1.Strategy research for improving teaching quality in clinical probation of hematology department

The current situation is that the study of blood system diseases is difficult, and the students lack both the initiative and enthusiasm to learn. Clinical teachers' energy is limited because of other clini-cal, management and research roles, so the quality of teaching varies greatly. Meanwhile, teaching quality has nothing to do with personal reviews. Furthermore, the relationship between doctors and patients is in trouble in current medical environment. All of these above affect the quality of clinical practice in hematology department. Aiming at these problems, we improve the teaching quality significantly by raising the students' interests, enhancing their initiative, improving teachers' teaching level and enthusiasm, carrying out the in-ternship system, and strictly enforce the elimination system.
2.Application of Quality of Life to Evaluate the Effect of Traditional Chinese Medicinein on Parkinson's Disease

3.Influence of dexmedetomidine on SAS score and Ramsay score in patients with hypertensive cerebral hemorrhage after borehole drainage

Objective:To explore influence of dexmedetomidine on Riker sedation -agitation scale (SAS) score and Ramsay sedation score in patients with hypertensive cerebral hemorrhage after borehole drainage . Methods :A total of 96 patients with hypertensive cerebral hemorrhage after borehole drainage treated in our hospital were selected . According to analgesic drugs taken after operation ,they were divided into dexmedetomidine group (n=48) and mid‐azolam group (n= 48) .Riker SAS score ,Ramsay sedation score and adverse reactions after operation were com‐pared between two groups . Results:Compared with midazolam group ,there were significant rise in Ramsay seda‐tion score [6h :(1.59 ± 0.52) scores vs .(2.13 ± 0.15) scores] and significant reduction in Riker SAS score [6h :(4.09 ± 0.82) scores vs .(3.73 ± 0.76) scores] on 6h ,12h and 24h after operation ,P<0.05 or <0.01 ;and signifi‐cant reduction in incidence rate of adverse reactions (37.5% vs .10.4% , P< 0.05 ) in dexmedetomidine group . Conclusion:Dexmedetomidine possesses definite analgesic and sedative effects on patients with hypertensive cerebral hemorrhage after borehole drainage ,it's safe andreliable ,which is worth extending .
4.Measure and Analysis on Rehabilitation Therapeutics Education Environment

Objective To assess the perception of rehabilitation therapeutics students to their learning environment.Methods The Dundee Ready Education Environment Measure (DREEM) inventory was used to measure rehabilitation therapeutics education environment by investigating two hundred and forty-seven baccalaureate rehabilitation therapeutics students.Results The rehabilitation therapeutics students perceived the total education environment and four subscales which included perceptions of learning and teachers, environment, and students' academic self-perceptions as relatively satisfactory. Moreover, they perceived that the teachers were knowledgeable, the relationships between teachers and students were better, and this school was well regulations. But the scores of students' social self-perceptions was lower (15.23 ±4.16). Especially, there were some problems in learning approach and accommodations.Conclusion To take advantage of competent teachers, good teacher-student relationship, and proper regulations and overcome unfavorable factors in social activities and approaches to learning is a guarantee of good rehabilitation therapeutics education environment.
5.Strengthening the medical graduate students' ideological and political education by relying on the scientific research groups

Aiming at the weakness of the medical graduate students' ideological and political education,we developed the new mode via investigation and surveying,allowing for the medical characteristics. We suggested that we should bring into full play the tutors and their groups function and excavate the connotation of groups’ morality education on the basis of the scientific research groups to develop the medical students' ideological and political education.

7.Clinical analysis of 26 neonatal pneumonia patients complicated with pneumothorax

Objective To explore the clinical features and prevention measures of neonatal pneumonia complicated with pneumothorax.Methods Clinical data of 26 neonatal pneumonia patients complicated with pneumothorax were retrospectively analyzed.Results In 26 cases of neonatal pneumonia complicated with pneumothorax,16 cases(61.4%) were aspiration pneumonia,10 cases(38.5%) were infectious pneumonia.2 cases of not obvious mild cyanosis and dyspnea were given conservative treatment,24 cases were shortness of breath cyanosis,groan,face pale,difficulty in breathing,restlessness,irritability.Breath sounds were reduced or disappear,sound shift,cyanosis to oxygen can not be sustained remission.Blood oxygen saturation were lower grade.One side or both sides of the chest profile were apparent uplift with thoracic puncture exhaust or thoracic closed drainage.24 cases(92.5%) suffered from pneumothorax were cured.2 cases (7.7%) gave up the treatment,including 1 case died and 1 case loss prevention.Conclusion Pneumonia of newborn pneumothorax is secondary to inhalation of most inappropriate,infection,airway pressure,suction.Once found,timely treatment is important.Mild cyanosis can spontaneous remission,cyanosis having oxygen can not be sustained remission.The prognosis of it by thoracic puncture exhaust or closed thoracic drainage is good.Actively control influence during pregnancy,birth and postpartum.Preventing meconium inhalation can reduce the occurrence of neonatal pneumonia complicated with pneumothorax.
8.13C-methacetin breath test for evaluating of children's liver disorder

Objective To explore the clinical value of 13C-methacetin breath test for the assessment of liver disorder and to analyze its predictive value to the severity of liver function injury in children.Methods Eighteen healthy children served as healthy control group,and 40 patients with different etiology and severity served as experimental group,and then the latter were divided into 2 subgroups,28 patients in Child-Pugh classification A,and 12 cases in below B(11 cases in B and 1 case in C).An oral dose of 2 mg/kg tracer 13C-methacetin was administered to each subject for the 13 C-methacetin breath test.At the same time,serum liver function markers including serum transaminase,bilirubin,albumin and prothrombin time were measured.The acquired data were analyzed by SPSS 17.0 software.Results (1) Metabolisation velocity (MV) max30 and cumulated dose (CUM) 120 in experimental group (46.64 ± 27.93,59.29 ± 30.73) were much lower than those of the healthy control group(73.56 ± 26.03,102.97 ± 41.80) (t =2.450,3.165,all P <0.05);(2) MVmax30 and CUM120 were closely correlated with the liver function markers of albumin,total bilirubin,direct bilirubin,prothrombin time (P < 0.05);(3) MVmax30 and CUM120 could predict liver diseases in children,especially the CUM120.With CUM120 =85.80 as a cut-off value to predict liver diseases,the Youden index was 0.578 at its maximum,and the sensitivity and specificity were 77.8% and 80.0%;(4) Compared with the Child-Pugh classification A,the CUM120 in Child-Pugh classification B and lower B was significantly lower(P < 0.001);(5) CUM120 could predict the severity of liver diseases.With CUM120 =56.15 as a cut off value to predict the severity of liver diseases,the Youden index was 0.857 at its maximum,and the sensitivity and specificity were 85.7% and 100.0%.Conclusion 13C-methacetin breath test index of CUM120 could predict liver diseases in children and the severity of liver function.
9.The pathogenesis of ischemic preconditioning to warm ischemia reperfusion injury of hepatocytes in rats

Objective To explore the pathogenesis of ischemic preconditioning to warm ischemia reperfusion injury of hepatocytes in rats. Methods Ninety SD rats were randomly divided into three experimental groups:sham operation group (group A), warm hepatic ischemia/reperfusion group(group B and group C). Group C was given ischemic preconditioning treatment. Serum levels of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were detected 0 h, 2 h, 6 h, 12 h and 24 h after ischemia reperfusion injury. Levels of TNF-α and IL-1β were tested detected by ELISA. Levels of malondialdehyde (MDA) and superoxide dismutase (SOD) of hepatocytes were detected at the same time points. Mitochondrial membrane potential was examined to assess ischemia reperfusion injury of hepatocytes in rats using chart of intensity of JC-1 in mitochondria. Results The serum levels of ALT, TNF-α, IL-1β, and MDA were significantly higher in hepatic warm ischemia reperfusion group and ischemic preconditioning group than those in sham operation group (P<0.05). Values of prothrombin activity and cholinesterase were significantly lower in liver warm ischemia reperfusion group and ischemic preconditioning group than those of sham operation group (P<0.05). The SOD level of liver was significantly lower in warm ischemia reperfusion group and ischemic preconditioning group than that in sham operation group. The indexes were better in ischemic preconditioning group than those of warm ischemia reperfusion group (P<0.05). The mitochondrial membrane potential level of liver cells reached the lowest value 0 hours after ischemia and reperfusion, and then increased gradually within 24 hours (P<0.05). And the level of mitochondrial membrane potential of liver cells was significantly higher in ischemic preconditioning group than that in warm ischemia reperfusion group (P<0.05). Conclusion Ischemic preconditioning may play a protective role in warm ischemia-reperfusion injury in rats. Ischemic preconditioning may significantly decrease the levels of ALT, AST, TNF-α, IL-1βand MDA, and increase the SOD activity in hepatocytes. Thedamage of mitochondrial membrane potential is decreased after ischemic preconditioning, which might be the pathogenesis of ischemic preconditioning to warm ischemia reperfusion injury of hepatocytes in rats.
10.Effects of arteriovenous fistula on structure and function of heart in patients with hemodialysis

Objective To investigate the effects of artervenous fistula on structure and function of heart in patients with hemodialysis in the Second Clinical Hospital of Chinese Medical University from 2004 to 2005.Methods AVF、LAD、RVD、LVD、LVEDV、SV and EF were examined and analysed before and 8 weeks after AVF in 55 patients with hemodialysis.Results LAD、RVD、LVD、LVEDV after AVF increased significantly and EF reduced evidently after AVF.Conclusion AVF can cause extension of LAD、 RVD、 LVD and decrease of EF.
