated positive Trousseau's and Chvostek's sign. Serum level of creatine kinase (CK) and lactate dehydrogenase (LDH) increased. The symptoms of myopathy released and the levels of CK and LDH decreased to normal level after enough calcium carbonate and calcitriol administration. Conclusion All types of HP (idiopathic, secondary and pseudo HP) can cause hypocalcemia and myopathy. Hypocalcemic myopathy secondary to HP would well recover by enough calcium carbonate and calcitriol treatment.

Objective To investigate the clinical significance of the expression of CD_(21) on peripheral blood B lymphocytes in different age periods.Methods All the cases was divided into newborn group,infant group and children group.The expression of CD_(21) on B lymphocytes were analyzed by the flow cytometry.Results 1.The number of CD_(21) on B lymphocytes of newborn group were obviously lo-(wer) than those in other two groups;and with growth of a child,the numer tended to grow up.2.Mean fluorescence intension(MFI),the expression of CD_(21) on B lymphocyte,also had the characteristic of increase with the age.Conclusions With the growing up of a child,the expression of CD_(21) on B lymphocyte has the tendency of up-regulation;which conforms to the feature of immune system development in childhood.Because of the higher expression of CD_(21) on B lymphocyte in certain age,it has the sensitivity to Epstein-Barr virus infection for children.

Objective To find out the relationship between the serum resistin levels and obesity, insulin sensitivity, blood-glucose as well as blood lipid in the elderly with diabetes. Methods Senile patients (51) suffering from type 2 diabetes and non-diabetic controls(42) were selected. Their serum resistin levels after an overnight fasting were measured by competitive ELISA. Other data including fasting blood-glucose, insulin, blood lipid, insulin sensitivity index, waistline, hipline and WHR were measured. Changes in respiratory function before and after CPB in 40 children with state of left-to-right shunt of VSD were studied. Among the cases, 20 of them were with pulmonary hypertension (PH group ) and 20 without PH (control group). Before CPB, resist ante of inspira tion (RI) and resistance of expiration (RE)in PH group (23.87?7.53,42.98?12.1) were significantly higher than those of control group(RI 17.95?7.53,RE 34.95?5.13);compliance of lung-thorax(CLT) in PH group(11.58?4.87) was significantly lower than that of control group(14.32?1.99). After CPB. both RI (30.94?11.89) and RE(46.43?9.9) in PH group were greatly higher than those in control group (28.00?9.29,46.43?9.9);CLT (10.98?4.6) was lower than that of control group (12.3?2.68). In PH group,the PaO_2 decreased and the magnitude of A-aDO2 increased much more than those in control group. The results suggest that respiratory function in the children with PH might not be improved immediately after CPB separation.

Objective To evaluate the applicative value of fiberoptic ductoscopy(FDS)in abnormal nipple discharge.Methods A total of 104 patients with nipple discharge were examined by fiberoptic ductoscopy(Germany Scholly Fiberoptic Gmbh 42.0500).The results were compared with postoperative pathological diagnosis.Results All the 104 cases were successfully diagnosed with FDS.The detectable rate for intraductal mass by ductoscopy is 94 %(49/52).The sensitivity and specificity of ductoscopy for mammary duct cancer is 50%(1/2)and 91%(43/47)respectively;the negative predictive value is 98%(43/44),positive predictive value is 20%(1/5),and accuracy is 90%(44/49).Five cases of papillomatosis were diagnosed by ductoscopy,in which 3 cases were consistent with postoperative pathological findings,therefore the diagnose accordance rate is 60%;the other 2 cases were proven to be multiple papilloma.Thirty-eight cases of papilloma were detected by ductoscopy and received lobectomy of bresat;55 cases of inflammatory duct disease did not receive any operation;and 3 cases(3%,3/104)were diagnosed papilloma by re-examination with ductoscopy due to persistent nipple discharge,and were confirmed after operation.Conclusions Fiberoptic ductoscopy is the first choice for examination of nipple discharge for its accuracy in finding pathogenic cause and locating lesion,and surgery can thereby be avoided for galactophoritis and mammarv duct ectasia patients.

Objective To study the curative effect and characteristics of all -transretinoic acid (ATRA) and arsenic acid (As2 O3 )combination chemotherapy for acute promyelocytic leukemia (APL).Methods ATRA (30 -60mg/d),As2 O3 (0.1%As2 O3 d1 -28),chemotherapy(anthracycline -based chemotherapy regimens and homoharringtonine -based chemotherapy regimens or high -doses of cytosine arabinoside and small -doses of metho-trexate)were used alternately 4 -5 cycles.Results 12 cases had complete response(CR),CR rate was 100%,first cases and relapse cases all got CR,the median time needed 30 days to get CR.Follow -up deadline,all cases 10 year disease -free survival (DFS )rate was 100%,all reexamination minimal residual disease (MRD)was negative. Conclusion Using this method to treat adult APL,the CR rate is high,has less adverse reaction,and did not produce resistance phenomenon.It can be used for APL consolidation and maintenance therapy.This is one of the ways that can be recommended in the treatment.

Objective To assess the value of dual-energy computed tomography myelography (CTM) on detecting leaks of cerebrospinal fluid (CSF) in patients with spontaneous intracranial hypotension (SIH). Methods Six patients with SIH underwent spinal CTM on a 2nd generation dual-source CT with tube voltage set at 100 and 140 kVp(with tin filter). The virtual non-contrast (VNC) and iodine map images were calculated from dual-energy images. The average weighted (AW) CTM images were mixed from two kVp images with mix factor of 0. 5. Two radiologists evaluated CSF leak using two sets of images respectively: VNC + iodine map images and AW-CTM images. The results from two reading methods were compared. The level of CSF leaks along the nerve roots, C1-2 retrospinal CSF collections, epidural CSF collections and spinal epidural venous plexus were marked. The consensus about leak sites and CSF collections was made by two radiologists in the third session Kappa statistics were used to measure the agreement between the two methods. Results Forty-one leaks were detected using VNC + iodine map images. Forty-three leaks were detected on AW images. The agreement between two methods was excellent (Kappa =0. 997 ,P ＜0. 01). There were no differences in the detection of C1-2 retrospinal CSF collections (n = 2), epidural CSF collections(n = 3) or spinal epidural venous plexus (n = 1). VNC and iodine map images demonstrated superior visual effects than AW images. Conclusion Dual-energy CTM can be used to diagnose spontaneous spinal cerebrospinal fluid leaks in SIH patient.
