4.A Morphologic Study on the Bile Duct Changes Induced by Common Bile Duct Ligation in Rats.

In an attempt to elucidate the pathological changes following common bile duct ligation, the present study was undertaken in male Sprague-Dawley rats. Morphologic studies of the livers were performed at 1, 2, 3, 5, 7, 9, 11, 13 and 15 weeks after common bile duct ligation. In an attempt to clarify the relationship between the process of bile duct formation and the nature of primitive cells observable around the primitive biliary structure, light microscopic, immunohistochemical and electron microscopic studies were performed. The results were noted as follows: 1) Light microscopically, proliferation of biliary cells began in the periphery of portal areas and expanded toward hepatic lobules. In severe cases of biliary structure proliferation, hepatocytic cords and classic hepatic lobules were inconspicuous. 2) Immunohistochemically, CK-19 expression was limited to biliary structures in protal areas and proliferated biliary epithelial cells. In the serial sections of paraffin block, proliferated intrahepatic biliary structures were associated with those of portal areas. Some oval cells in the ductular hepatocytes were stained for both CK-19 and MNF 116. 3) Ultrastructurally, the proliferated biliary epithelial cells divided into three patterns: absence of lumen, formation of incomplete lumen, and formation of complete lumen. Furthermore these patterns had spectral continuity of maturation in their structures. 4) In some biliary structures, individual biliary cells pushed the basement membrane toward neighboring tissue with accompanying destruction of basement membranes, patterns of budding. Sometimes these cells and hepatocytes comprised the same lumen. In summary, the results obtained by the present study indicate that proliferated biliary structures may be derived from the preexisting intralobular or portal biliary system.

5.Clinical Study of Operative Treatment of Medical Collateral Ligamentous Injuries of the Knee Joint

Rotatory Instability of the permitting abnormal external totation of the tibia on the femur is the result af forced abduction of the flexed knee and external rotation of the tibia. The basic lesion is a tear in the medlal capsular ligament. The authors have reported clinical study in 40 patients with medial collateral ligament injuries who were treated by operative methods in the Orthopedic Department at the Taegu Presbyterian Hospital from January 1973 to Octdber 1979. of 40 patients, pes anserinus transplantation was performed on 15 patients and direct repair on 25 patients. The results are as followings: 1. All the patients were seen with a complete rupture of the medial collateral ligament and of these 45% had rupture of the femoral attachement. 2. Exellent to good result were more often obtained in recent injuries of less than 2 weeks rather than those in older Injuries. 3. Exellent to good result were more often obtained in the groups of Pes Anserinus tranplantation than the groups of direct repair. 4. In all cases of groups of pes anserinus transplantation, permanent sensory palsy af the patellar branches of the saphenous nerve was present due to the location of the surgical incision.

7.Bronchioloalveolar carcinoma: a variety of radiographic patterns.

Bronchioloalveolar carcinoma may present with a variety of radiographic and clinical appearances. The authors reviewed the radiographs of 17 patients with pathologically proven bronchioloalveolar carcinoma, retrospectively. Seven cases were of the localized form. Among the seven five showed a localized mass and two showed localized consolidation. Ten cases were of the diffuse form. Among the ten, three showed diffuse nodules, two showed diffuse consolidation, and five showed a combined pattern of nodules and consolidations. Progression of disease was observed in 9 patients. In these cases, localized forms progressed to a diffuse forms and nodular patterns progressed to a consolidation pattern. Seven cases of the diffuse form were given antituberculous medications on hospital admission. In summary, bronchioloalveolar carcinoma has varied radiographic patterns, and often mimicks other pulmonary diseases such as pulmonary tuberculosis.
