2.Bone Mineral Densities in Ballerinas, Korean Traditional Dancers and Athletes.

Evidence in human studies of the association of long term habitual exercise with bone mineral content (BMC) and bone mineral density (BMD) mostly come from studies done on athletes. The highest BMC and BMD values in young adults have been found in strength and power-training athletes, while endurance activities such as long distance running and swimming seem to be less effective in obtaining peak bone mineral density. To compare the differences in BMD among ballet, Korean traditional dancing and athletics, we examined the BMD of 10 ballerinas, 10 Korean traditional dancers, 12 athletes and 12 volunteers as the control group. All participants were female. The average age were 20.6 years for the ballerinas, 21.2 for the Korean traditional dancers, 20.4 for the athletes and 21.3 for the control group. Average weights were 50.6 kg for the ballerina, 53.7 kg for the Korean traditional dancer, 59.2 kg for the athletes and 53.8 kg for the control group. Average heights were 162.0 cm for the ballerinas, 160.4 cm for the Korean traditional dancers, 162.3 cm for the athletes and 160.5 cm for the control group. There were no significant differences in the BMD values of the proximal femur and the lumbar spine between ballerinas and control group and between Korean traditional dancers and control group (p>0.05). There was, however, a significant increase of BMD in the proximal femur and lumbar spine of the athletes when compared with the control group (p<0.05) After adjusting for difference in body mass index (BMI) we recompared the BMD of ballerinas, Korean traditional dancer and athletes with BMD of control group and found significant higher lumbar spine BMD values in ballerinas, Korean traditional dancers and athletes. In athletes higher BMD levels were also obtained in the proximal femur including the neck, greater trochanter and Wards triangle regions (p<0.05). There was no significant increase of proximal femur BMD levels for Korean traditional dancers (p>0.05). Differences of BMD values between the ballerinas and control group was statistically significant only in femoral neck (p<0.05).

3.Unusual Type of Talar Body Fracture: A Case Report

Fractures of the body of the talus are extremely rare, constituting only about 0.1 percent of all fractures (Coltart 1952). Consequently, the prognostic problems have so far been elucidated only on the basis of few reports in the literature. The authors experienced an unusual type of the shearing fracture of the talar body with total disocation that the talar body was lying upside down in the ankle joint. Satisfactory results were obtained after follow up period of 20 months and some are reporting this case with bibliographic reviews.

5.Validity of Office-Based Ultrasonography in the Diagnosis of a Palpable Breast Mass: A prospective study .

PURPOSE: Current evidence indicates that ultrasonography of the breast is an important adjunct to mammography and clinical examination in the diagnosis of palpable breast abnormalities. An assessment of the value of office-based ultrasonography of a palpable breast mass performed by a breast surgeon was the aim of this work. METHODS: A 7.5 MHz linear probe was used to perform office-based ultrasonography in 109 consecutive female patients having a palpable solid breast mass between August 1998 and December 1999. Breast masses diagnosed histologically as fibroadenomas or breast cancer were included in this study. The clinical impression from the physical examination (PE), the result of officed-based ultrasonography done by a surgeon (OUSG), and comprehensive judgement engaging both PE and OUSG (PEUSG) were recorded in each patient's hospital record as benign or malignant at the first visit. The diagnostic values of PE, OUSG, PEUSG, mammography (MMG), and ultrasonography done by a radiologist (USG) were compared. RESULTS: Of 109 masses, a fibroadenoma was diagnosed in 73 patients while a carcinoma was established in 36 patients. The sensitivity, the specificity, the accuracy rate, and the kappa coefficient were 91.7%, 89.0%, 89.9%, and 0.780 for PE; 100%, 90.4%, 93.6%, and 0.862 for OUSG; 97.2%, 97.3%, 97.2%, and 0.938 for PEUSG; 90.4%, 89.4%, 90%, and 0.630 for MMG; and 88.9%, 90.9%, 90% and 0.798 for USG. The diagnostic accuracy of PEUSG was significantly higher than those of PE and MG (p<0.05). CONCLUSION: These data suggest that office-based ultrasonography of the breast performed by a surgeon is a useful adjunct to clinical evaluation of the breast mass and permits timely and cost-effective patient care.

9.Transplantation of Cultured Keratinocytes in Autologous Fibrin Glue Suspension.

The use of a cultured autologous keratinocyte sheet has become a recognized method for the coverage of extensive bums during recent years. The disadvantages of these sheet grafts are a long time-lag until keratinocyte sheets are available, the fragility and difficulty in handling of grafts, an unpredictable take rate and extremely high costs. In this study we investigated the transplantation of cultured keratinocytes as single cells suspended in autologous fibrin glue. In a rat model with standardized full thickness wounds, this new transplantation technique was evaluated and compared directly to the conventional keratinocyte sheet grafting technique. After transplantation, wounds were evaluated for the degree of epithelial coverage, and then microscopic structures were evaluated under light and electron microscopy. The results were as follows: 1) The fibrinogen solution prepared from autologous blood had 12 times more fibrinogen compared to the original blood. 2) After transplantation of cultured keratinocyt-es in fibrin glue, the degree of epithelial coverage was 79% at 2 weeks, which was comparable to 17% for cultured keratinocyte sheet graft 3) Typical basement membrane structures were consistently found at 2 weeks after transplantation of keratinocytes in fibrin glue. 4) Rete ridges were found at 4 weeks after transplantation of keratinocytes in fibrin glue. In conclusion, the transplantation technique of keratinocytes in fibrin glue is available earlier than sheet grafts, it transfers actively proliferating cells and it simplifies the grafting procedure. As well, this technique leads to an earlier epithelial covering and an earlier restoration of the dermo-epidermal junction than sheet grafting.
